The Study on Post Acupotomy Headache (침도 침술 후 나타나는 두통에 대한 임상적 고찰)

  • Objectives : This study was designed to investigate incidence rate, clinical features and treatment of headache followed after acupotomy. And based on that, we study correlating between post acupotomy headache and post-lumbar puncture headache(PLPH). Methods : From September 1st, 2007 to December 31st, 2007, the 73 inpatients who admitted to Daejeon Oriental hospital and underwent acupotomy on lumbar spine and cervical spine etc. more than once were observed. Then 9 inpatients who occurred headache after acupotomy was analyzed according to incidence rate, gender, age, clinical features. Results : We investigated clinical features of post acupotomy headache and made comparative study of post acupotomy headache and PLPH. The average frequency of post acupotomy headache was about 12.3%. The risk factors are undergoing acupotomy on lumbar spine, young age. Gender does not affect the incidence of post acupotomy headache. Patients complain of headache which worsen when the patients is upright and improve after the patients lie back down in a few minutes. Those clinical features are similar to PLPH. Conclusions : Although two type of headache have significant similarities, there is no enough evidence to support this study. In addition, the cause and treatment of post acupotomy headache are still unclear. Thus, based on the above study, more studies have to be investigated.

A Clinical Study of the Children and Adolescents Inpatients Admitted to the Oriental Medical Hospital (한방병원에 입원한 소아.청소년 환자에 관한 임상적 연구)

  • Objectives: The purpose of this study is to investigate clinical features of children and adolescents patients admitted to the oriental medical hospital. Methods: The study composed of 1284 inpatients aging from 0 to 19 who had been admitted in 4 oriental medical hospitals located in Daejeon and Chungcheong. Results: 1. In this study, 688 boys and 715 girls were examined, giving the ratio of 0.96:1. Average age of the children and adolescents was 11.5, and the number of inpatients has increased as getting older. 2. The percentage of children and adolescents inpatients has increased every year : 9.1% in 2005, 13.0% in 2006, 13.8% in 2007, 16.3% in 2008, 18.0% in 2009, with 22.2% in 2010, 7.8% until May 2011. The month scored the highest admittance was December and the lowest was March. 3. In systemic division, the musculoskeletal system was the most common, followed by neuromuscular system, respiratory system, digestive system, psychological system, and nervous system. 4. Most patients have admitted in acupuncture & moxibustion department, followed by oriental pediatrics, oriental rehabilitation medicine. 5. In Pediatrics, 196 boys and 166 girls were admitted. The age distribution showed that 6.4% were in infancy, 43.6% in preschool period, 30.1% in prepuberal period, 18.8% in early puberty and 1.1% in late puberty. The percentage of pediatrics inpatients has increased every year. Conclusions: More data of patients should be collated for further study. More detailed study with different kinds of diseases are expected based on this study.

Outbreaks of mumps: an observational study over two decades in a single hospital in Korea

  • Purpose: The introduction of the mumps vaccine has dramatically reduced the number of mumps cases, but outbreaks have recently occurred among highly vaccinated populations in developed countries. Epidemiological and clinical characteristics of patients with mumps admitted between 1989 and 2012 in a single hospital in Korea are described in the present study. Methods: We retrospectively evaluated inpatients with mumps between 1989 and 2012 and outpatients and inpatients with mumps in 2011-2012. Results: A total of 152 patients with mumps were admitted between 1989 and 2012, and 163 patients were recorded in 2011-2012. The highest number of admitted cases occurred in 1998 and 2012 (35 and 34 cases, respectively). Among the patients admitted in 2011-2012, the highest frequency was observed among people aged 15-19 years, and low frequency was observed in those aged <4 years and >20 years, compatible to the city data and national data. In patients admitted to our department in 1998 (35 cases) and in 2010-2012 (27 cases), there were significant differences in the mean age and the rate of secondary measles-mumps-rubella (MMR) vaccination, but had similar clinical features, including complications, except aseptic meningitis. Antimumps immunoglobulin (Ig) G was positive in 83% and 100%, and IgM was positive in 67% and 41%, respectively, in the two periods. Conclusion: In Korea, recent mumps outbreaks have occurred mainly among secondary school students who received two doses of the MMR vaccine. The vaccinees might have a modified immune reaction to viral insults, manifesting modified epidemiological and clinical features.

