일부 60-64세 농촌 거주자에서 혈압과 인지기능 장애와의 관계 (Relationship between Blood Pressure and Impairment of Cognitive Function In Some Rural Residents Aged 60-64)

  • 이 연구는 노인 인구에서의 혈압과 인지기능과의 관련성을 조사하기 위하여 대구광역시 달성군에 거주하고 있는 만60-64세의 노인을 대상으로 1996년 4월 부터 9월까지 면담조사를 실시하였다. 최종분석에 이용된 대상자는 원래 목표 한 1,329명에서 932명으로 70.1%이었다. 인지기능의 측정은 한국판 간이정신상태검사법(MMSEK)을 사용하였으며, 인지기능장애는 점수가 23점 이하인 자로 정의 하였다. 혈압은 누운 상태에서 측 상완부를 휴대용 자동혈압계로 1회 측정하였다. 남자에서 인지기능에 대한 단일변수 지수회귀분석의 결과에서 수축기 혈압은 통계적으로 유의한 비차비를 보이는 혈압 범주는 없었다. 확장기 혈압은 80-89 mmHg를 기준으로 79 mmHg 이하, 90-94, 95 mmHg이상 모두 1보다 큰 비차비를 나타내었으며 특히 79mmHg 이하는 1.68(95% 신뢰구간 1.02-2.75)로 통계적인 유의성을 나타내었다. 여자에서 수축기 및 확장기 혈압 모두 통계적인 유의성을 나타낸 비차비는 없었다. 연령, 교육 수준, 흡연, 음주, 비교체중, 죽상경화증을 시사하는 질환, 고혈압 약 투약력 등의 공변수를 통제한 다변수 지수회귀분석에서 남자의 수축기 혈압은 단일변수 분석과 비교하여 비차비의 변화가 거의 없었으며 통계적인 유의성도 없었다. 확장기 혈압에서 기준 혈압 범주에 비교해서 79mmHg 이하는 비차비가 2.01(95% 신뢰구간 1.15-3.52)로 증가하였다. 여자에서 수축기 혈압은 단일변수 분석과 비교하여 거의 차이가 없었으며 통계적인 유의성도 없었으나, 반면에 확장기 혈압에서 기준 혈압 범주에 비교하여 79 mmHg 이하는 비차비가 0.72에서 0.57 (95%신뢰 구간 0.37-0.89)로 유의하게 증가하였다. 이들 결과는 혈압과 인지기능 장애와의 관계는 수축기 혈압보다는 확장기 혈압이 연관성을 나타내며 확장기 혈압과 인지기능 장애간의 관련성의 방향이 성별간에 차이를 나타내어 혈압과 인지기능사이에 좀더 복합적인 관계가 있음을 시사해 준다.

한국판 섭식장애검사-자기보고형 6.0 (EDE-Q version 6.0) 및 한국판 임상손상평가(CIA)의 신뢰도와 타당도 연구 (A Reliability and Validity Study of the Korean versions of the Eating Disorder Examination Questionnaire version 6.0 (EDE-Q version 6.0) and the Clinical Impairment Assessment Questionnaire (CIA))

  • 연구목적 본 연구는 섭식장애 정신 병리의 주요 측면을 평가하는 한국판 섭식장애검사-자기보고형 6.0 (Eating Disorder Examination Questionnaire version 6.0, EDE-Q 6.0)과 이를 보완하는 도구로써 섭식장애로 인한 이차적인 기능손상 정도를 평가하는 한국판 임상손상평가(Clinical Impairment Assessment Questionnaire, CIA)를 국내에서 활용할 수 있도록 타당화하고자 진행되었다. 방 법 370명의 일반인군(여자 대학생) 및 49명의 섭식장애 환자군을 대상으로 한국판 EDE-Q 6.0, CIA, Body Shape Questionnaire (BSQ) 및 Weight Concern Scale (WCS)을 실시하였다. 결 과 EDE-Q 6.0의 Cronbach's ${\alpha}$는 0.92, CIA는 0.91로 높은 내적 신뢰도를 가지는 것으로 나타났다. CIA에 대한 탐색적 요인분석 결과 영문판과 동일하게 3요인이 추출되었으며, 각각 개인적 손상, 사회적 손상, 인지적 손상으로 명명되었다. EDE-Q 6.0, CIA는 BSQ 및 WCS와 유의한 정적 상관을 보여 수렴 타당도가 검증되었다. 섭식장애 환자군은 EDE-Q 6.0 및 CIA에서 일반인군에 비해 유의하게 높은 점수를 보여 해당 측정 도구가 좋은 판별 타당도를 가지는 것이 검증되었다. 결 론 본 연구를 통해 한국판 EDE-Q과 CIA가 타당하고 신뢰도가 높은 도구임을 확인하였다. 해당 도구는 향후 한국의 임상 장면 및 연구에서 섭식장애의 진단 및 개입에 유용하게 활용될 수 있을 것이다.

