간호학 교과과정 개선을 위한 조사 연구 (A Study on improvement of curriculum in Nursing)

  • This Study involved the development of a survey form and the collection of data in an effort-to provide information which can be used in the improvement of nursing curricula. The data examined were the kinds courses currently being taught in the curricula of nursing education institutions throughout Korea, credits required for course completion, and year in-which courses are taken. For the purposes of this study, curricula were classified into college, nursing school and vocational school categories. Courses were directed into the 3 major categories of general education courses, supporting science courses and professional education course, and further subdirector as. follows: 1) General education (following the classification of Philip H. phoenix): a) Symbolics, b) Empirics, c) Aesthetics. 4) Synthetics, e) Ethics, f) Synoptic. 2) Supporting science: a) physical science, b) biological science, c) social science, d) behavioral science, e) Health science, f) Educations 3) Professional Education; a) basic courses, b) courses in each of the respective fields of nursing. Ⅰ. General Education aimed at developing the individual as a person and as a member of society is relatively strong in college curricula compared with the other two. a) Courses included in the category of symbolics included Korean language, English, German. Chines. Mathematics. Statics: Economics and Computer most college curricula included 20 credits. of courses in this sub-category, while nursing schools required 12 credits and vocational school 10 units. English ordinarily receives particularly heavy emphasis. b) Research methodology, Domestic affair and women & courtney was included under the category of empirics in the college curricula, nursing and vocational school do not offer this at all. c) Courses classified under aesthetics were physical education, drill, music, recreation and fine arts. Most college curricula had 4 credits in these areas, nursing school provided for 2 credits, and most vocational schools offered 10 units. d) Synoptic included leadership, interpersonal relationship, and communications, Most schools did not offer courses of this nature. e) The category of ethics included citizenship. 2 credits are provided in college curricula, while vocational schools require 4 units. Nursing schools do not offer these courses. f) Courses included under synoptic were Korean history, cultural history, philosophy, Logics, and religion. Most college curricular 5 credits in these areas, nursing schools 4 credits. and vocational schools 2 units. g) Only physical education was given every Year in college curricula and only English was given in nursing schools and vocational schools in every of the curriculum. Most of the other courses were given during the first year of the curriculum. Ⅱ. Supporting science courses are fundamental to the practice and application of nursing theory. a) Physical science course include physics, chemistry and natural science. most colleges and nursing schools provided for 2 credits of physical science courses in their curricula, while most vocational schools did not offer t me. b) Courses included under biological science were anatomy, physiologic, biology and biochemistry. Most college curricula provided for 15 credits of biological science, nursing schools for the most part provided for 11 credits, and most vocational schools provided for 8 units. c) Courses included under social science were sociology and anthropology. Most colleges provided for 1 credit in courses of this category, which most nursing schools provided for 2 creates Most vocational school did not provide courses of this type. d) Courses included under behavioral science were general and clinical psychology, developmental psychology. mental hygiene and guidance. Most schools did not provide for these courses. e) Courses included under health science included pharmacy and pharmacology, microbiology, pathology, nutrition and dietetics, parasitology, and Chinese medicine. Most college curricula provided for 11 credits, while most nursing schools provide for 12 credits, most part provided 20 units of medical courses. f) Courses included under education included educational psychology, principles of education, philosophy of education, history of education, social education, educational evaluation, educational curricula, class management, guidance techniques and school & community. Host college softer 3 credits in courses in this category, while nursing schools provide 8 credits and vocational schools provide for 6 units, 50% of the colleges prepare these students to qualify as regular teachers of the second level, while 