A Case of Cytomegalovirus Colitis in Chronic Adult T-Cell Leukemia/Lymphoma (만성 성인T세포백혈병/림프종 환자에서 병발한 거대세포바이러스 장염 1예)

  • Adult T-cell leukemia/lymphoma (ATLL) is a malignancy of mature T-cells caused by the human T-cell lymphotrophic virus type I (HTLV-D. HTLV-I is endemic in some areas in Japan, the Caribbean basin, and Africa but has low prevalence in South Korea. Patients with ATLL are susceptible to opportunistic infections such as cytomegalovirus (CMV) infection, but CMV infection in chronic ATLL is uncommon. Reported herein is a case involving a 44-year-old woman with chronic ATLL who presented the symptoms of fever and diarrhea. She was suspected to have acute-type ATLL but was later diagnosed with CMV colitis.

  • In livestock, diagnosis of disease is found on body temperature variation like a human. In case of cattle, body temperature variation can estimate disease that milk fever, toxication, diarrhea, dyspepsia, chronic enteritis, influenza, pneumonia, anthrax. So we are suggested the temperature measurement system for livestock. This system will be useful to a stock farmer and alternative that a worker.

  • Eosinophilic gastroenteritis is a rare condition of unknown etiology characterized by peripheral eosinophilia, eosinophilic infiltration of the gastrointestinal tract, and gastrointestinal symptoms. Eosinophilic gastroenteritis is generally classified according to the Klain classification: predominant mucosal, muscular, and subserosal disease. Mucosal involvement may result in abdominal pain, nausea, vomiting, diarrhea, weight loss, anemia, protein-losing enteropathy, and intestinal perforation. Patients with muscular layer disease generally have obstructive symptoms. Subserosal eosinophilic infiltration may result in development of eosinophilic ascites. Most commonly, the stomach, duodenum, and small bowel are involved. A 13-year-old girl came to our hospital presenting with chronic, intermittent abdominal pain. She showed peripheral eosinophilia and biopsy specimen of the duodenum revealed eosinophilic infiltration of the mucosal layer. We here report a case of eosinophilic gastroenteritis.

Microscopic Identification of the 'Chung Kwei Won' (청쾌원의 현미감정 연구)

  • 'Chung Kwei Won(淸快園)' is a Chinese patent medicine, which is used for stomachic disorder, acute and chronic indigestion, dispepsia, vomiting and diarrhea in Korea. This preparation consists of 16 kinds of powdered crude drugs. For the identification of individual crude drugs in such powdery mixtures, microscopic method may be used conviniently as it requires only a small amount of specimens. In this paper, the effectiveness of this method is exemplified by the identification of the ingredients in 'Chung Kwei Won' which contains 16 powder crude drug ingredients.

A Canine Lymphoma Patient Diagnosed by the Cytologic Examination of Transudative Ascites (누출액성 복수의 세포학적 검사로 진단된 림프종 1예)

  • A 12-year old neutered male Yorkshire terrier dog was presented to the Veterinary Medical Teaching Hospital of Seoul National University with a history of chronic intermittent diarrhea, vomiting, anorexia and weight loss of 2-months duration. On presentation, he was very cachexic and had ascites. Abnormal findings on a complete blood count and chemistry profile included mild anemia, leukocytosis, panhypoproteinemia, hypocholesterolemia, decreased blood urea nitrogen (BUN) and increased serum bile acids. Radiographic findings indicated microhepatica. Peritoneal fluid analysis was consistent with transudates (total protein < 2.5 g/dl, total nucleated cell count = 2,200/ul) and cytologic examination of the fluid revealed neoplastic lymphoblasts. From these findings hepatic dysfunction and protein-losing enteropathy were attributable to abdominal lymphoma. This case suggests that cytologic examination is important in diagnosing underlying diseases of ascites, even if it is transudative effusion.

Intestinal intussusception in elefant

  • Five years old Asian male elephant (Elephas maximus), transported from Malaysia, had showed slight inappetence and mild diarrhea from begining of stay at the zoo, and died after three months clinical therapy. Necropsy of this elephant grossly demonstrated ileocecal intussusception as the cause of the death, coupled with purulent nephritis together and nephrolithiasis, mild liver degeneration, numerous subepiacrdial and subendocardial hemorrhages. Bacteriological culture was conducted from renal pelvis revealing Klebsiella pneumonia infection. Histological findings included diffuse chronic interstitial nephritis with numerous amount of lymphocytes and giant cells. This is a unique combination of pathological findings consisting of intestinal intussusception and purulent pyelonephritis in elephant. A Clinical Observation of Seroconversion on Chronic Hepatitis B Treated with a Herb Prescription

  • Objective : Report on a hepatitis B patient with abnormal AST/ALT, HBeAg(+), and habitual diarrhea stemming from irritable colon. Methods : Lab-evaluation of intervals of herb medicine treatment for a patient with hepatitis B with HBeAg and irritable colon. Result & Conclusion : AST/ALT level was normalized and did not rise again. Seroconversion to HBeAb repeatedly appeared just druing the time of the treatment, but typically repeated seroconversion between treatment period and no treatment period confirmed that the treatment was effective for the immune system against hepatitis B. The exact mechanism is not clear but the result provides an indication that oriental herbal medicine has potential capacity to treat liver diseases.

  • Sengwe Dan(生胃丹) is a Chinese patent medicine, which is used for stomachic disorder, heavy drinking, voracious eating, acute and chronic indigestion, diarrhea, nausea and vomiting in Korea. In the future, the demand for the drug is likely to increase, as the number of aged people increases. This medicine is in the large of pills consisting 19 kinds of powdered crude drugs. For the identification of individual ingredients in such powdery mixtures, microscopic method may advantageously be used as it requires only a small amount of specimens. In this paper, the effectiveness of this method is exemplified by the identification of the ingredients in Seng-We-Dan which contains 19 powdered crude drug ingredients.

  • A 69-year-old Korean man was admitted to emergency room with complaints of abdominal pain, vomiting, and diarrhea, Laboratory tests revealed eosinophilia, anemia, hypoproteinemia, and hyponatremia. The gastric mucosa showed whitish mottled and slightly elevated lesions on the body angle of antrum. Microscopically, chronic gastritis with incomplete intestinal metaplasia was observed. Many adult worms, larvae, and eggs in cross sections were located in the crypts. Furthermore, the filariform larvae of Strongyloides stercoralis with a notched tail were detected through the culture.
