  • Anterior open bite is one in which the teeth in the anterior portion of the maxilla and mandible are vertically apart and lack the overlapping necessary for the incisive function when the mandible is in closed position. Anterior open bite is a result of the interaction of many different etiologic factors including thumb and finger sucking, lip and tongue habits, airway obstruction, skeletal growth abnormalities and its tendency may appear with any type of skeletal patterns, such as Class I, II or III malocclusion types. Though the treatment methods for anterior open bite are various, the conventional FR-4, designed by Rolf Fr$\"{a}$nkel, is known to be effective in treating open bite cases with Class I or II skeletal patterns. It is due to that an incidence of skeletal Class II is high in the Occidentals, and open bite is accompanied by these malocclusion type in many cases. However, an incidence of skeletal Class III is high in the Orientals, and open bite is sometimes accompanied by skeletal Class III in many cases. Although the use of the conventional FR-4 was effective in the treatment of open bite, skeletal Class III would be worsened. So, a modified FR-4(placing the labial bow in the lower, the labial pads in the upper) was designed for the treatment of patients showing skeletal Class III and open bite.

An Epidemiologic study on the Orthodontic Patients Who Visited Department of Orthodontics, Chosun University Dental Hospital Last 10 Years(1990${\sim}$1999) (최근 10년간 조선대학교 부속치과병원 교정과에 내원한 부정교합 환자에 관한 역학적 연구(1990${\sim}$1999))

  • With socioeconomic development and change of esthetic recognition, the demand for orthodontic treatment and number of orthodontic patients has been increasing so rapidly. And frequency of malocclusion was changed. So this study was done in an attempt to provide an epidemiologic study so that we can accomodate their orthodontic needs adequately and to obtain the reliable quantitative information regarding the characteristics of orthodontic patients. Distribution and trends were examined in 3,070 malocclusion patients who had been examined and diagnosed at Department of Orthodontics, Dental Hospital, Chosun University over 10 year-period from 1990 to 1999. The results were as follows : 1. The number of patients per year was increasing trend and higher visiting rate in female(56.5%) than in male(43.5%). 2. Age distribution had shown 7${\sim}$ 12 year-old group being the largest(37.9%) and each percentage of 13${\sim}$18, 19${\sim}$24, above-19, 0${\sim}$6 year${\sim}$old group was 32.0%, 19.6%, 7.1%, 3.4%. 3. Hellman dental age IVa which is completion of the permanent dentition showed the highest percentage in male and female. 4. Geographic distribution showed a majority of patients in Kwang Ju(71.0%). Group within the distance 10km from Chosun Dental Hospital was 56.3% and group within 20km was 14.7%. 5. Anterior cross bite showed the highest percentage in chief complaints and percentage of Mn. prognathism and protrusion of Mx. teeth was 12.6%, 12.2%. 6. Distribution in the types of malocclusion according to the Angle's classification had shown; 38.9% for Class I, 20.7% for Class II division 1, 2.0% for Class III division 2, 38.4% for Class III. 7. In the dental vertical dysplasia according to the Angle's classification, deep bite was the most frequent in Class II div.1 and div. 2(24.3%, 56.7%) and open bite in Class III(21.4%). 8. In the skeletal sagittal dysplasia, 39.3% of skeletal Class II was due to the undergrowth of the mandible and 46.3% of skeletal Class III was due to the overgrowth of the mandible. 9. Distribution in orthodontic treatment acceding to the extraction and nonextraction had shown 66.9% for nonextraction case, 33.1% for extraction case, and four first bicuspids have been extracted in the highest percentage(38.6%). 10. Patients who had orthognathic surgery comprised 7.9%, with an increasing trend.