91% of the nursing schools and 60% of the vocational schools prepare their of the vocational schools prepare their students to qualify as school nurse. g) The majority of colleges start supporting science courses in the first year and complete them by the second year. Nursing schools and vocational schools usually complete them in the first year. Ⅲ. Professional Education courses are designed to develop professional nursing knowledge, attitudes and skills in the students. a) Basic courses include social nursing, nursing ethics, history of nursing professional control, nursing administration, social medicine, social welfare, introductory nursing, advanced nursing, medical regulations, efficient nursing, nursing english and basic nursing, College curricula devoted 13 credits to these subjects, nursing schools 14 credits, and vocational schools 26 units indicating a severe difference in the scope of education provided. b) There was noticeable tendency for the colleges to take a unified approach to the branches of nursing. 60% of the schools had courses in public health nursing, 80% in pediatric nursing, 60% in obstetric nursing, 90% in psychiatric nursing and 80% in medical-surgical nursing. The greatest number of schools provided 48 crudites in all of these fields combined. in most of the nursing schools, 52 credits were provided for courses divided according to disease. in the vocational schools, unified courses are provided in public health nursing, child nursing, maternal nursing, psychiatric nursing and adult nursing. In addition, one unit is provided for one hour a week of practice. The total number of units provided in the greatest number of vocational schools is thus Ⅲ units double the number provided in nursing schools and colleges. c) In th leges, the second year is devoted mainly to basic nursing courses, while the third and fourth years are used for advanced nursing courses. In nursing schools and vocational schools, the first year deals primarily with basic nursing and the second and third years are used to cover advanced nursing courses. The study yielded the following conclusions. 1. Instructional goals should be established for each courses in line with the idea of nursing, and curriculum improvements should be made accordingly. 2. Course that fall under the synthetics category should be strengthened and ways should be sought to develop the ability to cooperate with those who work for human welfare and health. 3. The ability to solve problems on the basis of scientific principles and knowledge and understanding of man society should be fostered through a strengthening of courses dealing with physical sciences, social sciences and behavioral sciences and redistribution of courses emphasizing biological and health sciences. 4. There should be more balanced curricula with less emphasis on courses in the major There is a need to establish courses necessary for the individual nurse by doing away with courses centered around specific diseases and combining them in unified courses. In addition it is possible to develop skill in dealing with people by using the social setting in comprehensive training. The most efficient ratio of the study experience should be studied to provide more effective, interesting education Elective course should be initiated to insure a man flexible, responsive educational program. 5. The curriculum stipulated in the education law should be examined.

기계 호흡 중 불안정한 호흡역학을 보인 환자에서 압력조절용적조정양식(Pressure-regulated Volume Control Mode)의 효용 (The Usefulness of Pressure-regulated Volume Control(PRVC) Mode in Mechanically Ventilated Patients with Unstable Respiratory Mechanics)

  • 연구배경 : 일정한 분시 유지하면서도 기도압의 하나인 압력조절용적조정양식(PRVC)의 입상적 유용성에 대한 보고는 없었다. 본 연구는 호흡역학이 불안정한 환자에서 PRVC 양식이 원래의 개발 목적대로 압력 조절 양식에 비해 일호흡량을 일정하게 유지하면서 용적조절양식에 비해 기도압을 낮게 유지하는지를 관찰하였다. 연구방법 : 기계 호흡 중이며 활력징후의 안정을 보이는 환자 중 15분간 관찰 결과 VCV에서 최고 홉기압의 변이계수가 10이상으로 최고 홉기압의 변화가 심하거나, PCV에서 상시 호흡량의 변이계수가 10이상으로 일 호흡량의 변화가 심한 환자 8 명(남 : 녀=6 : 2, $56{\pm}26$세)를 대상으로 하여 일호흡량을 비슷한 유지하도록 VCV, PCV, PRVC 양식을 설정한 후 각 환기양식에서 15분간 일호흡량과 최고 흡기압을 관찰하였다. 결 과 : 1) 평균 일호흡량은 VCV에서 $431{\pm}102ml$, PCV에서 $417{\pm}99ml$, PRVC에서 $414{\pm}97ml$였으며 각 환기양식 간에 차이가 없었다. 2) 상시 호흡량의 변이계수는 VCV에서 $5.2{\pm}3.9%$, PCV에서 $15.2{\pm}7.5%$ PRVC에서 $19.3{\pm}10.0%$로 PCV와 PRVC에서 상시 호흡량의 변화가 심하였다(p<0.01). 3) 평균 최고 홉기압은 VCV에서 $31.0{\pm}6.9cm$ $H_2O$, PCV에서 $26.0{\pm}6.5cm$ $H_2O$, PRVC에서 $27.0{\pm}6.4cm$ $H_2O$로 PCV와 PRVC의 최고 흡기압의 VCV의 경우보다 낮게 유지되었다(p<0.01). 4) 최고 홉기압의 변이계수는 VCV에서 $13.9{\pm}3.7%$, PCV에서 $4.9{\pm}2.6%$, PRVC에서 $12.2{\pm}7.0%$로 VCV와 PRVC에서 최고 홉기압의 변동이 심하였다(p<0.01). 결 론 : PRVC 양식은 호흡역학이 불안정한 환자에서는 VCV에 비해 평균 최고 홉기압은 낮게 유지하였으나 최고 흡기압의 변동이 심하였고, PCV에 비해 상시 호흡량의 변동이 심하여 이중조절환기양식 본래의 목적을 달성하기 어려울 것으로 사료된다.

