피하신경전기자극이 STZ-유도 당뇨 쥐의 척수신경원 흥분성에 미치는 효과 (Effect of Percutaneous Electrical Nerve Stimulation on the Spinal Neuron Excitability in STZ-induced Diabetic Rats.)

  • This study aimed at examining the effects of percutaneous electrical nerve stimulation(PENS) applied to different parts of the streptozotocin(STZ) induced diabetic rats on the change of glucose level and spinal neuron excitability. A total of twenty-eight SD rats, divided into four groups, were used as experiment animal. Experiment group I, the normal control group, was composed of normal rats without diabetes induction. Experiment group was composed II of the rats without any treatment after experimental diabetes induction. Experiment group III was composed of the rats with 2 Hz and $200\;{\mu}s$ of PENS to the acupuncture points related with diabetes for 20 minutes after diabetes induction. Experiment group IV was composed of the rats with 2 Hz and $200\;{\mu}s$ of PENS to the parts unrelated with diabetes for 20 minutes after diabetes induction. The results can be summarized as follow: As for glucose level, the group I showed no change within normal range, and the group III showed significant increase, compared with other groups (p<0.05). As for the change of H latency, M and H amplitude, the group III showed significant differences in decrease of latency and amplitude (p<0.05). As for Hmax/Mmax ratio, the normal and other groups showed no significant differences in decrease of amplitude. It can be concluded from the above results that PENS to the acupuncture points of the STZ-induced diabetic rats was effective for spinal neurone excitability, in particular, for those of the group with PENS to the acupuncture points. This study was conducted in the period of acute diabetes induction, and therefore, further study should be conducted in the period of chronic diabetes to research both acute and chronic diabetes.

대금음자(對金飮子)가 흰쥐의 만성 알콜성 근위축에 미치는 영향 (The Effects of Daekumeumja on Alcohol-induced Muscle Atrophy in Rats)

  • Chronic alcoholic myopathy is one of the most common skeletal muscle disorders. It is characterized by a reduction in the entire skeletal musculature, skeletal muscle weakness, and difficulties in gait. Patients with alcoholic hepatitis and cirrhosis have severe muscle loss that contributes to worsening outcome. Although the myopathy selectively affects Type II (fast twitch, glycolytic, anaerobic) skeletal muscle fibers, total skeletal musculature is reduced. The severity of the muscle atrophy is proportional to the duration and amount of alcohol consumed and leads to decreased muscle strength. The mechanisms for the myopathy are generally unknown but it is not due to overt nutritional deficiency, nor due to either neuropathy or severe liver disease. Skeletal muscle mass and protein content are maintained by a balance between protein synthesis and breakdown and in vivo animal models studies have shown that ethanol inhibits skeletal muscle protein synthesis. Daekumeumja is a traditional Korean medicine that is widely employed to treat various alcohol-induced diseases. Muscle diseases are often related to liver diseases and conditions. The main objective of this study was to assess that Daekumeumja extract could have protective effect against alcoholic myopathy in a Sprague-Dawley rat model. Rats were orally given 25% ethanol (5ml/kg, body weight) for 8 weeks. After 30 minutes, rats were administrated with Daekumeumja extract. Controls were similarly administrated with the vehicle alone. The weights of gastrocnemius, soleus and plantaris muscles were assessed and the morphologic changes of gastrocnemius and plantaris muscles were also assessed by hematoxylin and eosin staining. In results, The muscles from ethanol treated rats displayed a significant reduction in muscle weight and average cross section area compared to Normal group. Daekumeumja extract treated group showed increased muscle weight and muscle fiber compared to the ethanol treated group. It was concluded that Daekumeumja extract showed ameliorating effects on chronic alcohol myopathy in skeletal muscle.

Enhancement of Antinociception by Co-administrations of Nefopam, Morphine, and Nimesulide in a Rat Model of Neuropathic Pain

