Double burden of malnutrition in ethnic minority school-aged children living in mountainous areas of Vietnam and its association with nutritional behavior

  • BACKGROUND/OBJECTIVES: Although children of ethnic minority groups are experiencing a transition from a predominance of undernutrition to overnutrition, there is little evidence of a dual-malnutrition burden. Therefore, this study examined the double burden of malnutrition among school-aged ethnic minority children living in mountainous areas and its association with their diets. SUBJECTS/METHODS: A cross-sectional study was conducted from September 2019 to March 2020 in 3 mountainous areas of Northern Vietnam among 1,556 ethnic minority school-aged children. The prevalence of under-nutrition (stunting and thinness) and over-nutrition (overweight and obesity) were measured using the WHO 2006 child growth standards (height-for-age and BMI-for-age Z-score). Nutritional practices were evaluated by the frequency of food consumption based on a 4-level scale. RESULTS: The percentage of children with stunting and thinness were 14.0% and 5.4%, respectively, while the figure for overweight/obesity was 9.4%. The factors positively associated with stunting were living in a family with more than 2 children or being Muong/other ethnicities compared to the Tay ethnicity. Children who consumed fish/shrimps/crabs or milk weekly/daily were less likely to be undernourished compared to those who never consumed these foods. By contrast, children who never consumed foods rich in vitamin A precursors and vitamin A and fruit or consumed daily snacks/junk food were more likely to be overweight/obese. CONCLUSIONS: Undernutrition remains a common issue among school-aged children and adolescents of ethnic minority groups, while over-nutrition exists simultaneously. Public health nutrition programs promoting adequate diets and positive lifestyle changes related to nutrition are essential to tackle the double burden of malnutrition among ethnic minority children.

  • Although it is rapidly increasing the number of day-care centers in Korea, the quality of food and nutrition services is not improved sufficiently. The purpose of this study was to investigate the nutrition and health status of children in day-care center. Menus for lunch of 253 centers were collected and the content of nutrients was analyzed. Nutrient intakes from lunch and snacks of 90 children were investigated using weighing method and those from breakfast and dinner by 24-hour recall method surrogated from their mothers. Anthropometric indices of height, weight, skinfold thickness, the ratio of fat were measured. The lunch menus for children contained $437.0{\pm}138.9\;Kcal$, $17.6{\pm}6.7gr$ protein, $153.9{\pm}87.7mg\;Ca$, $3.63{\pm}1.6mg\;Fe$, $164.5{\pm}158.2RE\;Vit\;A$. and $4.46{\pm}3.04mg\;Vit\;B_1$. Energy, Ca and Fe supplied were below one third of RDAs. Mean daily calorie intake of children aged 3 years was $1303.0{\pm}474.0Kcal/day$, $1322.3{\pm}442.4Kcal$ in aged four, $1307.0{\pm}545.9Kcal$ in aged five and $1497.1{\pm}93.5kcal$ in six year of age. Intake of iron, vitamin A and vitamin $B_1$ were below RDAs.

Food sources of zinc and nutritional status with usual dietary zinc intake in Korean toddlers and preschool children

  • BACKGROUND/OBJECTIVES: This study aimed to evaluate the food sources of zinc and the usual intake of dietary zinc among Korean toddlers and preschool children. SUBJECTS/METHODS: A total of 2,679 children aged 1-5 years was selected from the 2009-2013 Korea National Health and Nutrition Examination Survey (KNHANES) data. Dietary data collected from a single 24-h recall were used to evaluate the food sources of zinc. To estimate usual zinc intake, the distribution obtained from single 24-h recall data in the total sample was adjusted using the ratio of within-to-between-person variance in zinc intake obtained from 2-day 24-h recall sub-sample data of the 2009 KNHANES. The proportion of children with usual zinc intake below the estimated average requirement (EAR) and above the tolerable upper intake level (UL) was assessed. RESULTS: The main sources of zinc in Korean children were grains, dairy products, and meat. The mean usual intakes of zinc among all individuals, those aged 1-2 yrs, and those aged 3-5 yrs were 5.50, 5.01, and 5.83 mg/d, respectively. In all participants, 1.1% of the children consumed zinc below the EAR, whereas 10.7% exceeded the UL. The proportion of children with excessive zinc intake was 25.6% in the 1-2 yrs age group and 0.6% in the 3-5 yrs age group. CONCLUSIONS: According to the current UL, the risk of excessive zinc intake appears to be high among Korean toddlers. Future studies that monitor the health effects of excessive zinc intake are needed to appropriately guide zinc intake in children.