폐상피세포에서 Triptolide에 의한 NF-${\kappa}B$ 의존성 IL-8 유전자 전사활성 억제기전 (Triptolide-induced Transrepression of IL-8 NF-${\kappa}B$ in Lung Epithelial Cells)

  • 연구배경 : 폐상피세포가 능동적으로 IL-8을 분비한다는 것은 주지의 사실이다. NF-${\kappa}B$는 IL-8 발현 조절에 있어서 가장 중요한 역할을 담당하는 전사인자이다. Triptolide는 최근 밝혀진 NF-${\kappa}B$ 억제제로서 중국한약제인 뇌공등 (Tripterygium Wilfordii)에서 추출된약제이다. 연자들은 새로운 NF-${\kappa}B$ 억제제인 triptolide가 폐상피세포에서 NF-${\kappa}B$ 의존성 IL-8 유전자의triptolide가 염증성 폐질환에서 새로운 치료제로서의 가능성을 확인하기 위하여 본 연구를 시행하였다. 방법 : 폐상피세포로서 A549 사용하였고 triptolide는 미국의 Pharamagenesis(Palo Alto, CA)사로부터 제공받았다. NF-${\kappa}B$ 활성유도물질로는 IL-$1{\beta}$(R&D)와 PMA(Sigma)를 이용하였다. IL-8 유전자의 발현은 RT-PCR과 ELISA를 이용하여 측정 하였다. NF-${\kappa}B$의존성 IL-8 유전자의 전사활성을 평가하기 위하여는 IL-8 NF-${\kappa}B$ luciferase construct를 안정적으로 유전자주입한 A549 IL-8 NF-${\kappa}B$ luciferase 세포주를 제조해 사용하였고 NF-${\kappa}B$ DNA 결합은 electromobility shift assay(EMSA)를 이용하였다. p65 전사활성을 assay하기 위해서는 Gal4-p65 fusion protein expression system을 유전자주입과 luciferase assay를 통하여 시행하였다. Transcriptional coactivator의 역할을 규명 하가 위하여서는 CBP(CREB-binding protein)와 SRC-1(steroid receptor coactivator-1) 발현 벡터를 유전자 주입하고 luciferase assay를 이용하여 확인하였다. 결과 : Luciferase assay로 triptolide가 PMA와 IL-$1{\beta}$자극에 의한 IL-8 NF-${\kappa}B$ 활성을 의미있게 감소시킴을 확인하였다. IL-8 ELISA와 RT-PCR로 triptolide가 PMA와 IL-$1{\beta}$ 자극에 의해 유도되는 IL-8 발현을 각각 단백질과 mRNA 수준에서 억제함을 관찰하였다. Triptolide가 PMA와 IL-$1{\beta}$에 의한 IL-8 NF-${\kappa}B$의 전사활성을 억제시킨 반면 EMSA와 $I{\kappa}B{\alpha}$ Western blot을 이용한 실험에서는 triptolide가 NF-${\kappa}B$ DNA 결합과 $I{\kappa}B{\alpha}$의 분해에 전혀 영향을 미치지 못함을 확인하였다. 이러한 전사활성 억제와 DNA 결합 간의 불일치의 원인으로서는 DNA 결합 이후에 발생하는 핵내 에서의 transactivation에 triptolide가 영향을 미치리라고 생각되어 p65 transactivation study를 Gal4-p65T A(p65의 transactivation domain) fusion protein 발현 시스템과 luciferase assay를 이용하여 시행한 결과 triptolide가 p65 transactivation을 억제함으로써 NF-${\kappa}B$를 억제함을 확인하였다. 그러나 CBP나 SRC-1과 같은 coactivator의 역할을 규명하기 위한 유전자주입 실험에서 triptolide에 의한 p65 transactivation 억제에 대해 CBP나 SRC-1의 과발현이 별다른 영향을 미치지 못하였다. 결론 : Triptolide는 폐상피세포에서 NF-${\kappa}B$ 의존성 IL-8 유전자의 전사활성을 억제하고 그 기전은 $I{\kappa}B{\alpha}$ 경로가 아닌 핵내에서의 p65 transactivation 억제에 의해 발생하며 이에는 CBP나 SRC-1과 같은 coactivator가 관여하지 않음을 확인하였다.

