노년기 건강에 있어 미병(未病) 관리의 의의: 한의학과 심리학의 연관성을 중심으로 (The Significance of Managing "Mibyeng" in Geriatric Health by Focusing on the Connection between Korean Medicine and Psychology: A Review)

  • Objectives: The aim of this study is to review the significance of managing "Mibyeong" in geriatric health in Korean medicine and psychology. Methods: We performed a literature search using OASIS, Koreantk, RISS, DBpia, and PubMed databases. We retrieved research articles using the keywords related to mibyeong (未病), sub-health (亞健康), Korean-medicine, psychology and geriatric health. Results: Both Korean medicine and psychology emphasize biological, psychological, and social balance and well-being. In addition, both value individual personality and constitution. Mibyeong is a Korean medical concept suggesting sub-health. Elderly individuals who are in a state of Mibyeong are frail and vulnerable to disorders and illness. Since depression, chronic diseases, low physical and mental activity have fatal outcomes in frail and elderly subjects, positive psychological support and active management of Mibyeong are very important. Conclusions: Geriatric health conditions are associated with chronic disease and multiple risk factors, including physiological, psychological, social, and economic conditions. A preventive and integrative approach is essential for geriatric health as prevention is better than cure for most geriatric diseases and illnesses.

산업분류와 만성질환 유무와의 관계 (The Relationship between Industrial Classification and Chronic Disease)

  • Purposes: The industry has specialized and fragmented than in the past. As a factor of economic growth and industrialization, the number of people employed in primary industry decreased and the number of people employed in secondary and third industry continuously increased. In modern times, incidence of chronic disease is increasing according to industrial development. So, the purpose of this study was to analyze the chronic disease according to Clark's industrial classification. Methodology: Data were derived from the 2012 Korea Health Panel. The sample was made up of 7,132 adult participants aged 20 or over selected Korea Health Panel by probability sampling from Korea. Binary logistic regression analysis was conducted to examine the main factors associated with chronic disease. Findings: The significant factors associated with chronic disease were gender, age, marital status, household member, education level, insurance type, disability, BMI, and industrial classification. Female, elderly, divorced(including bereavement, missing and separation), one-person households, less than high school graduation, medical aid, disability, obese and primary industry were confirmed chronic disease increases. Practical Implications: The study finds that primary industry's prevalence of chronic disease was higher than secondary and third industry. Therefore, this study aims to management and effort of the worker who engaged in the primary industry. Policy development is required to address inequality or popularization of the differences in these factors by conducting a study to define the working conditions and socio-economic factors between industry.

직업성 만성요통 환자와 정상성인에서 동요 유발 시 근 수축 개시시간 비교 (Comparison of Muscle Onset Times During Perturbation Between Subjects With and Without Work-Related Chronic Low Back Pain)

  • The purpose of this study was to compare the onset times of muscle activities and the order of muscle firing in erector spinae, multifidus, rectus abdominis and biceps brachii during perturbation between subjects with and without work-related chronic low back pain (LBP). Twenty-nine subjects, 14 with and 15 without LBP, participated in this study. The muscle responses were measured by surface EMG (electromyography) during perturbation in eye opened and eye closed conditions. The EMG onset times of the erector spinae, multifidus, rectus abdominis and biceps brachii were similar between groups in eye closed condition. But the onset times of the erector spinae, multifidus, rectus abdominis were significantly delayed in subjects with LBP in eye opened condition. The results provide an evidence for impaired feed-forward control of the trunk muscles in subjects with LBP. Further studies are needed to identify whether the impaired feed-forward control of the trunk muscles is the contributing factor to LBP.

  • Objectives : The purpose is to introduce the recent large-scale clinical researches for safety, efficacy and effectiveness of acupuncture in Germany. Results : In 2000, the German Federal Committee of Physicians and Health insurer proposed that large research initiatives on acupuncture, Acupuncture Model Projects(Modellvorhaben Akupunktur), could be conducted by health insurance companies for several pain that acupuncture is syndromes to justify the insurance-based reimbursement. Accordingly, 4 clinical researches were carried out; the Acupuncture Safety and Health economics studies(ASH), the Acupuncture Randomised Trial(ART), the Acupuncture in Routine Care studies(ARC), and the German Acupuncture trial(GERAC). Meanwhile, ASH is a prospective observational study for safety and costs. ART and GERAC are composed of RCTs for efficacy. ARC includes 6 pragmatic RCTs with additional non-randomized cohort study for effectiveness. We investigated the papers related to them and discussed about the outcomes. The researches showed that acupuncture is effective in practice for several chronic conditions such as migraine, tension-type headache, chronic low back pain, osteoarthritis of knee, dysmenorrhea, and allergic rhinitis. Based in part on them, the German health authorities decided that acupuncture would be included into routine reimbursement by social health insurance funds for chronic low back pain and chronic osteoarthritis of the knee in 2006. Conclusions : The German clinical researches may suggest the clues for establishing the evidence of acupuncture treatment.