Analysis of Treatment Period on the Intraoral Removable Appliance Utilizing Vertical Facial Growth on Class III Malocclusion (얼굴의 수직성장을 이용하여 III급 부정교합을 치료하는 구강내 가철식 장치의 치료기간분석)

  • Vertical facial growth triggers the rotation of mandible to move the chin point to the downward and backward direction, which showed remarkably effective result making the less prominent chin. Recently, the intraoral removable appliance utilizing class III elastic demonstrated the vertical growth trigger mechanism. The treatment change was very fast and wearing was quite easy, compared to extraoral appliances. The purpose of this study was to verify the duration of the treatment on class III malocclusion using intraoral removable appliances, which designed to accelerate vertical facial growth. 56 patients were selected with the complaint of the protruded mandible and class III malocclusion (overjet : -3 - 0 mm, overbite : 0 - 4 mm). Information like; age at start, duration of the treatment events, type of the treatment, overjet, overbite etc. was collected and calculated. The average age of the patients delivering the initial brace was $8.75{\pm}1.10year$. Most of the anterior crossbite was resolved within 6 months. The total treatment period was $21.79{\pm}10.73months$ with the additional procedures like the alignment of anterior teeth and torque control using additional removable and fixed orthodontic appliances. The correlation study showed that patient's cooperation (p = 0.000) and the use of fixed appliance (p = 0.032) were significantly influenced on treatment duration.

A ROENTGENOCEPHALOMETRIC STUDY ON THE EFFECTS OF THE CHINCAP IN THE SKELETAL CLASS III MALOCCLUSION (이모장치를 사용한 골격성 III급부정교합 아동의 두개악안면 형태변화에 대한 두부방사선계측학적 연구)

  • The purpose of this study was to evaluate the effects of the chincap therapy on the craniofacial structure in persons with skeletal Class III malocclusion. The patients selected for this study were treated with extra-oral chincap therapy only. Both control and treatment samples were obtained from Seoul National University Hospital where these longitudinal data were gathered. 55 treated patients and 14 control patients were studied. The mean ages at the 1st evaluation was 8 years 3 months in the treatment sample and 9 years 4 months in the control sample. The duration of chincap therapy was variable but averaged 2 years of treatment. Post-treatment observation procedeeded for 1 year 2 months. Active treatment and post treatment effects were evaluated. The results were as follows: 1. Neither significant restraint nor acceleration of growth was found in the cranial base and maxilla during treatment. 2. A distal rotation of the mandibular complex was seen. 3. Some amount of restraint of growth was found in mandibular body length, ramus height, mandibular length during treatment. 4. The genial angle was reduced. 5. After removal of the chin-cap, forward displacement of the mandible took place.

Study on Korean skeletal Class III craniofacial pattern by counterpart analysis (구조적 대응체 분석법에 의한 한국인 골격성 III급 부정교합의 특징)

  • Enlow's counterpart analysis reflects the characteristics of each individual sample to find out the cause of the malocclusion and further applying them to the clinic. Enlow's counterpart analysis was performed on 100 Korean samples (50 male, 50 female) with normal occlusion and 100 skeletal class III patients (50 male, 50 female) scheduled for orthognathic surgery. The following conclusions were obtained. 1. The cause of malocclusion in skeletal class III patients were complex and interrelated : backward upward rotation of the cranial base, forward inclination of the ramus, increase in the mandibular body length, and posteriorly located maxilla. 2. Seen on R2 (male-1.68mm, female-2.33mm), in skeletal class III, the maxilla Is mote posteriorly located than the normal group. 3. The cause of malocclusion In skeletal class III patients, consists of retrognathic maxilla(A1) male $22\%$, female $26\%$, prognathic mandible(B1) male $44\%$, female $34\%$, and combination of an retrognathic maxilla and prognathic mandible were male $28\%$, female $38\%$. 4. There was no significant difference in the anterior-posterior length of the maxilla(A4) between skeletal class III males with the normal group, while in the female subjects, the skeletal class m group showed a smaller maxilla(A4) compared to the normal group. 5. In skeletal class III patients the proganthic mandible was primarily caused by the Inclination of the ramus(R3, R4) and mandibular body length(B4, B6) rather than ramus width(B3).