Thyroglobulin 검사의 반응조건에 따른 결과 비교 분석 (Comparison of Results According to Reaction Conditions of Thyroglobulin Test)

  • Thyroglobulin (Tg)은 갑상선에서 특이적으로 생성되는 요오드단백질로써 분화갑상선 암(Differentiate Thyroid Carcinoma, DTC) 의 생물학적 표지자이다. 본 검사실의 Tg 검사법은 반응시간이 길어 진료과와 환자들의 결과보고 요청사항을 만족시키지 못하고 있는 상황이다. 이를 만족시키기 위해 Tg 검사의 반응시간을 단축하고자 현재 사용중인 B사 Kit와 타 병원에서 사용중인 C사 Kit의 반응시간을 단축시켜 실험을 진행하였다. 본원에 의뢰된 Tg 약 65명의 환자 검체를 대상으로 B사 Kit의 반응시간을 $37^{\circ}C$ shaking 2hr/2hr, 실온 shaking 3hr/2hr, $37^{\circ}C$ shaking 1hr/1hr으로, C사 Kit의 원법인 실온 shaking 3hr법과 반응시간을 단축한 실온 shaking 2hr, $37^{\circ}C$ shaking 2hr 방법으로 단축하여 기존에 시행하던 overnight법과 상관성검사를 실시하여 비교하였다. 그 결과 여섯가지 방법 모두 1.0 ng/mL 이상의 값에서는 높은 상관관계를 보였으나 1.0 ng/mL 이하의 값에서는 다소 흔들리는 경향이 있었다. 하지만 그 중 상대적으로 안정적이었던 B사 Kit $37^{\circ}C$ shaking 1hr/1hr 법과 C사 Kit 실온 shaking 2hr 법에 대해 다른 조건들을 재설정하여 적합한 방법을 찾는다면 진료과와 환자들의 요구 사항 충족은 물론, EIA법과의 경쟁력 면에서도 유용할 것이다.

식도암 세기조절방사선치료와 용적세기조절회전치료에 대한 Jaw-Tracking의 유용성 평가 (Effectiveness Assessment on Jaw-Tracking in Intensity Modulated Radiation Therapy and Volumetric Modulated Arc Therapy for Esophageal Cancer)

  • 목 적: 식도암 방사선치료 시 세기조절방사선치료(Intensity Modulated Radiation Therapy, IMRT) 및 용적세기조절회전치료(Volumetric Modulated Arc Therapy, VMAT)에서 Jaw-Tracking 기법 유 무에 따라 저선량 영역에 대한 주변 정상장기의 용적선량을 분석하여 그 유용성을 평가하고자 한다. 대상 및 방법: 본 원에서 사용하고 있는 선형가속기 VitalBeamTM(Varian Medical System, U.S.A)으로 식도암 방사선치료를 받은 27명을 대상으로 하였으며, 치료계획은 Eclipse(Ver. 13.6 Varian, U.S.A)를 이용하여 Jaw-Tracking(JT)을 사용한 치료계획과 Non Jaw-Tracking(NJT) 치료계획을 수립하였으며, 치료계획용적(Planning Target Volume, PTV)에 빗장위림프절(Supraclavicular Lymph Nodes, SCL)이 포함되어 있는 T자형 PTV를 가진 환자를 대상으로 하였다. 조사범위에 대한 영향을 확인하기 위해 복강(Celiac) 포함 여부로 비교군을 나누었다. 수립된 치료계획의 비교를 위해 손상위험장기는 양측 폐, 심장, 척수를 비교하였으며 Conformity Index(CI), Homogeneity Index(HI)를 비교하였다. 임상적용 검증을 위해 전자포탈영상장치(Electronic Portal Imaging Device, EPID)를 이용하여 Portal Dosimetry를 실시하였고, 선량 영역의 임계치(Threshold)를 10 %, 5 %, 0 %로 매개변수로 설정하여 감마분석을 실시하였다. 결 과: 모든 치료계획은 3 mm / 3 %, 감마통과율 95 % 기준에 대해 Threshold 10 %의 경우 95 % 이상으로 JT, NJT 모두 통과하였으며, IMRT는 Threshold가 5 %, 0 %로 줄어들수록 JT보다 NJT의 값이 1 % 이상 줄어 들었다. IMRT에서 양측 폐의 $V_5$$V_{10}$은 JT에서 Celiac을 포함하지 않을 때 최대 14.7 %, 평균 8.5 %, 5.3 % 만큼 감소했고, $D_{mean}$$72.3{\pm}51cGy$ 감소하였으며, Celiac을 포함할 때 JT에서 선량감소가 증가하였다. 심장의 $D_{mean}$$68.9{\pm}38.5cGy$, 척수의 $D_{max}$$39.7{\pm]30.1cGy$만큼 감소하였다. VMAT은 JT기법 사용 시 폐에서 $V_5$ 평균 2.5 % 감소하였고, 심장 및 척수에서 소량 감소하였으며, Celiac 포함 시 JT의 선량감소가 증가하였다. 결 론: 식도암 치료계획에서 IMRT가 JT 사용 시 양측 폐의 $V_5$, $V_{10}$에서 유의미한 감소가 나타났고, 저 선량영역에서 조사범위가 클수록 선량감소가 크게 나타났다. 따라서 식도암 방사선치료에는 IMRT가 VMAT보다 JT 적용 시 더 효과적이며, 저 선량영역에서의 MLC 누설 및 투과선량으로부터 정상장기를 보호할 수 있다.

폐암환자에서 혈청 CEA, SCC, Cyfra21-1, TPA-M 측정의 의의 (Clinical Application of Serum CEA, SCC, Cyfra21-1, and TPA in Lung Cancer)