  • Background: Neuropathic pain is a chronic pain due to disorder in the peripheral or central nervous system with different pathophysiological mechanisms. Current treatments are not effective. Analgesic drugs combined can reduce pain intensity and side effects. Here, we studied the analgesic effect of nimesulide, nefopam, and morphine with different mechanisms of action alone and in combination with other drugs in chronic constriction injury (CCI) model of neuropathic pain. Methods: Male Wistar rats (n = 8) weighing 150-200 g were divided into 3 different groups: 1- Saline-treated CCI group, 2- Saline-treated sham group, and 3- Drug-treated CCI groups. Nimesulide (1.25, 2.5, and 5 mg/kg), nefopam (10, 20, and 30 mg/kg), and morphine (1, 3, and 5 mg/kg) were injected 30 minutes before surgery and continued daily to day 14 post-ligation. In the combination strategy, a nonanalgesic dose of drugs was used in combination such as nefopam + morphine, nefopam + nimesulide, and nimesulide + morphine. Von Frey filaments for mechanical allodynia and acetone test for cold allodynia were, respectively, used as pain behavioral tests. Experiments were performed on day 0 (before surgery) and days 1, 3, 5, 7,10, and 14 post injury. Results: Nefopam (30 mg/kg) and nimesulide (5 mg/kg) blocked mechanical and thermal allodynia; the analgesic effects of morphine (5 mg/kg) lasted for 7 days. Allodynia was completely inhibited in combination with nonanalgesic doses of nefopam (10 mg/kg), nimesulide (1.25 mg/kg), and morphine (3 mg/kg). Conclusions: It seems that analgesic drugs used in combination, could effectively reduce pain behavior with reduced adverse effects.

Antinociceptive and neuroprotective effects of bromelain in chronic constriction injury-induced neuropathic pain in Wistar rats

  • Background: The continuous search for a novel neuropathic pain drug with few or no side effects has been a main focus of researchers for decades. This study investigated the antinociceptive and neuroprotective effects of bromelain in sciatic nerve ligation-induced neuropathic pain in Wistar rats. Methods: Forty-eight Wistar rats randomly divided into eight groups comprised of six animals each were used for this study. Peripheral neuropathy was induced via chronic constriction of the common sciatic nerve. Thermal hyperalgesic and mechanical allodynia were assessed using a hotplate and von Frey filaments, respectively. The functional recovery and structural architecture of the ligated sciatic nerve were evaluated using the sciatic functional index test and a histological examination of the transverse section of the sciatic nerve. The neuroprotective effects of bromelain were investigated in the proximal sciatic nerve tissue after 21 days of treatment. Results: Bromelain significantly (P < 0.05) attenuated both the thermal hyperalgesia and mechanical allodynic indices of neuropathic pain. There were improvements in sciatic function and structural integrity in rats treated with bromelain. These rats showed significant (P < 0.05) increases in sciatic nerve nuclear transcription factors (nuclear factor erythroid-derived-2-related factors-1 [NrF-1] and NrF-2), antioxidant enzymes (superoxide dismutase and glutathione), and reduced membranelipid peroxidation compared with the ligated control group. Conclusions: This study suggest that bromelain mitigated neuropathic pain by enhancing the activities of nuclear transcription factors (NrF-1 and NrF-2) which increases the antioxidant defense system that abolish neuronal stress and structural disorganization.

  • As the average life span has been lengthened and the rate of senile population has been raised, chronic degenerative diseases incident to aging have been increased rapidly and become a social problem. With this social background, recently, oxygen radicals(OR) have toxic effects on Central Nervous System and Peripheral Nervous System and cause neuropathy such as Parkinson's Disease, Alzheimer Disease. The purpose of this study is to examine the toxic effects caused by Xanthine Oxidase(XO) and the effects of herbal extracts such as Jokihaeatang(JHT) on the treatment of the toxic effects. For this purpose, experiments with the cultured cell from the cerebrums of new born mice were done. The results of these experiments were as follows. 1. X0, an oxygen radical, decreased the survival rate of the cultured cells on NR assay, MTT assay and amount of neurofilaments and increased the amount of lipid peroxidation. 2. JHT have efficacy of increasing the amount of neurofilaments.

Hirschsprung's Disease의 감별 진단 (Differential Diagnosis of Hirschsprung's Disease)

  • Hirschsprung's disease (HD) is usually diagnosed in the newborn period and early infancy. The common presentation of HD in newborns consists of a history of delayed passage of meconium within the first 48 hours of life. The differential diagnosis in newborns is one of the clinical challenges of this disorder. A number of medical conditions which cause functional obstruction of the intestines are easily excluded. Neonates with meconium ileus, meconium plug syndrome, distal ileal atresia and low imperforate anus often present in a manner similar to those with HD in the first few days of life. Abdominal radiographs may help to diagnose complete obstruction such as intestinal atresia. Microcolon on contrast enema can be shown in cases with total colonic aganglionosis, ileal atresia or meconium ileus. Suction rectal biopsy or frozen section biopsy at operation is essential for differential diagnosis in such cases. HD is also considered in any child who has a history of constipation regardless of age. Older children with functional constipation may have symptoms that resemble those of HD and contrast enema is usually diagnostic. However, children with other motility disorders generally referred to as chronic idiopathic intestinal pseudoobstruction present with very similar symptoms and radiographic findings. These disorders are classified according to their histologic characteristics.; visceral myopathy, visceral neuropathy, intestinal neuronal dysplasia (IND), hypoganglionosis, immature ganglia, internal sphincter achalasia. Therefore, the workup for motility disorders should include rectal biopsy not only to confirm the presence of ganglion cells but also evaluate the other pathologic conditions.

