Celiac Disease in South Jordan

  • Purpose: Celiac disease, an autoimmune enteropathy triggered by exposure to gluten, is not uncommon in South Jordan. However, its prevalence is underestimated due to lack of physician awareness of the diversity of disease presentation. The clinical spectrum includes classic gastrointestinal manifestations, as well as rickets, iron-deficiency anemia, short stature, elevated liver enzymes, and edema. Our goal was to evaluate celiac disease presentation in clinically diagnosed children. Methods: Retrospective study included all children diagnosed with celiac disease between September 2009 and September 2015. Hospital charts were reviewed. Demographic data, clinical characteristics, and follow-up were recorded. Results: Thirty-five children were diagnosed with celiac disease during the study period. Mean age${\pm}$standard deviation was $6.7{\pm}3.8$ years (range, 2.0-14 years). There were 17 (48.6%) female patients. The average duration between onset of symptoms and diagnosis was $16.3{\pm}18.7$ months. Fifteen (42.9%) patients presented with classic malabsorption symptoms, whereas 7 (20.0%) patients presented with short stature. Positive tissue transglutaminase antibodies (tTg)-immunoglobulin A (IgA) was seen in 34 (97.1%) patients. The one patient with negative tTg-IgA had IgA deficiency. Although tTG-IgA values were not available for objective documentation of compliance, clinical data (resolution of presenting abnormalities and growth improvement) assured acceptable compliance in 22 (62.9%) patients. Conclusion: CD in children may present with diverse picture. Although of the small number, the non-classical presentations are not uncommon in our rural community. Gluten-free diet is the main strategy for treatment and associated with usually correction of laboratory abnormalities and improvement of growth.

Sonographic Findings of Second Branchial Cleft Cysts; Variable Appearances and Helpful Diagnostic Findings (제2새열 낭종의 초음파 소견;다양한 소견들과 진단에 도움이 될 만한 소견들)

  • Objectives:The purpose of this study was to evaluate the variable sonographic features in addition to classic findings and to find the helpful characteristic findings for diagnosis of branchial cleft cysts. Subjects and Methods:We retrospectively analyzed the sonographic finding of 16 histopathologically proven branchial cleft cysts. Assessment involved the following variables : location related to carotid artery, size, cyst wall, border, margin, shape, internal echotexture, post echoic enhancement, orientation of longitudinal axis relative to the long axis of the neck, and tapering edge. Results:Most branchial cleft cysts were seen as well defined(15/16), oval shape(13/16), smooth inner (12/16) and outer margin(13/16), and post echoic enhancement(16/16). Their diameter varied from 1 to 6cm (mean 3.3cm). The branchial cleft cysts showed variable internal echopatterns;hyperechogenicity or pseudosolid appearance(1/16), heterogeneous echogenicity(4/16) in addition to classic form of anechoic(5/16) or hypoechoic internal echogenicity(6/16). Longitudinal axis of most branchial cleft cysts were arranged in the direction of the long axis of the neck(13/16) and some branchial cleft cysts had tapering edge(6/16). Conclusion:If both atypical sonographic findings and location are showed, longitudinal arrangement of long axis and tapering edge of branchial cleft cysts are helpful findings for differential diagnosis.

Modified Proximal Scarf Osteotomy for Hallux Valgus

  • Background: We developed a modified proximal scarf osteotomy technique for moderate to severe hallux valgus in an attempt to obtain better correction of the deformity. In addition, we compared the clinical and radiographic results of this modified technique with those of the classic scarf osteotomy reported in other studies. Methods: Between December 2004 and July 2009, 44 cases of modified proximal scarf osteotomy was performed in 35 patients with moderate hallux valgus. The American Orthopedic Foot and Ankle Society (AOFAS) score, visual analogue scale (VAS) score, range of motion of the first metatarsophalangeal joint, and radiographic results were evaluated. Results: The mean hallux valgus angle and the mean first intermetatarsal angle improved from an average of $32.2^{\circ}$ and $14.3^{\circ}$, respectively, to an average of $12.5^{\circ}$ and $8.6^{\circ}$, respectively. The distal metatarsal articular angle improved from an average of $18.7^{\circ}$ to $12.4^{\circ}$. The preoperative mean AOFAS and VAS scores were 47 points and 7 points, respectively, which improved to 86 points and 1 point, respectively, at the final follow-up. Limited range of motion occurred in two cases postoperatively. The height of the first metatarsal-cuneiform joint, which was an average of 15.9 mm preoperatively, did not change. The first metatarsal-talus angle increased from an average of $4.1^{\circ}$ to $7.1^{\circ}$. Conclusions: The modified proximal scarf osteotomy for the treatment of moderate hallux valgus showed similar results with the classic scarf osteotomy with regard to changes in the first intermetatarsal angle and postoperative satisfaction. Therefore, we suggest the modified proximal scarf osteotomy be considered as well as other proximal osteotomy in the treatment of moderate to severe hallux valgus.

