일부지역 치과위생사의 임상경력에 따른 치과 의료사고 및 분쟁 경험, 심리상태, 예방교육에 대한 인식정도 분석 (Analysis of the perception degree on dental medical accident and dispute experience, psychological status and preventive education according to clinical career of dental hygienist)

  • 연구목적: 본 연구에서는 임상에 근무하고 있는 치과위생사를 대상으로 임상경력에 따른 의료사고 및 분쟁 경험, 심리상태, 예방교육 대한 인식정도를 파악하고자 한다. 연구방법: 본 연구는 2012년 5월 1일부터 6월까지 경남 지역의 일부 치과 병·의원에 근무하는 치과위생사를 대상으로 자기기입식 설문지 총 330부를 대상으로 분석하였다. 수집된 자료는 SPSS (Statistical Package for the Social Science) ver 18.0 프로그램을 이용하여 빈도분석, 교차분석, ANOVA로 분석하였다. 연구결과: 1. 환자의 불평 및 불만으로 문제가 된 경험이 있는 경우와 치과위생사의 업무로 인한 불평 및 불만을 경험한 경우 모두 임상경력이 많은 6년 초과 군에서 각각 70.3%, 30.7%로 나타났다. 2. 환자의 불평, 불만 문제제기 되는 경우 중 진단, 치과진료기구 및 재료와 관련된 경우, 스케일링, 인상채득, 보철치료, 소아환자에서 통계적으로 유의하게 나타났으며, 전체적으로 임상경력이 높은 6년 초과 군에서 횟수가 많이 나타났다. 3. 환자의 불평 및 불만을 경험한 후의 심리상태 중 '과정은 힘들었지만 있을 수 있는 일이라 생각하고 잊었다'는 160명으로 그 중 임상경력이 6년 초과가 38.1%, 3년 미만이 37.5%으로 비슷하게 나타났고, 임상경력 3~6년은 24.4%으로 나타났으며, 통계적으로 유의한 차이를 보였다. 4. 치과위생사의 의료사고 및 분쟁 예방교육에 대한 사항에서 '진료 시 문제제기나 분쟁발생에 대한 의구심이 든다'에 '가끔 그렇다'고 답한 임상경력 3~6년 73.6%으로 높았으며, 예방교육 필요성 여부에서는 '필요하나 시급하지 않다'는 응답이 많았으며 그 중 임상경력 3년 미만이 60.0%로 가장 많았다. 의료분쟁 증가여부에서는 '예'가 많았으며 임상경력 6년 초과가 87.1%으로 가장 높았으나 통계적으로 유의하지는 않았다. 결론: 치과위생사를 대상으로 한 의료사고 및 분쟁에 대한 예방교육이 시급한 것으로 사료된다.

융복합 사회에서 응급구조(학)과 학생들의 임상실습 실태조사 및 개선방안 (Survey and Improvement Scheme for Clinical Practice of Emergency Medical Technology Students in the Convergence Society)

  • 본 연구는 응급구조(학)과 학생들의 임상실습 현실과 실태를 조사하여 임상실습 개선방안을 제시하고자 한 연구이다. 융복합 사회에서 임상실습은 다양한 직업을 경험할 권리만큼 중요한 것으로 간주되고 있고 최근 NCS(National Competency Standards)의 등장에 의해 더욱 활기를 띠고 있다. 응급구조(학)과 학생 115명이 대상이다. 자료는 2016년 6월 16-17일까지 수집하여 SPSS v. 18.0을 사용하여 분석하였다. 응급실만을 경험한 2학년은 취업에 대한 부정적 인식을 보였다. 반면 응급실과 119 구급대를 경험한 3학년은 119 구급대 임상실습의 후광효과로 취업에 대한 인식의 변화가 거의 나타나지 않았다. 두 그룹 모두 타직종에서의 다양한 임상실습 경험을 원한다. 연구 결과를 바탕으로 다양한 임상실습기관을 제공하여 취업에 대한 인식을 긍정적으로 바꾸고 다가올 1급 응급구조사의 취업난을 해결할 것을 제안한다.

