A Case of Acute Renal Failure due to Rhabdomyolysis Associated with Neuroleptic Malignant Syndrome (항정신병약물악성증후군에 동반한 횡문근융해증으로 인한 급성 신부전 1례)

  • Neuroleptic malignant syndrome (NMS) is a rare, but a potentially life threatening condition associated with the use of antipsychotics. The most frequent signs and symptoms of NMS include fever, muscle rigidity, autonomic dysfunction such as tachycardia, tachypnea, and labile blood pressure. Acute complications of NMS include disseminated intravascular coagulation, sepsis, seizure, myocardial infarction, acute renal failure due to rhabdomyolysis and death. We report a rare case of acute renal failure due to rhabdomyolysis associated with neuroleptic malignant syndrome.

Chemical Risk Factors for Children's Health and Research Strategy (어린이 건강관련 유해물질 연구방향)

  • To provide the research strategy for protection of children's health from hazardous chemical, we reviewed the hazardous chemicals can be exposed through maternity, children's life style and living environment. Recently, diseases related with children's living condition were focused as asthma, atopy, childhood developmental disability, congenital malformations and obesity. Children can be exposed to hazardous chemicals through an ambient air, water, soil, food, toys and other factors such as floor dust. Also children's health was deeply related with a wrong life style and neglectful caring by a lack of knowledge and information of harmful ones at parents and child care center's nursers. According to the previous study, the chemical risk factor of children's health were identified as inorganic arsenic, bisphenol A, 2,4-D, dichlorvos, methylmercury, PCBs, pesticide, phthalates, PFOA/PFOS, vinyl chloride, et al. Domestic studies for identification of causality between children exposure to chemicals and resulted hazardous effects were not implemented. The confirmation of chemical risk factors through simultaneously performing toxicological analysis, human effect study, environmental/human monitoring, and risk assessment is needed for good risk management. And also, inter-agency collaboration and sharing information can support confirming scientific evidence and good decision making.

Fire Safety Education Programs for Preventing Fire Accident of Young Children (유아 화재사고 예방을 위한 화재안전 교육프로그램 개발 방향 탐색)

  • The purpose of this study is to explore appropriate fire safety education programs for young children by both examining the actual damage condition and problems associated with fire accident in young children and considering fire safety education programs in America and legal basis for fire safety education in Korea. The results are as follows. First, the mortality of young children by fire accident in Korea is higher than that of U.K, Germany, and Sweden. Second, it is defined by the Child Welfare Law in Korea to implement the mandatory fire safety education for young children in kindergarten and preschool programs. Third, in order to activate fire safety education programs for young children, the fire safety education needs to be suitable for children's developmental characteristics, and various audiovisual materials and activity programs should be developed and provided. Fourth, fire safety education both for early childhood teachers and parents needs to be interrelated and systemized to reduce the risk of fire accident.

Chemokines Expression in Children with a Non-productive Cough (소아기 단순 기침 환아에서의 케모카인 발현 양상 연구)

  • Purpose : To evaluate the chemokine expression in children with a non-productive cough. Materials and Methods : Six children with a non-productive cough who visited Yeungnam University Hospital were evaluated for the mRNA expression of interferon-${\gamma}$-inducible protein 10(IP-10), macrophage cationic protein 1 and 3 (MCP-1, 3), interleukin (IL)-8, regulated upon activation in normal T cells expressed and secreted (RANTES), eotaxin and growth-related oncogene-${\alpha}$ (Gro-${\alpha}$) using the reverse transcription polymerase chain reaction. Results : The chemokines IP-10 and MCP-3 were expressed in all samples. The chemokine RANTES was expressed in five cases, and IL-8 was expressed in three among them. However, eotaxin, Gro-${\alpha}$ and MCP-1 were not expressed at all. The expression of chemokine MCP-3, RANTES and IL-8 were suppressed after the resolution of coughing in just one available case. Conclusion : The chemokines MCP-3, RANTES and IL-8 may contribute to airway inflammation in children with a non-productive cough, whereas IP-10 is of secondary importance in this condition.