  • The patient who has congenital hypothyroidism and pseudohypoparathyroidism could have mental retardation even though adequate hormone treatment and cannot endure conventional dental treatment. In this case, general anesthesia is selected to administer effective dental treatment. But, there could be symptoms such as anemia, neuropathy, associated pituitary or adrenal hypofunction, cardiac failure even in euthyroid state. And, bradycardia, mental dullness, hypothermia, slow reflexes can appear in case of inadequate thyroid hormone replacement. Especially, macroglosssia, slow drug metabolism, exaggerated responses to anesthetic agents and decreased ventilatory responses could be problem during general anesthesia. The presentation of hypoparathyroidism also varies depending on the chronicity of the result of hypocalcemia. Muscle spasms/tetany, paresthesias, and seizures may occur in an acute onset. Chronic hypocalcaemia causes fatigue, muscle cramps, lethargy, personality changes, and cerebration defects.

HiGANCNN: A Hybrid Generative Adversarial Network and Convolutional Neural Network for Glaucoma Detection

  • Glaucoma is a chronic neuropathy that affects the optic nerve which can lead to blindness. The detection and prediction of glaucoma become possible using deep neural networks. However, the detection performance relies on the availability of a large number of data. Therefore, we propose different frameworks, including a hybrid of a generative adversarial network and a convolutional neural network to automate and increase the performance of glaucoma detection. The proposed frameworks are evaluated using five public glaucoma datasets. The framework which uses a Deconvolutional Generative Adversarial Network (DCGAN) and a DenseNet pre-trained model achieves 99.6%, 99.08%, 99.4%, 98.69%, and 92.95% of classification accuracy on RIMONE, Drishti-GS, ACRIMA, ORIGA-light, and HRF datasets respectively. Based on the experimental results and evaluation, the proposed framework closely competes with the state-of-the-art methods using the five public glaucoma datasets without requiring any manually preprocessing step.

Neuromodulation of the median nerve in carpal tunnel syndrome, a single-blind, randomized controlled study

  • Background: This study aimed to evaluate the efficacy of pulsed radiofrequency applied using transcutaneous electrodes in carpal tunnel syndrome (CTS). Methods: After randomization, the patients received two cycles of noninvasive pulsed radiofrequency (NiPRF), once weekly, or splinting (the control group) for three months. Clinical evaluations were recorded at baseline and weeks 4 and 8. The Boston Carpal Tunnel Syndrome Questionnaire (BCTQ) was used to determine the functional status and symptom severity. Results: Sixty-two patients were followed up for three months. There was no difference between the groups in the BCTQ scores before and after treatment. The NiPRF group found a significant difference between the BCTQ measurements at all time intervals (paired sample t-test; P < 0.001). In the splint group, there was a significant difference only between the basal-1st month and basal-3rd month (paired samples t-test; P < 0.001). The main effect of the time variable was statistically significant (ANOVA; P < 0.001), but the group variable was not. There was no correlation between the BCTQ results measured at any time and the electroneuromyelogragphy findings in either group. Conclusions: NiPRF effectively improves symptoms and functionality in patients with CTS for up to 3 months. Thus, NiPRF can be considered an easy, safe, and useful alternative treatment modality for CTS.

  • Diabetes is a condition that can be brought on by a variety of different factors, some of which include, but are not limited to, the following: age, a lack of physical activity, a sedentary lifestyle, a family history of diabetes, high blood pressure, depression and stress, inappropriate eating habits, and so on. Diabetes is a disorder that can be brought on by a number of different factors. A chronic disorder that may lead to a wide range of complications. Diabetes mellitus is synonymous with diabetes. There is a correlation between diabetes and an increased chance of having a variety of various ailments, some of which include, but are not limited to, cardiovascular disease, nerve damage, and eye difficulties. There are a number of illnesses that are connected to kidney dysfunction, including stroke. According to the figures provided by the International Diabetes Federation, there are more than 382 million people all over the world who are afflicted with diabetes. This number will have risen during the years in order to reach 592 million by the year 2035. There are a substantial number of people who become victims on a regular basis, and a significant percentage of those people are uninformed of whether or not they have it. The individuals who are most adversely impacted by it are those who are between the ages of 25 and 74 years old. This paper reviews about various machine learning techniques used to detect diabetes mellitus.
