An Investigation on Concentration of Airborne Microbes in a Hospital (병원내 공기중 미생물의 농도에 관한 조사연구)

  • A survey was conducted to measure concentration of airborne microbe in a hospital using RSC air sampler during October~November 1991.The result was as follows: 1) In an agar strip GK-A media for total counts of microbial particles. The highest count were 1384 CFU/m$^3$ in the main lobby, followed by 912 CFU/m$^3$, in the obstetric room, 688 CFU/m$^3$ in 1CU. By gram staining, the distribution for organisms in the air were shown 74.1% in gram possitive cocci followed by 16.8%, in gram possitive bacilli 6.7% in gram negative bacilli and 4.7% in yeast, but low organism was detected in recovery room with 194 CFU/m$^3$. 2) In agar strip S media for Staphylococci the count at the main lobby was detected in the recovery room with 92 CFU/m$^3$, Tests of coagulase were negative Staphylococci with 78%, and positive Staphylococci with 22%. The Staphylococci were highly resistance to penicillin, ampicillin and sensitive to amikacin, cefazolin, gentamycin and chloramphenicol. 3) In agar strip C media for coliform bacteria the colony counts at the main lobby was 139 CFU/m$^3$ and treatment room was 190 CFU/m$^3$, most frequently isolated microorganisms were non fermentative bacilli. 4) In agar strip HS media for yeast and molds. Most frequently colony counts 17~76 CFU/m$^3$, 0.5% lactophenol cotton blue stains were shown unidentified 77.2%, 8.1%, in Penicillium 8.1% in Aspergillus, and 3.8% in mucor.

Increases of Antibiotic Resistance in Excessive Use of Antibiotics in Smallholder Dairy Farms in Northern Thailand

  • Antibiotic resistance patterns of bacterial isolates from both quarter teat-tip swabs and their quarter milk samples were evaluated in smallholder dairy farms in northern Thailand with excessive use of antibiotics (HIGH) compared with normal use (NORM). Results from teat-tip swab samples showed that the percentage of Bacillus spp. resistance to overall antibiotics was significantly lower in the NORM group than that of the HIGH group, whereas, the resistance percentage of coagulase-negative staphylococci in the NORM group was higher than that of the HIGH one. The overall mastitis-causing bacteria isolated from milk samples were environmental streptococci (13.8%), coagulase-negative staphylococci (9.9%), Staphylococcus aureus (5.4%), and Corynebacterium bovis (4.5%). Both staphylococci and streptococci had significantly higher percentages of resistance to cloxacillin and oxacillin in the HIGH group when compared to the NORM one. An occurrence of vancomycin-resistant bacteria was also observed in the HIGH group. In conclusion, the smallholder dairy farms with excessive use of antibiotics had a higher probability of antibiotic-resistant pattern than the farms with normal use.

  • A study was carried out to define the relationship between the N-acetyl-$\beta$-D-glucosaminidase(NAGase) levels and isolated pathogenic bacteria in 379 quarter fore milk of mastitis suspected samples collected in this clinics. All samples were tested the NAGase, California mastitis test(CMT), Somatic cell count(SCC) and bacterial culture. Except 111 from 379 samples, 268 bacteria-positive quarter fore milk samples were classified into the latent and mastitis infection group by SCC($500,000cells\;per\;m{\ell}$), and the mean NAGase levels($nmol/min/m{\ell}$) of each isolated pathogen in mastitis infection group were Staphylococcus aureus 3.067, Coagulase-negative staphylococci 4.083, Staphylococcus aureus 3.594, Str. uberis 3.513, Str. dysgalactiae 1.640, E coli 4.441 and gram negative rods 4.560, respectively. Most of the relationship between mean SCC and NAGase in each pathogen group were highly significant using a student t test(p<0.05). When the mastitis pathogens were classified into minor(Coagulase-negative staphylococci, Corynebacterium sp.) and major pathogen group(Staphylococcus aureus, Streptococcus agalactiae, Str. uberis, Str. dysgalactiae, gram negative rods), the NAGase levels were higher at major than minor pathogen group. On the other hand, when the mastitis milk samples were classified by SCC($500,000cells\;per\;m{\ell}$) and by the presence of pathogen(IDF scheme), the NAGase levels were also higher at the mastitis than latent infection. The possibility of combining SCC and NAGase data in order to give the more difinitive diagnosis is discussed.

