Validity of Korean Version of Denver II in Screening Children with Developmental Risk (발달문제 위험 아동 선별을 위한 한국형 Denver II 검사의 타당성에 관한 연구)

  • Purpose: This study was done to evaluate the validity of Korean Denver ll developmental screening test in screening children with developmental risk. Method: The participants in the study were 113 children referred for developmental assessment in the department of rehabilitation of D University hospital. They were examined with the BSID (Bayley Scales of Infant Development) II and Korean Denver II by one occupational therapist. The data was analyzed by $x^2$ test and discriminant analysis. Results: There was a significant relationship between the Mental Developmental Index (MDI) of the BSID II and the result of the Korean Denver II. The sensitivity and specificity of Korean Denver II were 0.87 and 0.62 when abnormal MDI was defined as lower than 85 was used as a diagnosis of developmental delay. There was a significant relationship between the Psychomotor Developmental Index (PDI) of the BSID ll and the result 0.7 Korean Denver ll. The sensitivity and specificity of Korean Denver II were 0.83 and 0.51 The over-all hit ratio was $79.6\%$. Conclusion: The result showed that the Korean Denver II was valid and has good sensitivity and moderate specificity in screening developmental delay.

Simulation and Health Risk Evaluation of Indoor Air Quality Changes by Ventilation System in New Apartment (신축아파트 환기방식에 따른 실내공기질 변화와 이에 대한 시뮬레이션 및 건강 위해성 평가)

  • In this study, air quality conditions were identified and analyzed in real time, at the same time, living habits and ventilation methods were maintained in the daily life of residents, and thus, this present study focuses on the lifestyles of residents. Previous studies showed a difference from this study, focusing on the study on the effects of changes in indoor air quality on human health according to the indoor air quality process test standards of the Ministry of Environment. Formaldehyde concentrations exceeded all ventilation standards, but satisfied the organic standards of the Ministry of Environment when ventilation devices and air purifiers were activated. As such, it was investigated that a large amount of formaldehyde emission in the condo is initially ventilated, but a certain concentration is maintained. The change in PM2.5 concentration according to the ventilation method showed a clear difference. As a result of simulating indoor air flow during natural ventilation, the effects of wind speed and wind direction affect the flow rate of indoor air, and indoor polluted air is stagnant even in the presence of wind and is not completely discharged. When the risk assessment results are averaged on the day of measurement, the trends of change between adults and children are almost equivalent, but the results address that children are more sensitive to risk than adults.

Exposure Assessment of Phthalates from House Dust and Organic Films in the Indoor Environment (실내환경 중 집먼지 및 유기필름에서 기인한 프탈레이트 노출평가)

  • Background: Various types of semi-volatile organic compounds (SVOCs) exist in the public's living environment. They occur in different forms in terms of their physical and chemical properties and partition coefficients. As a consequence, indoor exposure to SVOCs occurs via various routes, including inhalation of air and airborne particles, skin contact, and dust intake. Objectives: To propose a method for assessing human exposure to the SVOCs occurring in the air of an indoor environment, the concentrations of SVOCs in house dust and organic films measured in a real residential environment were estimated in terms of gas-phase concentration using the partition coefficient. Assessment of inhalation exposure to SVOCs was performed using this method. Methods: Phthalates were collected from samples of house dust and organic films from 110 households in a real residential environment. To perform an exposures assessment of the phthalates present in organic films, gas-phase concentration was calculated using the partition coefficient. The airborne gas-phase concentrations of phthalates from the house dust and organic films were estimated and exposure assessment was performed based on the assumption of inhalation exposure from air. Results: As a result of the exposure assessment for gas-phase phthalates from house dust and organic films, preschool children showed the highest level of inhalation of phthalates, followed by school children, adults, and adolescents. Conclusions: This study includes the limitation of not considering different SVOCs exposure pathways in the health impact assessment, including those of phthalates in the indoor living environment. However, this study has the significance of performing exposure assessment based on exposure to SVOCs present in indoor air that originated from organic films in the indoor residential environment. Therefore, the results of this study should be useful as basic data for exposure and health risk assessments of SVOCs associated with organic films in the indoor environment.

