The effect of supportive periodontal therapy after periodontal treatment : A 1-year follow up. (치주치료 후 유지치주치료의 효과에 관한 평가 : 1년 관찰)

  • Purpose : Preservation of the periodontal health of the treated patient requires supportive periodontal therapy for the elimination of periodontal disease. After Phase I therapy is completed, patients are placed on a schedule of periodic recall visits for maintenance care to prevent recurrence of the disease. The amount of tooth loss would be the most relevant criterion in an evaluation of the effect of periodontal treatment, but this would require studies with extremely long follow-up periods. Thus the most commonly used outcome criteria in clinical research have been clinical attachment level change, reduction of pocket depth and bleeding frequency. The purpose of this case study is to identify the effect of supportive periodontal therapy after periodontal flap surgery. Materials & Methods : Following routine hygienic phase of treatment, patients with chronic periodontitis received surgical periodontal treatment. Bleeding on probing, pocket depth and clinical attachment level were measured at baseline, pre-operation and 1 year follow up. All procedures were performed by one operator. Results : One year a total of 28 patients (58sites) to recheck remained, when conducted maintenance program after periodontal flap surgery was observed reduction of bleeding frequency, pocket depth and improvement of clinical attachment level. Conclusion : The results from this study indicate that supportive periodontal therapy after periodontal flap surgery is effective for reduction of bleeding frequency, pocket depth and gain of clinical attachment level.

The Regeneration Effect of Platelet-Rich Plasma on the Treatment of Grade II Furcation Involvement Using Synthetic Bone in Human (2급 치근이개부 병변의 합성골 이식 시 혈소판 농축 혈장의 재생에 관한 효과)

  • The present study evaluated of regeneration effect of platelet rich plasma on the treatment of grade II furcation involvement, with coralline calcium phosphate bone in humans. 30 teeth with grade II furcation defect were selected and 15 teeth(control) were treated with coralline calcium phosphate bone, the others(test) were treated with coralline calcium phosphate bone and platelet rich plasma. Pocket depth, clinical attachment level, width of keratinized gingiva width were measured at baseline, postoperative 3, 6months. from cementoenamel junction to alveolar crest and fundus were measured at baseline, 6months(re-entry). Both groups were statistically analyzed by Wilcoxon signed Ranks Test & Mann-whitney Test using SPSS program(5% significance level). The results were as follows: 1. Pocket depth, clinical attachment level, keratinized gingva width, cementoenamel junction - alveolar crest, cementoenamel junction - fundus were not differ significantly in both groups at baseline 2. The change of pocket depth, clinical attachment level, keratinized gingva width, cementoenamel junction - alveolar crest, cementoenamel junction - fundus in both groups were decreased significantly at 3, 6months(p<0.05). 3. The change of pocket depth, clinical attachment level in test group decreased significantly than control at 3, 6months(p<0.05). 4. The change of keratinized gingiva width, cementoenamel junctional - alveolar crest, cementoenamel junction - fundus were not differ significantly in both groups at 3, 6months. 5. The pocket depth, clinical attachment level, keratinized gingiva width exhibited marked changes at 3 months in both groups. In conclusion, the results of this study suggest that platelet rich plasma have adjunctive clinical treatment effect to periodontal regeneration in grade II furcation defects.