Clinical and molecular characteristics of Korean children with Cornelia de Lange syndrome

  • Purpose: Cornelia de Lange syndrome (CdLS) is a rare genetically heterogeneous disorder caused by genetic variants of the cohesin complex. However, the diverse genetic etiologies and their phenotypic correlations in Korean patients with CdLS are still largely unknown. Hence, this study aimed to clarify the clinical characteristics and genetic background of Korean patients with CdLS. Materials and Methods: The medical records of 15 unrelated patients (3 males and 12 females) genetically confirmed to have CdLS were retrospectively reviewed. All individuals were diagnosed with CdLS using target gene analysis, whole-exome sequencing, and/or chromosomal microarray analysis. The clinical score (CS) was calculated to assess disease severity. Results: The median age at diagnosis was 1.7 (range, 0.0-11.8) years, and median follow-up duration was 3.8 (range, 0.4-11.7) years. Eight (53.3%) patients showed classic phenotypes of CdLS, two (13.3%) showed non-classic phenotypes, and five (33.3%) had other phenotypes sharing limited signs of CdLS. Fifteen causative variants were identified: NIPBL in five (33.3%, including 3 males), SMC1A in three (20.0%), SMC3 in three (20.0%), and HDAC8 in four (26.7%) patients. The CS was significantly higher in the NIPBL group than in the non-NIPBL group (14.2±1.3 vs. 8.7±2.9, P<0.001). Conclusion: We identified the clinical and genetic heterogeneity of CdLS in Korean patients. Patients with variants of NIPBL had a more distinctive phenotype than those carrying variants of other cohesin complex genes (SMC1A, SMC3, and HDAC8). However, further studies are warranted to understand the pathogenesis of CdLS as a cohesinopathy and its genotype-phenotype correlations.

Review on the origin of herbal name, Sik-bang-pung, for the root of Peucedanum japonicum Thunberg (갯기름나물 약재명(식방풍(植防風))의 연원에 대한 고찰)

  • Objectives : Sik-bang-pung (植防風, Peucedani Japonici Radix), the root of Peucedanum japonicum Thunberg (Apiaceae), has often been used as a substitute for Bang-pung (防風, Saposhnikoviae Radix) in the past, but recently it is treated as a counterfeit of Bang-pung or a kind of 'Jeonho (Peucedani Radix)'. Hence, it is crucial to investigate the origin of herbal name to provide appropriate usage of Sik-bang-pung. Methods : The classic and contemporary literatures regarding to the herbal medicines and botanical nomenclature were searched to find the herbal and botanical origins of Bang-pung and Sik-bang-pung, respectively. Result : The botanical descriptions of Bang-pung (防風) were consistent throughout the classic literatures and its botanical species was determined as Saposhnikovia divaricata Schischkin (Apiaceae) in the compendium. In the literatures published in the Joseon Dynasty, 'Byeong-pung-na-mul' was recorded as the medicinal plant of Bang-pung, but its botanical species could not be confirmed. In Japanese literatures, Bang-gyu (防葵) was confirmed as Mok-dan-bang-pung (牡丹防風); botan-bōfū in Japanese), which was determined as the herbal name of the root of 'Gaet-girum-namul (P. japonicum)' in Korea during the period of Japanese rule. The roots of Gaet-girum-namul was medicinally named as Mok-dan-bang-pung (牡丹防風), Sik-bang-pung (植防風), or San-bang-pung (山防風) in the past decades in Korea. Among them, Sik-bang-pung might be chosen as the herbal name, but its nominal origin could not be found. Conclusions : The herbal name, Sik-bang-pung, was presumably affected by Japanese botanical nomenclature. Although its medicinal application is still controversial, Sik-bang-pung should be considered an independent herbal medicine.

Groundwater vulnerability assessment in the southern coastal sedimentary basin of Benin using DRASTIC, modified DRASTIC, Entropy Weight DRASTIC and AVI