  • Staphyylococci are responsible for over 80 per cents of the suppurative diseases encountered in medical practice. They cause most suppurative infections of the skin but may also invade and produce severe infections in any other parts of the body. In order to know the carrier rate of staphylococci between the parahospital workers and in the hospital workers, the author undertook isolation of S aureus from nasal cavity on 68 cases of freshmen, 31 cases of sophomores, and 37 cases of juniors in Busan National University, and 30 cases of nursing students, 30 cases of nurses, 30 cases of nurses in charge of operating room and 30 cases of doctors in Busan National University Hospital. The tested total cases were 256 cases which were 136 Cases in parahospital workers and which were 120 cases in hospital workers. The biochemical characters of S aureus strains isolated were studied on coagulase test, mannitol test, hemolysis test and sensitivity test to antimicrobial agents. The results obtained were as follows: 1. S aureus were isolated 49 cases(29.7%) from nasal cavity parahospital workers and were isolated 67 cases(55.8%) from nasal cavity on hospital workers. 2. Among 40 strains of S aureus isolated from parahospital worker's nasal cavity coagulase positive were 29 cases(72.1%), and coagulase negative were 11 cases(27.5%). And mannitol positive were 29 cases(72.5%), and mannitol negative were 11 cases(27.5%). 3. Among 67 strains of S aureus isolated from hospital worker's nasal cavity coagulase positive were 59 cases(88.1%), and coagulase negative were 8 cases(11.9%). And mannitol positive were 49 cases(73.1%), and mannitol negative were 18 cases(26.9%). 4. The hemolysis test of each erythocytes on coagulase and mannitol positive S aureus isolated were sensitive to rabbit(40 cases: 81.6%), guinea pig(26 cases: 53.6%), sheep(13 case: 26.5%), and were not sensitive to chicken and human erythrocytes, respectively. 5. The hemolysis test of each erythocytes on 10 strains of coagulase and mannitol negative S aureus isolated were not sensitive to all of, erythrocytes. 6. The sensitivity test to the various chemotherapeutic agents was almost sensitive to the strains isolated from parahospital workers, but was almost resistant to the strains isolated from hospital workers.

An Easy Method of Disk Diffusion Antibiotic Susceptibility Test for Detection of Erythromycin-induced Resistance to Clindamycin in Staphylococci (포도구균의 Erythromycin 유도성 Clindamycin 내성검출을 위한 간편한 디스크 확산법의 유용성)

  • A simple and easy modification of AST by disk diffusion was tested for the detection of induced clindamycin resistant Staphylococci and their antimicrobial susceptibility at the same time. The incidence of inducible clindamycin resistant staphylococci in blood culture and their MIC characterization at Seoul National University Hospital was analyzed by an AST contained disk approximation test (D-zone test) and Etest, respectively. Of the total 309 staphylococcal isolates, 139 (45%) isolates presented constitutive resistance to ERY and CLI (ERY-R, CLI-R phenotype), and 59 were ERY-I/R and CLI-S phenotypes. Of the 59 isolates, 19 (32%) isolates were inducible resistant to CLI. The incidence was higher in S. aureus (66.7%) than coagulase-negative staphylococci (CNS, 26.0%). Especially, methicillin-resistant staphylococci (MRSA, 100%; MRCNS, 45.5%) presented higher inducibility than methicillin susceptible (MSSA, 50%; MSCNS, 20%). For most of the inducible clindamycin resistant staphylococci (15 of 19 isolates), their ERY MIC were high (>$128_{\mu}g/mL$) and were methicillin resistant. The remaining 4 isolates were methicillin susceptible and their ERY MIC were of intermediate concentrations ($1-4_{\mu}g/mL$). We concluded that suscetibility testing of staphylococci, especially methicillin resistant, should include the D-zone test.

  • The purpose of this study is to investigate the prevalence of resistance to macrolide, lincosamide, streptogramin and ketolide antibiotics in Korea. The antibiotic susceptibility test was performed to the macrolide erythromycin, clarithromycin, azithromycin, josamycin, the lincosamide clindamycin, the streptogramin synercid and the ketolide ABT -773 against 337 clinical Staphylococcus aureus(SAU). Coagulase-negative Staphylococci (CNS) and Enterococci isolates exhibited an average percentage of 64%, 56%, and 81 % of resistance to erythromycin, respectively.

Antimicrobial Resistance of Methicillin-Resistant Staphylococci Isolates from Dog Ears in Korea

  • Methicillin-resistant staphylococci (MRS), which are often multi-drug resistant (MDR), are important pathogens in both human and veterinary healthcare. The purpose of this study was to characterize the antimicrobial resistance of MRS isolated from dog ears in Korea. From 827 dog ears, staphylococci were cultured from both ears with otitis externa (n = 161, 41.0%) and healthy ears (n = 135, 31.1%). The prevalence of coagulase-positive staphylococci (CoPS) in ears with otitis externa (58.4%) was significantly higher (p < 0.05) than in healthy ears (28.2%), while the prevalence of coagulase-negative staphylococci (CoNS) in healthy ears (74.8%) was higher (P < 0.05) than in ears with otitis externa (41.6%). Forty-six (35.1%) and 74 (44.8%) CoPS and CoNS isolates, respectively, were determined to be MRS. Antimicrobial resistance in MRS was most frequently observed for penicillin (76.7%), ampicillin (61.7%), kanamycin (61.7%), erythromycin (47.5%), tetracycline (47.5%), and trimethoprim/sulfamethoxazole (46.7%). Overall, the MDR isolates were resistant to significantly more (P < 0.05) antimicrobial agents tested than methicillin-sensitive staphylococci in this study. These results provide therapeutic guidelines for the treatment of otitis externa in dogs from Korean veterinary hospitals, and the significant associated health concern to companion animals and their human contacts.

