Helkimos Indices and Frictions Craniomandibular Index in Korean Young Population

  • To evaluate Helhimo's Anamnestic Dysfunction Index(AI), Helkimo's Clinical Dysfunction Index(HDI), and Friction's Craniomandibular Index(CMI) in Korean Young population, clinical examinations were performed in 207 dental college students who were healthy and had no evidence of the craniomandibular disorders. The obtained results were as follows; 1. The mean values of the Helkimo's Anamnestic Index(AI), Clinical Dysfunction Index(HDI), Fricton's Palpation Index(PI), Dysfunction Index(DI), and Craniomandibular Index(CMI) were 0.35, 0.71, 0.03, 0.05, and 0.04 in male subjects. 2. The mean values of the Helkimo's Anamnestic Index(AI), Clinical Dysfunction Index(HDI), Fricton's Palpation Index(PT), Dysfunction Index(DI), and Fricton's Craniomandibular Index(CMI) were 0.42, 0.72, 0.02, 0.04, and 0.03 in female subjects. 3. There was no statistically significant difference between male and female subjects in each parameter. 4. The palpation index(PI) observed in Korean Young Population was lower than that in American population.

체질 임상 정보를 분석하기 위한 임상 지수 프로그램 개발 (Clinical Index Program for Analyzing Clinical Information of Sasang Constitutional Medicine)

  • In Sasang Constitution Medicine (SCM), it is most important that a personal SCM type is determined accurately ahead of applying any Sasang treatments. Although SCM doctors have districted personal SCM types in many hospitals and universities via their own discriminant, it still lacks objective criteria on diagnosis of SCM type. Therefore, many researchers have been studied to diagnose the SCM type using constitutional clinical data. Previous work, we have developed decision tree program to analyze the clinical information. In this paper, we developed a clinical index program based on the web to analyze the correlation among clinical information. Finally, we identified useful factors(4 clinical indexes) which have significant influence on SCM types using clinical index program with previously developed decision tree program.

소음인 소증 진단평가지표 개발 연구 (A Study on the Development of Diagnosis and Assessment Index by Soeumin's Ordinary Symptoms)

  • Objective This study was devised to create a new diagnosis and an assessment index by Soeumin's Ordinary Symptoms. Method First, a Working Committee and an Advisory Committee were formed for development, and the development goals were set. Then, the Soeumin's disease diagnosis indicators were extracted from the previous Clinical Practice Guidelines for Sasang Constitutional Medicine. To increase the clinical applicability, the extracted diagnosis indicators were focused on ordinary symptoms, and translated into Korean. The translated diagnosis and assessment indicators for Soeumin were surveyed to the Advisory Committee using the Delphi technique, and the inclusion, importance, and validity of each indicator were investigated accordingly. Result The translated diagnosis and assessment indicators were finally revised based on the surveyed inclusion and translation validity opinions, and the weight of each indicator was set based on the investigated importance, and a draft of the diagnosis and assessment index was developed. Conclusion This developed index can help to make effective diagnose about Soeumin's diseases by clinical doctor. In the future, a clinical study of this index can be conducted to consider the reliability, validity, and cut-point, and through this process, the actual clinical applicability will be improved.

Development and Validation of a Simple Index Based on Non-Enhanced CT and Clinical Factors for Prediction of Non-Alcoholic Fatty Liver Disease

  • Objective: A widely applicable, non-invasive screening method for non-alcoholic fatty liver disease (NAFLD) is needed. We aimed to develop and validate an index combining computed tomography (CT) and routine clinical data for screening for NAFLD in a large cohort of adults with pathologically proven NAFLD. Materials and Methods: This retrospective study included 2218 living liver donors who had undergone liver biopsy and CT within a span of 3 days. Donors were randomized 2:1 into development and test cohorts. CTL-S was measured by subtracting splenic attenuation from hepatic attenuation on non-enhanced CT. Multivariable logistic regression analysis of the development cohort was utilized to develop a clinical-CT index predicting pathologically proven NAFLD. The diagnostic performance was evaluated by analyzing the areas under the receiver operating characteristic curve (AUC). The cutoffs for the clinical-CT index were determined for 90% sensitivity and 90% specificity in the development cohort, and their diagnostic performance was evaluated in the test cohort. Results: The clinical-CT index included CTL-S, body mass index, and aspartate transaminase and triglyceride concentrations. In the test cohort, the clinical-CT index (AUC, 0.81) outperformed CTL-S (0.74; p < 0.001) and clinical indices (0.73-0.75; p < 0.001) in diagnosing NAFLD. A cutoff of ≥ 46 had a sensitivity of 89% and a specificity of 41%, whereas a cutoff of ≥ 56.5 had a sensitivity of 57% and a specificity of 89%. Conclusion: The clinical-CT index is more accurate than CTL-S and clinical indices alone for the diagnosis of NAFLD and may be clinically useful in screening for NAFLD.