Different Clinical Features and Lower Scores in Clinical Scoring Systems for Appendicitis in Preschool Children: Comparison with School Age Onset

  • Purpose: To clarify the clinical features of appendicitis in preschool children and to explore clinical appendicitis scoring systems in this age group. Methods: We retrospectively collected data on 142 children, aged 10 years or younger, with confirmed diagnosis of appendicitis based on surgical and pathologic findings. Enrolled subjects were divided into two groups: Group 1 (preschool children aged ${\leq}5$ years, n=41) and Group 2 (school children aged >5 to ${\leq}10$ years, n=101). Data analyzed included clinical presentation, laboratory findings, the pediatric appendicitis score (PAS), and the modified Alvarado score (MAS). Results: The most common presenting symptom was abdominal pain in both groups (92.7% vs. 97.0%). Other presenting symptoms were as follows: fever (65.9%), vomiting (68.3%), right lower quadrant (RLQ) localization (24.4%), anorexia (14.6%), and diarrhea (7.3%) in Group 1, and RLQ localization (74.3%), vomiting (71.3%), anorexia (52.5%), fever (47.5%), and diarrhea (11.9%) in Group 2. Perforation and abscess occurred more frequently in Group 1 than in Group 2 (43.9% vs. 12.9%, p<0.001; 34.1% vs. 5.0%, p<0.001; respectively). PAS and MAS were lower in Group 1 than in Group 2 ($4.09{\pm}1.97$ vs. $6.91{\pm}1.61$, p=0.048; $4.65{\pm}1.79$ vs. $6.51{\pm}1.39$, p=0.012; respectively). Conclusion: In preschool children, appendicitis often presents with atypical features, more rapid progression, and higher incidence of complications. This age group is more likely to have lower PAS and MAS than those of school children.

학령기 아동의 문제행동과 관련요인 (Problems Behavior and Its Related Factors in School-aged Children)

  • Purpose: This study aimed to investigate problem behavior and its related factors in school-aged children. Methods: A descriptive correlational study was conducted with 221 mothers of children at 1st to 6th grades. Data were collected from March to April, 2015 using self-report structured questionnaires. Questionnaires included characteristics of subjects, Korean Child Behavior Checklist for measuring problem behavior and Children's Sleep Habits Questionnaire for assessing sleeping habits. Collected data were analyzed by independent t-test, $x^2$ test, and multiple logistic regression. Results: In logistic regression analysis, having any maternal job (aOR=2.8, 95% CI=1.1~6.9) and higher daytime sleepiness (aOR=2.9, 95% CI=1.2~7.4) were statistically significant contributors of internal problems. Higher bedtime resistance (aOR=2.2, 95% CI=1.0~4.9) and higher daytime sleepiness (aOR=2.8, 95% CI=1.2~6.3) were statistically significant contributors of external problems. Higher bedtime resistance (aOR=4.3, 95% CI=1.5~12.4) was statistically significant contributors of total problem behavior. Conclusion: School-aged children with sleeping problem are at risk for problem behavior. More attention is need to consider sleeping habits for supporting behavioral improvement in school-aged children.

노부모-성인자녀 간의 결속도 및 노부모의 자아존중감에 관한 연구 (A Study on the Solidarity between Aged Parent & Their Adult Children, and Self-Esteem of Aged Parents)

  • This study is to investigate the factors which affect self- esteem of the aged parents. In this study questionaires were used to measure solidarity between aged parents and adults children, self- esteem of aged parents. Subjects were 234 male and 235 female parents who were aged over 60. The data were analyzed by using frequency, percentage, mean, standard deviation, ANOVA, and stepwise regression analysis. The results are as follows : 1) The solidarity between the aged parents and their adult children is average 65.5(Conversion value by 100 : 51.9) and shows the average level. The relations between solidarity and background variables are variables affected by education, pocket money, health, social activity and religion. 2) The degree of self- esteem in the aged parents is presented to average 26.1(Conversion value by 100 : 56.5) and shows the average level and it according to the background variables has a significant difference depending on education, pocket money, family living together, health, social activity, religion and the number of children. 3) To investigate the degree of self- esteem according to the degree of solidarity, the sub-areas of solidarity are subdivided into five groups. higher solodarity the group has, higher self- esteem it has. 4) The degree of effects of the variables which affect self- esteem in the aged parents is in such order as affectional solidarity($\beta$=0.32), helping($\beta$=0.14), education($\beta$=0.10), solidarity by contact($\beta$=0.09) and solidarity by argument($\beta$=0.09) and $R^2$ of such variables are 30%.