  • 연구배경 : 종양 표지자는 각종 악성 종양의 진단, 진행정도, 절제 가능성, 예후추정, 치료후 추적 검사 등에도 사용될 수 있으나 민감도와 특이도가 만족스럽지 못한 경우가 많아서 임상에서 보조적인 수단으로 이용되고 있는 경우가 대부분이다. 최근 Cyfra21-1이 비소세포 폐암의 진단에 CEA, SCC보다 우수하다는 보고가 있었고 단일클론 항체를 이용한 TPA-M(Tissue polypeptide antigen)이 비소세포 폐암은 진단하는데 상당히 우수한 결과를 보인다고 보고하고 있다. 이에 폐암환자에서 혈청 CEA, SCC, Cyfra21-1, TPA-M의 임상적 유용성을 서로 비교해 보고자 이 연구를 시행하였다. 방 법 : 1996년 4월 1일 부터 19961건 8월 31일까지 계명대학교 의과대학 병원에 내원한 환자중 조직 생검으로 확진된 비소세포 폐암 37예와 소세포 폐암 12예를 총 49예를 폐암 환자군으로 같은 기간의 입원 환자 중 양성 양성 폐질환으로 확진된 29예를 대조군으로 선정하였고 폐암 환자의 경우 수술, 항암요법 및 방사선치료를 전혀 시행하지 않은 상태에서 혈청을 채취하여 검사를 시행하였다. CEA는 일본 Eiken사의 Ab bead CEA, SCC는 미국 DAINABOT사의 SCC RIA BEAD kit, Cyfra21-1은 일본 TFB사의 CA 21-1 kit, TPA-M은 일본 DAIICHI사의 TPA-M kit를 사용하였다. 결 과 : 전체 폐암환자와 대조군을 비교해보면 CEA가 $10.05{\pm}38.39{\mu}/L$, $1.59{\pm}0.94{\mu}/L$, SCC가 $3.04{\pm}5.79{\mu}/L$, $1.58{\pm}2.85{\mu}/L$, Cyfra21-1이 $8.27{\pm}11.96{\mu}/L$, $1.77{\pm}2.72{\mu}/L$, TPA-M이 $132.02{\pm}209.35U/L$, $45.86{\pm}75.86U/L$으로 나타났으며 Cyfra21-1과 TPA-M은 폐암환자에서 대조군보다 종양 표지자의 측정치가 유의하게 높았다.(p<0.05) CEA, Cyfra21-1, TPA-M, SCC의 cutoff value를 회사에서 권장하는 $2.5{\mu}/L$, $3.0{\mu}/L$, 70.0U/L, $2.0{\mu}/L$로 하였을 때 전체폐암에 대한 민감도와 특이도는 CEA 33.3%와 78.6%, Cyfra21-1 50.0%와 89.7%, TPA-M 52.3%와 89.7%, SCC 23.8%와 89.3%이었으며 비소세포 폐암에 대하여서는 CEA 36.1%와 78.1%, Cyfra21-1 50.1%와 89.7%, TPA-M 53.1%와 89.7%, SCC 33.8%와 89.3% 소세포 폐암의 경우에도 CEA 25.0%와 78.5%, Cyfra21-1 50.0%와 89.6%, TPA-M 50.0%와 89.6%, SCC 0%와 89.2%로 폐암환자에서 Cyfra21-1과 TPA-M이 민감도와 특이도에 있어 더 우수한 결과를 보였고 비소세포 폐암환자에서 SCC도 높은 특이도를 나타내었다. ROC(Receiver operating characteristic) curve에 따라 민감도와 특이도를 얻어 가장 정확도가 높은 cutoff value를 CEA $1.25{\mu}/L$, Cyfra21-1 $1.5{\mu}/L$, TPA-M 35U/L, SCC $0.6{\mu}/L$로 하여 민감도와 특이도, 정확도 및 kappa index를 비교해 보아도 Cyfra21-1과 TPA-M이 더 우수한 결과를 나타내었다. 병기에 따른 비소세포 폐암에서의 종양 표지자를 3A 병기까지와 3B 병기이상으로 나누어 비교해보면 3B 병기이상에서 SCC에서만 통계적 유의성을 보였다. (p<0.05) 그러나 종양크기에 따른 종양표지자의 측정치는 통제적 유의성이 없었다.(p>0.05) 결 론 : 혈청 TPA-M과 Cyfra21-1은 폐암환자에 있어 CEA나 SCC보다 민감도와 특이도가 높으며 SCC의 경우 비소세포 폐암에 있어 높은 특이도를 보였으며 SCC의 측정치가 병기와 유관하게 나타났으나 종양의 크기와 병기의 진행과의 관계에 있어서는 더 많은 환자들을 대상으로 한 연구가 행해져야 할 것으로 생각된다.

  • 1. The 'Kao Zheng Pai(考證派) comes from the 'Zhe Zhong Pai' and is a school that is influenced by the confucianism of the Qing dynasty. In Japan Inoue Kinga(井上金娥), Yoshida Koton(吉田篁墩) became central members, and the rise of the methodology of historical research(考證學) influenced the members of the 'Zhe Zhong Pai', and the trend of historical research changed from confucianism to medicine, making a school of medicine based on the study of texts and proving that the classics were right. 2. Based on the function of 'Nei Qu Li '(內驅力) the 'Kao Zheng Pai', in the spirit of 'use confucianism as the base', researched letters, meanings and historical origins. Because they were influenced by the methodology of historical research(考證學) of the Qing era, they valued the evidential research of classic texts, and there was even one branch that did only historical research, the 'Rue Xue Kao Zheng Pai'(儒學考證派). Also, the 'Yi Xue Kao Zheng Pai'(醫學考證派) appeared by the influence of Yoshida Kouton and Kariya Ekisai(狩谷掖齋). 