A CLINICAL STUDY ON CHILD SEXUAL ABUSE (소아 성학대에 대한 임상 연구)

  • This study was purposed to find out clinical characteristic of intra and extrafamilial sexual abuse, nature of offenders, difference of symptoms describe that sexually abused children and adolescents were experienced. Subjects were 10 sexually abused children and adolescents who visited Seoul National Mental Hospital, from Jan. 1992 to Dec. 1993. In intrafamilial sexual abuse, they had more chronic course than extrafamilial abuse and had more family psychopathology. In symptomatology, The preschool children mainly presented to somatic symptoms and anxiety, and The school children prominently exhibited depressive reaction and withdrawal. Most common reaction of parents was anger and guilt. They were treated with crisis intervention, play therapy, and hospitalization, if needed. The maintenance of treatment was difficult due to parent's avoidance and repression.

Preschool Children's Judgment on Moral and Conventional Rules (유아의 도덕적 및 인습적 규칙에 대한 판단)

  • This paper is to examine the development of Korean young children's judgement on moral and conventional rules. The subjects are 120 children, 30 each at four age levels; age 3(2.8-3.5), age 4(3.7-4.4), age 5(4.8-5.5), and age 6(5.7-6.5) in a day care center in Taegu. Results are summarized as follows: 1. In terms of nonpermissibility, there was a significant difference in regard to the moral and the conventional rule transgression between the group of age 3 years and three groups of ages 4, 5, and 6. 2. In terms of seriousness, there was a significant difference according to domain. Three groups ages 4, 5 and 6 years evaluate moral transgressions to be more serious than conventional transgressions. 3. In terms of rule contingency and generalizability, there was a significant difference in regard to the moral and conventional transgression between the group of age 3 years and three groups of ages 4, 5 and 6. 4. In terms of punishment, there was significant difference according to domain. Three groups of age 4, 5, and 6 years evaluate moral transgressions to be more punishable than conventional transgressions. 5. Children of age 5 with institutional experience do not make a distinction between moral and conventional rules on punishment criterion.

The Effects of Home Literacy Environments on Preschoolers' Vocabulary and Reading Skills (가정문해환경과 유아의 어휘 및 읽기 능력)

  • We examined the effects of family and home literacy environments on the vocabulary and reading abilities of 3 to 5-year-olds. In particular, SES was considered against home literacy environment. The sample consisted of 366 Korean children and their mothers. We found that the level of mother's education was related to the child's vocabulary test score. After controlling for family factors, home literacy environment score positively predicted children's performance both in vocabulary and reading tests. Magnitudes of differences between the two literacy groups (high and low home literacy environment) became larger in the children from relative lower income families. Magnitudes of differences for vocabulary and reading tests was strikingly large in the low-income group both in vocabulary and reading tests. These findings shed lights on the importance of parent education and family services for low-income families with preschool children.

Usefulness of Ultrasonography in Potential Bilateral Inguinal Hernia of Children (양측성 서혜부 탈장 고위험군 영유아에서 수술 전 초음파 검사의 유용성)

  • Inguinal hernia is the most frequent problem requiring surgery in children. Moreover, subsequent contralateral occurrence after repair of the symptomatic unilateral inguinal hernia(UIH) is not rare, This study is to evaluate the diagnostic value of inguinal ultrasonography (IUS) for potential bilateral inguinal hernia (BIH). A prospective study was performed for preschool children less than 6 years of age who were diagnosed as UIH from July 1999 to December 2000. We selected 58 cases with potential BIH, based on the past history, such as prematurity, ventriculo peritoneal shunt, family history of BIH. hernia on the left side (LIH), age below 2, female, and contralateral positive silk glove sign on the physical examination, Screening with IUS and bilateral surgical exploration were applied on these cases. Forty-seven cases were males (81.0%) and 11 cases were females (19.0%). Thirty four were infants. Symptomatic right inguinal hernia (RIH) were 28 (48.3%), and LIH were 30 cases (51.7%). Six cases had no evidence of contralateral patent process vaginalis (PPV) by IUS but showed contralateral PPV by operation. Two cases were suspicious to contralateral PPV under IUS, but operative findings were negative. Fifty cases showed contralateral PPV by IUS as well as operation. The detection rate of contralateral PPV under IUS was 86.2%. The preoperative IUS may reduce contralateral exploration.