Comparison of Sleep Parameter according to Apnea-Hypopnea Index

  • Obstructive sleep apnea (OSA) is the most common type of sleep apnea and is caused by obstruction of the upper airway. It is characterized by repetitive pauses in breathing during sleep despite the effort to breathe. Apnea is closely related to clinical findings and respiratory disturbance index (RDI). The total subjects were 42 (male 26, female 16) and examined by polysomography (PSG) in terms of RDI above 5 and below 5 at Mok-Dong Hospital in Ewha Womans University from January to June, 2012. The study revealed the followings: The comparison of clinical findings and RDI above 5 showed significant increase in age, BMI, and snoring sound although lowest $SpO_2$ level decreased. The correlation coefficient analysis between clinical findings and RDI showed statistically significant correlation in age, BMI, lowest $SpO_2$ although snoring sound and average $SpO_2$ showed statistically insignificant correlation.

파킨슨병 변증 유형 및 지표 분포에 대한 전향적 다기관 관찰연구 프로토콜 (An Observational Multi-Center Study Protocol for Distribution of Pattern Identification and Clinical Index in Parkinson's Disease)

  • Objectives: This study investigated the pattern identification (PI) and clinical index of Parkinson's disease (PD) for personalized diagnosis and treatment. Methods: This prospective observational multi-center study recruited 100 patients diagnosed with PD from two Korean medicine hospitals. To cluster new subtypes of PD, items on a PI questionnaire (heat and cold, deficiency and excess, visceral PI) were evaluated along with pulse and tongue analysis. Gait analysis was performed and blood and feces molecular signature changes were assessed to explore biomarkers for new subtypes. In addition, unified PD rating scale II and III scores and the European quality of life 5-dimension questionnaire were assessed. Results: The clinical index obtained in this study analyzed the frequency statistics and hierarchical clustering analysis to classify new subtypes based on PI. Moreover, the biomarkers and current status of herbal medicine treatment were analyzed using the new subtypes. The results provide comprehensive data to investigate new subtypes and subtype-based biomarkers for the personalized diagnosis and treatment of PD patients. Ethical approval was obtained from the medical ethics committees of the two Korean medicine hospitals. All amendments to the research protocol were submitted and approved. Conclusions: An objective and standardized diagnostic tool is needed for the personalized treatment of PD by traditional Korean medicine. Therefore, we developed a clinical index as the basis for the PI clinical evaluation of PD. 하악 2급 치근 이개부 병소에서 키토산 나노 차폐막을 이용한 치주조직 재생의 임상적 효과에 관한 비교 연구 (A comparative study of the clinical effects of chitosan nanofiber membrane in the treatment of mandibular class II furcation defects)

  • The purpose of this study was to evaluate the clinical efficacy of guided tissue regeneration(GTR) technique using chitosan nanofiber membrane and to compare it to the clinical efficacy following GTR using PLA/PLGA(copolymer of polvlactic acid and polylacticglycolic acid) membrane in mandibular class II furcation defects in human. The chitosan nanofiber membranes were applied to the mandibular class II furcation defects of 13 patients(test group) and PLA/PLGA membranes were applied to those of 11 patients(control group). Probing pocket depth, clinical attachment level, gingival recession, plaque index and gingival index were measured at baseline and 3 months postoperatively. Vertical and horizontal furcation defect depth were measured at surgery. Both groups were statistically analyzed by Wilcoxon signed Ranks Test and Mann-Whitney Test using SPSS program. The results were as follows: 1. Probing pocket depth, clinical attachment loss and gingival index were significantly reduced at 3 months postoperatively compared to values of baseline in both groups(p<0.05). 2. Gingival recession and plaque index were not significantly decreased at 3 months postoperatively compared to values of baseline in both groups. 3. No significant difference between two groups could be detected with regard to changes of probing pocket depth, gingival recession, clinical attachment level, plaque index and gingival index at 3 months postoperatively. In conclusion, chitosan nanofiber membrane is effective in the treatment of human mandibular class II furcation defects and a longer period study is needed to fully evaluate the outcomes.

Roles of Ki67 in Breast Cancer - Important for Management?