3. In the 'Kao Zheng Pai(考證派)'s theories and views the 'Yi Xue Kao Zheng Pai' did not look at medical scriptures like the "Huang Di Nei Jing"("黃帝內經") and did not do research on 'medical' related areas like acupuncture, the meridian and medicinal herbs. Since they were doctors that used medicine, they naturally were based on 'formulas'(方劑) and since their thoughts were based on the historical ideologies, they valued the "Shang Han Ja Bing Lun" which was revered as the 'ancestor of all formulas'(衆方之祖). 4. The lives of the important doctors of the 'Kao Zheng Pai' Meguro Dotaku(目黑道琢) Yamada Seichin(山田正珍), Yamada Kyoko(山田業廣), Mori Ritsi(森立之) Kitamura Naohara(喜多村直寬) are as follows. 1) Meguro Dotaku(目黑道琢 1739${\sim}$1798) was born of lowly descent but, using his intelligence and knowledge, became a professor as a Shi Jing Yi(市井醫) and as a professor for 34 years at Ji Shou Guan mastered the "Huang Di Nei Jing" after giving over 300 lectures. Since his pupil, Isawara Ken taught the Lan Men Wu Zhe(蘭門五哲) and Shibue Chusai, Mori Ritsi(森立之), Okanishi Gentei(岡西玄亭), Kiyokawa Gendoh(淸川玄道) and Yamada Kyoko(山田業廣), Meguro Dotaku is considered the founder of the 'Yi Xue Kao Zheng Pai'. 2) The family of Yamada Seichin(山田正珍 1749${\sim}$1787) had been medical officials in the Makufu(幕府) and the many books that his ancestors had left were the base of his art. Seichin learned from Shan Ben Bei Shan(山本北山), a 'Zhe Zhong Pai' scholar, and put his efforts into learning, teaching and researching the "Shang Han Lun"("傷寒論"). Living in a time between 'Gu Fang Pai'(古方派) member Nakanishi Goretada(中西惟忠) and 'Kao Zheng Pai' member Taki Motohiro(多紀元簡), he wrote 11 books, 2 of which express his thoughts and research clearly, the "Shang Han Lun Ji Cheng"("傷寒論集成") and "Shang Han Kao"("傷寒考"). His comparison of the 'six meridians'(3 yin, 3 yang) between the "Shang Han Lun" and the "Su Wen Re Lun"("素問 熱論) and his acknowledgement of the need and rationality of the concept of Yin-Yang and Deficient-Replete distinguishes him from the other 'Gu Fang Pai'. Also, his dissertation of the need for the concept doesn't use the theories of latter schools but uses the theory of the "Shang Han Lun" itself. He even researched the historical parts, such as terms like 'Shen Nong Chang Bai Cao'(神農嘗百草) and 'Cheng Qi Tang'(承氣湯) 3) The ancestor of Yamada Kyoko(山田業廣) was a court physician, and learned confucianism from Kao Zheng Pai 's Ashikawa Genan(朝川善庵) and medicine from Isawa Ranken and Taki Motokata(多紀元堅), and the secret to smallpox from Ikeda Keisui(池田京水). He also became the first owner of the Wen Zhi She(溫知社), whose main purpose was the revival of kampo, and launched the monthly magazine Wen Zi Yi Tan(溫知醫談). He also diagnosed and prescribed for the prince Ming Gong(明宮). He usually analyzed the 'symptoms' of diseases but the composition, measurement, processing and application of medicine were all in the spectrum of 'analystic research' and 'researching analysis'. 