A Statistical Study of the Patients Admitted to the Department of Acupuncture and Moxibustion Medicine (침구의학과에 입원한 환자들에 대한 통계적 고찰)

  • Objectives : The aim of this study was to investigate clinical features of the patients admitted to the department of acupuncture and moxibustion in an oriental medicine hospital in order to provide clinical preliminary data for advancement of acupuncture and moxibustion Methods : Total 526 patients were admitted to the department of Acupuncture and Moxibustion in Dunsan Oriental Hospital of Daejeon University from September 1st, 2011 to August 31st, 2012. Of these patients 11 were discharged on the day that they were admitted and the remaining 526 patients who were hospitalized for over 2 days were investigated this study. We analyzed inpatients by discharge progress notes and medical records, using PASW statistics 18.0 for statistical analysis. Results : 1. There were 361 females(62.9%) and 213 males(37.1%). The age distribution was skewed to 40s and 50s and the average age of the patients was 42.91 years. 2. The prevalence of the diseases groups was in the order of musculoskeletal disease, neuromuscular disease, neuro-psychological disease, and EENT disease. The musculoskeletal disease and neuromuscular disease together made up 92% of all diseases. 3. The prevalence of individual diagnosis was in the order of Bell's palsy, cervical sprain, lumbar sprain, herniated lumbar disc, herniated cervical disc, and lumbar spinal stenosis. 4. There were 261 patients with single diagnosis and 193 with two and 120 with three or more diagnoses. The days of hospitalization significantly increased with the number of diseases the patients were diagnosed with. Conclusions : With high concentration of patients seen in certain disease groups, continuous efforts needed to expand the usage of acupuncture and moxibustion treatments to other disease groups and to explore and develop other diverse treatment methods for better outcome of the disease treated.

Epidemiology and Clinical Features of Respiratory Viruses in Pediatric Inpatients in a Single Medical Center in Daegu from 2010 to 2012 (대구지역 단일병원에서 입원 환아의 호흡기 바이러스 역학 및 임상 양상: 2010-2012년)

  • Background: This study was performed to investigate the epidemiologic and clinical features of acute respiratory viral infection in hospitalized children. Methods: From 2010 to 2012, we tested nasopharyngeal swab specimen in 1,584 hospitalized children with multiple real-time polymerase chain reactions to identify 10 kinds of respiratory viruses (including influenza virus A, B (FluA, FluB), respiratory syncytial virus (RSV), human metapneumovirus (MPV), adenovirus (AdV), human coronavirus (CoronaV), human enterovirus (HEV), human bocavirus (HBoV), parainfluenza virus (PIV), and human rhinovirus (Rhinovirus)). We analyzed the positive rate, annual and seasonal variations, and clinical features (respiratory tract and non-respiratory tract) according to the retrospective review of medical records. Results: Respiratory viruses were detected from 678 (42.8%) of 1,584 patients. The most common detected virus was RSV (35.0%), and then AdV (19.0%), HEV (18.1%). The critical period of the respiratory viral infection was during the first 12 months of a child's life. PIV increased by 8.4%, 12.1%, and 21.1% annually. Bronchiolitis was most frequently caused by RSV, and croup was frequently caused by PIV. The most common cause of meningitis was HEV. Hepatitis-associated respiratory virus was developed 111 in 678 cases. Conclusion: Although this study was confined to a single medical center for three years, we identified the epidemiology and clinical feature of respiratory viruses in Daegu from 2010 to 2012. Future surveillance will be necessary for annual and seasonal variations.

