A Direction of Child-care Development through the View of Home Economics (가정학적 접근을 통한 보육의 발전 방향 제시)

  • The purpose of this study is to seek for a direction of child-care development through the basic concepts of home economics which have taken charge of much parts of the studies in child-care in the early years. When the problem of child-care is studied in view of the developmental process or the basic concepts of home economics, it is summed up as follows; First, on the basis of human ecological approach, when we study child-care, it needs to emphasize the mutual relationship between the macro-environment in connection with home and child-care. The study of child-care have to find out the relationship between the micro-environment in connection with children and the macro-environment in connection with home. And on the basis of this study, we must endeavor to build up the desirable environment of child-care. Second, on the basis of the concept of Institutional household, we mud be concerned about the developmental stages of young children, and emphasize 'the principle of love and care'. This means that the institution of child-care must be considered as a place which enlarges the merits of home and complements the defects of home through the strengthener of the function of home. Third, on the basis of healthy family, child-care service must support and complement the function of bringing of home in view of 'total healthiness of home'. Child-care service must be offered to be able to be chosen according to the total conditions of family such as the employment of mother, the family life cycle, the life style, the socioeconomic status of family.

Long-Distance Mothers' Foster Care Types, Separation Anxiety, and Guilt in Foster Care (주말부모 어머니의 자녀양육실태, 격리불안과 죄책감)

  • The purpose of this study was to find out the characteristics of foster care of long-distance parents who meet their children on weekend and to examine the relationships among the separation anxiety and maternal guilt in foster care. The subject consisted of 138 employed mothers who are living separately with their children. The instruments used for this study were the Separation Anxiety Scale(Cho & Park,1992), the Maternal Guilt Scale(Kim & Kang, 1997), the Parental Satisfaction Scale(Hyun & Cho,1994), and the Parental Stress Scale(Park,1994). The main results of this study were as followings: 1. Mothers had a tend to rely on family members expecially grandparents for foster care of their children. They usually have visited to meet their children weekend and made a phone call once a day. Parental satisfaction in foster care was reported to be moderately high level. 2. The subject’s separation anxiety was found to be high, and it was strong positive relationships to maternal guilt feeling in foster care. The results of this study have implications for both formal and informal support systems of employed mothers with children. The findings of this study may used as basis for understanding long-distance parents’problems in foster care, developing support programs, and public policy for employed mothers.

Development of a Home Health Care Model in the Public Health System -Visiting Health Service by Community Health Practitioners (공공보건조직에서의 방문간호사업모형개발 : 보건진료소 방문간호사업)

  • The purpose of this study was to develop a home health care model in the public health system and to test the effectiveness of the model. Seven com-munity health practitioners in Yon- Cheon county. Kyunggi province, carried out home health care service for this research. The subjects of the home health care were a total of 111 community residents with chronic health problems and risk-prone infants and children; 29 persons with hypertension, 18 persons with diabetes, 12 persons with neurologic problems, 12 elderly, and 40 infants and children. During the period of study, from December, 1993 to March, 1995, a demonstrative home health care model was developed in the Yon-Cheon County community health centers with the cooperation of the Yon-Cheon Medical Center and Yon-Cheon Public Health Center for the first six months. A home care practice manual and recording system for home visits were also co-developed by the researchers and community health practitioners. Four workshops and monthly conferences were held for this purpose. Actual home care practice took place for two months, and on-going evaluation and replanning accompanied this process. The result of the evaluation of home care service were as follows. 1) For persons with hypertension, diabetes, neurologic problems, there was significant improvement in knowledge of disease and care, but no significant difference was seen in health behavior or symptoms after home care service. 2) No significant difference was seen in level of self esteem or depression after reminiscence therapy among 12 elderly subjects. 3) There were significant differences in satis-faction toward child rearing and parental sup-port, but no significant difference In education needs for parental role after home care service among parents of infants and children. 4) There was significant improvement in the quality of life among the subjects after the home care service. 5) Subjects responded that they were highly satisfied with the home care service given by the community health practitioners. Although, the actual implementation period was very short, and not all of the evaluation outcomes showed significant improvement, the home health care model of community health practitioners was, in general, positively evaluated. Through this re-search, the possibility of community health practitioners working as active home care personnel in the public health care system is supported. Further research with an expanded area and subjects for a longer period is recommended. Cost effectiveness research is also needed.