가족부담감 측정도구의 신뢰도와 타당도 (The Reliability and Validity of Family Burden Questionnaire)

  • Purpose: This study was conducted to test the reliability and validity of Family Burden Questionnaire. Method: The subjects were 156 caregivers of children with acute or chronic disease. To test the reliability, internal consistency using Cronbach's α coefficient was analyzed, and factor analysis, known-group technique, and concurrent validity were utilized for validity test. Result: Cronbach's α coefficient of the tool was .89. Six factors were identified and explained 58.7% of the total variance. Through analysis using known-group technique, the difference of family burden between acute and chronic disease groups was statistically significant(t= -4.09, p<.001). Correlations with mood state, other family burden score by Family Burden Interview Schedule, quality of life, and health symptoms were also relatively high and statistically significant. Conclusion: Family Burden Questionnaire showed a relatively high validity and reliability to measure the burden of caregivers caring patients with various chronic conditions in Korea. For the further study, it may be reconsidered to identify the factor structure of the Family Burden Questionnaire with the various subjects from different age groups.

The Impact of Air Pollution, Including Asian Sand Dust, on Respiratory Symptoms and Health-related Quality of Life in Outpatients With Chronic Respiratory Disease in Korea: A Panel Study

  • Objectives: Air pollution is a growing concern in Korea because of transboundary air pollution from mainland China. A panel study was conducted to clarify the effects of air pollution on respiratory symptoms and health-related quality of life (HR-QoL) in outpatients with and without chronic obstructive pulmonary disease (COPD) in Korea. Methods: Patients filled out a questionnaire including self-reported HR-QoL in February and were followed up in May and July. The study was conducted from 2013 to 2015, with different participants each year. Air quality parameters were applied in a generalized estimating equation as independent variables to predict factors affecting HR-QoL. Results: Lower physical fitness scores were associated with Asian sand dust events. Daily activity scores were worse when there were high concentrations of particulate matter (PM) less than $10{\mu}m$ in diameter ($PM_{10}$). Lower social functioning scores were associated with high PM less than $2.5{\mu}m$ in diameter and nitrogen dioxide ($NO_2$) concentrations. High $NO_2$ concentrations also showed a significant association with mental health scores. Weather-related cough was prevalent when $PM_{10}$, $NO_2$, or ozone ($O_3$) concentrations were high, regardless of COPD severity. High $PM_{10}$ concentrations were associated with worsened wheezing, particularly in COPD patients. Conclusions: The results suggest that PM, $NO_2$, and $O_3$ cause respiratory symptoms leading to HR-QoL deterioration. While some adverse effects of air pollution appeared to occur regardless of COPD, others occurred more often and more intensely in COPD patients. The public sector, therefore, needs to consider tailoring air pollution countermeasures to people with different conditions to minimize adverse health effects.

  • 통증 생리의 생물학적 측면에 대한 새로운 지식이 폭발적으로 늘어나게 되면서, 통증에 대한 개념도 매우 많이 바뀌고 있다. 통증의 원인에 심리적, 정서적 요인들이 많이 작용하며 개인과 가족역동의 중요성, 빈번한 정신과 질환의 공존, 만성통증의 치료에 있어서 많은 정신과 약물의 사용, 그리고 다양한 치료적 접근의 중요성의 부각 동을 고려할 때 정신과 의사들은 만성통증 환자의 치료에 참여할 수 있는 능력이 있으므로 적극적으로 참여해야 한다. 그리고 현재 효과적인 통증치료에 다양한 환자의 치료가 필요한 것이 인정되고 있으며, 통증치료에서 좋은 치료 결과를 보이는 외국의 대부분의 통증센터나 통증크리닉에서는 정신과 의사의 역할이 중요하게 인식되고 있으나 아직까지 국내에서는 이러한 인식의 부족으로 정신과 의사의 참여가 거의 없어서 다양한 치료적 접근을 하지 못하고 있다. 이에 저자는 통증장애에 대하여 정신과 의사들의 관심을 환기하고 적극적인 참여를 유도하기 위하여 통증장애 환자 3 증례를 보고하고, 이에 관하여 고찰하였다. 통증장애를 다루기 위해서는 통증에 대한 생물-정신-사회적인 관점 모두에서의 전반적인 고찰을 다루어야 하지만, 본 논문에서는 심리적 요인과 관련된 통증장애 환자에 대한 종례만을 보고하므로 이를 중심으로 다루었다.