  • Regeneration of Periodontium with PRP does not only improve regeneration rate and density of bone but have a possibility to estimate faster healing process for soft tissue. And also, synthetic bone and xenogenic bone graft are effective on regeneration of periodontium. The purpose of this study is to evaluate the effectiveness of synthetic bone ($Biogran^{(R)}$) and xenogenic bone ($BBP^{(R)}$) grafts with the PRP technique on regeneration of periodontium. 52 Generally healthy Pt. who had pocket depth 5mm at any of 6 surfaces of the teeth were in the study at Dept. of Perio. in Dankook Dental Hospital. Open Flap was treated for 18 infra-bony pockets as control group, $Biogran^{(R)}$ with PRP was inserted for 25 infrabony pockets as first test group, and $BBP^{(R)}$ with PRP was inserted for 22 infrabony pockets as 2nd test group. Then evaluation was made after 3 and 6 months 1. 6 months after surgery, each difference of average probing pocket depth was $2.61{\pm}0.23$ for control, $3.40{\pm}0.30$ for 1st test, and $3.45{\pm}0.37$ for 2nd test group. 2. 6 months after surgery, each difference of clinical probing attachment level was $1.39{\pm}0.12$ for control, $2.88{\pm}0,24$ for 1st, and $2.86{\pm}0,27$ for 2nd test group. 3. 6 months after surgery, each difference of Maximal probing attachment level was $1.11{\pm}0.16$ for control, $3.28{\pm}0.30$ for 1st, and $3.27{\pm}0.35$ for 2nd test group. 4. There were significant differences for clinical change of each three group which were between average probing pocket depth and clinical attachment level of 3,6 months and minimal and maximal attachment level after 6 months 5. There were significant differences for average probing pocket depth which were only at control group and 2nd test group between 1 and 6months. For clinical attachment level and minimal and maximal proving attachment level, there was a significant difference after 6month of surgery. 6. There was no significant difference between two test groups for average probing depth, clinical attachment level, and minima1 and maximal probing attachment level. As the result, PRP with bone graft is very effective for regeneration of periodontium and there is no difference between xenogenic bone and synthetic bone.

Determining the Relationship among Organizational Commitment, Occupational Stress, and Interpersonal Relations according to Adult Attachment Styles of Clinical Dental Hygienists

  • Background: This study aimed to identify the types of adult attachment and determine the relationship between adult attachment and job factors in dental hygienists. Ultimately, it was necessary to identify the need for a secure attachment to improve the quality of clinical dental hygienist's services. Methods: Data of 454 clinical dental hygienists working in dental hospitals or clinics were collected. The research tools consisted of items related to the general and work characteristics of dental hygienists (9 items), adult attachment styles (36 items), organizational commitments (12 items), occupational stress (15 items), and interpersonal relations (18 items). Cronbach's ${\alpha}$ of each tool was ${\geq}0.7$. Results: Most of the participants had fearful attachment styles, followed by dismissing-avoidance, security, and preoccupation. Security was the highest level of organizational commitment according to the adult attachment style, although the differences of the levels were insignificant. For occupational stress, preoccupation was the highest, followed by fearful, security, and dismissing-avoidance, and the differences were significant (p<0.001). For interpersonal relations, security was the highest, followed by preoccupation, dismissing-avoidance, and fearful in order, and the differences were significant (p<0.001). Conclusion: Job stress and interpersonal relation ability according to the adult attachment style of clinical dental hygienists had significant results. Thus, the development of attachment improvement programs by personal style, development of differentiated clinical education and its application, and improvements in the adult attachment styles of clinical dental hygienists would be required rather than simply presenting the needs to collectively improvement the working environment.

A comparative study of clinical effects following treatment of class II furcations using allograft and PR with and without bioabsorbable membrane (2급 이개부 병변을 동종골과 혈소판 농축 혈장으로 치료시 차폐막 사용에 따른 임상적 효과의 비교 연구)

Effect of root planing on the reduction of probing depth and the gain of clinical attachment depending on the mode of interproximal bone resorption

  • Purpose: The purpose of the present study was to evaluate the effect of root planing on the reduction of probing pocket depth and the gain of clinical attachment depending on the pattern of bone resorption (vertical versus horizontal bone loss) in the interproximal aspect of premolar teeth that showed an initial probing pocket depth of 4-6 mm. Methods: In this study, we analyzed 68 teeth (15 from the maxilla and 53 from the mandible) from 32 patients with chronic periodontitis (17 men and 15 women; mean age, 53.6 years). The probing pocket depth and clinical attachment level at all six sites around each tooth were recorded before treatment to establish a baseline value, and then three months and six months after root planing. Results: The reduction in interdental pocket depth was 1.1 mm in teeth that experienced horizontal bone loss and 0.7 mm in teeth that experienced vertical bone loss. Interdental attachment was increased by 1.0 mm in teeth with horizontal bone loss and by 0.7 mm in teeth with vertical bone loss. The reduction of probing pocket depth and the gain of clinical attachment occurred regardless of defect patterns three and six months after root planing. Conclusions: The reduction of pocket depth and gain in the clinical attachment level were significantly larger in horizontally patterned interproximal bone defects than in vertical bone defects.