Need Assessment for Central Food Production in Child Care Center Foodservices (보육시설 급식소의 공동조리 요구도 조사)

  • Nutrition during childhood is essential for growth and maintenance of health. Good food habits developed during the childhood will contribute both to the healthy growth and the prevention of the degenerative disease of later life. Both parents and the providers in child care centers play an important role for children's good eating behavior. Therefore all child care programs should achieve recommended standards for meeting children's nutritional and educational needs in a safe, sanitary, and supportive environment to promote the healthy growth and development of children. The purposes of this study were to evaluate the foodservice management practices and assess the needs for a Central Production Unit by contacting the child care center' providers. This approach was achieved using a variety of qualitative and quantitative information including the general foodservice management practices and the needs for a Central Production Unit. An indepth face-to-face interview with structured-questionnaires was undertaken at 32 representative child-care centers in Seoul. Statistical data analysis was done using the SAS program for descriptive analysis and ANOVA. The number of national/public and private sectors were 11 respectively, followed by 10 licensed home day-care centers. Total average number of children in child-care centers was 54.3 $\pm$48.5. The foodservice productivity index in child-care centers was 4.8 minutes per meal for public child care centers, 6.0 for private child-care centers, and 9.8 for home child care centers. Home child care centers were found to have the lowest productivity index which indicated inefficient foodservice practice. The important factors in group purchasing were menus(39.6%) or close distance(39.6%) > type of foodservice operation(32.8%) > total number of meals(19.9%) > food costs(16.2%) in order. Average score of the efficiency for central food production in child-care centers was 3.80 $\pm$0.84 out of 5.

A Survey on the Demand for Home Care of the Mother's of the Hospitalized Children (입원 환아 어머니의 가정간호 요구도 조사)

  • The subjects of the study were the 120 mothers who had children hospitalized in 6 hospitals in P city during the period from Feb. 1. to May 30. 1996. The purpose of the study are(1) the problems encountered when they carried out Home Care after discharge and (2) the degrees of demand that they had against Home Care Services. In order to investigate the purposes above-mentioned the researchers used the 55 items of Home Care services belonging to 10 areas selected by Korea Home-Care Association in 1994. The items were modified and supplemented to 4-point scale. The selected data were analyzed by SPSS : PC+ and the following results were derived. 1. The subjects payed great attention to 5 items such as 'pain control, OPD visits, diet-control problems' among the 15 sources of anxiety that they had against Home Care services after discharge. Especially, 'OPD visits and knowledge deficit' would to be overcome by the nursing activities of the Home Care Nurses. 2. The acknowledge level of the subjects to the Home Care Project that governmental plans to carry out was surveyed and found as under 50. 0% responded they heard about Home Care Nursing System. 60.8% said that they thought the system was 'very necessary' and 'somewhat necessary'. 65.0% said they would like to use the system. However, the acknowledge level of 50.0% is still low and is thought to need further national promotion and propagation. 3. The demand of the subjects against 55 Home Care Services was investigated and found as

    . 'Assessment, Lab, basic nursing technology, relay ' transfer, training ' education and curative nursing' showed comparatively high scores. However, the items belonging to pediatric Home Care Services such as 'umbilical cord care, breast-feeding, infant care, and post-partum care & loan of breast-pump' showed relatively low scores.

  • Nutritional Assessment and Management in Long-term Care Insurance's Home Visit Care Service (경기도 일부 노인의 노인장기요양보험 방문요양서비스의 영양상태 평가 및 영양관리 현황)