Assessment of Elementary Teachers' Knowledge about ADHD, Attitude, Stress Level and Intention for Inclusion Education (초등교사의 ADHD 관련 지식, 태도, 스트레스 수준 및 통합교육의지에 대한 인식)

  • Attention deficit hyperactivity disorder (ADHD) is a relatively common condition of childhood onset and is of significant educational and social concern in elementary school. Investigating the determinants of teachers' attitudes and behavior and their relative importance is crucial for improving teaching practices and initial teacher education for effective inclusion of ADHD children. Based on two hundred and twenty seven survey responses from elementary school teachers, having experienced in dealing with ADHD students, this paper examines the age, gender, career and training-hours effects on the knowledge, attitude, stress and inclusive education involving ADHD children. The regression analysis shows that the gender influences the level of stress and that the training hours has some impact on the knowledge about ADHD at 5% significance level. However, the number of ADHD children experienced, age, gender, career and training-hours were identified as the non-significant predictors of teaching behavior in relation to ADHD children. As a conclusion, a brief discussion concerning the critical needs of elementary school teachers is also presented.

Clinical and laboratory findings of childhood buckwheat allergy in a single tertiary hospital

  • Purpose: Buckwheat allergy is one of the most severe types of food allergy in some countries, especially among children. However, few studies have investigated this condition. The aim of this study was to report the clinical and laboratory findings in Korean children with buckwheat allergy. Methods: Thirty-seven subjects, aged 1 to 14 years, were enrolled by retrospective medical record review from January 2000 through May 2015 at the Department of Pediatrics in Ajou University Hospital. The demographic profile, previous exposure to buckwheat pillows, clinical symptoms, and laboratory findings of each subject were recorded. Results: Twenty-six of the 37 children had immediate-type allergic symptoms to buckwheat, while 11 subjects were tolerant to buckwheat. Seventeen out of 26 buckwheat allergic children (65.4%) had anaphylaxis. The median buckwheat specific IgE level in the buckwheat allergic group ($7.71kU_A/L$) was significantly higher (P<0.001) than in the buckwheat tolerant group ($0.08kU_A/L$) with an optimal cutoff value of $1.27kU_A/L$ (sensitivity 84.6%, specificity 100%). When adjusted for age, the difference between the 2 groups showed no statistical significance (P=0.063). In subjects who had anaphylaxis, buckwheat-specific IgE levels ranged from 0.37 to $100kU_A/L$. Conclusion: Almost two-thirds of buckwheat-allergic children had anaphylaxis, and a wide-range of buckwheat specific IgE levels were observed in these children. Anaphylaxis occurred in a subject with a remarkably low IgE level ($0.37kU_A/L$).

Congenital Thoracic Ectopic Kidney associated with Diaphragmatic Hernia in a 15-month-old Boy (선천성 횡격막 탈장과 동반된 이소성 흉강내 신장 1례)

  • Congenital thoracic ectopic kidney is a very rare developmental disorder and the rarest type of ectopic kidney. This condition is usually asymptomatic and detected incidentally on routine chest radiography. Most cases of thoracic ectopic kidney develop in adulthood and during the neonatal period, and congenital thoracic ectopic kidney rarely develops in children. Most patients are asymptomatic, and the treatment depends on the diagnosis. Herein, we report a rare case of ectopic thoracic kidney associated with a diaphragmatic hernia in a 15-month-old male infant, who presented with periodic severe irritability. The thoracic ectopic kidney was detected as a mass in the right base of the chest on routine chest radiography.

