Weekly Cisplatin-Based Concurrent Chemoradiotherapy for Treatment of Locally Advanced Head and Neck Cancer: a Single Institution Study

  • Background: The organ preservation approach of choice for the treatment of locally advanced head and neck cancers is concurrent chemoradiation with three weekly high doses of cisplatin. Although this is an efficacious treatment policy, it has high acute systemic and mucosal toxicities, which lead to frequent treatment breaks and increased overall treatment time. Hence, the current study was undertaken to evaluate the efficacy of concurrent chemoradiation using 40 mg/m2 weekly cisplatin. Materials and Methods: This is a single institutional retrospective study including the data of 266 locally advanced head and neck cancer patients who were treated with concurrent chemoradiation using 40 mg/m2 weekly cisplatin from January 2012 to January 2014. A p-value of < 0.05 was taken to be significant statistically for all purposes in the study. Results: The mean age of the study patients was 48.8 years. Some 36.1% of the patients had oral cavity primary tumors. The mean overall treatment time was 57.2 days. With a mean follow up of 15.2 months for all study patients and 17.5 months for survivors, 3 year local control, locoregional control and disease free survival were seen in 62.8%, 42.8% and 42.1% of the study patients. Primary tumor site, nodal stage of disease, AJCC stage of the disease and number of cycles of weekly cisplatin demonstrated statistically significant correlations with 3 year local control, locoregional control and disease free survival. Conclusions: Concurrent chemoradiotherapy with moderate dose weekly cisplatin is an efficacious treatment regime for locally advanced head and neck cancers with tolerable toxicity which can be used in developing countries with limited resources.

Pu-erh Tea Powder Preventive Effects on Cisplatin-Induced Liver Oxidative Damage in Wistar Rats

  • Background: Chemotherapy is one of the major means for control of malignancies, with cisplatin (CDDP) as one of the main agents, widely used for the treatment of various malignant solid tumors. However, prevention of hepatotoxicity from cisplatin is one of the urgent issues in cancer chemotherapy. In this study, we aimed to investigate the effects of pu-erh tea on hepatotoxicity through body weight and tissue antioxidant parameters like, liver coefficient, serum alanine aminotransferase (ALT), serum aspartate aminotransferase (AST), superoxide dismutase (SOD), glutathione peroxidase (GSH-PX), malondialdehyde(MDA) and glutathione (GSH) levels, and light microscopic evaluation by histological findings. Materials and Methods: The rats were randomly divided into five groups: Control (n=10), cisplatin (3 mg/kg p.i., n=10), cisplatin+pu-erh (0.32 g/kg/day i.g., n=10), cisplatin+pu-erh (0.8 g/kg/day i.g., n=10) and cisplatin+pu-erh (1.6 g/kg/day i.g., n=10). Pu-erh tea powder was administrated for 31 consecutive days. The rats were sacrificed at the end on the second day after a single dose of cisplatin treatment for measuring indices. Results: Pu-erh tea powder exhibited a protective effect by decreasing MDA and GSH and increasing the SOD and GSH-PX levels and GSH-PX/MDA ratio in camparison with the control group. Besides, pu-erh tea was also able to alleviate the pathological damage to some extent. Conclusion: Pu-erh tea powder is protective against cisplatin-induced liver oxidative damages, especially at the medium dosage (0.8 g/kg/d).

Early Efficacy of Endostar Combined with Chemoradiotherapy for Advanced Cervical Cancers

  • The aim of this study was to investigate the early outcome of Endostar combined with chemoradiotherapy for advanced cervical cancer. Fifty-two cases (FIGO IIb to IVa) were divided randomly into two groups, receiving chemoradiotherapy alone (CRT group) and Endostar combined with chemoradiotherapy (CRT+E group). For the patients in the CRT+E group, Endostar was administered daily with the dosage of 7.5 $mg/m^2$, and cisplatin was administered weekly with the dosage of 20 $mg/m^2$ during the radiation. The regimens lasted for 4 weeks with no difference in chemoradiotherapy between the two groups. The early outcome complete remission rate was 73.1%, partial remission rate was 23.1% and the total response rate was 96.2% in CRT+E group, a significant improvement on the 34.6%, 42.3% and 76.9%, respectively, in the CRT group. One year survive rates were 100% and 84.6% in the CRT+E group and CRT groups, the difference being significant. Endostar combined with chemoradiotherapy can improve the early outcome of the advanced cervical cancer, and adverse effects were not encountered.

  • In this study, the antitumor activities were investigated using $\beta$-lmmunan, a proteoglycan obtained from cultured mycelia of the IY009 strain of Ganoderma lucidum belonging to basidiomycetes. The result showed the significant effect of cytotoxicity test against murine sarcoma 180 and murine lymphocytic leukemia L1210 using immunized macrophage cultures by $\beta$-lmmunan. When intraperitoneally injected at 40 mg/kg/day daily for 10 days, $\beta$-lmmunan inhibited the growth of sarcoma 180 solid tumor in ICR mice by 88.8% (p<0.05). It was also observed that $\beta$-lmmunan increased life span by 85.2% (p<0.01) after treatment of 100 mg/kg/day in BDF1 mice bearing lymphocytic leukemia L1210. And combination therapy with cisplatin (dosage: 4 mg/kg) increased life span by 140.4% (p<0.05) after treatment of 100 mg/kg/day daily in BDF1 mice bearing lymphocytic leukemia L1210.

