Gastric cancer is heterogeneous in morphology, biology, genomics, and treatment response. Alterations in human epidermal growth factor receptor 2 (HER2) overexpression, microsatellite instability (MSI) status, programmed death-ligand 1 (PD-L1) levels, and fibroblast growth factor receptor 2 (FGFR2) can be used as biomarkers. Since the combination of fluoropyrimidine/platinum plus trastuzumab that was investigated in the ToGA trial was approved as a standard of care in HER2-positive patients in 2010, no other agents showed efficacy in the first- (HELOISE, LOGiC, JACOB trials) and second- (TyTAN, GATSBY, T-ACT trials) line treatments. Despite the success in treating breast cancer, various anti-HER2 agents, including a monoclonal antibody (pertuzumab), an antibody-drug conjugate (ADC; trastuzumab emtansine [T-DM1]), and a small molecule (lapatinib) failed to translate into clinical benefits until the KEYNOTE-811 (first-line) and DESTINY-Gastri01 (≥second-line) trials were conducted. The incorporation of HER2-directed treatment with immune checkpoint inhibitors in the form of a monoclonal antibody or ADC is now approved as a standard treatment. Despite the promising results of new agents (engineered monoclonal antibodies, bi-specific antibodies, fusion proteins, and small molecules) in the early phase of development, the management of HER2-positive gastric cancer requires further optimization to achieve precision medicine with a chemotherapeutic backbone. Treatment resistance is a complex process that can be overcome using a combination of chemotherapy, targeted agents, and immune checkpoint inhibitors, including novel agents. HER2 status must be reassessed in patients undergoing anti-HER2 treatment with disease progression after the first-line treatment. As a general guideline, patients who need systemic treatment should receive chemotherapy plus targeted agents, anti-angiogenic agents, immune checkpoint inhibitors, or their combinations.

The microdeletion syndrome of chromosome 2p15p16.1 (MIM: 612513) is an extremely rare contiguous gene deletion syndrome. Microdeletions of varying sizes in the 2p15-16.1 region are associated with developmental delay, intellectual disability, autism spectrum disorder, hypotonia, and craniofacial dysmorphism. Previous studies have identified two critical regions: the proximal 2p15 and distal 2p16.1 regions. BCL11A, PAPOLG, and REL genes play crucial roles in patients with 2p16.1 microdeletion. To our knowledge, only 39 patients have been reported as having 2p15p16.1 microdeletion syndrome. Here, we present another patient with 2p15p16.1 microdeletion syndrome. A nine-month-old boy was referred to our clinic for the psychomotor delay, facial dysmorphism, and congenital hypothyroidism. During his follow-up visits, he was diagnosed with global developmental delay, intellectual disability, abnormal behavior, hypotonia, microcephaly, and abnormal electroencephalography. Using a chromosomal microarray for genetic analysis, a novel, de novo, 622 kb microdeletion of 2p16.1 was identified as one of the critical regions of the 2p15p16.1 microdeletion syndrome. This is the first case of its kind in Korea. We have discussed our case and literature reviews to clarify the relationship between the genes involved and clinical phenotypes in 2p15p16.1 microdeletion syndrome.

Coronavirus disease 2019 (COVID-19) is an inflammatory and infectious disease caused by severe acute respiratory syndrome coronavirus 2 virus with a complex pathophysiology. While COVID-19 vaccines and boosters are available, treatment of the disease is primarily supportive and symptomatic. Several research have suggested the potential of herbal medicines as an adjunctive treatment for the disease. A popular herbal medicine approved in the Philippines for the treatment of acute respiratory disease is Vitex negundo L. In fact, the Department of Science and Technology of the Philippines has funded a clinical trial to establish its potential as an adjunctive treatment for COVID-19. Here, we utilized network pharmacology and molecular docking in determining pivotal targets of Vitex negundo compounds against COVID-19. The results showed that significant targets of Vitex negundo compounds in COVID-19 are CSB, SERPINE1, and PLG which code for cathepsin B, plasminogen activator inhibitor-1, and plasminogen, respectively. Molecular docking revealed that α-terpinyl acetate and geranyl acetate have good binding affinity in cathepsin B; 6,7,4-trimethoxyflavanone, 5,6,7,8,3',4',5'-heptamethoxyflavone, artemetin, demethylnobiletin, gardenin A, geranyl acetate in plasminogen; and 7,8,4-trimethoxyflavanone in plasminogen activator inhibitor-1. While the results are promising, these are bound to the limitations of computational methods and further experimentation are needed to completely establish the molecular mechanisms of Vitex negundo against COVID-19.