신규간호사의 임상수행능력 예측요인 (Predictors of Clinical Competence in New Graduate Nurses)

  • Purpose: The purpose of this study was to identify the predictors of clinical competence in new graduate nurses. Methods: The subjects of this study were 238 nurses at 13 general hospitals who have had less than 12 months of nursing experience. The data were collected by structured questionnaire from August 5 to August 31 of 2009 and analyzed by the SPSS Win 12.0 program. Results: The total mean score for clinical competence was $181.05{\pm}15.17$, critical thinking disposition was $94.65{\pm}8.12$, and practice environment was $41.00{\pm}5.55$. There were significant differences of clinical competence according to the GPA (t=-3.58, p<.001), the number of beds in the hospital (t=-3.22, p=.001), instruction by preceptor (t=-2.32, p=.021), and previous experience of clinical practice in the hospital (t=-2.21, p=.028). Additionally, critical thinking disposition and practice environment were positively correlated to clinical competence (r=.50, p<.001; r=.20, p=.002). In multivariate approach, predictors included in this study explained 43% of variance in clinical competence. Significant predictors of clinical competence were critical thinking disposition ($\beta=.50$, p<.001), practice environment ($\beta=.14$, p=.012), and working duration ($\beta=.13$, p=.018). Conclusions: Based on these findings, it is needed that providing supportive practice environment and developing curriculum for enhancing the critical thinking disposition to improve the clinical competence in new graduate nurses.

The Effect of Clinical Dental Hygienist's Moral Sensitivity and Psychological Well-Being on Job Engagement

  • Background: The present study aimed to provide basic data to establish a foundation for efficient operation of the organization by identifying the effects of factors related to moral sensitivity and psychological well-being on job engagement of clinical dental hygienists. Methods: This cross-sectional survey consisted of self-administration questionnaires. After the approval by the Institutional Review Board, a survey was conducted from September 2019 to November 2019, targeting dental hygienists with clinical experience of 1 year or more. A total of 189 questionnaires were analyzed using SPSS 18.0 for Windows. The data were analyzed using average standard deviation, t-test, one-way analysis of variance, Scheffe test, Pearson's correlation analysis, and multiple regression analysis. Results: According to job-related characteristics, higher job engagement was associated with more amount of clinical experience, less turnover intention, higher positions, and occasional job fatigue. Job engagement was improved by increasing moral sensitivity and psychological well-being of clinical dental hygienists. The factors affecting job engagement revealed that along with clinical career, the sub-areas of job engagement, namely turnover intention, moral sensitivity, and psychological well-being had statistically significant influence on job engagement and the explanatory power of the model was approximately 44.8%. Conclusion: Joy of working as a clinical dental hygienist, clinical career, turnover intention, moral sensitivity, and psychological well-being were confirmed as influencing factors that affected job engagement of clinical dental hygienists. It is necessary to establish an effective plan to improve job engagement by increasing moral sensitivity and psychological well-being of clinical dental hygienists.

임상전단계실습 교과목의 개설시기 및 치과경험에 따른 참여태도, 스트레스, 만족도 분석 (Analysis of the relationship of the time of offering pre-clinical practicums and dental experience to participation attitude, stress and satisfaction)

  • Objectives : The purpose of this study was to examine the participation attitude of students toward pre-clinical practicums, their satisfaction with the practicums and related stress, and to investigate the influential factors for their participation attitude to practicums. Methods : The subjects in this study were three different groups of students who took pre-clinical practicums at different times. One group of students who entered college in 2008 took a practicum in December, 2009, and another group of students who entered college in 2009 took a practicum in June, 2010. The third group of students who entered college in 2010 took a practicum in June, 2011. Results : 1. As for participation attitude, the students who completed a practicum before they underwent field training in the second semester of their sophomore year got a higher mark of 3.44(p<0.05). 2 Concerning the operation of practicum, they felt a lot of stress when they took a practicum before they underwent field training in the first semester of their sophomore year and when they had no dental experience(p<0.001). 3. Regarding satisfaction with practicum, they scored higher when they took a practicum before they underwent field training in the second semester of their sophomore year(p<0.05). And the students who were more gratified with dental experience expressed better satisfaction(p<0.001). 4. As to the influential factors for the participation attitude of the students, those who were more satisfied with the content of practicum, the operation of practicum and evaluation took a more positive participation attitude(p<0.001). More stress about the operation of practicum and more stress about evaluation exerted a greater negative influence on participation attitude(p<0.05). Conclusions : Given the findings of the study, a pre-clinical practicum should be offered before field training is provided in the second semester of the sophomore year in order to ensure the efficiency of education, and a wide variety of information should be offered so that students could gain dental experience before they take a practicum.

