The work of visiting nursing personnel consists mostly of direct nursing and it is absolutely necessary that training of the nursing personnel for infection control be emphasized because most of patients are composed of elderly people with chronic diseases and malnutrition who are highly susceptible to infection. Accordingly. this study was designed to provide basic data for the development of an infection control program in public health centers as well as the promotion of perception and performance of infection control by analysing the perception of importance and performance in the infection control of visiting nursing personnel who carry out visiting nursing services. The subjects of this study consisted of a number of visiting nursing personnel working for 15 public health centers. 15 health-center branches. and 16 health clinics. The collection of data was grounded on a questionnaire method. The questionnaire breaks down to 8 questions about general characteristics and 108 questions in connection with the perception and performance of infection control. The results of this study are as follows: 1. 82 subjects (78.1%) responded that it is necessary to control infection. 79 subjects(75.3%) responded that education is necessary for infection control. 50 respondents (47.6%) answered in the affirmative in regard to the role of infection carriers. 2. The mean score for the perception of importance in infection control was 4.55 and that performance accounts for 4.03. this turns out to be very high. 3. In terms of the perception of importance per each field of infection control. medication. the management of contamination and sterilization, the control of environments and instruments. washing hands and individual hygiene as well as the prevention of cross-infection and the supervision of infection stand at 4.72. 4.64. 4.60. 4.58. 4.52 and 4.21 respectively. In terms of performance. the supervision of infection. the control of environments and instruments, individual hygiene and the prevention of cross infection. medication. washing hands, and the management of contamination and sterilization was 4.21. 4.12. 4.10. 4.03. 3.91 and 3.81 respectively. 4. In terms of the correlation between the perception of importance and the performance of infection control. the score stands high in all fields: this means the high perception of importance leads to high performance statistically (P< .05). 5. In terms of the perception of importance in general characteristics. there is no significant difference while the form of working places. working career and the career as visiting nursing personnel show a significant difference in terms of performance(P< .05). From the results. it is suggested that the visiting nursing personnel should be constantly educated with the aid of the development of infection control programs and that instructions and instruments for the management of infection should be worked out so as to enhance the performance of infection control on the part of nurses in applicable communities.
Isolated sphenoid sinusitis is a rare disease in children, and its symptoms are often nonspecific and confusing. Rarely, severe headache can be the first or only symptom of isolated sphenoid sinusitis. New daily persistent headache (NDPH) is a form of chronic daily headache that may have features of both migraines and tension-type headaches. NDPH is difficult to diagnose and requires a multifaceted approach. Here, we report on a 10-year-old boy and an 11-year-old girl who both presented with typical NDPH symptoms. These patients had no nasal symptoms or signs of infection. Neither nonsteroidal anti-inflammatory drugs nor topiramate had any effect on the headaches. Their neurological and ophthalmological examinations were normal. The results of routine blood work, including thyroid function tests, inflammatory markers, complete blood count, tests for viral infection, and a metabolic panel, were normal. A brain magnetic resonance imaging scan showed isolated sphenoid sinusitis. Both patients' symptoms resolved completely after approximately 1 month of oral antibiotics for sinusitis.
From 1968 through September 1986, the authors have experienced 34 cases of peripheral arterial surgery using various vascular grafts. Almost all patients [32] were men, and age distribution was variable according to the disease entities. There were twenty eight cases of chronic occlusive peripheral vascular disease including ASO [21], Buerger`s disease [6], Aortoenteric fistula complicating infrarenal abdominal aortic aneurysm [1], four cases of vascular trauma, one case of acute arterial embolism [1] and one case of unknown etiology. The indications of operations for chronic vascular disease was intermittent claudication in 48%, rest pain in 45%, ischemic pregangrene or gangrene in 28%, and sensory change in 10% of patients. Types of operation used were arterial bypass in 28 cases [Aortobifemoral in 5, Aortoiliac in 3, Aortofemoral in 4, Aortoiliac with Aortofemoral in 1, Femorofemoral in 1, Femoropopliteal in 8, Femoroperoneal in 2, Axillofemoral in 3 cases of patients], graft interposition in four and patch angioplasty in three cases. Thirty four prosthetic vascular grafts including Dacron, Gore-Tex, Nylon and two autogenous saphenous vein graft and patch were used for vascular reconstruction in thirty four patients. Unfortunately recently performed one vein bypass was failed immediate postoperatively due to severity of disease and poor case selection. The authors experienced five post operative complications: wound infection [1], graft infection [1], bleeding [1], great saphenous neuralgia [1], pseudoaneurysm [1]. Twenty two of thirty four patients were followed up for more than one month and their cumulative patency rate was 81% [17/22] at 1 month and, 31% [7/22] at 5 month.
