  • Chronic obstructive pulmonary disease (COPD) is a chronic airway inflammation characterized by fixed airflow limitation and chronic respiratory symptoms, such as cough, sputum, and dyspnea. COPD is a progressive disease characterized by a decline in lung function. During the natural course of the disease, acute deterioration of symptoms leading to hospital visits can occur and influence further disease progression and subsequent exacerbation. Moreover, COPD is not only restricted to pulmonary manifestations but can present with other systemic diseases as comorbidities or systemic manifestations, including lung cancer, cardiovascular disease, pulmonary hypertension, sarcopenia, and metabolic abnormalities. These pulmonary and extrapulmonary conditions lead to the aggravation of dyspnea, physical inactivity, decreased exercise capacity, functional decline, reduced quality of life, and increased mortality. In addition, pneumonia, which is attributed to both COPD itself and an adverse effect of treatment (especially the use of inhaled and/or systemic steroids), can occur and lead to further deterioration in the prognosis of COPD. This review summarizes the long-term outcomes of patients with COPD. In addition, recent studies on the prediction of adverse outcomes are summarized in the last part of the review.

  • Infantile Convulsion, one of common emergency symptoms in pediatrics, arises from sudden derangement of the central nerve system, and can cause a sudden loss of consciousness and spasm. It falls into three categories: Acute Infantile Convulsion, Chronic Infantile Convulsion and Chronic Spleen Convulsion. According to research, approximately 6~7% of all babies undergo spasm more than once. Since the treatment must be done immediately, acupuncture & moxibustion treatment can be one of the most important treatments in this Particular case. Therefore, the focus of this study is on how acupuncture & moxibustion can be utilized in the treatment of Infantile Convulsion, and the literary findings are as follows: 1. The meridian points used on acute infantile convulsion are Sugu(GV26), T'aech'ung(Liv3), Hapkok(LI4). 2. The meridians used on acute infantile convulsion are Governor Vessel(GV), Bladder Meridian(BL), Stomach Meridian(ST). 3. The meridian points used on accompanied symptoms with acute infantile convulsion are Haenggan(Liv2), Yangnungch'on(Liv3) on spasm, Paek'oe(GV14) on opisthotonus, Kokchi(LI11), Taech'u(GV14) on fever, Nogung(P8), Yongch'on(K1) on fainting spell, Chok-samri(S36) on body weakness. 4. The meridian points used on chronic infantile convulsion are Shinguol(CV8), Ch'onchj'u(S25), T'aech'ung(Liv3), Kwanwon(CV4), Ch'ukt'aek(L5). 5. The meridians used on chronic infantile convulsion are Conception Vessel(CV), Governor Vessel(GV), Stomach Meridian(ST). 6. The meridian points used on accompanied symptoms with chronic infantile convulsion are Ch'onchj'u(S25), Kolli(CV11) on diarrhea, Taenung(P7), Shinmun(H7) on fainting spell, Kansu(B18), T'aech'ung(Liv3) on spasm. 7. • Objectives: The aim of this review is to ascertain whether Bojungikgi-tang and its modifications is more effective than non-specific management in chronic fatigue syndrome (CFS). Methods: We collected clinical trials to investigate the effects of Bojungikgi-tang and non-specific management on general symptoms, fatigue, and sleep quality in CFS patients. The databases used for data retrieval were Pubmed, Central Cochrane, CNKI, VIP, CiNii, OASIS, RISS, and Koreamed. We performed selection/exclusion process from the found records according to prespecified criteria, and assessed the final included trials according to the Cochrane risk of bias tool. The included studies were classified on the basis of interventions in experimental group. Results: Seven randomized controlled trials (476 participants) were eligible and their results were synthesized in the meta-analysis. The synthesis showed a substantial effect of Bojungikgi-tang (relative risk 0.20 [95% CI 0.13 to 0.31], Z=7.44, P<0.00001; I2=0%) and combination of Bojungikgi-tang and Sosiho-tang (relative risk 0.15 [95% CI 0.08 to 0.28], Z=5.85, P<0.00001; I2=0%) compared with non-specific management on improvement of general symptoms. However, we could not find sufficient clinical research records to determine their effects of improvement on fatigue and sleep quality. Conclusions: Bojungikgi-tang and its modifications are more effective than non-specific management for improvement of general symptoms in CFS post-treatment.

  • A cross-sectional study was conducted in an effort to investigate the effect of air pollution on respiratory symptoms. Two groups of female aged more than 20 living in the unpolluted rural area of Taebul (n=159) and urban area of Taegu (n=140) were selected. The ATS-DLD-78 questionnaire was translated into Korean and administered with minor modification. The proportion of smoker was less than 1% in both area. Exposure to smoking and higher educational level were more frequent in Taegu. Age-adjusted prevalence rates of 'chronic cough', 'chronic sputum', 'wheezing', and 'dyspnea' were higher in Taegu than in Taebul. In particular, the prevalence rate of 'chronic sputum' in Taegu was found to be higher, which was statistically significant. Exposure to smoking and education level were not concerned with all respiratory symptom prevalence rates. In conclusion, this study Indicates that an urban factor is related to the rates of respiratory symptoms in Korea.

  • Chronic subdural hematoma (CSDH), which rarely happens in the young, is thought to be a disease of the elderly. Whereas unspecific symptoms and insidious onset in juveniles and young adults, as a result of its relative low morbidity, CSDH is usually neglected even undertreated in the young. Through the three cases and review of the current literature on this subject, we tried to illustrate the clinical and etiopathological characteristics of this entity and find out the most appropriate treatment strategy. We report three young CSDH patients with different but similar symptoms. The present histories, tests and examinations revealed different predisposing factors accounting for the genesis of CSDH. Their preoperative symptoms were all resolved with burr hole and drainage operation. Juveniles and young adults suffering from CSDH differ from that of their elderly counterparts in their clinical and etiopathological characteristics. Although trauma is the most important risk factor in young and old CSDH patients, some other predisposing factors may exist. Burr hole and drainage surgery could resolve the problem most of the time. But further tests and examinations even specific management should be made in some cases.