Impact of radiation dose on concurrent chemoradiotherapy for limited-stage small-cell lung cancer

  • Purpose: To evaluate clinical outcomes according to radiation dose in patients with limited-stage small-cell lung cancer (LS-SCLC) treated with concurrent chemoradiotherapy (CCRT). Materials and Methods: From January 2006 to December 2015, 38 patients with LS-SCLC were treated with CCRT with etoposide and cisplatin. Total radiation doses ranged from 45 Gy to 66 Gy (1.8-2 Gy/fraction) and were classified into three groups: 45-54 Gy, 60-63 Gy, and 66 Gy. The impact of radiation dose on survival outcomes were evaluated. Toxicities were evaluated according to the Common Terminology Criteria for Adverse Events version 4.03. Results: The median follow-up period was 21 months. The 2-year overall survival (OS) and local failure-free survival (LFFS) rates were 45.8% and 67.5%, respectively. The 2-year LFFS rates were 33.3% for 45-54 Gy group, 68.6% for 60-63 Gy group, and 87.1% for 66 Gy group (p = 0.014). In multivariate analysis, radiation dose was a significant factor for LFFS (p = 0.015). Although radiation dose was not a significant factor for OS and disease-free survival (DFS) in multivariate analysis, both OS and DFS of 66 Gy group tended to be better than that of 45-63 Gy group in univariate analysis. However, there were no differences in severe toxicities among three groups. Conclusion: Higher radiation dose achieved better local control in patients with LS-SCLC treated with CCRT. In addition, a total dose of 66 Gy tended to improve OS and DFS.

Characterization of a conjugated polysuccinimide-carboplatin compound

  • Carboplatin, an advanced anticancer drug with excellent efficacy against ovarian cancer, was developed to alleviate the side effects that often occur with cisplatin and other platinum-based compounds. Our study reports the in vitro characteristics, viability, and activity of cells expressing the inducible nitric oxide synthase (iNOS) gene after carboplatin was conjugated with polysuccinimide (PSI) and administered in combination with other widely used anticancer drugs. PSI, which has promising properties as a drug delivery material, could provide a platform for prolonging carboplatin release, regulating its dosage, and improving its side effects. The iNOS gene has been shown to play an important role in both cancer cell survival and inhibition. Herein, we synthesized a PSI-carboplatin conjugate to create a modified anticancer agent and confirmed its successful conjugation. To ensure its solubility in water, we further modified the structure of the PSI-carboplatin conjugate with 2-aminoethanol groups. To validate its biological characteristics, the ovarian cancer cell line SKOV-3 and normal ovarian Chinese hamster ovary cells were treated with the PSI-carboplatin conjugate alone and in combination with paclitaxel and topotecan, both of which are used in conventional chemotherapy. Notably, PSI-carboplatin conjugation can be used to predict changes in the genes involved in cancer growth and inhibition. In conclusion, combination treatment with the newly synthesized polymer-carboplatin conjugate and paclitaxel displayed anticancer activity against ovarian cancer cells but was not toxic to normal ovarian cancer cells, resulting in the development of an effective candidate anticancer drug without severe side effects.

상악동암에서 수술 전 방사선 치료의 효과 (Preoperative Radiotherapy of Maxillary Sinus Cancer)

  • 목적 :상악동암에서 수술 전 방사선 치료를 시행함으로써 안면 보존의 가능성이 어느 정도 되는지를 알아보고자 하였다. 대상 및 방법 : 1986년 6월부터 1990년 9월까지 상악동암으로 방사선치료를 받은 42례의 환자를 대상으로 후향적 분석을 시행하였다. 환자의 연령은 24세에서 75세로 중간값은 56세였으며, 남자 27예, 여자 15예였다. 병기별 분포는 T2 2예, T3 19예, T4 21예로 진단 당시 대부분 진행된 상태였다. 조직학적으로는 편평 상피암 38예, 미분화 세포암 1예, 이행 상피암 1예, 선양 낭종암 2예였다. 모든 환자에 대해 동측 부비동을 포함하는 쌍쐐기빔 방법의 방사선치료를 시행하였다. 방사선 치료선량은 50.4-70.2 Gy로 중간값은 70.2 Gy였다. 11례의 환자에 대해 방사선치료와 유도 화학요법을 병행하였다. 화학요법은 5-fluorouracil과 cisplatin으로 2회 시행하였다. 대상 환자의 추적기간은 3개월에서 92개월로 중간값은 16개월이었다. 방사선치료 후 6 내지 8개월 경과 후 전산화단층촬영을 실시하여 병소의 반응을 평가하였다. 완전관해가 된 경우는 계속 추적검진만 하였으며 병변이 의심되는 경우는 콜드웰-뤼크 수술을 시행하여 확인하였다. 병변이 확인된 경우는 상악동전절제술을 시행하였다. 결과 : 방사선 및 화학요법 시행 후 9예 (21.4$\%$)에서 완전관해를 보였고 33예 (88.6$\%$)에서 잔여병변이 확인되었다. 완전관해를 보인 9예의 환자 중 7예가 국소제어되었으며 2예에서 국소재발하였다. 잔여병변이 있는 33예의 환자 중 10예에서 근치적 수술이 가능하였고 그 중 2예에서 국소제어가 되었다. 전체 환자의 5년 생존율은 23.1$\%$였고 5년 무병생존율은 16.7$\%$였다. 방사선치료에 대한 반응 (P<0.05) 만이 생존율에 영향을 미치는 유의한 인자였다. 결론 : 방사선치료에 대한 반응만이 생존율에 영향을 미치는 유의한 인자였다. 전체적으로 7예(16.7$\%$) 에서 수술 전 방사선치료로 안면보존이 가능하였다.