Maternal Perceptions of the Conditions of Happiness across the Life-Cycles of Their Children (자녀 생애주기별 어머니의 행복 조건에 대한 인식 경향)

  • The purpose of this study was to examine the differences in maternal perceptions of mothers in regards to the conditions of happiness across the life-cycles of their children. The participants consisted of a total of 961 women who were the mothers of preschool children, school aged children, and adolescents. The mothers completed a questionnaire on the perceptions of the conditions of happiness which consisted of a total of 56 items. The results of this study revealed that mothers in their 20's and 30's, with high levels of education, and on high incomes were more likely to have high expectations regarding most of the conditions of happiness compared to mothers in their 40's and 50's, with lower levels of education, and on low incomes. In addition, the results further revealed that mothers perceived that 'children grow up well and healthy' as being the most important factor in their happiness regardless of the life cycles of their children. In particular, mothers with younger children tended to give greater priority to a close mother-child relationship in their happiness. However, when mothers' demographic variables were controlled, there were no differences in maternal perceptions of the conditions of happiness across the life-cycles of their children. The implications of parent education and the directions of following research were also discussed.

Support for Child Care: The Financial and Social Support Received from Government, Employers, and Grandparents (가구 보육지원의 결정요인: 정부, 직장, 조부모의 재정적 지원 및 서비스 지원을 중심으로)

  • This study examined how much support families with pre-school children receive for child care along with the types of support they receive. The paper also investigated the factors that influence the financial and social support received from government, employers and grandparents of the children. Data for this study were obtained from a questionnaire completed by mothers of pre-school children (n=641) during the spring of 2009. The data were analyzed by the following methods: $\chi^2$, t-test, ANOVA, Logit analysis, and Tobit analysis using SPSS and STATA software. The profile of support for child care showed that approximately 33 percent of families with pre-school children received free or subsidized child care from the government, 12 percent received some forms of support for child care from the parents' employers, and 26 percent received support from grandparents either financially or in the form of free child care. The results of the Logit analysis demonstrated that families with pre-school children were more likely to receive child care subsides from the government when they had a higher number of pre-school children and a lower level of family income. The significant factors that influence support from employers were mother's educational level, the number of pre-school children, and family income. The significant factors that affect support from grandparents of the children were the mother's age, the mother's employment status, the type of residence, and the family type.

A Study on the Growth, Quality of Sleep of Children with Chronic Rhinitis (만성비염 환아의 수면의 질과 성장에 관한 임상적 연구)

  • Objectives : The study was designed to find out the relationships between rhinitis, quality of sleep, growth among children with chronic rhinitis. Methods : This research was performed in 50 children from November 2007 to May 2008. We measured height, weight, body mass index of those children and asked to answer to the questionnaires that consist of nasal symptoms, period of time with rhinitis, sleep symptoms, parent's height, present illness, and medical history. We also analyzed correlations with this factors obtained from questionnaires. Results : 1. In 50 subjects, there were significant correlations between nasal obstruction, the quality of sleep, and the between quality of sleep and growth. But, there were not significant correlations between nasal symptoms and growth. 2. In 24 preschool children, frequency of waking up during sleeping and the number of snorers were higher and quality of sleeping was worse than those in the children who were attending school. And there were significant correlations between quality of sleep and nasal obstruction. An average of RH-MPH(Recent Height-Mid-Parental Height) is $-7.74{\pm}36.51(%)$ which means they aren't as tall as MPH(Mid-Parental Height). 3. In 26 school children, there were significant correlations between symptoms and period of rhinitis, quality of sleep and growth. Conclusions : Children with chronic rhinitis have low quality of sleep and low growth. But more studies are needed.