치매환자 여부와 혈중 납, 카드뮴, 수은 및 망간 농도와의 상관성 (Correlation between Dementia and Lead, Cadmium, Mercury, and Manganese Concentrations in the Blood)

  • Objectives: The causes of dementia have been reported in various ways, but there has been little research on the interrelationship between heavy metals and dementia, and the results also show little consistency. Therefore, it is imperative to compare the levels of heavy metal exposure between the dementia-suffering group and a control group to confirm the correlation between the level of heavy metal exposure and the likelihood of dementia. Methods: In order to assess the dementia level of the elderly, the Global Deterioration Scale (GDS) and Mini Mental State Examination (MMSE) were applied. To analyze the concentration of heavy metals in the blood, blood was collected from the veins of study subjects and measured using Inductively Coupled Plasma-mass spectrometry (ICP-MS). Results: There was a statistically significant correlation between lead and manganese concentrations in the blood and the MMSE and GDS. It was found that there was a statistically significant correlation between cadmium concentration in the blood and the GDS, but the MMSE was less relevant. It was found that the blood mercury concentration and the MMSE and GDS were less relevant. The lead concentration in the blood was 0.95±0.74 ㎍/dL in the dementia patient group and 0.33±0.22 ㎍/dL in the normal group, while cadmium was 0.69±0.37 ㎍/L in the dementia group and 0.18±0.10 ㎍/L in the normal group. Mercury was 0.81±0.31 ㎍/L in the dementia group and 1.16±0.80 ㎍/L in the normal group. Manganese was 6.83±2.01 ㎍/L in the dementia group and 4.78±1.59 ㎍/L in the normal group. All of these show statistically significant differences. Conclusions: As the concentration of lead, cadmium and manganese in the blood increases, the MMSE scores and GDS scores were found to worsen, and it was confirmed that there is a correlation between heavy metal exposure and cognitive degradation.

영아기 어머니역할 교육 프로그램이 모아상호작용과 영아발달에 미치는 효과 (Effects of Maternal Role Education Program on the Mother-Infant Interaction and Infant Development)

  • The impact of childhood experience has lifelong significance on subsequent health and development. Especially, the experience of infant is mostly affected by the quality of parental care and rearing environment. But the new mothers usually do not know what to do because of the lack of experience in these days. Therefore, an educational program regarding maternal role would be necessary. This study was conducted to evaluate the effectiveness of the maternal role education program for mother-infant interaction, child-rearing environment, and infant development. Non-equivalent control group time-series design was used, and Barnard's mother-infant interaction model was used as a conceptual framework of this study. The subjects were the healthy infants weighing over 2,500gm at birth, whose gestational age was more than 37 weeks, and their mothers. The final sample consisted of 19 mother-infant dyads for intervention group and 18 dyads for control group. Data were collected from March 15th to September 3rd in 1999. For the intervention group, programmed education which focused on mother-infant interaction, breast feeding, and infant care was provided before discharge. Telephone counselling was provided within one week after discharge. Home visiting for maternal role education was provided twice, one month and three months postpartum. For the control group, home visiting was also conducted but only for data collection. The data were analyzed using chi-square test and t-test to test the equivalence of two groups, and the effectiveness of intervention program was determined with repeated measure ANCOVA and t-test. The results were as follows: 1. Significant differences were found in mother-infant interaction between two groups(p=.000). It indicates that intervention program was effective in improving mother- infant interaction. In subscale analysis, four out of six subscale showed significant differences between the groups: sensitivity to cues (p=.000), social-emotional growth fostering (p=.000), cognitive growth fostering(p=.000) in mothers, and responsiveness to caregiver (p=.019) in infants. 2. The difference in the mean score of childrearing environment (HOME) between the intervention group and control group was significant(p=.003). When each subscale of HOME was examined individually, intervention group showed significantly higher scores in the diversity of stimulation(p=.000), and mother's involvement(p=.001). 3. Three-month-Infants of the intervention group showed higher GQ in the Griffiths mental development scale(p=.026). In subscale analysis, significant differences were found in the personal-social(p=.005), and the hearing and speech(p=.003). In conclusion, the maternal role education program proved to be effective in promoting the mother-infant interaction, organizing the childrearing environment, and fostering the infant development. These results are very meaningful that we found maternal role education necessary for normal infants' mothers, and that nurses can make a great contribution in promoting health of infants and mothers.

