원유시료에서 분리된 반코마이신 저항성 Coagulase-Negative Staphylococcus 균주의 특성 (Characterization of a Coagulase-Negative Staphylococcus sp. Isolated from Raw-Milk having Reduced Susceptibility to Vancomycin)

  • Coagulase-negative Staphylococcus sp #39, isolated from raw-milk showed reduced susceptibility to vancomycin. The minimun inhibitory concentration for strain #39 was at 8$\mu\textrm{g}$ of vancomycin per ml. Transmitting electron microscopy displayed that this strain had a 2.5-3.5 times thicker cell wall than a vancomucin sensitive strain of Staphylococcus sp. The strain #39 also had an increased cell volume. These data indicate that the reduced susceptibility may be due to the thickness of the cell wall of the test strain.

  • Osteomyelitis of the jaw infected with Coagulase-Negative Staphylococci (CNS) is rarely reported in the Oral and Maxillofacial Region. Staphylococcus is a part of the normal body flora, but it may be cause severe infections and CNS are often described as the important pathogens in nosocomial infections. Although many studies on prevalence and antibiotics of Staphylococcus aureus have been done, but many of these studies focus only on Methicillin-resistant S. aureus and not on methicillin-resistant coagulase-negative Staphylococci (MRCNS). There was a less study about CNS or MRCNS infections in the Oral and Maxillofacial Region. This report describes a case of a 41-year-old male patient who developed osteomyelitis caused by MRCNS on condyle after open reduction and internal fixation and suggests guideline for the prevention of postoperative infection and appropriate recommendation for treatment and control.

Comparative Genomic Analysis of Food-Originated Coagulase-Negative Staphylococcus: Analysis of Conserved Core Genes and Diversity of the Pan-Genome

  • To shed light on the genetic differences among food-originated coagulase-negative Staphylococcus (CNS), we performed pan-genome analysis of five species: Staphylococcus carnosus (two strains), Staphylococcus equorum (two strains), Staphylococcus succinus (three strains), Staphylococcus xylosus (two strains), and Staphylococcus saprophyticus (one strain). The pan-genome size increases with each new strain and currently holds about 4,500 genes from 10 genomes. Specific genes were shown to be strain dependent but not species dependent. Most specific genes were of unknown function or encoded restriction-modification enzymes, transposases, or prophages. Our results indicate that unique genes have been acquired or lost by convergent evolution within individual strains.

기관절개술 환자의 Povidone 소독에 따른 포도상구균의 양상 (Aspects of Staphylococcus in Povidone Disinfection of Patients with a Tracheostomy)

  • 본 연구는 기관절개술을 받은 환자의 기관절개부위에서 발견되는 포도상구균에 대해 povidon의 살균효과와 소독제 사용 횟수, 경과일수에 따른 변화를 관찰하는 것을 목표로 하고 있다. 연구대상은 2001년 1월 16일부터 2001년 2월 26일까지 D시내에 소재한 2개 종합병원의 신경외과 중환자실에 입원한 35명의 기관절개환자였으며, 수집된 자료는 SAS를 이용하여 백분율, t-test로 분석하였다. 연구결과는 다음과 같다. 포도상구균에 대한 povidone의 살균효과는 농도와 시간에 관계없이 강하게 나타났다. 1일 1회 소독 후 3일째부터 Staphylococcus aureus균이 검출되었고, 6일째에 가장 많이 검출되었다. Coagulase negative Staphylococcus균은 1일부터 3일까지는 거의 검출되지 않았으나, 4일째에 가장 많이 검출되었다. 소독횟수에 따른 균집락화 양상은 1일 1회 소독할 때보다 2회 소독시 황색포도상구균이 현저히 감소하였다. 항생제에 대한 Staphylococcus aureus와 Coagulase negative Staphylococcus의 내성검사를 한결과 Staphylococcus aureus는 Methicillin에서 72.7%, Imipenem에서 63.6%가 내성을 나타냈으며, Coagulase negative Staphylococcus는 Methicillin에서 37.5%, Imipenem에서 12.5%가 내성을 나타내었고, Vancomycin에는 모두 내성을 나타내지 않았다. 이상의 본 연구결과로써 기관절개환자의 절개부위에 대한 철저한 소독을 통해 병원감염을 예방할 수 있는 기초자료가 될 수 있는 것으로 사료되며, 다음과 같은 제언을 하고자 한다. 앞으로 더 많은 소독제와 다양한 균종과의 관계에 대한 연구가 시행되어야 할 것이며, 모든 의료인은 철저한 병원감염예방에 힘써야 할 것이다. 또한, 병원감염 예방과 간호 발전을 위한 소독전문인력을 양성하여 활성화시키는 것이 필요할 것으로 사료된다.