Clinical Study of Tetracycline HCl Conditioning in the Treatment of Gingival Recession. A Comparative Study (치근 피개 술식시 치근에 도포된 테트라사이클린의 효과에 관한 임상적 연구)

  • Histological studies indicate a tetracycline HCl similar to citric acid to induce connetive tissue repair in animals. When tetracycline HCl was used as a root conditioning agent in humans, there was a trend toward more connective tissue attachment than in root planing alone. The purpose of this study was to evaluate clinical effect of tetracycline HCl in the treatment of gingival recession. 44 teeth in 12 patients with bilaterally gingival recession & Miller classification I, II gingival recession were selected and 22 teeth were treated with 125mg/ml tetracycline HCl , the others was not treated with tetracycline HCl. Gingival recession, pocket depth, clinical attachment level, width of keratinized gingiva were observed at baseline, postoperative 4, 12, 20weeks. Both groups were statistically analyzed by Wilcoxon signed Ranks Test & Wilcoxon's rank sum test(Mann-whitney test) using SPSS program.(5% significance level) The results were as follows: 1........The change of gingival recession, clinical attachment level, keratinized gingiva in both groups were increased significantly at 4, 12, 20 weeks. 2.......The pocket depth exhibited no marked changes throughout the entire investigation in both groups. 3........The change of gingival recession in tetracycline group was increased significantly than control group at 4, 12, 20 weeks and the percentage of root coverage was 93% in tetracycline group and 83% in control group. 4........The change of clinical attachment level, pocket depth, keratinized tissue from baseline to 4, 12, 20 weeks was not differ significanltly in both group.

  • Periodontitis is the most prevalent noncontagious disease. Periodontal probing for measuring probing depth (PD) and clinical attachment level (CAL) has been an important diagnostic tool to determine the presence and severity of periodontal diseases. For probing reproducibility, adroit probe handling with constant force is required.

The comparison of clinical changes during maintenance phase after non-surgical or surgical therapy of chronic periodontitis (만성 치주염에서 비외과적 또는 외과적 치주치료 후 유지관리기 동안 임상적 변화의 비교)

  • Reports on the comparison of clinical effect between non-surgical and surgical therapy, and the change of the clinical parameters during maintenance phase have been rarely presented in Korea. This study was to observe the clinical changes during maintenance phase of 6 months in patients with chronic periodontitis treated by non-surgical or surgical therapy in Department of Periodontics, Chonnam National University Hospital. Among the systemically healthy and non-smoking patients with moderate to severe chronic periodontitis, twenty eight patients (mean age: 47.5 years) treated by non-surgical therapy (scaling and root planning) and nineteen patients (mean age: 47.3 years) treated by surgical therapy (flap surgery) were included in this study. The periodontal supportive therapy including recall check and oral hygiene reinforcement was started as maintenance phase since 1 month of healing after treatment. Probing depth, gingival recession. clinical attachment level and tooth mobility were recorded at initial, baseline and 1, 2, 3 and 6 month of maintenance phase. The clinical parameters were compared between the non-surgical and surgical therapies using Student t-test and repeated measure ANOVA by initial probing depth and surfaces. Surgical therapy resulted in greater change in clinical parameters than non-surgical therapy. During the maintenance phase of 6 months, the clinical effects after treatment had been changed in different pattern according to initial probing depth and tooth surface. During maintenance phase, probing depth increased more and gingival recession increased less after surgical therapy, compared to non-surgical therapy. The sites of initial probing depth less than 3 mm lost more clinical attachment level, and the sites of initial probing depth more than 7 mm gained clinical attachment level during maintenance phase after non-surgical therapy, compared to surgical therapy. Non-surgical therapy resulted in greater reduction of tooth mobility than surgical therapy during maintenance phase. These results indicate that the clinical effects of non-surgical or surgical therapy may be different and may change during the maintenance phase.