The Treatment Change of PAR(Peer Assessment Rating) Index and Cephalometric Measurements in Class I Malocclusion Patients (제 I 급 부정교합 환자에서 치료전후 PAR지수와 두부방사선계측치의 변화)

  • The purpose of this study was to evaluate the relation between the treatment duration and cephalometric measurements and the PAR index in Class I malocclusion patients. In 100 Class I malocclusion patients, PAR score and cephalometric measurements were taken from study model and cephalometric radiographs and analyzed statistically. The results of this study were obtained as follows: 1. treatment duration was correlated with extraction and pre PAR index. 2. ANB, FMA, FMIA and IMPA exhibited positive correlation between pre PAR index and pretreatment cephalometric measurements. 3. $\underline{1}$ to FP exhibited positive correlation between post PAR index and posttreatment cephalometric measurements, and $\overline{1}$ to FP exhibited negative correlation. 4. $\underline{1}$ to SN, IIA and $\overline{1}$ to FP exhibited positive correlation between ${\%}\;PAR$ reduction and the change of cephalometric measurements and FMA FMIA, WITS and UL exhibited negative correlation. The results of this study indicate that PAR index taken from study model relate with items concerned with upper and lower incisors, and there are the tendency that pretreatment PAR index are larger in the patients with large Am value and hyperdivergent face.

A STUDY ON THE MORPHOLOGICAL CHANGES IN CHILDREN WITH CLASS I AND III MALOCCLUSIONS: CEPHALOMETRIC ANALYSIS (I급과 III급 부정교합 어린이의 두개안면골 형태의 변화에 관한 연구)

  • The present study was designed to compare the morphological and structural differences of craniofacial structures among 146 children with Class I and Class III malocclusions. The results below were obtained from the study. 1. Sphenoethmoidal synchondrosis continues to grow later in Class III. 2. Anteroposterior length of the nasomaxillary complex was significantly shorter in Class III, but the height of the nasomaxillary complex was similar. 3. Mandibular length and mandibular body length were longer in Class III, but had no statistical significance. Lower anterior facial height was shorter in Class III, but had no statistical significance. 4. Dentoalveolar height was similar between Class I and Class III. 5. In Class I, anterior cranial base took part in the anteroposterior length of the nasomaxillary complex and the mandible. 6. In Class III, anterior cranial base and middle cranial base had higher correlation with the mandible with aging. These results suggest that there exist a little differences between Class I and Class III malocclusions at age $7{\sim}11$, but growth patterns are mostly similar. Therefore it is necessary to correct Class III malocclusions at an early age before skeletal differences appear.

THE CASE REPORT OF THE SKELETAL ANGLE'S CLASS II MALOCCLUSION WITH THE UPPER CENTRAL INCISOR MISSING (상악 중절치 결손을 동반한 골격성 II급 부정교합 환자의 치험예)

  • The causes of the missing teeth are classified as congenital missing, trauma and extraction due to dental caries, variable problems are occured clinically by the missing teeth. The missing of the upper incisors especially would assume a serious aspect, and could be treated by three methods of orthodontic treatment, prosthodontic treatment and autotransplantation of the premolar teeth. The patient of this report had the skeletal class II malocclusion with the left upper central incisor missing, and have been treated with the fixed appliance after extraction of the right upper central incisor and both lower second premolars. The results were obtained as follows: 1. Treatment was done for 1 year 6 months. 2. Normal overbite and overjet were achieved. 3. Cuspal interdigitation was obtained normally. 4. Space problem was resolved with resin restoration of the upper lateral incisors. 5. The upper canines were used as the upper laterals after cuspal contouring. 6. Retention would be required with adequate retainers for a long time to prevent relapsing after treatment.

FACIAL GROWTH CHANGE AFTER ORTHODONTIC TREATMENT IN CHILDREN (Second Report) (소아(小兒)에 있어 교정전후에 악골의 변화(變化)에 관(關)한연구(2차보고))

  • This study was undertaker to observe the longitudinal change by orthodontic treatment for early Class III malocclusion in primary and mixed dentition. Cephalometric roentgenograms of 8 children with Class III malocclusion obtained during activator therapy were measured and compared with those obtained before activator therapy. The following results were observed. During treatment with activator; 1. The maxilla became retrueded in A,C,E, and K and protruded in F, G, H, and J. 2. The mandible became retruded in A,C,E, and F and F and protruded in H and J. No difference was observed in G and K. 3. Gonial angle became reduced in A,C,E,F, and K and increased in H. No difference was observed in G and J. 4. The steepness of mandibular plane became reduced in C,E,F,G,H, and J and increased in A. No difference was observed in K. 5. The inclination of upper incisor became more labially in A,C,E,G,H,J, and K. No difference was observed in F. 6. The inclination of lower incisor became more lingually in all cases.