만성질환 진단이 노인의 우울수준에 미치는 영향: 주요 5대 만성질환의 초기 진단기를 중심으로 (The Effect of Transition to Living with Chronic Diseases on Depressive Symptoms)

  • 본 연구는 노인인구의 급속한 증가에 따른 만성질환자 증가 추이를 고려하여, 만성질환에 의한 정신건강 악화에 대해 다루고자 하였다. 이를 위해, 한국인의 사망원인 중에서 상위를 차지하는 5가지의 주요 만성질환(당뇨병, 암, 만성폐질환, 심장질환, 뇌혈관 질환)을 선정하였다. 1-2차년도 고령화연구패널조사(KLoSA)에 참여한 45세 이상의 중고령자를 대상으로, 만성질환 진단자로의 전이와 우울 수준이 어떠한 관계가 있는지 살펴보았다. 연구방법은 SPSS 25.0을 사용한 다중회귀분석을 실시하여, 만성질환자로 전이한 경우 및 5가지 각 질환별 전이한 경우가 우울에 어떠한 영향을 미치는지 분석하였다. 본 연구결과에 따르면, 만성질환 진단자로 전이는 우울수준을 유의미하게 증가시키는 것으로 나타났다. 질환별로 살펴보았을 때, 암, 만성 폐질환, 뇌혈관 질환이 진단된 중고령자는 우울 수준이 높아졌으나, 당뇨병과 심장질환이 진단된 경우는 우울에 유의한 영향을 미친다는 증거가 발견되지 않았다. 본 연구결과를 바탕으로 만성질환자로의 전이와 우울관리에 대한 논의가 이루어졌다.

실시간 압력정보 제공 진동 촉각 피드백이 만성 뇌졸중 환자의 정적균형능력과 체중 지지율에 미치는 영향 - 예비실험연구 (Effects of Vibrotactile Bio-Feedback Providing Pressure Information in Real Time on Static Balance and Weight Bearing Rate in Chronic Stroke Patients - Pilot Study)

  • Purpose : The purpose of this study is to find out if it helps to improve static balance ability and weight bearing rate for chronic stroke patients with poor balance in clinical intervention through a method of correcting movement errors while performing a task by vibrotactile bio-feedback providing pressure information. Methods : Fifteen chronic stroke patients (12 male and 3 female) were participated in this study. To examine the effects of vibrotactile bio-feedback and general standing without bio-feedback on static balance ability and weight distribution symmetric index in all subjects randomized with R Studio. The static balance ability and weight distribution symmetric index of the participants was evaluated using a force plate. A paired t-test was used for comparison of each conditions. Statistical significance was set at α=0.05. Results : The comparisons of static balance ability and weight distribution symmetric index in chronic stroke patients after two different condition are as follows. In the static balance ability and weight distribution symmetric index, the vibrotactile feedback providing pressure information showed a significant difference compared to none feedback (p<.001). Conclusion : The vibrotactile bio-feedback providing pressure information in real time can support an improve in static balance ability, uniform weight bearing rehabilitation in chronic stroke patients. In the future, it is hoped that a follow-up study that provides a better direction of intervention compared to various feedback interventions commonly used in clinical practice.

최근 1년 내 한방의료이용에 영향을 미치는 요인 - 2020년 한방의료이용 실태조사를 활용하여 (Factors Affecting the Utilization of Korean Medicine in Last 1 Year : Analysis of the National Survey on Korean Medicine Utilization, 2020)

  • Objectives : This study aimed to identify the factors that affect the utilization of Korean Medicine within the past year. Methods : I analyzed micro-data from the Korean Medicine Utilization Survey (2020), which surveyed the general public. "User (in last 1 year)" were defined as those who had used Korean Medicine within the past year, while "Non-user (in last 1 year)" were defined as those who had used it in their lifetime but not within the past year. I examined several factors, including general characteristics, perceptions of Korean Medicine, satisfaction with Korean Medicine, and intention for future utilization. I conducted the Rao-Scott chi-squared test and complex sample logistic regression analysis. Results : I analyzed a total of 3,762 respondents, of which 1,442 (36.3%) were User (in last 1 year) and 2,320 (63.7%) were Non-user (in last 1 year). User (in last 1 year) and Non-user (in last 1 year) differed significantly in terms of demographic characteristics, perceptions, and satisfaction with Korean Medicine. Women, the elderly, those with low education levels, non-married individuals, those with low-income, chronic diseases, and poor subjective health conditions were significant factors. Those who used Korean Medicine within the past year were more knowledgeable about Korean Medicine than those who had not used it recently, and recognized herbal medicine as safer. In terms of satisfaction, User (in last 1 year) were more satisfied than Non-user (in last 1 year). The more women, the older, the more aware of Korean Medicine, the safer they perceived herbal medicine, the more satisfied they were with its use, the better their subjective health condition, and the absence of chronic diseases, the less they experienced Korean Medicine. Conclusions : The study showed that positive experiences and perceptions of Korean Medicine increase the likelihood of its utilization. Individuals with better subjective health conditions and without chronic diseases have a lower possibility of using Korean Medicine.