Microbial Risk Assessment in Reclaimed Wastewater Irrigation on a Paddy Field (하수의 농업적 재이용에 따른 논 담수 내 미생물 위해성 평가)

  • Water stress has become a major concern in agriculture. Korea suffers from limited agricultural water supply, and wastewater reuse has been recommended as an alternative solution. A study was performed to examine the effects of microorganism concentration in the ponded-water of a paddy rice field with reclaimed-water irrigation for evaluating the microbial risk to farmers and neighborhood children. Most epidemiological studies were performed based on an upland field, and they may not directly applicable to paddy fields. Beta-Poisson model was used to estimate the microbial risk of pathogen ingestion. Their risk value increased significantly high level after irrigation and precipitation. It implies that agricultural activities such as plowing, and fertilizing, and precipitation need be practiced a few days after irrigation considering health risks. The results about field application of the microbial risk assessment using E. coli showed difference according to monitoring time and treatment plot. Result of the microbial risk assessment showed that risk values of ground-water and reclaimed secondary wastewater irrigation were lower than directly use of wastewater treatment plants' effluent. This paper should be viewed as a first step in the application of quantitative microbial risk assessment of E. coli to wastewater reuse in a paddy rice farming.

Childhood Arsenic Exposure and Health Risk Assessment from Rice Cultivated Near the Mining Areas in Korea

  • Domestic studies for identification of causality between children exposure to toxic chemicals, such as arsenic (As) and resulted hazardous effects were not implemented. This study was conducted to probabilistically estimate dietary As intake and health risk assessment for young children and all age-specific populations from the consumption of As-contaminated rice of Korea. Arsenic intakes for young children (1 to 6 years old) from As-contaminated rice were higher than other age-specific groups, based on a dose-per-body weight basis. Based on the current EPA cancer slope factor for As, estimated cancer risks (to the skin cancer) associated with dietary intake of As-contaminated rice for 1 to 2 years old group and 3 to years old group are 1.76 per 10,000 and 3.16 per 10,000, respectively, at the 50th percentile. Based on possible reference levels (0.005 mg/kg/day) for children, mean and $95^{th}$ percentile value of HQ from rice for young children are very below 1.0, which is a regulatory limit of non-carcinogenic risks for human.

Mytilus Galloprovincialis and Metal Contaminants: Health Risk Assessment from Sinop Coasts

  • The goal of this study was to quantify the content of heavy metals (Zn, Cu, Cd, Pb, Hg) in the bivalve Mytilus galloprovincialis, which was obtained from the Black Sea's Sinop peninsula, as well as estimate the health risks. Concentrations of heavy metals in mussels' soft tissues ranged between 16.4-21.8, 0.52-0.93, 0.13-0.45; 0.015-0.019 and 0.006-0.011mg kg-1 wet weight for Zn, Cu, Pb, Hg and Cd, respectively. In general, metal concentrations in mussels were quite low. In mussels, Zn was the most prevalent element, followed by Cu. Regarding the monthly variation, although relatively higher tissue metal concentrations were observed in August compared to July, no statistical difference was found. The estimated daily intakes did not exceed the tolerable intakes. There was no health risk since the total hazard index was far below critical value 1. This demonstrates that eating mussels from the Sinop coasts in 2019 does not cause any health risks in infants, children, or adult. Mussels have a risk index of less than 10-6, which is considered insignificant. In conclusion, the results of this study show that there is no heavy metal pollution in the mussels collected from Sinop coasts.

  • The incidence of childhood obesity has increased dramatically. Childhood obesity is an increasing health problem because of its strong associations with chronic health problems in children and adults. These health problems significantly contribute to the development of common chronic diseases in later life, including hypertension, type2 diabetes, hyperinsulinemia, coronary heart disease, and other psychological disorders. So it is an important issue to prevent and treat obesity during childhood and adolescent. Diet and exercise are the cornerstones of treatment for obesity and related complications. For obese children, some clinical trials have shown improvement with diet, exercise, and /or behavioral interventions. Promising interventions for high-risk individuals, such as bariatric surgery and novel pharmacological agents, also require rigorous assessment with attention to long-term patient important outcomes. There are various pharmacological approaches to the treatment of obesity in the adolescent population some of which have FDA approval. In the article we discuss pharmacological approaches to guide the treatment of obesity in the pediatric population, including risks of treatment, monitoring of potential side effects.

