노년기 우울증 환자의 하지불안증후군의 임상적 중요성 (Clinical Significance of Restless Legs Syndrome in Patients with Late Life Depression)

  • Objectives Restless legs syndrome (RLS) is a sleep disorder characterized by uncomfortable and unpleasant sensations in the legs and an urge to move the legs, usually at night. The aim of this study is to investigate the incidence of RLS in patients with late life depression and its influence on various clinical outcomes such as severity of depression, sleep quality, cognitive function, and quality of life and accordingly, to elucidate the clinical significance of RLS in patients with late life depression (LLD). Methods This study enlisted 170 depressive patients aged 65 years or older from an outpatient clinic. Structured diagnostic interviews were performed using the Korean version of the Mini-International Neuropsychiatric Interview. All patients completed the questionnaires, including the International RLS Severity Scale, the Korean version of Short-Form 36-Item Health Survey (SF-36), and the Pittsburgh Sleep Quality Index (PSQI). The severity of depression was evaluated by the Korean form of the Geriatric Depression Scale (KGDS) and the level of global cognition was assessed by the Mini-Mental State Examination in the Korean version of The Consortium to Establish a Registry for Alzheimer's Disease Assessment Packet (MMSE-KC). Results The incidence of RLS was 17.6% in LLD patients. RLS was more prevalent among the subjects with major depressive disorder (MDD) than those with minor depressive disorder or subsyndromal depressive disorder. The RLS group showed higher score in the KGDS than the Non-RLS group but the difference did not reach the statistical significance (p = 0.095, Student t-test). The mean PSQI score was significantly higher in the RLS group than in the Non-RLS group (p = 0.001, Student t-test). The MMSE-KC score was also lower in the RLS group than in Non-RLS group (p = 0.009, analysis of covariance). But, there was no difference in the score of SF-36 between the RLS group and the Non-RLS group. Conclusions RLS is common in LLD patients, especially in the patients with MDD and is associated with poor sleep quality and cognitive dysfunction, indicating that RLS is clinically significant in patients with LLD. Therefore, RLS should be considered as an important clinical issue in the management of LLD.

뇌 자기공명영상에서 대뇌 피질 두께 분석법을 이용한 파킨슨병의 경도인지장애 신경심리검사와 신경해부학적 상관관계 (Correlation of the Neuropsychological Screening Battery (NSB) and Neuroanatomy for the Parkinson's Disease with Mild Cognitive Impairment by Using the Analysis of Cerebral Cortex Thickness in the Brain MRI)

  • 본 연구는 경도인지장애가 동반된 환자를 대상으로 뇌 자기공명영상에서 인지 영역에 대한 대뇌 피질 두께를 측정하고 신경심리검사 결과와 비교하여 신경해부학적 상관관계를 규명하고자 하였다. 이를 위하여 파킨슨병으로 최초 진단 받고 신경심리검사를 시행한 78명(경도인지장애군: 39명; 비인지장애군: 39명)과 정상인 그룹 32명을 선정하였다. 신경심리검사에서 경도인지장애군과 비인지장애군의 상관관계와 신경심리검사와 뇌 자기공명영상에서 대뇌 피질 두께의 상관관계는 독립표본 T 검증 또는 피어슨 상관분석을 실시하였으며 수집된 자료의 유의 수준은 p<0.05에서 검증하였다. 결론적으로 경도인지장애를 동반한 파킨슨병 환자는 뇌 자기공명영상에서 양측 쐐기압소엽과 우하측두엽의 대뇌 피질 두께가 통계적으로 유의하게 감소하였으며 인지 기능 평가를 위한 신경심리검사에서 시공간 기능, 언어 및 시각 기억력 기능이 저하되었다. 특히 언어 및 시각 기억력 영역에 대한 신경심리검사와 신경해부학적으로 좌측 쐐기앞소엽에서 상관관계가 있었다.

  • Information culture is a concept with multiple and sometimes conflicting definitions. Nevertheless at a time when social and cognitive aspects of information and communication systems are recognized as being of similar significance as the technological functionality, the concept of information culture is demonstrating considerable value. This paper explains the concept of information culture and considers its value from two main perspectives: firstly as providing an analytical tool, and secondly as a means of providing practitioners with insight and sensitivity to the organisational contexts of their workplaces.