4) The ancestors of Mori Rits(森立之 1807${\sim}$ 1885) were warriors but he became a doctor by the will of his mother, and he learned from Shibue Chosai(澁江抽齋) and Isawaran Ken and later became a pupil of Shou Gu Yi Zhai, a historical research scholar. He then became a lecturer of medical herbs at the Yi Xue Guan, and later participated in the proofreading of "Yi Xin Fang"("醫心方") and with Chosai compiled the "Jing Ji Fang Gu Zhi"("神農本草經"). He visited the Chinese scholar Yang Shou Jing(楊守敬) in 1881 and exchanged books and ideas. Of his works, there are the collections(輯複本) of "Shen Nong Ben Cao Jing"(神農本草經) and "You Xiang Yi Hwa"("遊相醫話") and the records, notes, poems, and diaries such as "Zhi Yuan Man Lu"("枳園漫錄") and "Zhi Yuan Sui Bi"("枳園隨筆") that were not published. His thoughts were that in restoring the "Shen Nong Ben Cao Jing", "the herb to the doctor is like the "Shuo Wen Jie Zi"("說文解字") to the scholar", and he tried to restore the ancient herbal text using knowledge of medicine and investigation(考據). Also with Chosai he compiled the "Jing Ji Fang Gu Zhi"("經籍訪古志") using knowledge of ancient text. Ritzi left works on pure investigation, paid much attention to social problems, and through 12 years of poverty treated all people and animals in all branches of medicine, so he is called a 'half confucianist half doctor'(半儒半醫). 5) Kitamurana Ohira(喜多村直寬 1804${\sim}$1876) learned scriptures and ancient texts from confucian scholar Asaka Gonsai, and learned medicine from his father Huai Yaun(槐園). He became a teacher in the Yi Xue Guan in his middle ages, and to repay his country, he printed 266 volumes of "Yi Fang Lei Ju("醫方類聚") and 1000 volumes of "Tai Ping Yu Lan"("太平禦覽") and devoted it to his country to be spread. His works are about 40 volumes including "Jin Qui Yao Lue Shu Yi" and "Lao Yi Zhi Yan" but most of them are researches on the "Shang Han Za Bing Lun". In his "Shang Han Lun Shu Yi"("傷寒論疏義") he shows the concept of the six meridians through the Yin-Yang, Superficial or internal, cold or hot, deficient or replete state of diseases, but did not match the names with the six meridians of the meridian theory, and this has something in common with the research based on the confucianism of Song(宋儒). In clinical treatment he was positive toward old and new methods and also the experience of civilians, but was negative toward western medicine. 