Clinical Investigation of Burns from Caramelized Sugar Candy (Dalgona) (달고나에 의한 화상의 임상적 특징)

  • Purpose: Dalgona, a kind of candy made of caramelized sugar, is a popular snack for children. Given the popularity of preparing dalgona, increasingly many patients are treated for burns sustained while preparing dalgona. We report the clinical features and dangers of burns from dalgona. Methods: We retrospectively reviewed the clinical records of 11 inpatients and outpatients who had been treated for burns they received while preparing dalgona from March 2020 to December 2020. The data reviewed were age, sex, the severity of the burn, the size and location of the burn, the type of treatment, and the place where the injury occurred. Results: The age of the patients ranged from 3 to 19 years, and the average age was 10.2 years (2 male, 9 female). Three patients had superficial second-degree burns, while eight had deep second-degree or third-degree burns. Most of the cases were treated with a local skin flap or skin graft. All the burned lesions were on the hands and feet. In all cases, the burns occurred at home due to accidental spillage. Conclusion: Most of the patients were children and teenagers, and they had serious burns. Therefore, we report these findings to emphasize the need for public awareness of the potential for burn injuries to occur during dalgona preparation.

Obsessive-Compulsive Symptoms in Relation to Duration of Schizophrenia (조현병 환자의 유병기간에 따른 강박증상)

  • Objectives The aim of this study was to evaluate the psychopathological features such as depression and anxiety in schizophrenics with obsessive-compulsive symptoms (OCS) as well as the severity of OCS according to duration of schizophrenia. Methods We randomly selected sixty four inpatients with schizophrenia. We classified the patients into two groups (OCS group, non-OCS group) according to the Yale-Brown Obsessive-Compulsive Scale (Y-BOCS). Clinical and demographic features were evaluated. To assess OCS, Y-BOCS were performed. The Korean version of the Positive and Negative Syndrome Scale(K-PANSS), the Brief Psychiatric Rating Scale (BPRS), the Korean version of the Calgary Depression Scale for Schizophrenia (K-CDSS), the Beck Anxiety Inventory (BAI) and the Satisfaction With Life Scale (SWLS) were conducted. Independent t-test and chi-square test were conducted to compare the two groups. Pearson correlation analysis was performed to examine the relationship between the duration of schizophrenia and the Y-BOCS score. Results The Y-BOCS, K-CDSS, and BAI scores were higher in the OCS group. There was a significant correlation between the duration of schizophrenia and the Y-BOCS score. Conclusions Anxiety and depression symptoms were severe in the OCS group. In addition, the results of this study indicate that the longer duration of schizophrenia, the more severe the OCS. Therefore, the evaluation of OCS in schizophrenics should be accompanied by treatment intervention.

A Preliminary Study for the Application of Michigan Alcoholism Screening Test-Geriatric Version in Korea (한국에서 노인용 미시간주정의존선별검사의 적용을 위한 예비연구)