젓갈 유래 Coagulase-Negative Staphylococci의 안전성 평가 (Safety Assessment of Coagulase-Negative Staphylococci from Jeotgal, a Korean High-Salt-Fermented Seafood)

  • 젓갈로부터 분리된 ampicillin 감수성, coagulase 음성 Staphylococcus 속 17 균주의 항생물질내성 및 위해성을 평가하였다. 15 균주는 한 종류 이상의 항생물질에 대한 내성을 나타내었고, penicillin G 내성 균주가 가장 높은 빈도로 검출되었다. PCR 증폭에 의한 항생물질내성 유전자 존재 확인 결과, trimethoprim 내성 관련 dfrA 유전자와 tetracycline 내성 관련 tetK 유전자를 각각 보유한 두 균주가 확인되었다. α 형 용혈활성은 검출되지 않았지만, 다섯 균주가 δ 형의 용혈을 나타내었고, 그 중 두 균주는 β 형 용혈활성을 나타냈으며, 12 균주가 biofilm을 형성하였다. 본 실험에서 수행한 모든 안전성 평가 결과는 균주 특이적으로 나타났다.

부산지역 치과환경에서 분리된 coagulase-negative staphylococci의 특성 (Characteristics of Coagulase-negative Staphylococci Isolates from Dental Clinic Environments in Busan, Korea)

  • 본 연구는 치과 병의원 진료실 주변환경과 치과종사자의 휴대전화에서 분리된 coagulase-negative staphylococci (CNS)의 분리율과 항균제 내성양상 및 분자 역학적 특성을 분석하고자 하였다. 2014년 12월부터 2015년 1월까지 부산지역 10개의 치과 병의원에서 총 154개의 샘플을 수집하여 MALDI-TOF분석법을 통해 동정하였다. 항균제 감수성검사는 디스크 확산법을 시행하였고 mupA, mecA 유전자 보유현황 및 SCCmec type은 PCR과 염기 서열분석에 의해 결정하였다. 154개의 샘플 중 10개(6.5%)에서 CNS 균주(Staphylococcus epidermidis 5주, Staphylococcus capitis 2주, Staphylococcus warneri 2주, Staphylococcus haemolyticus 1주) 가 분리되었다. 항균제 감수성검사에서 penicillin 10주, mupirocin 6주, gentamicin 5주, tetracycline 3주 및 cefoxitin과 erythromycin 2주가 내성이었고 clindamycin, ciprofloxacin, teicoplanin, trimethoprim-sulfamethoxazole에 내성인 세균은 없었다. 2개의 CNS균주(S. warneri, S. haemlyticus)에서 mecA 유전자가 검출되었고 1개의 CNS균주(S. epidermidis)에서 mupA가 확인되었다. Methicillin 내성 CNS균주 가운데 1주는 SCCmec I형이었고 1균주는 SCCmec의 유전형이 구분되지 않았다. 본 연구를 통하여 다약제 내성을 보이는 CNS균주가 더 이상 우리나라 치과병의원 환경에서 드물지 않음을 알 수 있었다.

  • 대부분의 식중독은 단체 급식으로부터 발생하는 경우가 많으며, 특히 위생상태와 연관되어 식중독을 야기 시키는 병인 물질 중 포도상 구균은 많은 부분을 차지하고 있다 따라서 본 연구에서는 서부경남지역의 5개 초등학교 급식시설에서 총 98개의 샘플 중 A 급식소의 식수, D 급식소의 손, E 급식소의 냉장고와 앞치마에서 4개의 포도상 구균을 분리하였다. 분리된 균주들은 1개의 메티실린 저항성 혈장응고 효소 음성 황색포도상구균(Methicilline Resistant Coagulase Negative Staphylococcus aureus; MRCPS)과 3개의 메치실린 민감성 혈장응고효소 양성 황색포도상구균 (Methicilline sensitive Coagulase positive Staphylococcus aureus; MSCPS)으로 구분되었다. 한편 포도상 구균은 내열성 내독소로서, 이 중 가장 문제가 되는 내독소 B(enterotoxin B)를 검색하기 위한 PCR을 실시한 결과, A 장소의 식수, D 장소의 손, E 장소의 냉장고와 앞치마에서 분리된 균주로부터 477bp의 생성물을 갖는 sob gene을 확인할 수 있었다. 이들 분리된 황색포도상구균에 대한 항생제 민감성 실험에서는 ampicillin과 penicillin에 대하여 전체적으로 저항성을 가졌으며, 특히 A 식수에서 분리된 균주는 옥사실린 저항성(oxacilline resistant)균주로 나타나 MRSA(methicilline resistant staphylococcus aureus)균주로 확인되었다.