6) The ancestor of the Taki family Tanbano Yasuyori(丹波康賴 912-955) became a Yi Bo Shi(醫博士) by his medical skills and compiled the "Yi Xin Fang"("醫心方"). His first son Tanbano Shigeaki(丹波重明) inherited the Shi Yao Yuan(施藥院) and the third son Tanbano Masatada(丹波雅忠) inherited the Dian You Tou(典藥頭). Masatada's descendents succeeded him for 25 generations until the family name was changed to Jin Bao(金保) and five generations later it was changed again to Duo Ji(多紀). The research scholar Taki Motohiro was in the third generation after the last name was changed to Taki, and his family kept an important part in the line of medical officers in Japan. Taki Motohiro(多紀元簡 1755-1810) was a teacher in the Yi Xue Guan where his father was residing, and became the physician for the general Jia Qi(家齊). He had a short temper and was not good at getting on in the world, and went against the will of the king and was banished from Ao Yi Shi(奧醫師). His most famous works, the "Shang Han Lun Ji Yi" and "Jin Qui Yao Lue Ji Yi" are the work of 20 years of collecting the theories of many schools and discussing, and is one of the most famous books on the "Shang Han Lun" in Japan. "Yi Sheng" is a collection of essays on research. Also there are the "Su Wen Shi"("素問識"), "Ling Shu Shi"("靈樞識"), and the "Guan lu Fang Yao Bu"("觀聚方要補"). Taki Motohiro(多紀元簡)'s position was succeeded by his third son Yuan Yin(元胤 1789-1827), and his works include works of research such as "Nan Jing Shu Jeng"("難經疏證"), "Ti Ya"("體雅"), "Yao Ya"("藥雅"), "Ji Ya"("疾雅"), "Ming Yi Gong An"("名醫公案"), and "Yi Ji Kao"("醫籍考"). The "Yi Ji Kao" is 80 volumes in length and lists about 3000 books on medicine in China before the Qing Dao Guang(道光), and under each title are the origin, number of volumes, state of existence, and, if possible, the preface, Ba Yu(跋語) and biography of the author. The younger sibling of Yuan Yin(元胤 1789-1827), Yuan Jian(元堅 1795-1857) expounded ancient writings at the Yi Xue Guan only after he reached middle age, was chosen for the Ao Yi Shi(奧醫師) and later became a Fa Yan(法眼), Fa Yin(法印) and Yu Chi(樂匙). He left about 15 texts, including "Su Wen Shao Shi"("素間紹識"), "Yi Xin Fang"("醫心方"), published in school, "Za Bing Guang Yao"("雜病廣要"), "Shang Han Guang Yao"(傷寒廣要), and "Zhen Fu Yao Jue"("該腹要訣"). On the Taki family's founding and working of the Yi Xue Guan Yasuka Doumei(失數道明) said they were "the people who took the initiative in Edo era kampo medicine" and evaluated their deeds in the fields of 'research of ancient text', 'the founding of Ji Shou Guan and medical education', 'publication business', 'writing of medical text'. 5. The doctors of the 'Kao Zheng Pai ' based their operations on the Edo Yi Xue Guan, and made groups with people with similar ideas to them, making a relationship 'net'. For example the three families of Duo Ji(多紀), Tang Chuan(湯川) and Xi Duo Cun(喜多村) married and adopted with and from each other and made prefaces and epitaphs for each other. Thus, the Taki family, the state science of the Makufu, the tendency of thinking, one's own interests and glory, one's own knowledge, the need of the society all played a role in the development of kampo medicine in the 18th and 19th century.