    • The purpose of the study was to examine nutritional management and nutritional care demands of home-visit care service and the nutritional status of serviced elderly. The survey was carried out from $5^{th}$ to $21^{st}$ of January, 2011 among 37 In-Home Service institutions. The number of elderly surveyed by 143 care helpers was 281. Analysis was performed for 203 subjects (male: 44, female: 159). The age distribution was from 50 to 99 years. The grading by long-term care insuranceshowed 18,2% on level 1 and 2, and 81.8% on level 3. For the disease treatment, the proportion of implementing diet treatment turned out to be very low. The proportion of subjects living with their children was 45.3%, and living alone was 29.1%. The proportion of home-visit care among types of In-Home Service was 70%. Subjects who needed additional necessary diet management service turned out to be 40.9%. Diet management was the most necessary services from all levels. MNA (specifiy MNA) scores were significantly (p < 0.001) associated with BMI, mid-arm circumference (MAC), calf circumference (CC), intake problems, and weight loss during the last 3 months. For studied elderly, 45.3% were malnourished, and 46.8% were at the risk of malnutrition by MNA score. Based on the result of this survey, the nutritional care was not systematic on the In-Home Service. Prevention and management of the disease was much better than the treatment to improve the quality of life. We conclude that nutrition management is necessary for the elderly. To improve nutritional status of elderly in home care services, systematic nutrition management should be implemented.

    영유아의 보육환경에 관한 실태조사 -부산지역을 중심으로-

    • This study attemped to identify the desirable early childhood education and investigate the current situation of the child care centers. The survey was conducted by means of interviews or mailing method using the structured questionnaire. The sample size was 176 teachers in child care centers in Pusan. The questionnaire was built on previous studies and bibliographies. The results were as follows; (1) Most of the teachers were unqualified and under 30 years of age. (2) The facilities and the equipments in the child care centers were insufficient for children's development. (3)The nutritional knowledge of the teachers were far short and the emergency discipline was seldom carried out. (4) No additional efforts were made for the parent -education. It is required to increase the facilities & the equipments for children's development. It appears that re-education of teachers must be carried out in a near future prior to child educaion.

    A Basic Study on Public Nanny Service Characteristics and Improvement Strategies (이용자관점에서 본 아이돌보미지원사업의 특성과 개선방안에 관한 기초연구)

    • In order to find out better policy strategies for public nanny service, this study tried to find out the characteristics and the major problems of this service based on 24 in-depth interviews and 7 focus group interviews with nannies, beneficiaries, managers, and public agents. Research findings showed that public nanny service was effectively filling the gap between nursery care services and parent's care services(especially who are both working). Also, this service provided tailored support to parents who have urgent or unexpected problems which prohibit them from properly caring their children. Especially this service effectively worked for temporary needs of caring young children. This study proposes these two strategies as follows: First, the beneficiary selection criteria should be updated. Second, the level and types of available time should be raised to fit the diverse needs of parents.

    • The Purpose of this study was to develop a comprehensive and integrated child-care services model for children both with and without disabilities. The comprehensive perspective included several dimensions such as education, child welfare, family welfare, and community welfare. Survey research based on theories and models regarding the integrated child-care services was carried out to determine the overall needs of child-care institutions, parents, and community members. The results revealed the need for development in the following three areas: (1) edu-care curriculums for integrated programs, (2) programs for supporting family members who have disabilities children, and (3) improved community members' perception about integrated child-care services. A model was developed for fulfil these identified needs.

    Improved Nutritional Status of Children by Nutritional Management Programs at Child Care Centers in Korea (영유아 보육시설의 영양관리 시범 사업(II) : 영양상태 개선 효과)

    • The study was carried out to assess whether the nutritional management program by dietician at child center can affect nutritional status of children at child care center. The diet intakes were measured by mother\`s record at home and by direct weighing at child care centers, and the hight and weight of children were measured before and after nutritional management program for 3months to see the effects. The energy intakes were significantly increased 56.29 Kcal after program, and protein 3.61g, lipid 3.12 g, calcium 49.71mg, iron 0.39 mg, vitamin B$_1$ 0.084 mg, vitamin B$_2$ 0.116 mg, and vitamin C 10.10 mg, respectively, except vitamin A and niacin. the significant changes of nutrient intakes were more at higher age group compared to at the lower age group, and more at girls compared to boys, Although the significant increase fo nutrient intakes at child care center, most nutrient intakes at home were not significantly changed, except energy, lipid, iron, vitamin A. The changes at changes of fat and calcium intakes for 3 months were positively correlated to the change of Z-score for weight, and the change of A-score for weight was positively correlated to changes of Z-score for height. The results showed that the nutritional management program for 3 month at child care center can increase nutrient intakes significantly and the incresed nutrient intakes can enhance children\`s growth.