  • Thirty-one dairy cow from two farm(more than 500,000 cells/ml of bulk milk) in Kyongbuk northern province were selected because of their high somatic cell(more than 500,000 cells/ml of milk In individual cow). Each cow received. staphylococcus aureus vaccine(Labac Staph) and immunostimulant(Ultracon) by intramuscular injection to be repeated every fifteen days for S times. The present study was investigated variation of somatic cell after administration of Labac Staph and Ultracon, and antibiotics resistance of isolated staphylococcus spp from milk in selected cow. The results obtained through the survey were summarized as follows ; 1. Ten dairy cow was injected in A farm. Chronic mastitic two cow after 2rd injection was weeded out the herd. Decrease rate of somatic cell after 1st, 2nd, ,3rd, 4th and 5th administration were 41.4%, 35.6%, 56.4%, 65.4% and 36.7%, respectively. Twenty-one cow was injected in B farm. Chronic mastitic five cow after ,3rd injection was weeded out the herd. Decrease rate of somatic cell after 1st, 2nd, 3rd, 4th and 5th administration were 36.9%, 59.9%, 24.5%, 62.6% and 78.4%, respectively. 2. In A farm, isolated staphylococcus spp were identified as S hyicus 2 strains(11.8%), coagulase negative staphylococcus 15 stains(89.2%) and S epidermidis 6strain(35.3%). In B farm, isolated staphylococcus spp were identified as S aureus 19 strains(55.98%) and coagulase negative staphylococcus 15 strains (44.2%). 3. In A fm, antibiotics resistant rate of isolated staphylococcus spp was high at ampicillin, penicillin and kanamycin, and middle at neomycin, streptomycin and erythromycin. in B farm, antibiotics resistant rate was moderate at ampicillin, penicillin, gentamicin, ka-namycin, neomycin, streptomycin, erythromycin and tetracycline, and coagulase negative staphylococcus spp was moderate at streptomycin.

정맥내 주사요법시 수액세트에 연결된 3 way-stopcock의 오염정도에 관한 실태조사 (Degree of contamination in 3-way stopcock connected to infusion set on the intravenous therapy)

  • This study is to evaluate contamination of the 3-way stopcocks connected to infusion set, which is used to the patients admitted to emergency room in a general hospital in D city. The data were collected from Oct. 1, 2001 to Feb. 25, 2002. First of all, in order to select microorganisms, From the 50 patients were randomized, Coagulase Negative Staphylococcus, Staphylococcus aureus, micrococcus, Pseudomonas aeruginosa, Acinetobacter detected. Coagulase Negative Staphylococcus, Staphylococcus aureus, micrococcus were determined to be evaluated in this study. As a result, 8 of the patients were Coagulase Negative Staphylococcus positive(>15 colonies), 4 were Staphylococcus aureus positive(>15 colonies). 1 was micrococcus positive(>15 colonies). Among the patients who were Coagulase Negative Staphylococcus positive 112(average) colonies were detected on the first day, 429 were on the second day, and 563 were on the third day. In case of patients of Staphylococcus aureus positive, 85(average) colonies were detected on the first day, 151 were on the second day, and the 203 were on the third day. CNS was cultured using API kit for the 8 patients who were in CNS positive. One case was detected Staphylococcus capitis, another one case was Staphylococcus chromogenes. Two cases were Staphylococcus xylosus, another two were Staphylococcus hominis, and the remainer were Staphylococcus epidermidis. As a result, the API codes of two Staphylococcus epidermidis had shown the same pattern, and the resistance patterns of the them were the same, too. As a result of resistance test among 5 patients who have shown that the same resistance pattern in 02SAK1, 02SAK5, 02SAK2, 02SAK4. As a result of this study, aseptic technique of 3-way stopcock intravenous therapy can protect infections, and it is needed the sterilization of the 3-way stopcock just before injection using disinfectants. It needs to improve the 3 way stopcock change intervals from 48 hours.

  • Mastitis is a common and serious infection of the mammary gland in dairy cattle and has a major economic impact on the production of milk and dairy products. Bacterial mastitis is caused by several pathogens and is most frequently associated with coagulase-negative staphylococci (CNS). Although CNS are typically associated with subclinical or mild mastitis, the importance of CNS has increased as these pathogens have emerged as predominant mastitis-related pathogens in many countries. CNS can cause persistent infections, resulting in increased milk somatic cell counts and thereby affecting milk quality and decreasing milk production. Globally, Staphylococcus chromogenes, S. epidermidis, and S. simulans are the predominant CNS species in dairy cattle mastitis. Antibacterial resistance of CNS varies with species, and most CNS are susceptible to vancomycin and resistant to penicillin and tetracycline. As the most frequently isolated CNS species, some strains of S. chromogenes exhibit phenotypic resistance to ampicillin, erythromycin, oxacillin, penicillin, and tetracycline. Some strains of S. epidermidis and S. haemolyticus are only susceptible to vancomycin and rifampicin. Therefore, more studies are needed to achieve the control and prevention of CNS as environmental pathogens.