A Study on the Nutritional Status of the Children and the Household Food and Nutrients Consumption Level in Rural Farm Household (농촌가구의 식품 및 영양소 섭취수준과 가구내 어린이의 건강상태에 대하여)

  • It has been a long tradition in the field of nutrition that the degree of malnutrition in a community used to be estimated by the degree of the adequacy of nutrients consumption in the households. The adequacy of nutrient consumption in the households then used to be applied to estimate the degree of malnutrition of the children. Such a practice is theoretically acceptable in the community where the food sharing within the household is individual serving style such as in European countries. However, the community where the dietary habit in the household is communal such as in Korea, the estimation of single serving portion from household consumption is quite uncertain. Because of such an uncertainty this study is intended to test the validity of the household food consumption level as an indication of nutritional status of the children in the rural farm households in Korea. The date from the household consumption survey in 1982 by the Korean Rural Nutrition Institute has been used for the analysis of food consumption levels of the households. The survey on the nutritional status of children of 127 preschool children from 78 farm households, has been conducted during May 1983. The nutritional status of the children has been directly measured by weight, height, skinfold thickness, haemoglobin, haematocrit and morbidity of influenza and diarrhoea. The analysis has been conducted to distinguish the differences in prevalence of malnutrition of children from the estimated of household consumption and that of from the direct measurement of nutritional status of children. The results show that the degree of malnutrition of the estimated by the household consumption level has not related with the prevalence of malnutrition by the direct measures of nutritional status of the children. Therefore, the validity of household consumption level as an indication of nutrional status in a same household is quite uncertain for the population studied.

Analysis of Factors Associated with The Preschool Children's Nutrition Awareness -III. Dietary intakes and nutrition awareness of children- (유아기 어린이의 영양 인식과 관련된 요인 분석 -III. 식이 섭취와 영양 인식-)

  • This study estimated the status of dietary intake of 258 children from ages 3 to 6 living in Seoul and analyzed the correlation between the degree of children's nutrition awareness and dietary intake, to provide some basic informations on nutrition education program for their health promotion. Average height and weight of the children were 110.5 cm and 19.7 kg. And their body mass index was 16.2 and obesity index was 6.5%. Proportions of carbohydrate, protein and fat to the total calorie intakes were on the average 55.8%, 15.6% and 28.6%. Calorie intake proportion of breakfast, lunch, dinner and snack were 20.9%, 21.9%, 22.3% and 34.9%. In protein intake, proportion of animal and vegetable protein was 1.4, in fat intake pattern, P/M/S ratio was 1.2/1.2/1.0. As a consequence of analyzing the correlation between children's nutrition awareness and dietary intake, children of high awareness degree to the food value did not intake much calorie and carbohydrate, and did not choose cereals and starches, condensed fat food. As a result, dietary intake of pre-school children showed very good but partially showed a problem of over and under nutrition in quantity or in quality we hope that the result of this study could be helpful for the nutrition education program for the health and nutrition of the pre-school children.

The Cut Off Values of Body Fat Percentage for Diagnose Obese among Selected Number of Preschool Children in Yangsan (우리나라 양산지역 학동전기 아동에서 비만 진단을 위한 체지방률의 절단값)

  • Objectives The objectives of this study were to evaluate the correlations between the indices of obesity and percentage body fat by bioelectrical impedance analysis(BIA). Also, this study was designed to define the cut off values of percentage body fat by BIA which would represent BMI and degree of obesity in order to detect overweight and obesity in preschool children. Methods The height and weight of 683 children aged four to six years old were measured. Also, percentages of body fat were measured by BIA. Results There were high correlations between the boy's body fat percentages and degree of obesity(r=790). Also, there were high correlations between the girl's body fatpercentages and BMI(r=778). The cut off values of body fat percentage, which would correspond to those of BMI percentiles, were calculated by ROC curve analysis. To classify as an overweight, the correspondent cut off values of body fat percentage were 19.55% in boys and 22.35% in girls. For obesity, the cut off values were 28.80% in boys and 27.60% in girls. The cut off values of body fat percentage, which would correspond to degree of obesity were calculated by ROC curve analysis. To classify as an overweight, the correspondent cut off values of body fat percentage were 19.75% in boys and 22.35% in girls. For obesity, the cut off values were 29.05% in boys and 28.75% in girls. Conclusions According to the BMI criteria by the Korean pediatrics society in 2007, the most appropriate cut off values of body fat percentage in for four to six years old children were follows; for boys, 19.55% considered as overweight, and 28.80% as obese while for girls, 22.35% considered as overweight and 27.60% as obese. According to suggested degree of obesity criteria, the most appropriate cut off values of body fat percentage in for four to six old children were were follows; For boys, 19.75% was considered as overweight, and 29.05% as obesity. For girls, 22.35% was considered as overweight, and 28.75% as obese.