Non-small cell lung cancer (NSCLC) is an important cause of cancer-associated deaths worldwide. Therefore, the exact molecular mechanisms of NSCLC are unidentified. The present investigation aims to identify the miRNAs with predictive value in NSCLC. The two datasets were downloaded from the Gene Expression Omnibus (GEO) database. Differentially expressed miRNAs (DEmiRNA) and mRNAs (DEmRNA) were selected from the normalized data. Next, miRNA-mRNA interactions were determined. Then, co-expression network analysis was completed using the WGCNA package in R software. The co-expression network between DEmiRNAs and DEmRNAs was calculated to prioritize the miRNAs. Next, the enrichment analysis was performed for DEmiRNA and DEmRNA. Finally, the drug-gene interaction network was constructed by importing the gene list to dgidb database. A total of 3,033 differentially expressed genes and 58 DEmiRNA were recognized from two datasets. The co-expression network analysis was utilized to build a gene co- expression network. Next, four modules were selected based on the Zsummary score. In the next step, a bipartite miRNA-gene network was constructed and hub miRNAs (let-7a-2-3p, let-7d-5p, let-7b-5p, let-7a-5p, and let-7b-3p) were selected. Finally, a drug-gene network was constructed while SUNITINIB, MEDROXYPROGESTERONE ACETATE, DOFETILIDE, HALOPERIDOL, and CALCITRIOL drugs were recognized as a beneficial drug in NSCLC. The hub miRNAs and repurposed drugs may act a vital role in NSCLC progression and treatment, respectively; however, these results must validate in further clinical and experimental assessments.

Ephs belong to the largest family of receptor tyrosine kinase and are highly conserved both sequentially and structurally. The structural organization of Eph is similar to other receptor tyrosine kinases; constituting the extracellular ligand binding domain, a fibronectin domain followed by intracellular juxtamembrane kinase, and SAM domain. Eph binds to respective ephrin ligand, through the ligand binding domain and forms a tetrameric complex to activate the kinase domain. Eph-ephrin regulates many downstream pathways that lead to physiological events such as cell migration, proliferation, and growth. Therefore, considering the importance of Eph-ephrin class of protein in tumorigenesis, 7,620 clinically reported missense mutations belonging to the class of variables of unknown significance were retrieved from cBioPortal and evaluated for pathogenicity. Thirty-two mutations predicted to be pathogenic using SIFT, Polyphen-2, PROVEAN, SNPs&GO, PMut, iSTABLE, and PremPS in-silico tools were found located either in critical functional regions or encompassing interactions at the binding interface of Eph-ephrin. However, seven were reported in nonsmall cell lung cancer (NSCLC). Considering the relevance of receptor tyrosine kinases and Eph in NSCLC, these seven mutations were assessed for change in the folding pattern using molecular dynamic simulation. Structural alterations, stability, flexibility, compactness, and solvent-exposed area was observed in EphA3 Trp790Cys, EphA7 Leu749Phe, EphB1 Gly685Cys, EphB4 Val748Ala, and Ephrin A2 Trp112Cys. Hence, it can be concluded that the evaluated mutations have potential to alter the folding pattern and thus can be further validated by in-vitro, structural and in-vivo studies for clinical management.
Background: ABO typing is crucial for ensuring safe blood transfusion and is commonly performed by examining antigen-antibody interactions. Determining ABO blood group can be difficult when dealing with ABO discrepancy and ABO subgroups. ABO genotyping may be necessary to resolve ABO discrepancy. ABO genotyping primarily involves direct sequencing, with the possibility of using other molecular methods. Methods: PCR and direct sequencing of exons 6 and 7 were performed for total 108 samples from June 2010 to December 2019. Also, other molecular methods including cloning sequencing and short tandem repeat analysis were carried out just in case. Sequencing data were compared with allele information of blood group antigen mutation databases. Results: The predominant causal allele among 108 ABO discrepant cases was cis-AB01, with 28 cases. This was followed by rare ABO alleles (B309, B306, A204, Bw29, and Ax01) with 14 cases, and blood chimera with 5 cases. Five new alleles were identified during the investigation. Conclusion: This study reaffirms that cis-AB is the most common cause of inherited ABO discrepancies, and cis-AB01 is the most prevalent cis-AB allele in the Korean population, also in the southeastern region. In addition, we discovered five new alleles and five blood chimeras by adopting sequencing analysis and additional molecular techniques to resolve ABO discrepancies, which provide regional data on rare alleles. This study presents rare and new ABO alleles and blood chimeras identified over a ten-year period at two major university hospitals in Southeastern Korea.