Novel SLC5A2 Mutations and Genetic Characterization in Korean Patients with Familial Renal Glucosuria

  • Purpose: Familial renal glucosuria (FRG, OMIM #233100) is a rare but relatively benign genetic condition characterized by persistent isolated glucosuria with a normal blood glucose level. We report three additional SLC5A2 mutations and examine their phenotypic and genetic characteristics in a Korean FRG cohort. We also reviewed the literature and summarized the genotypes of all Korean patients with FRG. Methods: A genetic analysis was conducted by directly sequencing all 14 exons of the SLC5A2 gene and their flanking regions in six unrelated Korean children with FRG and their family members. Novel non-synonymous single-nucleotide polymorphisms were identified and compared with known mutations that are repeatedly detected in the Korean population. Results: We found two novel mutations [c.274G>A (G92S) and c.1168C>T (L390F)] and one known [c.1382G>A (S461N)] mutation in each family and one recurrent mutation [c.1346G>A (G449D) (rs768392222)] in two pedigrees. The recurrent G449D was predicted to be "possibly damaging," with a score of 0.883 in Polyphen-2, while G92S, L390F, and S461N were predicted to be "probably damaging," with scores of 1.000, 0.999, and 0.996, respectively. Conclusions: Two novel, one previously reported, and one recurrent mutation were identified in six Korean FRG pedigrees as causative mutations of renal glucosuria. Sequence variations in the SLC5A2 gene were frequently detected in children with persistent isolated glucosuria. A long-term follow-up of this FRG cohort is needed to understand how these specific SGLT2 mutations impair kidney function and energy homeostasis.

A Machine Learning Approach for Stress Status Identification of Early Childhood by Using Bio-Signals (생체신호를 활용한 학습기반 영유아 스트레스 상태 식별 모델 연구)

  • Recently, identification of the extremely stressed condition of children is an essential skill for real-time recognition of a dangerous situation because incidents of children have been dramatically increased. In this paper, therefore, we present a model based on machine learning techniques for stress status identification of a child by using bio-signals such as voice and heart rate that are major factors for presenting a child's emotion. In addition, a smart band for collecting such bio-signals and a mobile application for monitoring child's stress status are also suggested. Specifically, the proposed method utilizes stress patterns of children that are obtained in advance for the purpose of training stress status identification model. Then, the model is used to predict the current stress status for a child and is designed based on conventional machine learning algorithms. The experiment results conducted by using a real-world dataset showed that the possibility of automated detection of a child's stress status with a satisfactory level of accuracy. Furthermore, the research results are expected to be used for preventing child's dangerous situations.

A Case of Neuroblastoma Presenting with Sudden Blindness (실명을 주소로 한 신경아세포종 1예)

  • Neuroblastoma is the most common extracranial solid tumor of childhood which presents various clinical symptoms depending on the primary and metastatic sites. However, it has been rarely reported that sudden onset of blindness was the chief complaint of neuroblastoma. A four years old boy was admitted to the Yeungnam University Hospital with the chief complaint of a sudden onset of blindness due to a distant metastasis of abdominal neuroblastoma to the sphenoid sinus. On admission, both side pupils were dilated without light reflex, fundoscopy showed pale optic disk, electroretinogram was subnormal and visual evoked potential showed no response. The liver was palpable in $3{\frac{1}{2}}$ finger breadth from the right costal margin and adult fist sized mass was palpable in the right flank. Skull X-ray showed destructed sphenoid bone and clinoid process and brain CT scan showed tumor mass in the sphenoid sinus and left orbit. Ultrasonogram and CT scan of the abdomen showed large tumor masses around the right kidney and para-aortic and retropancreatic lymph node. IVP showed displaced right calyceal system with preserved contour. Left supraclavicular lymph node which appeared after admission was biopsied and it showed poorly differentiated neuroblasts. He was treated according to the multiagent chemotherapy schedule for stage IV neuroblastoma patient of children's cancer study group. Abdominal tumor masses and sphenoid sinus mass were markedly reduced after 2 courses of the combination chemotherapy of cyclophosphamide, vincristine, DTIC, adriamycin and VM-26. Eventhough the blindness was not improved, the patient has been in good clinical condition.