아마추어 야구선수의 경쟁불안과 수행자신감과의 관계: 성격강점과 인지평가의 조절된 매개효과 (The Relationship between Competition Anxiety and Performance Confidence in Amateur Baseball Player: The Moderated Mediation Effects of Cognitive Appraisal and Character Strength)

  • 본 연구에서는 국내 아마추어 야구선수를 대상으로 경쟁불안이 인지평가와 수행자신감에 미치는 영향, 그리고 이러한 관계에서 인지평가의 매개효과를 검증하고, 성격강점이 이와 관련한 조절적인 역할을 하는지의 여부를 살펴보았다. 특히 긍정심리학적 관점에서 경쟁불안과 인지평가의 관계에서 성격강점적 특성에 따른 조절효과를 알아보고자 6개의 미덕으로 나누어 검증을 실시하였다. 이를 검증하기 위해서 만 19세 이상의 아마추어 야구선수를 201명을 대상으로 온라인 설문조사를 실시하였다. 이러한 과정에 따라 도출 된 본연구의 주요 결과는 다음과 같다. 첫째, 경쟁불안과 수행자신감과의 관계를 인지평가가 매개하는 것으로 나타났다 둘째, 경쟁불안과 인지평가와의 관계에서 성격강점이 조절효과를 보이는 것으로 나타났다. 셋째, 조절된 매개효과는 유의미한 것으로 나타났다. 그 중 에서도 정의, 초월의 성격 강점이 유의미하게 조절하는 것으로 검증되었는데, 정의, 초월 성격강점이 경쟁불안에 의해 인지평가가 감소 되는 정도를 완화 시켜 주는 것으로 나타났다. 끝으로 본 연구결과를 토대로 시사점 및 한계점, 추후 연구 방향에 대해 논의하였다.

일부 여자 중학생 대상 사회인지이론기반 영양교육 프로그램의 적용 및 효과 (Application and the Effect of Nutrition Education Program Based on the Social Cognitive Theory Among Middle School Girls)

  • Objectives: The purpose of this study was to evaluate the effect of nutrition education using materials based on social cognitive theory. Education topics focused on improving health-related and dietary self-awareness and behavior capability in adolescents. Methods: Participants were recruited from a middle school for girls; 67 students (educated group, n=34 and control group, n=33) participated. The education group received 12 lessons in club activity class. Self-administered surveys were conducted for each group before and after the nutrition education program. The questionnaires consisted of variables such as self-efficacy, outcome expectation, outcome expectancy, knowledge, and dietary practices based on the social cognitive theory. Education satisfaction was evaluated using a five-point Likert scale for two sections: a) teaching and learning and b) education results. The data were analyzed using a t-test and Chi Square-test (significance level: p < 0.05). Results: In the education group, post-education, there were significant differences in self-efficacy (p < 0.05), knowledge (p < 0.01), and dietary practices (p < 0.05), whereas outcome expectation and expectancy did not show any significant differences. None of the variables showed any significant differences in the control group. Educational satisfaction scores were $4.38{\pm}0.12$ (teaching and learning) and $4.14{\pm}0.15$ (education results). Conclusions: This study showed that improving adolescent's awareness and behavior capability has a positive effect on their dietary practices. Moreover, this study suggested that a theory-based determinant should be considered to improve dietary behavior among adolescents.

1년간의 복합 운동프로그램이 남성 치매환자의 운동능력과 인지기능에 미치는 영향 (The Effects of One Year Exercise Program on Exercise Capacity & Cognitive Function in Male Patients with Dementia)

  • 1년간의 운동이 남성 치매환자의 운동능력과 인지기능에 미치는 영향을 연구한 결과, 운동군의 경우 심폐지구력, 근력, 근지구력 및 MMSE는 운동전에 비해 운동중과 운동후 유의하게 증가하였고, 평형성과 민철성은 운동후 유의하게 증가하였으나 유연성은 유의한 차이가 없었다. 반면, 통제군은 모든 시기에서 유의한 차이가 나타나지 않았다. 이상의 결과를 볼 때, 1년간의 운동은 남성 치매환자의 운동능력과 인지기능을 향상시켜 환자들에게 자신감과 성취감을 얻을 수 있는 기회를 주었고, 나아가 독립적인 일상생활을 영위하고 삶의 질을 높이는데 크게 이바지 할 것으로 기대된다.