  • The importance of groundwater has long been recognized, but the ground water potential to become contaminated as a result of human activities has only been recognized in recently. Before 1980 it was thought that soils served as filters, preventing harmful substances deposited at the surface from migrating into groundwater. Today it is known that soils have a finite capacity to protect groundwater. It can be contaminated from divers sources. Therefore, Assessment of aquifer vulnerability to pollution is essential for the protection and management of groundwater and land use planning. In this study, we used DRASTIC and AVI for groundwater vulnerability to contamination assessment. the different methods were applied to the southern coastal sedimentary basin of Benin and DRASTIC method was modified in two different steps. First, we modified DRASTIC by adding land use parameter to include the actual pollution sources (DRASTICLcLu) and second, classic DRASTIC weights was modified using Shannon's entropy (Entropy weight DRASTIC). The reliability of the applied approaches was verified using nitrate (NO3-) concentration and by comparing the overall vulnerability maps to the previous researches in the study area and in the world. The results from validation showed that the addition of landcover/land use parameter to the classic DRASTIC helps to improve the method for better definition of the vulnerable areas in the basin and also, the weight modification using entropy improved better the method because Entropy weight DRASTICLcLu showed the highest correlation with nitrate concentration in the study basin. In summary the weight modification using entropy approach reduced the uncertainty of the human subjectivity in assigning weights and ratings in the standard DRASTIC.

  • As the rapid development of quantum computing compromises current public key encryption methods, the National Institute of Standards and Technology (NIST) in the United States has initiated the Post-Quantum Cryptography(PQC) project to develop new encryption standards that can withstand quantum computer attacks. This project involves reviewing and evaluating various cryptographic algorithms proposed by researchers worldwide. The initially selected quantum-resistant cryptographic algorithms were developed based on lattices and hash functions. Currently, algorithms offering diverse technical approaches, such as BIKE, Classic McEliece, and HQC, are under review in the fourth round. CRYSTALS-KYBER, CRYSTALS-Dilithium, FALCON, and SPHINCS+ were selected for standardization in the third round. In 2024, a final decision will be made regarding the algorithms selected in the fourth round and those currently under evaluation. Strengthening the security of public key cryptosystems in preparation for the quantum computing era is a crucial step expected to have a significant impact on protecting future digital communication systems from threats. This paper analyzes the security and efficiency of quantum-resistant cryptographic algorithms, presenting trends in this field.

Automated Classification of Audio Genre using Sequential Forward Selection Method

  • In this paper, we propose a content-based audio genre classification algorithm that automatically classifies the query audio into five genres such as Classic, Hiphop, Jazz, Rock, Speech using digital signal processing approach. From the 20 second query audio file, 54 dimensional feature vectors, including Spectral Centroid, Rolloff, Flux, LPC, MFCC, is extracted from each query audio. For the classification algorithm, k-NN, Gaussian, GMM classifier is used. In order to choose optimum features from the 54 dimension feature vectors, SFS (Sequential Forward Selection) method is applied to draw 10 dimension optimum features and these are used for the genre classification algorithm. From the experimental result, we verify the superior performance of the SFS method that provides near $90{\%}$ success rate for the genre classification which means $10{\%}$-$20{\%}$ improvements over the previous methods

A Study on Korean Translation of the Pathway of Lung Meridian in Miraculous Pivot·Meridian Vessel (영추·경맥편 수태음폐경 유주의 한글번역에 대한 고찰)

  • Objectives : It aims to establish a basic rule in Korean translation of the pathway of lung meridian in Miraculous Pivot Meridian vessel. Based on the rule, We tried to make standard translation of the pathway of lung meridian in Miraculous Pivot Meridian vessel. Methods : Books needed for this study were collected through searching Kyunghee University Library(http:// khis.khu.ac.kr). Keywords included "Miraculous Pivot of Huangdi's Internal Classic". We also include the book which is generally used as a textbook in Colleges of Korean Medicine. Results : In five Chinese books, the word-spacing was used differently in four phrases. Six Korean-translated books had the different translation in three phrases. We suggested a standard Korean translation of the pathway of lung meridian in Miraculous Pivot Meridian vessel. Conclusions : This result of the study would be expected to not only be published in Korean Journal of Acupuncture but be studied more about Korean translation by experts in this field.

Literatual Study on Cause and Treatment of Amnesia (건망(健忘)의 병인(病因) 치료(治療)에 관(關)한 문헌고찰(文獻考察))

  • Objectives : We suggest the method of oriental neuropsychiatry treatment about amnesia through herb therapy. Methods : We investigate cause of disease, component of herbs about amnesia with classic current oriental medicine books. Results : Amnesia is due to simsinbulgyo(心腎不交), biwieyangher(脾胃陽虛), dammisimgyoo(痰迷心竅), emotional damage(七情所傷), extravasated blood(瘀血), deficiency of kidney (腎虛). There is 138 kind of herbs are used in our study that we find out that most frequently used herb is ginseng(人蔘). Heart meridian is the highest use in the all meridians. Sungon(性溫) is the highest use in the all kimi(氣味) Conclusions : We could confirm that herbs of amnesia treatment was related to the three vital organs(臟) named of spleen(脾), lung(肺), kidney(腎).