Seven cases of canine babesiasis were observed by clinically, hematologically and immunologically in the Kyungbook and Pusan area from May 1961 to October 1963. And a survey was conducted on the rising pups for their babesia, infection in the broken out districts of the disease. The results obtained were as follows: 1. Severe cases of canine babesiasis due to the same agent have been encountered in various breed, of dogs in widely separated location in the Kyungpook and Pusan area. 2. The principal symptoms oserved in infected dogs were severe anemia, weakness, inappetence, moderate fever or subnormal body temperature, increased respiration and pulse, palpitation, icterus, hemogrobinuria and redish yellow defecation. 3. Of hematological findings, decrease in erythrocytes count, hemoglobin content was evident and severe anemic changes occured which were associated with macrocytic anisocytosis. The differential leucocytes counts showed a tendency to increased monocytes and decreased monocytes and decreased basophil. 4. The principal anatomic chabges observed in two cases of infected dogs were severe anemic and icteric changes, markedly enlarged spleen, and enlarged liver with distended gall bladder. 5. Haemaphysalis bispinosa was suspected as being the principal vector of the infection. 6. It was suspected that chronic canine babesiasis would prevailed widely in Kyungpook and Pusan area, and that dogs are raised in such infected environment usually suffer only from the mild, chronic form, which may be practically symptomless, while imported dogs usually suffer from the acute form of the disease.
Epilepsy is one of the major chronic neurological diseases affecting many patients. Resection surgery is the most effective therapy for medically intractable epilepsy, but it is not feasible in all patients. Vagus nerve stimulation (VNS) is an adjunctive neuromodulation therapy that was approved in 1997 for the alleviation of seizures; however, efforts to control epilepsy by stimulating the vagus nerve have been studied for over 100 years. Although its exact mechanism is still under investigation, VNS is thought to affect various brain areas. Hence, VNS has a wide indication for various intractable epileptic syndromes and epilepsy-related comorbidities. Moreover, recent studies have shown anti-inflammatory effects of VNS, and the indication is expanding beyond epilepsy to rheumatoid arthritis, chronic headaches, and depression. VNS yields a more than 50% reduction in seizures in approximately 60% of recipients, with an increase in reduction rates as the follow-up duration increases. The complication rate of VNS is 3-6%, and infection is the most important complication to consider. However, revision surgery was reported to be feasible and safe with appropriate measures. Recently, noninvasive VNS (nVNS) has been introduced, which can be performed transcutaneously without implantation surgery. Although more clinical trials are being conducted, nVNS can reduce the risk of infection and subsequent device failure. In conclusion, VNS has been demonstrated to be beneficial and effective in the treatment of epilepsy and various diseases, and more development is expected in the future.
To assess the contribution of tumor necrosis factor $(TNF){\beta}$ +252 polymorphisms to risk and prognosis of hepatocellular carcinoma (HCC), we enrolled 150 pairs of sex- and age-matched patients with HCC, patients with cirrhosis alone, and unrelated healthy controls. $TNF{\beta}$ +252 genotypes were determined by polymerase chain reaction with restriction fragment length polymorphism. Multivariate analysis indicated that $TNF{\beta}$ G/G genotype [odds ratio (OR), 3.64; 95%CI, 1.49-8.91], hepatitis B surface antigen (OR, 16.38; 95%CI, 8.30-32.33), and antibodies to hepatitis C virus (HCV) (OR, 39.11; 95%CI, 14.83-103.14) were independent risk factors for HCC. There was an additive interaction between $TNF{\beta}$ G/G genotype and chronic hepatitis B virus (HBV)/HCV infection (synergy index=1.15). Multivariate analysis indicated that factors associated with $TNF{\beta}$ G/G genotype included cirrhosis with Child-Pugh C (OR, 4.06; 95%CI, 1.34-12.29), thrombocytopenia (OR, 6.55; 95%CI, 1.46-29.43), and higher serum ${\alpha}$-fetoprotein concentration (OR, 2.53; 95%CI, 1.14-5.62). Patients with $TNF{\beta}$ G/G genotype had poor cumulative survival (p=0.005). Cox proportional hazard model indicated that $TNF{\beta}$ G/G genotype was a biomarker for poor HCC survival (hazard ratio, 1.70; 95%CI, 1.07-2.69). In conclusion, there are independent and additive effects between $TNF{\beta}$ G/G genotype and chronic HBV/HCV infection on risk for HCC. It is a biomarker for poor HCC survival. Carriage of this genotype correlates with disease severity and advanced hepatic fibrosis, which may contribute to a higher risk and poor survival of HCC. Chronic HBV/HCV infected subjects with this genotype should receive more intensive surveillance for early detection of HCC.