스트레스 관리 시 호흡치료의 이론적 근거와 기법 적용 (Theoretical Bases and Technical Application of Breathing Therapy in Stress Management)

  • Breathing is essential for life and at the same time takes a role as a antidote for stress. In the Orient, it was recognized early that respiration, mind, and body have a relation that is inseparable and therefore proper breathing is so important. However, since the mechanism of therapeutic effect by breathing have not been verified, the treatment has been continued till recent years. From that which originated in the Orient, several techniques in the west have been developed to regulate breathing, and have been applying to the clinical situation and to studies, however scientific studies are still lacking. Recently, relaxed breathing has been used as an efficient strategy for breathing therapy as it has an effect on reducing physiological tension and arousal, and, therefore can be used as a basic technique to control or manage stress. In this study, in order to provide basic information and guidelines for clinical application, which will aid in the application of the theoretical basics of breathing therapy and its technique, a review of the literative was conducted. The findings are as follows: 1. Since proper breathing not only has, physically, the important function in supplying oxygen to the body but also gives a good emotional, or pleasant state of mind, it is the first step in controlling physical and mental health. 2. The basic types of breathing can be classified into two types; ‘diaphragmatic breathing(relaxed breathing)’ and ‘chest breathing(stress breathing)’. In yoga type breathing, there are four kinds of breathing, ‘upper breathing’, ‘mid breathing’, ‘down breathing’, and ‘complete breathing’. 3. The theoretical explanation of the positive thera peutic effect of breathing therapy techniques exemplifies good brain function, sufficient air flow through the nasal passages, diaphragmatic movement, light vagal stimulation, CO2 changes and cognitive diversion but in most studies, the hypothesis of CO2 is supported. 4. The technique of breathing is designated with many names according to the muscles and techniques used for breathing, and for control of stress, diaphragmatic breathing(relaxed breathing) is explained as a basic technique best used to manage of stress. 5. The relaxed-breathing includes slow diaphragmatic breathing, breath meditation, nasal breathing, yogic abdominal breathing, Benson's relaxed response, and quiet response.

대학생의 애착불안이 우울에 미치는 영향: 자기몰입의 매개효과 (The Effects of Anxious Attachment on Depression among College Students: Mediation Effect of Self-Absorption)

  • 애착불안이 우울을 완전하게 설명할 수 없다는 Hazan과 Shaver의 주장을 근거로 애착불안과 우울과의 관계를 매개하는 요인을 탐색하는 국내외연구들이 활발히 진행되어 왔지만 자기몰입의 매개효과를 살펴본 연구는 거의 없었다. 그러므로 본 연구는 대학생들의 성인 애착불안과 우울의 관계에서 자기몰입의 매개효과를 검증하고자 하였다. 이를 위해 대학교 재학생 283명을 대상으로 자료를 수집하였다. 본 연구에서는 연구 변인들 사이의 관계를 검증하기 위하여 구조방정식을 사용하였고 애착불안과 우울의 관계에서 자기몰입의 매개효과를 검증한 결과, 자기몰입은 부분매개효과를 나타내는 것으로 확인되었다. 이러한 연구결과는 애착불안이 지각단계에까지 영향을 미치며 자기몰입이라는 비-적응적인 지각과정을 통해 우울이 심화될 수 있다는 것을 의미한다. 본 연구결과는 과도기에 해당하는 대학생의 정신건강과 밀접한 관련이 있는 위험요인과 인지적인 매개요인에 대한 기초자료를 제공하는데 그 의의가 있다. 이러한 연구결과를 바탕으로 대학생을 대상으로 하여 애착불안과 우울의 관계에서의 여러 가지 위험요인과 보호요인과의 상관관계와 매개효과를 살펴보는 후속연구가 필요할 것으로 사료된다.

미취학 아동의 감성 발달을 위한 연령별 놀이가구 유형에 관한 연구 (A Study on the Type of Playable Furniture for Emotional Development of Preschool Children)

  • Nowadays the preschool children spend much of the day playing indoors. Therefore, it needs the indoor environment that helps a variety of fun activities and physical development, and it requires the space configuration and playable furniture considering the emotional development for mental health. However, the furniture for fast growing preschoolers has not various types because the domestic furniture market for children is mostly baby beds and the furniture for the education of children. Therefore, this study presents the concepts and types of the playable furniture reflecting emotional design for preschool children's healthy emotion and suggests the most appropriate type of playable furniture considering play behavior by age. In this study, we investigated the physical, cognitive, social, emotional and linguistic development characteristics and play behavior of preschoolers, and derived the right type of playground equipment and furniture, and examined the types of playable furniture to help the emotional development. We derived the items to be checked for developing the playable furniture by age for emotional development, and classified preschoolers' playable furniture into the use of learning, relaxation and storage, and suggested its basic type focusing on the cases of various playable furniture developed at home and abroad. As a result, the playable furniture was divided into three types. The first is the self-play type making possible self amusement, the second is module built-up-type that consists of furniture and modules or units and creates various patterns and can be modified through the self-assembly and disassembly, and the third is IT game type grafting IT skills and a variety of electronic games to furniture. We sorted these types into three classes (1-3 years old, 4-5, 6-7) according to age and presented the type of play for each age, the play element and representative image that can be introduced to this furniture. In this study, we provided the basic design types of age-specific emotional playable furniture by analyzing these results.

