  • Background: This study was planned to assess and compare the effectiveness of cervical region stabilization exercise and strengthening exercise on pain and range of motion in patient with chronic neck pain. Methods: Fifty-one patient with chronic neck pain were randomly allocated into strengthening and stabilization exercise (SSE) group (n=27) and Strengthening exercise (SE) group (n=24). The program was carried for 12 sessions, 3 days/week in 4weeks. Pain intensity with visual analog scale (VAS), cervical range of motion, deep cervical flexor strengthening were evaluated before and after the intervention. Results: After the program, pain intensity decreased in group SE and SSE were found (p<.05). Range of motion and deep cervical muscle strength increased in group SE and SSE were found(p<.05). The intergroup comparison showed that significant difference in range of motion and deep cervical muscle strength (p<.05). Conclusions: The study suggest that both SSE program and SE program might be a useful treatment for patients with chronic neck pain. However SSE program might be superior in improving cervical range of motion and deep cervical flexor strength compare to SE program.

  • Background: The aim of this study was to investigate the relationship between pain catastrophizing, acceptance, and quality of life in relation to chronic low back pain in Jakarta, the capital city of Indonesia. We also analyze the effect of personality in catastrophizing and acceptance. Methods: A total of 52 chronic low back pain patients were enrolled as participants from 2 hospitals in Jakarta (43 females, 9 males, mean age 54.38 years). Participants completed a set of self-reported questionnaires: the NEO Five Factor Inventory (NEO-FFI), Chronic Pain Acceptance Questionnaire-Revised (CPAQ-R), Pain Catastrophizing Scale (PCS), and Pain Discomfort Module (PDM). Results: This study showed that acceptance increased the patient's quality of life by giving physical relief from pain. In contrast, pain catastrophizing decreased the quality of life, and increased the patients' tendency to get frustrated, irritated, and anxious about the pain. From a personality perspective, the trait neuroticism may lead to a higher level of pain catastrophizing. Conclusions: This study showed that catastrophizing, compared with acceptance, had a greater impact on the patient's life by reducing its quality.

  • Objectives : This study is to report two cases of chronic skin pruritus improved by Chijasi-tang. Methods : Chijasi-tang was prescribed to two chronic skin pruritus patients. One patient took Chijasi-tang for 30 days and the other took it for 15 days. We compared patients' symptoms before and after treatment using self-made standards. Results : Patients who complained of pruritus on the elbow and thigh, pruritus decreased from NRS 8 to 1, and skin symptoms improved from 16 to 2. The other patient who complained of pruritus in the neck and chest, pruritus improved from NRS 9 to 2, and skin symptoms at the lesion site improved from 12 to 2. Also, accompanied symptoms such as heat of the feet and heartburn almost disappeared. Conclusions : After prescribing Chijasi-tang to two chronic skin pruritus patients, significant improvements were observed. It is thought that Chijasi-tang can be used as the treatment of other skin diseases in the future.

  • The purpose of this study was to examine changes in the swallowing function with chronic stroke patient. Ten chronic stroke patients with dysphagia followed up for 3 months. Eight normal age match subjects were also participated. A total of 18 participated in this study and they were classified into two groups (stroke patient group and normal control group). Participants underwent videofluoroscopic swallowing examination. The swallowing function was evaluated over time. Oral transit duration, pharyngeal transit duration, laryngeal response duration, Modified Barium Swallowing Impairment Profile ($MBSImP^{TM(c)}$), and Functional Oral Intake Scale (FOIS) were applied. Chronic stroke patient group were showed significantly longer oral transit duration, pharyngeal transit duration and laryngeal response duration than normal control group at baseline. After 3 months in stroke patient group, the mean oral components score of MBSImP (1~5) significantly decreased and the mean score on FOIS increased compared to baseline. In conclusion, oral swallowing function and oral intake function improved by the 3 month evaluation. These results describe changes in swallowing function with chronic stroke patients over time and provide basic informations to understand dysphagia.

The growing number of patients with chronic renal failure wake us up to the need to prepare for proper management. There are not only the prevalence of coronary artery disease, but also a great amount of valve dysfunction exist in this group. Peritoneal dialysis may be one of the obstacles for cardiopulmonary bypass but it is not a great hindrance in cardiac surgery with careful preparation and well organized perioperative care. The author has performed mitral valve replacement in a 33-year-old anuric female patient with chronic renal failure and severe mitral insufficiency. Preoperatively, the patient was kept in adequate fluid and electrolyte balance using peritoneal dialysis. Peritoneal dialysis continued and regulated according to the laboratory data in this patient during and after the surgery. She recovered well showing an uneventful course and was discharged on postoperative 1 th day.