Assessment of Bacterial Contamination of Raw Meats Sold in Korea, 2007 (2007년 한국에서 판매된 식육의 미생물 오염도 평가)

  • 본 연구는 인간의 건강과 식품의 오염을 예방하기 위한 유용한 자료를 얻기 위하여 육류(소고기, 닭고기, 돼지고기)로부터 분리된 staphylococci를 가지고 한국에서 광범위하게 사용되는 6가지 항생제와 daptomycin, linezolid, quinupristin/dalfopristin and tigecycline과 같은 4가지의 신항생제에 대한 항생제 감수성 시험을 수행하였다. 이에 더하여 육류의 세균 오염실태를 조사하기 위하여 총 세균수와 대장균수를 측정하였다. 그 결과 147개의 육류 샘플 중 48%의 샘플에서 일반세균수의 기준에 부적합한 결과가 나타났으며, 대장균수에 대해서는 31%의 샘플에서 기준에 부적합한 결과가 나타났다. 또한 육류의 91%에서 stapylococci가 검출되었으며 이들 staphylococci는 Coagulase Negative Staphylococci (CNS)와 S. aureus로 동정되었다. 분리된 staphylococci의 5.4%가 methicillin (oxacillin)에 내성을 나타내었고, 특히 mupirocin ($MIC_{90},\;>\;12\;{\mu}g/mL$) gentamicin ($MIC_{90},\;64\;{\mu}g/mL$)에 높은 내성을 나타내었다. 그러나 신항생제에 대해서는 methicillin 내성 staphylococci를 포함한 모든 staphylococci 분리균주가 높은 감수성을 나타내었다. 결론적으로 본 연구의 결과들은 육류가 소비자들의 건강에 중요한 위험요소로 작용할 수 있음을 보여주고 있다.

Characterization of the Cloned Staphylococcal Peptidoglycan Hydrolase Gene Product

  • Cloned staphylococcal peptidoglycan hydrolase was used in determining the physiological characteristics of peptidoglycan hydrolase. This enzyme hydrolyzed the bacterial cell walls and released the N-terminal alanine, but not the reducing groups. This cloned gene product was localized in the cytoplasm of transformed Escherichia coli. Activity gels indicated the enzyme had an Mr of about 54,000, which was consistent with the deduced Mr from sequencing of the cloned gene. The activity bound to CM-cellulose but not DEAE-cellulose resin, indicating it as a basic protein. Enhanced enzyme activity in a low concentration of cations, and inhibited enzyme activity in a solution with dissolved phospholipids, suggested that the activity and the availability of this basic protein may be regulated between negatively charged and positively charged cellular molecules. The activity against boiled crude cell wall was much greater than against purifed cell wall, suggesting protein associated with crude cell wall may aid in the binding of the peptidoglycan hydrolase The cloned peptidoglycan hydrolase showed positive activity on whole cells of some lysostaphin-resistant coagulase-negative staphylococci. The cloned enzyme may be an alternative for lysostaphin for lysis of staphylococci.

Nasal Carriage of 200 Patients with Nasal Bone Fracture in Korea

  • Background Pathogens in the nasal cavity during nasal surgery could lead to a systemic infectious condition, such as bacteremia, nosocomial infection, or toxic shock syndrome. However, there is no research about the prevalence of nasal carriage in patients with nasal bone fracture. Methods This was a prospective, double-blind, randomized study about the rate of nasal carriage in 200 patients with nasal bone fracture in Korea. Nasal secretions were taken from both the middle nasal meatus and colonized. All analyses were carried out using SPSS software. Results Pathogens were identified in 178 of the 200 cases. Coagulase-negative staphylococci (CNS) were the most cultured bacteria in 127 (66.84%) of the 190 total patients after excluding 10 cases of contaminated samples, and methicillin-resistant coagulase-negative staphylococci (MRCNS) were found in 48 (25.26%). Staphylococcus aureus was the second most identified pathogen, found in 36 (18.95%), followed by 7 cases (3.68%) of methicillin-resistant Staphylococcus aureus (MRSA). The prevalence rate of MRSA in the females was higher than that in the males (RR=4.70; 95% CI, 1.09-20.18), but other demographic factors had no effect on the prevalence rate of MRSA and MRCNS. Conclusions The prevalence rate of these pathogens in patients with nasal bone fracture in Korea was similar to other reports. However, few studies have addressed the prevalence rate of CNS and MRCNS in accordance with risk factors or the change in prevalence according to specific prophylaxis against infectious complications. Additional research is needed on the potential connections between clinical factors and microbiological data.