목적: 자연 유산에 대한 세포유전학적 분석은 유산의 원인을 규명하여 부모에게 정확한 유전상담을 위한 귀중한 정보를 제공한다. 또한 그 분석결과를 통해 염색체 이상의 빈도와 유형을 알 수 있으며 그에 따른 재발 위험도를 산정할 수 있다. 하지만 아직까지 한국인에서는 자연유산에서 관찰되는 염색체 이상에 대해 몇몇의 보고들이 있으나 그 대상군의 수가 적다. 이에 저자들은 자연유산에 의한 수태산물 470예에서 관찰되는 염색체 이상의 발생 빈도와 종류에 대해 보고하고자 한다. 대상 및 방법: 2005년부터 2010년까지 국내 병원이나 개인 산부인과에서 서울의과학연구소에 염색체 분석이 의뢰된 자연유산에 의한 수태산물 470예를 대상으로 염색체 핵형 결과와 함께 유산시 임신 주수와 산모나이에 따른 염색체 이상의 종류와 발생빈도를 분석하였다. 결과: 총 470예의 자연 유산에 의한 수태산물의 염색체 핵형을 분석한 결과 염색체 이상 빈도는 54.3% (255/470)였다. 그 중 수적 이상이 89.3% (228/470)로 대부분을 차지하였는데, 수적 이상 중에는 상염색체의 삼염색체가 67.0%였고, 단일염색체 X가 12.5%, 다배수성은 8.2%, 삼염색체X가 0.8%, 상염색체의 단일염색체가 0.8%였다. 구조적 이상은 10.7% (27/470)으로 균형전좌가 3예, 불균형전좌는 24예이었다. 남녀 성비는 정상핵형과 비정상핵형 모두 0.58과 0.65으로 여아에서 높게 나타났다. 상염색체의 삼염색체는 1번, 3번, 19번 염색체를 제외하고 모두 관찰되었는데, 16번 삼염색체가 19.9%로 가장 많았으며, 22번 염색체가 13.5%, 21번 염색체가 12.3%, 15번 염색체가 9.9%, 13번과 18번 염색체가 각각5.3% 순으로 관찰되었다. 염색체 이상의 빈도는 임신 주수가 낮을수록, 산모나이가 많을수록 높았는데, 산모나이가 많을수록 삼염색체와 수적이상과 구조적 이상이 같이 있는 염색체 이상의 빈도가 높았다. 결론: 본 연구는 최근 6년간의 자연유산에 의한 수태산물 470예에 대해 세포유전학적 결과를 분석하였고, 이는 국내 자연유산 환자에게 적절한 산전유전상담을 위한 기초자료로서 유용할 것으로 생각된다.
전형적 갈락토오스혈증(classical galactosemia)은 상염색체 열성으로 유전되는 galactose-1-phosphate uridyltransferase(GALT) 효소의 결핍에 의한 갈락토오스 대사 장애 질환이며 galactose-1-phosphate가 축적되어 간, 뇌, 신경에 심각한 영향을 끼친다. 본 증례는 GALT 효소 결핍 신생아에서, 출생 후 락토오스가 함유된 일반 수유를 시작하면서 심한 황달, 구토, 심한 출혈경향 및 간 부전 등이 발생하였다. 신생아 선별검사에서 갈락토오스의 증가로 재검을 의뢰하였고, 그 사이에 일반수유로 인해 임상증상이 악화 되어 생후 11일째 본원으로 전원되었다. 효소분석검사와 유전자검사로 확진 전에 임상적으로 갈락토오스혈증이 의심이 되어 소이 분유로 수유를 시작하였고, 이후 심한 황달과 출혈경향의 호전, 간기능 호전, 체중 증가와 전신상태가 회복되어 11일간의 입원 이후 퇴원하였다. 갈락토오스혈증의 임상 증상에 대한 올바른 이해를 통해 조기 진단하여 식이요법 등의 치료로 사망률을 줄이고, 유전자검사로 갈락토오스혈증을 확진하여 유전상담 및 산전진단에 유용하게 이용할 수 있는데, 본 증례에서는 효소분석 결과 GALT 효소 결핍증과 유전자검사에서 아직까지 보고된 적이 없는 GALT 유전자의 이형 돌연변이를 경험 하였기에 이를 보고하는 바이다.