Whole Brain Radiation-Induced Cognitive Impairment: Pathophysiological Mechanisms and Therapeutic Targets

  • Radiation therapy, the most commonly used for the treatment of brain tumors, has been shown to be of major significance in tumor control and survival rate of brain tumor patients. About 200,000 patients with brain tumor are treated with either partial large field or whole brain radiation every year in the United States. The use of radiation therapy for treatment of brain tumors, however, may lead to devastating functional deficits in brain several months to years after treatment. In particular, whole brain radiation therapy results in a significant reduction in learning and memory in brain tumor patients as long-term consequences of treatment. Although a number of in vitro and in vivo studies have demonstrated the pathogenesis of radiation-mediated brain injury, the cellular and molecular mechanisms by which radiation induces damage to normal tissue in brain remain largely unknown. Therefore, this review focuses on the pathophysiological mechanisms of whole brain radiation-induced cognitive impairment and the identification of novel therapeutic targets. Specifically, we review the current knowledge about the effects of whole brain radiation on pro-oxidative and pro-inflammatory pathways, matrix metalloproteinases (MMPs)/tissue inhibitors of metalloproteinases (TIMPs) system and extracellular matrix (ECM), and physiological angiogenesis in brain. These studies may provide a foundation for defining a new cellular and molecular basis related to the etiology of cognitive impairment that occurs among patients in response to whole brain radiation therapy. It may also lead to new opportunities for therapeutic interventions for brain tumor patients who are undergoing whole brain radiation therapy.

Effects of the Forest Healing Program on Depression, Cognition, and the Autonomic Nervous System in the Elderly with Cognitive Decline

  • Background and objective: The purpose of this study is to investigate the effect of a forest healing program in terms of depression, neuropsychological and physiological benefits for the elderly. Methods: For this purpose, we developed a forest therapy program for the elderly who are vulnerable to dementia and conducted a total of 11 sessions of forest therapy activities in a forest once a week. We measured the changes in depression, resting-state Electroencephalography(EEG) and heart rate variability (HRV) before and after the program. There were 60 subjects aged over 65 yesrs old. 30 subjects participated in the forest therapy program, and the other were in the control group. The Geriatric Depression Scale was used to measure the level of depression, neuroNicle FX2 (Laxtha, Korea) was used to measure the resting-state EEG, and photoplethymogram (ubpulse T1, Laxtha, Korea) was used to measure the HRV. Results: The results showed that the depression index of the experimental group improved with statistical significance after the program (experiment group = 3.267 decrease of the mean). In the EEG measurement, the alpha-peak frequency at rest (experimental group = 0.227 Hz increase of the mean) was improved (mean increase = 0.23 in the experimental group, p < .05). The high frequency of HRV, which represents the parasympathetic nerve activity of the body's autonomous response, was also significantly improved (mean increase = 0.396 in the experimental group, p < .05). Conclusion: The results suggest that the forest therapy program can reduce the cognitive, psychological and physical risk factors of dementia for the elderly at risk of cognitive decline. Therefore, forest therapy activities may be suitable for the prevention of dementia in the elderly.

모아상호작용에 관한 연구 - 교육과 수유상황을 중심으로 (A study on the mother-infant intercation in teaching and feeding situation)

  • This study was conducted to provide the basic information for nursing intervention in infant teach. ing and feeding situation. The subjects were 30 pairs of mother and her infant(9 month) who were normally delivered at 3 university hospital in Seoul area. The data collection was conducted by observation using videotaperecord from September 27, 1989 to Feburary 26, 1990 in their home. The measulsement tools used by this researcher were Nursing child Assessment Teaching scale (NCATS) and Nursing child Assessment Feeding Scale(NCAFS) which was developed by Barnard. The higher sum of "yes" score means the higher mother-infant interaction level. Mother-Infant interaction behaviors in both sitution were based on 6 subcategories; sensitivity to cue, response to distress, social-emotional growth fostering, cognitive growth 'fostering, clarity of + cues, and responsiveness to parent. The data collected through above method were analyzed by mean and t-test and the results were as follows. 1. The sum of maternal behavior score was 40.2 out of 50.0 in teaching situation and 37.9 out of 50.0 in feeding situation. Out of the 4 subcategories on maternal behavior in both situation, the highest subcategory was shown by response to infant's distress and the lowest subcategory by cegnitive growth fostering. 2. The sum of infant behavior score was 14.7 out of 23 in teaching situation and 17.9 out of 26 in feeding situation. Out of the 2 categories on infant behavior in both situation, the highest sub categroy was shown by the clarity of infant's cue in both situation. 3. There was a statistical significance between father's educational level and mother-infant interaction in teaching situation; college group was higher than high school group, particulary in category of sensitivity to infant's cue and congnitive growth fostering. 4. There was a statistical significance in teaching situation between mother's educational level and mother-infant interaction;college group was higher than high school group, particulary sensitivity to infant cue, social-emotional growth fostering and cognitive fostering. 5. There was no statistical difference in between parent educational level and mother.infant interaction in feeding situation.situation.
