  • Background: Neuroimaging can play a crucial role in discovering potential abnormalities to cause secondary headache. There has been a progress in the fields of headache diagnosis and neuroimaging in the past two decades. We sought to investigate neuroimaging findings according to headache disorders, age, sex, and imaging modalities in first-visit headache patients. Methods: We used data of consecutive first-visit headache patients from 9 university and 2 general referral hospitals. The International Classification of Headache Disorders, third edition, beta version was used in headache diagnosis. We finally enrolled 1,080 patients undertook neuroimaging in this study. Results: Among 1,080 patients (mean age: $47.7{\pm}14.3$, female: 60.8%), proportions of headache diagnosis were as follows: primary headaches, n=926 (85.7%); secondary headaches, n=110 (10.2%); and cranial neuropathies and other headaches, n=43 (4.1%). Of them, 591 patients (54.7%) received magnetic resonance imaging (MRI). Neuroimaging abnormalities were found in 232 patients (21.5%), and their proportions were higher in older age groups and male sex. Chronic cerebral ischemia was the most common finding (n=88, 8.1%), whereas 76 patients (7.0%) were found to have clinically significant abnormalities such as primary brain tumor, cancer metastasis, and headache-relevant cerebrovascular disease. Patients underwent MRI were four times more likely to have neuroimaging abnormalities than those underwent computed tomography (33.3% vs. 7.2%, p<0.001). Conclusions: In this study, the findings of neuroimaging differed according to headache disorders, age, sex, and imaging modalities. MRI can be a preferable neuroimaging modality to identify potential causes of headache.

  • A case of intradural extramedullary cavernous angioma is presented with headache, dizziness, and bilateral sensorineural hearing loss caused by an intracranial superficial hemosiderosis. It was incidentally found in a patient with a 3-month history of sustained headache, dizziness and a 3-year history of hearing difficulty. The neurological examination was unremarkable in the lower extremity. MR images showed an intracranial superficial hemosiderosis mostly in the cerebellar region. Myelography and MR images of the thoracolumbar spine revealed an intradural extramedullary mass, which was pathologically proven to be a cavernous angioma. T12 total laminoplastic laminotomy and total tumor removal were performed without any neurologic deficits. The patient's symptoms, including headache and dizziness, have been absent for three years. Intradural extramedullary cavernous angioma can present with an intracranial superficial hemosiderosis as a result of chronic subarachnoid hemorrhage.

  • Epidural steroid injection is one method of releiving chronic back pain. However, problems with the loss of resistance to air technique include the possible subarachnoid or subdural injection of air resulting in headache, venous air embolism, and the introduction of air bubbles into the epidural space. Pneumocephalus is a rare complication of epidural block for epidural steroid injection. We report a case of a 58-year-old woman who developed a severe headache and posterior nuchal pain with incomplete oculomotor palsy due to pneumocephalus occuring after an epidural steroid injection.

  • Objective: This study aimed to produce a description of features of chronic fatigue patients visiting an Oriental Hospital. Methods: 121 patients (75 male and 46 female) who visited a fatigue-care center were enrolled in this study, and the main complaints besides fatigue feeling, symptom differentiation, and health-related factors were analyzed. Results: On a visual analogue scale (VAS), average severity of fatigue was 7.0 (male 6.77 and female 7.4; 0 indicates no fatigue vs. 10 indicates intolerable fatigue), and the main complaints included headache (male and female) and dyspepsia (female). Patients generally slept about 7 hrs and caught a cold 2.4 times per year (same in both male and female). 54.6 % of patients used functional supplements, and red ginseng, vitamin and herbal medicine were chosen in that order. Conclusions: Chronic fatigue is a main target of traditional Korean medicine (TKM). This study can be helpful to design TKM-based treatment for chronic fatigue-related symptoms.

  • In most cases, subdural hematoma (SDH) is regarded as a complication of head injury and nontraumatic causes are rare. Moreover, spontaneous chronic SDH in child or adolescent is very unusual. Here, we present the case of a healthy 14-year-old girl who was diagnosed as a spontaneous chronic SDH. The patient presented with severe headache following blurring of vision two weeks ago without any history of trivial head injury. Computed tomography and magnetic resonance imaging depicted a chronic SDH. The cause of the hematoma was not established. After performing burr hole drainage of the hematoma, the patient made an uneventful recovery. We explore the potential risk factors and pathophysiology implicated in this condition. Possible pathogenic mechanisms of this unique case are discussed and a review of the pertinent literature is included.

  • Objectives : The purpose of this study is to analyze the characteristics of tinnitus patients according to the presence of headache. Methods : The medical records of 110 tinnitus patients who visited oo Korean Medicine Clinic from June 2019 to June 2021 were retrospectively reviewed. Clinical characteristics including gender, age, onset, cause, tinnitus site, severity of tinnitus and accompanied symptoms were analyzed. Results : Among all patients, 40s and 50s and chronic tinnitus over 1 year was the most frequent group. After an average of 2 months of treatment, 61 patients(55.5%) showed improvement, and there were prescribed more frequently 50s and 60s and with tinnitus grade mild to moderate group. The proportions of female(65%, p=0.012) and herbal dialectics which 'tonify and replenish' and 'tranquilize'(32.5%, p=0.043) were prescribed more frequently in patients with headache than those without headache. Dizziness, ear pain, heart symptoms, fatigue, and neck stiffness were more frequent in patients with headache(p<0.05). In patients with improvement in tinnitus, tinnitus grade improved significantly in patients with improvement in headache compared to those with no improvement in headache(1.83±0.79 vs 0.80±0.84, p=0.018). Conclusions : In tinnitus patients treated with Korean medicine, there was no significant difference in the improvement of tinnitus according to the presence or improvement of headache, but the patients with improvement of both tinnitus and headache showed significantly more degree of improvement in tinnitus severity than patients with only tinnitus improved. Based on these findings, further study is needed regarding the correlation headache and tinnitus in clinical setting.