심폐소생술금지 교육 및 임상 경험에 따른 윤리적 태도 (Ethical Attitudes according to Education and Clinical Experience of Do-Not-Resuscitate (DNR))

  • 목적: 본 연구는 심폐소생술 비시행(DNR)에 관해 간호사와 의사의 DNR에 대한 인식 및 태도가 DNR 관련 교육 및 임상경험 여부에 따른 차이가 있는지 분석하고자 하였다. 방법: 연구대상은 단일대학병원에서 근무하는 간호사와 의사로서 총 310명이었으며, 자료수집 기간은 2013년 8월 19일부터 8월 30일까지였다. 연구도구는 DNR에 관해 간호사와 의사의 임상 경험과 윤리문제에 관한 인식 및 태도를 조사하기 위해 연구자들의 문헌고찰을 통해 30문항으로 구성된 설문지를 사용하였다. 결과: 본 연구의 참여자 중에 간호사는 238명(77%), 의사는 72명(23%)이었다. 대부분의 참가자들(99%)이 DNR의 필요성에 동의하였고 이유로는 환자의 편안하고 품위 있는 죽음(52%), 회복이 불가능한 의학적 상태(23%), 환자 본인의 죽음에 대한 선택(19%)의 순이었다. DNR 임상경험에 따른 차이는 DNR 필요성의 동의, DNR 시기 및 DNR 이후의 다른 치료의 허용에 대해서 차이를 보였다. 그러나 DNR 교육 여부에 따른 차이는 DNR 시기 이외에는 유의한 차이를 보이지 못하였다. 결론: 본 연구를 통해 DNR에 대한 교육보다는 임상에서의 경험이 실제 DNR에 대한 인식 및 태도에 유의한 영향을 미치는 것을 보여주었기에 효과적인 DNR 교육을 위하여는 실제 임상 실습을 통한 교육이 필요할 것으로 생각된다.

간호대학생의 의사소통능력, 임상수행능력, 인수인계 경험이 인수인계 자기효능감에 미치는 영향 (The Effects of Communication Competence, Clinical Competence and Experience of Handover on Self-efficacy of Handover Reporting among Nursing Students)

  • 본 연구는 간호대학생의 의사소통능력, 임상수행능력, 인수인계 경험, 인수인계 자기효능감을 파악하고 인수인계 자기효능감에 영향을 미치는 요인을 규명하여 인수인계 교육프로그램을 개발하는데 기초자료를 제공하고자 수행되었다. 연구방법은 서술적 조사연구로서 G시에 소재한 4년제 2개교 간호대학생 255명을 2019년 6월부터 9월까지 구조화된 설문지를 통해 자료수집하였다. 자료분석은 t-test, ANOVA, Pearson's correlation coefficents 그리고 단계적 다중회귀분석을 활용하였다. 연구결과, 간호대학생들의 의사소통능력은 57.3점(100점 만점에 76.4점), 임상수행능력은 69.8점(100점 만점에 73.5점), 인수인계 자기효능감은 33.8점(100점 만점에 51.2점)으로 나타났다. 간호대학생의 인수인계 자기효능감은 성(F=4.60, p<.001), 연령(F=16.72, p<.001), 학년(t=-6.39, p<.001), 임상실습 만족도(F=3.68, p=.027), 인수인계 교육 경험(t=26.44, p<.001), 인수인계 경험(t=4.84, p<.001), 인수인계에 대한 두려움(F=16.97, p<.001), 환자안전에 있어서 인수인계 중요도(F=6.42, p=.002)에 따라 유의한 차이를 보였다. 간호대학생의 의사소통능력, 임상수행능력, 인수인계 자기효능감 간의 상관관계를 분석한 결과, 인수인계 자기효능감은 의사소통능력(r=.249, p<.001)과 임상수행능력(r=.426, p<.001) 간에 유의한 순 상관 관계가 있었다. 간호대학생의 인수인계 자기효능감에 영향을 미치는 요인으로 인수인계에 대한 두려움(β=-.294, p<.001), 임상수행능력(β=.252, p<.001), 학년(β=.191, p=.001), 인수인계 경험(β=.185, p<.001), 성별(β=.150, p=.003), 인수인계 교육 경험(β=.126, p=.017)순으로 모형의 설명력은 40.0%이었다. 결론적으로 간호대학생들의 인수인계 자기효능감을 높이기 위해서는 임상수행능력을 향상시키고 임상실습 시 인수인계 경험을 포함한 인수인계 교육을 제공하여 인수인계 두려움을 감소시키는 인수인계 교육프로그램의 개발이 필요하다.

