A clear cell hepatocellular carcinoma in an obese dog with hyperlipidemia: a case report

  • An 11-year-old obese dog was referred for a liver mass. Cytologic examination revealed vacuolated hepatocytes with mild pleomorphism. A partial liver lobectomy was performed. On histopathologic examination, the mass was diagnosed as hepatocellular carcinoma composed of hepatocytes with clear vacuoles. These findings were consistent with clear cell hepatocellular carcinoma (CCHCC). The CCHCC is a rare subtype of hepatocellular carcinoma in dogs, and clinical features are poorly defined. This is the first report on the cytological, histological and clinical aspects of CCHCC, suggesting that obesity and hyperlipidemia are potential risk factors for CCHCC in dogs.

  • Objectives: This report presents a case of late onset chorea hyperglycemia basal ganglia syndrome treated using traditional Korean medicine therapies. Methods: A patient was treated with Korean medicine therapies, including the herbal medication, Yokukan-san. We evaluated the improvements in the patient's symptoms based on the total score of the Abnormal Involuntary Movement Scale, the activities of daily living portion of the Universidade Federal de Minas Gerais (UFMG) Sydenham's Chorea Rating Scale, the numeric rating scale, and an assessment of the patient's writing. Results: After undergoing Korean medicine treatment for 15 days, there were improvements in the patient's chorea and writing as well as a decrease in the scores for all three scales. Conclusions: This clinical case study suggests that Yokukan-san might be effective for treating dyskinesia in patients with chorea hyperglycemia basal ganglia syndrome.

The Effect of Public Report on Antibiotics Prescribing Rate (급성상기도감염 항생제 처방률 공개 효과 분석)

  • Controlling inappropriate antibiotics prescribing for acute upper respiratory infections(URI) is a very important for prudent use of antibiotics and resistance control. Health Insurance Review and Assessment Service (HIRA) introduced Prescribing Evaluation Program and publicly reported antibiotics prescribing rate for URI of each health institution. We performed segmented regression analysis of interrupted time series to estimate the effect of public report on antibiotics prescribing rate using national health insurance claims data. The results indicate that just before the public report period, clinics' monthly antibiotics prescribing rate for URI was 66.7%. Right after the public report, the estimated antibiotics prescribing rate dropped abruptly by 12.3%p. There was no significant changes in month-to-month trend in the prescribing rate before and after the intervention.

  • Peripheral gangrene, characterized by distal ischemia of the extremities, is a rare complication in patients with falciparum malaria. Patients with this complication have generally undergone early amputation of the affected areas. In this report, we describe 3 adult Thai patients presented at the Hospital for Tropical Diseases, Bangkok, with high grade of fever ranged 6-9 days, jaundice, acute renal failure, respiratory failure, alteration of consciousness and shock. Two patients had gangrene developed at the lower extremities on day 1 of hospitalization and 1 patient had gangrene developed on day 3. Blood smears revealed hyperparasitemia with Plasmodium falciparum. These patients were diagnosed as having severe malaria with peripheral gangrene. The resolution of gangrene was successfully achieved by treatment with artesunate and conservative treatment in 2 of 3 cases.

  • Objectives This study was perfomed to assess the adherance to CARE (CAse REport) guideline of case reports in the Journal of Korean Medicine Rehabilitation Methods We searched the case reports published in the Journal of Korean Medicine Rehabilitation from January 2016 to April 2019 in the database of oriental medicine advanced searching integrated system (OASIS). Then we evaluated the quality of the searched case reports based on the CARE guideline. Results Totally 31 papers were selected after the screening the case reports by the inclusion and exclusion criteria. The report rate of the sub-items of the CARE guideline was 78.26% at the maximum, 60.87% at the maximum, and 70.97% on the average. The following items were reported only in less than 50% of them; 'Timeline', 'Diagnostic challenges', 'Diagnostic reasoning including other diagnoses considered' 'Prognostic characteristics', 'Follow-up and Outcomes', 'Patient Perspective', 'Informed Consent' Conclusions This study is expected to contribute to the overall improvement of the level of case reports in the Journal of Korean Medicine Rehabilitation.

Immediate loading on mandibular edentulous patient with SFI Bar$^{(R)}$ overdenture

  • Despite the greater retention and low maintenance of bar attachment system, the longer clinical time and increased number of visits were the assignments to overcome in bar attachment system. This case report describes SFI-Bar$^{(R)}$ (Cendres et Me$\'{e}$taux, Biel/Bienne, Switzerland) to be solve those problems. A 65-year-old female, who had severely absorbed mandible, hoped to wear a stable mandibular denture without pain. As soon as two implants were placed on mandible, a tube bar was connected to two adaptors connected to each implant. The length of the tube bar was adjusted considering inter implant distance, and reconnected to the adaptors. Finally a female part was seated beneath the denture. This case report showed that a satisfactory clinical result was achieved by delivering bar overdenture immediately after implant placement without laboratory procedure.

A Case Report of Lower Extremity Paresthesia with Polyneuropathy Treated with Traditional Korean Complex Treatment (양하지 이상감각을 호소하는 다발신경병증 환자에 대한 침구-한약 복합한방 치료 치험 1례)

  • Objectives: This report presents the case of a 21-year-old woman with lower extremity paresthesia in both feet due to polyneuropathy. Methods: The patient was treated using a Korean traditional complex treatment approach that included herbal medication, acupuncture, electroacupuncture, and moxibustion. We evaluated the improvements in the pain symptoms in both feet using the numeric rating scale (NRS); we also assessed for gait disturbance and used Digital Infrared Thermographic Imaging (D.I.T.I.) to evaluate minute changes in body temperature in diseased areas. Results: After administering the Korean traditional complex treatment, we observed a decrease in pain levels in both feet based on the NRS scores. We also observed improvements in gait disturbance and D.I.T.I. Conclusions: This case showed that the use of a Korean traditional complex treatment approach, consisting of herbal medication, acupuncture, electroacupuncture, moxibustion, had a positive effect on decreasing polyneuropathy symptoms.

  • Objective : This study was designed to report the clinical effect of Rectal Injection therapy on chronic prostatitis. Methods : We analyzed 20 manuscripts contributed to the Chinese medical journals from 1999 to 2003 that presenting report of Rectal Injection therapy on chronic prostatitis. Results : The results are summarized as follows 1. Operation of rectal injection therapy was treated 2~3 times and one course take time 5~30 days. 2. During therapy, average amount of fluid is 100~150 ml and it takes 1~2 hours. 3. Using medication was 'chung-yol' medicine, 'hwal-hyul-gu-er' medicine, 'bo-yik' medicine, 'lee-soo-sam-sup' medicine. Conclusion : We conclusion that Rectal Injection therapy was clinical effect on chronic prostatitis.

A Clinical Case of Tinea Pedis Treated with Korean Medicine (한방치료로 호전된 족부백선 환자 1례)

  • Objectives : This study is to report a case of tinea pedis accompanied with erythema and pruritus, treated with Korean medicine treatment. Methods : A 26-year-old female patient has suffered from erythema and pruritus on her right 3rd toe. She applied desoximetasone ointment on it for a month, but the effect didn't last long and the symptoms got worse. After disappointing application of the corticosteroid, she received Korean medicine treatment including herbal medicine and herbal external medication for 1.5 months. Results : The skin symptoms such as erythema and pruritus were remarkably improved. Also, the Visual Analogue Scale(VAS) of pruritus changed from 4 to 0. Conclusions : This case report suggests that Korean medicine may have a role in treating tinea pedis with some skin symptoms such as erythema and pruritus.