  • Objectives : The alcohol dependence in elderly people has been prevalent because of increase in geriatric population. However, it is difficult to find out alcohol dependence in the aged, because they have less specific clinical features as compared with adult alcoholics. The aims of this study were to screen alcohol dependence among elderly Koreans and to know the clinical characteristics of Korean delerly alcoholics. Methods : The questionnaires translated into Korean such as Michigan Alcoholism Screening Test(MAST-K), the Brief MAST and the MAST-Geriatirc Version (MAST-KG) were used to screen alcohol dependence in the elderly alcoholic inpatients aged over 60(N=43), adult alcoholic inpatients within 20-59 Yrs of age(N=60), which were compared with age matched normal healthy aged(N=18) or adult controls(N=45). The demographic data such as sex, age, education, occupation, socioeconomic status, marital status, numbers of children, dwelling and religion as well as alcohol history such as duration of alcohol drinking, onset age, family history, impulsivity, somatic illness and motivation were also obtained to identify characteristic features of Korean aged alcoholics by structured interviews. Results : 1) The aged alcoholics had the charateristic features of more in males, lower age, low education levels, more in blue-collar workers, lower socioeconomic class, more in single, few babies, more living alone, having no religion without statistical significance. 2) The onset age of alcohol dependence was significantly higher in the aged alcoholics($45.3{\pm}13.6Yrs$) than in the adult alcoholics($27.7{\pm}8.7Yrs$)(p<0.0001). The duration of problematic alcohol drinking was significantly longer in the aged alcoholics($22.0{\pm}15.1Yrs$) than in adult alcoholics($14.2{\pm}8.4Yrs$)(p<0.01). Otherwise, there were no significant difference between aged and adult alcoholics in the family history, imulsivity, somatic illness and motivation. 3) The mean score of the MAST-K was significantly higher in the aged alcoholics($20.6{\pm}5.4$) than in the normal aged($6.7{\pm}4.4$)(p<0.0001), which was significantly lower than in the adult alcoholics($26.2{\pm}8.0$) and in normal adult controls($9.5{\pm}3.2$)(p<0.05). The mean score of the Brief MAST was significantly lower in the aged alcoholics($9.3{\pm}3.5$) than in the adult alcoholics($14.5{\pm}6.6$)(p<0.0001). The mean score of the MAST-KG was significantly higher in the aged alcoholics($10.6{\pm}3.5$) than in the normal aged($4.8{\pm}4.3$)(p<0.0001). The former was significantly lower than in the adult alcoholics($12.9{\pm}4.3$)(p<0.005), and the mean score was $4.5{\pm}2.8$ in normal adult controls. 4) The items which showed statistically significant differences between aged alcoholics and normal aged controls could be found in 10 items of the MAST-K(items 2, 3, 4, 5, 11, 14, 17, 21, 22 and 23), 2 items of the Brief MAST(items 2 and 9), and 7 items of the MAST-KG(items 6, 13, 18, 19, 22, 23 and 24)(p<0.01). Conclusions : The scores of the MAST-K, the Brief MAST and the MAST-KG were significantly lower in the aged alcoholics than those in the adult alcoholics (p<0.05). The statistically significant differences between aged alcoholics and normal aged controls could be found in 10 items of the MAST-K, 2 items of the Brief MAST and 7 items of the MAST-KG. Therefore, a briefer rating scales around 10 items are needed to screen alcohol dependence among Korean elderly people.

Clinical Characteristics of Misdiagnosed Delirium (오진된 섬망 환자의 임상적 특징)

  • Objectives : Delirium of hospitalized patients is common and it is significantly associated with increased mortality rate. Misdiagnosis rates of delirium are reported in a range of 36.7 to 63% in clinical setting. We aimed to identify the clinical features and symptomatic characteristics associated with misdiagnosis of delirium. Methods : Subjects were 256 inpatients who were referred for psychiatric consultation and diagnosed with delirium by a psychiatrist at a university hospital between January 1 and June 30, 2012. Clinical data were obtained with retrospective chart review. Patients were divided into misdiagnosed group and correct diagnosed group, after reviewing the reason which were described in the consultation request form. Results : Sixty nine(27%) subjects of the 256 patients were referred for other reasons(mood, substance, anxiety, dementia etc.) than "delirium/confusion" by clinician(misdiagnosed group). In misdiagnosed group, use of antipsychotics was more common. There were no differences between the two groups in age, gender, and department of referring clinician. Fluctuation score of DRS-R-98 was higher in the correct diagnosed group. Conclusions : In this study, the risk of misdiagnosis was higher when the patients have taken antipsychotics or less symptom fluctuation. Careful clinical attention is needed for diagnosis for delirium in these patients.

Clinical Features of Kawasaki Disease with Pyuria (농뇨 동반 가와사키병의 임상적 특징)

  • Purpose: Clinical and laboratory features of two Kawasaki disease (KD) groups were evaluated; the patient with pyuria and those without pyuria. Methods: From January 2015 to December 2016, the medical records of 140 (86 males and 54 females) inpatients with KD were retro-spectively analyzed. Results: Forty-eight KD patients (34.3%) presented with pyuria. KD patients with pyuria showed a higher level of C-reactive protein (CRP) and a higher proportion of elevated liver enzymes than those without pyuria. There were no differences in the proportions of unresponsiveness to intravenous immunoglobulin and coronary artery lesions between the two groups. Six KD patients (12.5%) with pyuria underwent a renal imaging study to rule out the possibility of a urinary tract infections. Thirty-two KD patients (66.7%) with pyuria received treatment with antibiotics in addition to the standard treatment for KD. Conclusions: KD patients with pyuria showed a higher level of CRP and elevated levels of liver enzymes than those without pyuria. These findings suggest that KD patients with pyuria have more severe systemic inflammation than those without pyuria.