  • Background: The three standard biomarkers used in breast cancer are the estrogen receptor (ER), progesterone receptor (PR) and human epidermal growth factor receptor 2 (HER2). The Ki-67 index, a proliferative marker, has been shown to be associated with a poorer outcome, and despite absence of standardization of pathological assessment, is widely used for therapy decision making. We aim to study the role of the Ki-67 index in a group of Asian women with breast cancer. Materials and Methods: A total of 450 women newly diagnosed with Stage 1 to 3 invasive breast cancer in a single centre from July 2013 to Dec 2014 were included in this study. Univariable and multivariable logistic regression was used to determine the association between Ki-67 (positive defined as 14% and above) and age, ethnicity, grade, mitotic index, ER, PR, HER2, lymph node status and size. All analyses were performed using SPSS Version 22. Results: In univariable analysis, Ki -67 index was associated with younger age, higher grade, ER and PR negativity, HER2 positivity, high mitotic index and positive lymph nodes. However on multivariable analysis only tumour size, grade, PR and HER2 remained significant. Out of 102 stage 1 patients who had ER positive/PR positive/HER2 negative tumours and non-grade 3, only 5 (4.9%) had a positive Ki-67 index and may have been offered chemotherapy. However, it is interesting to note that none of these patients received chemotherapy. Conclusions: Information on Ki67 would have potentially changed management in an insignificant proportion of patients with stage 1 breast cancer.

폐쇄성수면무호흡증후군 환자의 무호흡-저호흡지수와 임상양상간의 상관성 (Correlation between Clinical Characteristics and Apnea-Hypopnea Index with Obstructive Sleep Apnea Syndrome)

  • Obstructive sleep apena syndrome (OSAS) is accompanied by the following symptoms: apnea caused by upper respiratory tract obstruction while sleeping, repetitive lowering of $SpO_2$, severely affected excessive daytime sleepiness (EDS), suffocation/frequent awakeness while sleeping, daytime lethargy, and lack of concentration. OSAS was investigated with sex, age, body weight, body mass index (BMI), neck circumference and snoring sound as clinical characteristics and the anticipating factors of OSAS were studied in relation with the apnea-hypopnea index (AHI). The subjects were 42 people (male 34/female 8) who visited the clinic due to snoring and had polysomnography evaluation. AHI was differenciated into normal (less than 0~5/hr), mild (5~15/hr), moderate (15~30/hr) and severe (more than 30/hr). As the apnea-hypopnea index (AHI) gets higher, the snoring sound was louder (p<0.01), neck circumference was thicker (p<0.05) and also there were relative correlations with body weight (p<0.01), body mass index (p<0.05), snoring sound (p<0.01) and neck circumference (p<0.01). Since the snoring sound and neck circumference explained 32.8% of the AHI distribution, if the patient was severely snoring or had a thick neck circumference due to obesity, the apnea-hypopnea index showed a predisposition to the obstructive sleep apnea syndrome.

Distribution Dynamics and Proposed Determinants: Exploring Morphological, Clinical Laboratory, and Lifestyle Factors in the Coexistence of Age-Related Skeletal Muscle Mass Loss and Obesity among Young Men: A Nationwide Cross-Sectional Study

  • PURPOSE: This study examined the distribution dynamics and proposed determinants, including morphological measurements, clinical laboratory tests, and lifestyle factors among young Korean men aged 20 to 29 years with the coexistence of age-related loss of skeletal muscle mass and obesity (CALSMO). METHODS: Six hundred and sixty-six participants were divided into two groups based on their skeletal muscle mass index, with 12 individuals categorized in the CALSMO group and the remaining 654 in the normal group. The proposed determinants variables consisted of three main components: morphological measurements, clinical laboratory tests, and lifestyle factors. The morphological measurement variables were height, weight, body mass index, waist circumference, and skeletal muscle mass index. The clinical laboratory tests were fasting glucose, triglyceride, total cholesterol levels, and systolic and diastolic blood pressure. The lifestyle factors considered were alcohol consumption and tobacco use. Complex sampling analysis was performed for the evaluation. RESULTS: The distribution dynamics were determined to be 1.81(1.02-3.18) %. Morphological factors, such as height, weight, body mass index, waist circumference, and skeletal muscle mass index, showed significant differences (p < .05). The clinical laboratory test variables, specifically the fasting glucose, triglyceride, and total cholesterol levels, also exhibited significant differences (p < .05). The lifestyle factor, alcohol consumption, also showed a significance (p < .05). CONCLUSION: This study provides insights into the distribution dynamics. The proposed determinants in young Korean individuals with CALSMO are height, weight, body mass index, waist circumference, skeletal muscle mass index, fasting glucose, triglyceride, total cholesterol levels, and alcohol consumption.