Purpose: Human papillomavirus (HPV) infection plays an important role in the development of cervical cancer, but the prevalence of HPV infection in women of Shenzhen city remains unclear. The present study was performed to describe the change of cervical HPV infection in females who participated in voluntary cervical cancer screening from 2006 to 2010 in Shenzhen city, China. Methods: A total of 4, 413 women were recruited. HPV infections were genotyped by polymerase chain reaction (PCR) and reversed dot blot hybridization in Shenzhen Maternity and Child Health Hospital. Results: The prevalence of HPV infection was 13.8%. The five most commonly found HPV types were HPV16 (3.47%), HPV58 (1.68%), HPV33 (1.38%), HPV43 (1.36%) and HPV18 (1.27%). The secular trends of major HPV type-specific were diverse. Among of them, the prevalence of HPV18 increased sharply while others increased slowly or even decreased in the period. The change of total HPV, single HPV and multiple HPV infection were similar during the five years. Conclusions: Our findings suggested that HPV infection is common with HPV16 and HPV 58 as the primary subtypes in women in Shenzhen city.The prevalence of HPV 18 infection is increasing faster than any others, which will lead it to be one of the main subtypes in this city in the future.
Background: Brain abscess is a life-threatening condition that occurs due to complications during a neurosurgical procedure, direct cranial trauma, or the presence of local or distal infection. Infection in the oral cavity can also be considered a source of brain abscess. Case presentation: A 45-year-old male patient was transported with brain abscess in the subcortical white matter. Navigation-guided abscess aspiration and drainage was performed in the right mid-frontal lobe, but the symptoms continued to worsen after the procedure. A panoramic radiograph showed alveolar bone resorption around the maxillary molars. The compromised maxillary molars were extracted under local anesthesia, and antibiotics were applied based on findings from bacterial culture. A brain MRI confirmed that the three brain abscesses in the frontal lobe were reduced in size, and the patient's symptoms began to improve after the extractions. Conclusion: This is a rare case report about multiple uncontrolled brain abscesses treated by removal of infection through the extraction of maxillary molars with odontogenic infection. Untreated odontogenic infection can also be considered a cause of brain abscess. Therefore, it is necessary to recognize the possibility that untreated odontogenic infection can lead to serious systemic inflammatory diseases such as brain abscess. Through a multidisciplinary approach to diagnosis and treatment, physicians should be encouraged to consider odontogenic infections as a potential cause of brain abscesses.
Helicobacter pylori (H. pylori) infection is uncommon in developed countries, yet is common in underdeveloped and developing countries. Infection rate of H. pylori is minimally influenced by economic, environmental, and public health status and genetic factors. Korea is a developing country with a high incidence of H. pylori infection and gastric carcinoma, which is one of the leading causes of death. For this reason, accurate clinical and pathologic data on H. pylori-associated disease are very important. Intestinal metaplasia accompanies chronic gastritis and increases the risk of gastric carcinoma. For this reason, the relationship between H. pylori infection and intestinal metaplasia is very closely linked. Because of this, as the antecedent condition is guessed, it examines the relationship of the H. pylori and the intestinal metaplasia. Intestinal metaplasia is thought to be the basis in the development of intestinal type gastric carcinomas. Recent investigations showed that inflammatory reaction in the gastric fundus affect the development of gastric carcinogenesis. To verify neutrophilic activity in the gastric fundus and development of intestinal metaplasia in both gastric fundus and antral mucosa, their relationship was studied using 159 healthy patients who had undergone gastric endoscopic biopsies without any identifiable pathologic disesaes. When neutrophilic activity accompanied, incidence of intestinal metaplasia was significantly increased (p<0.05). H. pylori infection was statistically and significantly associated with the presence of intestinal metaplasia (p<0.05). These results suggest that H. pylori infection affected the development of intestinal metaplasia in the stomach. These results will help our understanding of H. pylori infection in the pathogenesis of intestinal metaplasia, a preneoplastic condition of the stomach. To reduce the incidence of gastric adenocarcinoma, eradication treatment of H. pylori is recommended when there's a neutrophilic activity in the gastric fundus.

Background: Herpes simplex virus type 1 (HSV-1), known to latently infect the host's trigeminal ganglion, can lead to severe herpes encephalitis or asymptomatic infection, potentially contributing to neurodegenerative diseases like Alzheimer's. The virus generates reactive oxygen species (ROS) that significantly impact viral replication and induce chronic inflammation through NF-κB activation. Nuclear factor E2-related factor 2 (Nrf2), an oxidative stress regulator, can prevent and treat HSV-1 infection by activating the passive defense response in the early stages of infection. Methods and results: Our study investigated the antiviral effects of ginsenoside Rg5, an Nrf2 activator, on HSV-1 replication and several host cell signaling pathways. We found that HSV-1 infection inhibited Nrf2 activity in host cells, induced ROS/NF-κB signaling, and triggered inflammatory cytokines. However, treatment with ginsenoside Rg5 inhibited ROS/NF-κB signaling and reduced inflammatory cytokines through NRF2 induction. Interestingly, the Nrf2 inhibitor ML385 suppressed the expression of NAD(P)H quinone oxidoreductase 1(NQO1) and enhanced the expression of KEAP1 in HSV-1 infected cells. This led to the reversal of VP16 expression inhibition, a protein factor associated with HSV-1 infection, thereby promoting HSV-1 replication. Conclusion: These findings suggest for the first time that ginsenoside Rg5 may serve as an antiviral against HSV-1 infection and could be a novel therapeutic agent for HSV-1-induced neuroinflammation.