저소득 가정과 일반 가정 아동의 우울성향에 대한 부모간 갈등과 양육행동의 영향 (The Relationship between Parents' Conflict and Child-Rearing and School-Aged Children's Depression in Low Income Family and Non-Low Income Family)

  • The purpose of this study was to investigate the relationship between parents' conflict and child-rearing and school-aged children's depression in low income family and non-low income family. The subjects were 357 school-aged children who attended elementary school and who were 5th 176(49.3%) and 6th grade 181(50.7%)(boys were 206(57.7%), girls 151(42.3%)). The data were analyzed by t-test, Pearson's correlation, simple regression, enter regression(using SPSS 12.1). Major findings were as follows: (1) Parents' conflict was significantly associated with children's depression in low income and non-low income family. (2) Parents' conflict was significantly associated with parents' child-rearing in low income and non-low income family. (3) Parents' conflict partially exerted indirect effect on children's depression, controlled by parents' child-rearing in low income and non-low income family. The effect of parents' child-rearing as mediation was higher in low-income family than in non-low income family.

학령기 아동의 일반적 특성에 따른 학교생활적응정도 예측요인 - 5개 초등학교를 중심으로 - (The Estimated Factors for School Adaptation on School Aged Children by General Characteristics - 5 Elementary Schools -)

  • Purpose: This study was to examine estimation variables and predicted probability for school-aged children's school adaptation. Methods: The subjects of this study consisted of 614 children. Data were analyzed by logistic regression using SPSS Windows 11.0. Results: 1. Girls adjusted themselves 1.65 times easier than boys, and 5th-and 6th-graders adjusted themselves slightly easier than 4th-graders. Children with monthly pocket money lower than 10,000 won adjust adjusted 2.92 times easier than those with a larger amount of pocket money, unhealthy children adjusted themselves 2.44 times easier than healthy ones, and children with low academic achievement adjusted themselves 2.68 times easier than those with high academic achievements. 2. As for family economic status, children who thought the economic condition of their family poor adjusted themselves 2.41 times easier than those who thought the economic condition of their family fine. As for conversation with people, children conversing with families and relatives adjusted themselves 2.69 times easier and those conversing with friends and others adjusted themselves 2.71 times easier than those conversing with none. Children from normal families adjusted themselves 3.44 times easier than those from deficiency families. 4. The overall prediction rate was 73.9%. Conclusions: It is concluded that nurses, teachers and parents are important in improving school-aged children's adaptation to school life.

로봇 강아지에 대한 유아의 생명현상 인지 및 추론 (Children's Cognition and Inference of the Life Phenomenon about Robotic Dogs)

  • The purpose of this study was to investigate cognition of life phenomenon on robotic dogs according to properties of robotic dogs amongst aged 3, 4 and 5. 95 Children aged 3 to 5 years participated in this study. Each child was interviewed individually and responded to questions designed to measure his/her cognition of life phenomenon. The major findings of this study are as follows. First, there was a significant difference in children's cognition of the life phenomenon according to their age. 3 years were more likely to refer the robotic dog as alive, compared to the 5 years. Secondly there was a significant difference in children's inference of the life phenomenon according to properties of robotic dogs. Such as high activeness and responsiveness, preschoolers considered robotic dogs to be alive and have more biological and psychological traits. This result shows the activeness and responsiveness of robotic dogs influences young children's inference of life phenomenon.

학령기 아동의 자기조절능력과 관련변인들 간의 관계 구조분석 (A Structural Analysis on School-Aged Children's Self-Regulation Competence and Its Related Variables)

  • The purpose of this study was to investigate the structural relationships between of school-aged children's self-regulation competence, self-determination, self-efficacy, and mother's acceptive parenting. The subjects for this study were 544 Children in 5th and 6th grade randomly selected from three elementary schools located in Ulsan-city. The collected data were analyzed with AMOS 19.0. The results showed that 1) children's self-determination and self-efficacy were found to affect children's self-regulation competence directly. 2) children's self-efficacy were found to have indirect effect on children's self-regulation competence mediated by children's self-determination. 3) mother's acceptive parenting were found to have indirect effect on children's self-regulation competence mediated by children's self-efficacy or children's self-determination.