목 적 : 터너증후군 환자들에게는 다양한 핵형과 표현형이 나타나지만 우리나라에서는 그 연관성에 대한 연구가 미미한 실정이다. 그리하여 본 연구에서는 터너증후군으로 진단받은 환자들의 염색체 이상, 임상양상, 동반 질환에 대해 조사하였다. 대상 및 방 법 : 경북대학교병원에서 터너증후군으로 진단받은 환자 92명을 대상으로 염색체 핵형을 분류하였으며, 그 중 62명을 대상으로 임상 양상 및 동반 질환을 조사하였다. 결 과 : 핵형이 45,X인 환자는 54.3%였고. 섞임증 및 구조 이상이 나머지를 차지하였다. 섞임증의 경우 45,X에 비하여 Turner stigmata의 빈도가 낮았다. 46,X,del(Xp) 및 45,X/46,X,del(Xq)에서는 모두 골격 이상이 나타난 반면, 46,X, del(Xq)에서는 나타나지 않았다. 46,X,del(Xp)에서는 성적 유치증이 나타나지 않았지만, 46,X,del(Xq) 및 45,X/46,X,del(Xq)의 경우에는 이차 성징 지연이 지연 및 무월경이 나타났다. 46,X,i(Xq) 및 45,X/46,X,i(Xq)의 경우 이차 성징이 발현되지 않았고 모두 일차 무월경을 보였다. 그 외에 장완의 isochromosome이 있는 경우 청력 장애 및 갑상선 질환이 더 빈번하게 나타났다. 결 론 : 터너증후군 환자들의 핵형과 표현형 사이의 연관성을 조사하는 작업은 성염색체에 위치하는 유전자자리를 예측하는 정보를 얻을 수 있다는 점에서 중요하다고 생각한다.
목 적: 국내실정에 맞는 전문 유전상담사 교육프로그램과 교육기관의 인정 및 유전상담사의 전문 자격 인증 제도를 설립에 근거자료로 활용하고자 하였다. 대상 및 방법: 국외의 유전상담사 교육 프로그램 인정과 유전상담사 인증 제도를 파악하고, 관련 업무 종사자에게 설문조사를 실시하였으며, 전문가 회의를 통해 도출된 의견을 분석하였다. 결 과: 인정받은 교육기관 수료자만을 인증시험요건으로 규정한 미국과는 달리, 일본은 관련 분야의 임상경력을 갖춘자에게도 한시적으로 인증시험 자격을 부여하였다. 일본은 필기와 면접시험으로, 미국은 필기시험으로 인증 시험이 진행되고 있으며, 주기는 미국 2년, 일본 1년, 인증기간은 미국 10년, 일본 5년이었다. 설문조사에서는 '대한의학유전학회에서 위원회를 구성하여 교육기관 인정과 전문 유전상담사 인증을 담당하며, 관련학회의 의견수렴과 자문을 얻는다'에 대다수가 찬성하였다. 지원자의 전공은 제한할 필요가 없으며, 인증시험은 필기와 실기로 이루어져야하고 주기는 2년, 인증기간은 5년이 적당하다는 응답이 가장 많았다. 결 론: 유전상담 교육기관 및 유전상담사 인증 제도는 대한 의학유전학회에서 주관하며, 미국과 일본의 프로그램을 참고하여 국내실정에 맞게 도입한다. 대한의학유전학회에서는 위원회를 구성하여 관련 학회 등의 의견수렴과 자문을 얻어 교육프로그램 개발과 인증 제도를 구체화 할 것을 촉구하는 동시에 정부에서도 유전상담의 급여화와 필요한 제도적인 지원이 있어야만 국내의 유전의료의 발전에 필요한 인프라 구축의 내실을 다질 수 있게 될 것이다.

② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