편입과정을 거쳐 입사한 신규간호사의 적응경험 (Adaptation to Clinical Experience by Older Graduate Nurses who Entered Nursing after Graduation in Another Field)

  • Purpose: With the increasing opportunities for students to transfer into nursing from other departments or after graduation, it is important to examine how these students adapt to clinical work in the hospital setting after graduation. This study was done to describe the adaptation experience of older graduate nurses in hospital settings and to understand the meanings and search the nature of their experience. Method: For this study, the participants were 6 nurses who entered nursing after having graduated from college or university in another field. The data were collected through in-depth interviews from March to April 2007 and analyzed by Colaizzi's phenomenological method. Results: The essential themes of the adaptation experience for older graduate nurses were the following 6 categories: taking responsibilities for the expected role according to one's age, following the rules of senior-junior hierarchy, being ambivalent about one's older age, expanding one's support system, getting rid of the concept of age difference and establishing the expected position appropriate for one's career. Conclusion: For older graduate nurses, interpersonal relationship factors were more important than work related factors. Therefore, it is important to create a more flexible nursing culture to ease the adaptation of these older graduate nurses.

만성 폐쇄성 폐질환 환자의 증상경험, 자기효능감, 우울과 약물복용 이행 간의 관계 (Symptom Experience, Self-Efficacy, Depression, and Medication Adherence in Patients with Chronic Obstructive Pulmonary Disease)

  • Purpose: This study was done to investigate the correlation among symptom experience, self-efficacy, depression, and medication adherence in patients with chronic obstructive pulmonary disease (COPD). Methods: Participants were 100 patients with COPD recruited in one general hospital in Seoul. A structured questionnaire was used to measure the study variables. Data were analyzed using descriptive statistics, t-test, ANOVA, and Spearman correlation coefficient. Results: Symptom experience and depression were positively correlated (r=.41, p<.001), symptom experience and self-efficacy were negatively correlated (r=-.21, p=.035). Depression was negatively correlated with self-efficacy (r=-.60, p<.001) and medication adherence (r=-.48, p<.001). Self-efficacy and medication adherence were positively correlated(r=.76 p<.001). Conclusion: Findings from this study indicate that depression and self-efficacy are important variables related to medication adherence in patients with chronic obstructive pulmonary disease. Therefore, depression and self-efficacy should be assessed, and customized nursing interventions should be provided in order to increase medication adherence.

Barriers and Attitudes to Research Among Nurses in One Hospital in Korea

  • Purpose. Nursing research is recognized as an essential basis for the continuous development of the scientific nursing knowledge and practice. An understanding of the barriers and attitudes to research among nurses is important to improve clinical nursing research activities. Method. Data were collected from nurses at a general hospital, located in D city, using a self-reported questionnaire, from March 15th to April 4th, 2003, and 133 questionnaires were used for the analyses. A 5 point-scale instrument was used to measure barriers and attitudes towards nursing research, with a Cronbach's alpha of .85 and .89, respectively. Results. Nurses had limited experience in research and were not well prepared to conduct it. Lack of experience, training and time were the major barriers for conducting research. However, the nurses had positive attitudes toward research. There was a difference in barriers (t = 2.68, P = .02) and research attitudes (t =- 2.74, P = .00) according to the nurses' position. Also, the nurses' research experience influenced research attitudes (t = -3.27, P = .00). The degree of research preparation (F = 6.98, P = .00) had an effect on the attitudes toward nursing research but not on the barriers. Conclusion. There is an urgent need to promote both the ability and environment for clinical nurses to actively participate in research. As the majority of nurses had little experience and low confidence in research and perceived many barriers, this promotion can be accomplished by enhancing their confidence in research through continuing education and by gradually eliminating the barriers to research.