한국치매노인의 문화를 반영하는 회상: 개념분석 (Reminiscence Reflecting Cultural Background in Korean Older Adults with Dementia: A Concept Analysis)

  • Purpose: Reminiscence therapy is a widely used intervention for older adults and those with dementia. However, the attributes of the concept of reminiscence is not clearly proposed. The aim of this study was to clarify the concept of reminiscence in Korean older adults with dementia. Methods: The study applied the concept analysis method of Walker and Avant. The literature review included the previous studies of reminiscence from philosophy, literature, art, neuroscience, medicine, psychology, sociology, and nursing. Results: A total of 43 Korean or international papers were analyzed in this study. Attributes of reminiscence in Korean older adults with dementia were regeneration of memories, internal and external communications, expression of suppressed emotions, and recognition of ego identity. The suppressed emotions and ego identity may result from Korean cultural characteristics based on the suppressed environments of many wars and dictatorships in the past. The consequences of reminiscence in Korean older adults with dementia are cognitive enhancement, relief of depression, and improvements of social interactions, ego integrity, and quality of life. Conclusion: The results of this study suggest that the historical and cultural characteristics should be considered to develop effective nursing interventions including reminiscence for Korean older adults with dementia.

병역판정검사를 위해 일 대학병원 정신건강의학과에 내원한 환자들의 초기 부적응 스키마와 관련 인지 기능 특성 (Characteristics of Early Maladaptive Schemas and Associated Cognitive Functions in Visitors to the Psychiatric Department in a University Hospital for Military Designation Process)

  • Objective : The goal of this study was to investigate the characteristics of early maladaptive schemas (EMSs), and associated neurocognitive functions as seen in visitors for military designation process. Methods : This retrospective study included 111 males aged 18 to 24 years among three groups: 41 visitors for military designation process (VMD), 21 patients with obsessive-compulsive disorder (OCD), and 49 healthy subjects. We collected the results of the Young Schema Questionnaire, Symptom Checklist-90-Revised, three neurocognitive tests as well as their clinicodemographic data. We analyzed the differences in EMSs between these three groups, and the correlations among the identified EMSs and neurocognitive performances within the VMD group. Results : Compared with both the OCD and healthy groups, the evaluation of the VMD group showed significantly higher scores in mistrust/abuse (F=6.4, p=0.002), vulnerability to harm (F=6.6, p<0.0001) and negativity/pessimism schema (F=7.3, p<0.0001), even when controlling for depression scores and levels of education. These three schemas also exhibited significant negative correlations with the score of Stroop test with r ranging from -0.34 to -0.44. Conclusion : These findings suggest that people who are likely to have difficulties adjusting to living in a military life may have psychological vulnerabilities related to certain EMSs. Further studies are warranted to test the clinical potentials of these findings, such as a treatment target and a predictor factor.

Effect of Mindful Self-Compassion Training on Anxiety, Depression and Emotion Regulation

  • Objective : The present study examined the effectiveness of the mindful self-compassion (MSC) program on anxiety, depression, self-compassion and emotion regulation. Methods : A total of 29 subjects (mean age 27.5±6.5 years, 15 males and 14 females) participated in a standardized 8-week MSC program. The control group consisted of age- and sex-matched twenty participants (mean age 26.0±2.9 years, 11 males and 9 females). All subjects completed self-report measurements at two weeks before and after the MSC program. Results : MSC training improved self-compassion as demonstrated by the significant group x time interaction effects on the total Self-Compassion Scale scores (F[1, 47]=8.324, p<0.01). Regarding the subscale scores, a significant improvement in self-kindness, isolation and mindfulness components of self-compassion was observed after MSC training. A significant group x time interaction was observed on the self-kindness subscale (F[1, 47]=4.664, p<0.05), with a significant main effect of time (F[1, 47]=23.723, p<0.001). The isolation subscale showed a significant group x time interaction (F[1, 47]=8.698, p<0.001). For the mindfulness subscale, there was a significant group x time interaction (F[1, 47]=6.611, p<0.05) and main effect of time (F[1, 47]=6.611, p<0.05). MSC training also improved the acceptance emotion regulation strategy, as demonstrated by the significant group x time interaction in the acceptance subscale scores of the Cognitive Emotion Regulation Questionnaire (F[1, 47]=6.845, p<0.05). Conclusion : MSC training showed efficacy in fostering self-compassion and improving emotion regulation. Thus, this program might be applicable to improve mental health.