Quantitative Microbial Risk Assessment of Wastewater Reuse for Irrigation in Paddy Field (하수처리수의 논 관개용수 재이용을 위한 미생물 위해성 평가)

  • The reuse of wastewater for agricultural irrigation may cause human health risk as a result of exposure to pathogens. This study conducted the quantitative microbial risk assessment in paddy field irrigated with treated wastewater. Six treatments were used to irrigate the paddy field from Year 2003 to Year 2005: biofilter-effluent, UV-disinfected water (6, 16, 40, 68 $mW s cm^{-2}$), pond-treated water, wetland-treated water, conventional irrigation water and tap water. Total coliforms, fecal coliforms and E. coli were monitored during rice growing period. Beta - Poisson model was employed to calculate the microbial risk of pathogens ingestion that may occur to farmers and neighbor children. Uncertainty of risk was estimated using Monte Carlo simulation. In this study, the microbial risk was higher during initial cultivation (end of May$\sim$June), and it decreased with time. Biofilter effluent (secondary effluent) irrigation showed higher risk values than others (>$10^{-4}$) and irrigation with UV-disinfected water has the lowest risk range ($10^{-6}{\sim}10^{-5}$). The risk value estimated in 2005 was lower than risk value in 2003 and 2004, it is likely due to clean tap water irrigation in initial transplanting stage. It is suggested that irrigation with UV-disinfected water and pond-treated water would reduce the microbial risk associated with wastewater irrigation in paddy field. In addition, the first irrigation water quality significantly affected the subsequent microbial risk.

Analyses on Nutritional and Behavioral Risk Factors of Atopic Dermatitis (AD) in Korean Preschoolers (학령 전 아동에서 아토피피부염의 영양.행동 위험 요인 분석)

  • In order to examine nutritional and behavioral risk factors of atopic dermatitis (AD) in Korean preschoolers, we analyzed data on 144 children aged 3-6 years with AD and their 434 healthy counterparts. The data included breast-feeding history, current weight, food behaviors assessed by the Mini Dietary Assessment (MDA), food intake by a semi-quantitative food frequency questionnaire, and behavior problems by the Preschool and Kindergarten Behavior Scale (PKBS). Multivariate logistic regression analyses were performed after controlling for preschool location and child's age, gender and total energy intake, as appropriate. There was no group difference of child and household characteristics. Breastfeeding history was related to lower AD risk (OR = 0.63, 95% Cl = 0.40-0.99), yet no statistically significant association was found with overweight status. Regarding food behaviors, AD risk was lower in children who drank milk at least one cup per day (OR = 0.52, 95% Cl = 0.35-0.78) and had regular meals (OR = 0.62, 95% Cl =0.42-0.92). Moreover, there were lower risks of AD in the second (OR =0.48, 95% Cl = 0.28-0.82) and the highest (OR = 0.55, 95% Cl = 0.32-0.94) intake quartiles as compared with the lowest quartile of kimchi intake. Similarly, AD risk was lower in the highest quartile of rice (OR = 0.51 Cl = 0.28-0.93) and the second quartile of fruit (OR =0.45, 95% Cl = 0.25-0.82) intakes. AD children had more problems in social interaction (OR = 1.97,95% Cl = 1.26-3.07) and independence (OR = 1.60, 95% Cl = 1.01 -2.54) measures than the healthy controls. Likewise, AD children tended to show more problem behaviors such as anxiety (OR = 1.63, 95% Cl = 0.99-2.69). Our results suggest that nutritional and behavioral dimensions are related to AD risk, yet the case control study design may preclude generalization of these results.

Investigation of Labeling Status and Toxicity Data of Environmentally Hazardous Substances in Children's Products (어린이용품의 환경유해인자 표시 현황과 독성자료에 대한 연구)

  • Objectives: Children are exposed to various environmental pollutants through contact with children's products. We investigated the KC mark, certification number, and contained substances labeled on children's products through market research and collected the toxicological data on these substances. Methods: The environmentally hazardous substances labeled on children's products (n=6576), including toys (n=2812), personal care products (n=2212), stationary/books (n=1333), and playground equipment (n=219) were examined. For the components that could be identified by CAS number, toxicological data on oral, inhalation, and dermal routes, cancer slope factor, and reference dose were collected. Results: Among the investigated products, KC marks or certification numbers were found for 4557 products (69.3%). Except for cosmetics and cleansers, the material information was labeled on most of the products. The frequency of labeling substance information in toys and stationary/books was low since this information could be omitted if KC certification was obtained. In the target products, 617 substances were identified by CAS number, and polypropylene, acrylonitrile butadiene styrene, and polyester were the most frequently displayed. Chronic toxicity data was found for only 32.4% of individual components, and information on toxicity through the dermal route was also highly limited. Conclusion: Our study suggested that labeling guidelines should be required to identify the environmentally hazardous substances contained in children's products. In addition, the toxicological data on many ingredients in children's products were insufficient. The data gap for toxicity data should be filled for future risk assessment.
