The effect of prepositioned upper cervical traction mobilization and therapeutic exercise on cervicogenic headache: A case study

  • The International Headache Society (IHS) has validated cervicogenic headache (CGH) as a secondary headache type that is hypothesized to originate due to nociception in the cervical area. CGH is a common form of headache and accounts for 15% to 20% of all chronic and recurrent headaches. CGH is commonly treated with manual and exercise therapy. To date, no studies have isolated only one manual intervention in an attempt to determine its effectiveness. In this case study we present a 28-year-old patient with right upper cervical (UC) and occipital pain who responded well to a single manual intervention technique. This technique was applied in isolation for the first three visits and two therapeutic exercises prescribed on the fourth and fifth visit. In total, manual and exercise intervention occurred over 8 visits at which point in time the patient was discharged with no UC motion impairments, an NPRS rating of 0, a NDI and HDI demonstrating a 100% improvement and a 37% improvement in FOTO score. The traction based manual intervention and two therapeutic exercises prescribed for this patient were successful in relieving UC pain and CGH. At six months follow up, the patient was still symptom free.

척추 측만증에 대한 견인 치료의 효과 (Traction Effects for the Treatment of Scoliosis)

  • The purpose of this research is to identify traction effects for the treatment of scoliosis and compare the effects among patients as their own physical characters. In this research, the patients with scoliosis hospitalized for ten days have treated in the way of the static traction for twenty hours a day, and alternating lumbar and cervical traction for twenty minutes two times a day in the department of rehabilitaton of Yong-Dong Severance Hospital. The followings are the conclusions of this research: 1) The traction for the patients with scoliosis is effective treatment method to reduce the curved angle (p<0.001). 2) Statistically there is no significant difference of the reduced curved angle between male and female after the traction (P>0.05). 3) Statistically there is significant difference of the reduced curved angle among age groups after the traction (P<0.05). 4) Statistically there is significant difference of the reduced curved angle among curved angle groups after the traction (p<0.001). 5) Statistically there is no significant difference of the reduced curved angle among shapes of curved angle after the traction (P>0.05).

Management of Unilateral Facet Dislocation of the Cervical Spine

  • Objective : Unilateral facet dislocation of the cervical spine occurs by flexion and rotation injuries and cannot be easily reduced by axial traction. We analyzed 14 consecutive patients with unilateral facet dislocation of the cervical spine to increase knowledge about anatomical reduction of locked facet and factors for successful reduction. Methods : Fourteen patients [10 men and 4 women] with unilateral facet dislocation of the cervical spine were retrospectively analyzed. Plain X-ray, computerized tomography scan, and magnetic resonance imaging were performed. All patients underwent manual reduction and surgery with anterior interbody fusion and plate fixation. The manual reduction was performed by neck flexion and rotation to the opposite side of dislocation, followed by rotation and flexion of the head toward the side of dislocation and extension with relaxation of traction. Mean follow-up period was 17 months. The level of spine, amount of subluxation, combined facet fracture, and time from injury to initial reduction were analyzed using the data obtained from medical records. Results : Thirteen [93%] patients were reduced successfully. Immediate reduction was achieved in 7 patients but failed in 7 patients. Seven patients underwent delayed closed reduction under general anesthesia, and successful reduction was achieved in 6 patients. Only one patient with bone chips between articular facets failed to achieve anatomical reduction. Conclusion : In order to reduce the locked facet more easily and safely, we recommend manipulative traction with anterior interbody fusion and plate fixation under general anesthesia after being aware of spinal cord injury with magnetic resonance imaging.

  • Objects : This study was to report a clinical effect of Chuna Treatment for Cervical Myelopathy due to disc herniation. Methods : The patient was diagnosed as Cervical Myelopathy due to disc herniation through Cervical spine MRI and treated by cervical traction technique of Chuna Treatment with conservative management. JOA score(Japanese Orthopedic Association score), Recovery rate of Hirabayashi, VAS(Visual Analogue Scale), Dynamometer score, ROM(Range of Motion) and Motor weakness Grade were used. Results and Conclusion : JOA score, VAS, Dynamometer score, ROM and Motor weakness Grade were improved and Recovery rate was 100%.

Diplopia developed by cervical traction after cervical spine surgery

  • Diplopia is a rare complication of spine surgery. The abducens nerve is one of the cranial nerves most commonly related to diplopia caused by traction injury. We report a case of a 71-year-old woman who presented with diplopia developing from abducens nerve palsy after C1-C2 fixation and fusion due to atlantoaxial subluxation with cord compression. As soon as we discovered the symptoms, we suspected excessive traction by the instrument and subsequently performed reoperation. Subsequently, the patient's symptoms improved. In other reported cases we reviewed, most were transient. However, we thought that our rapid response also helped the patient's fast recovery in this case. The mechanisms by which postoperative diplopia develops vary and, thus, remain unclear. We should pay attention to the fact that the condition is sometimes an indicator of an underlying, life-threatening condition. Therefore, all patients with postoperative diplopia should undergo thorough ophthalmological and neurological evaluations as well as careful observation by a multidisciplinary team.

  • Objectives : This study was peformed to evaluate the effect of Chuna Traction, Correction and conservative treatment. Methods : The patient was diagnosed as cervical bulging disc, protrusion and Uncovertebral joint arthrosis through Cervical spine MRI and treated with conservative treatment(Chuna, Acupunture etc.). We measured Visual Analog Score(VAS) and Range Of Motion(ROM) to evaluate treatment effects. Results and Conclusions : ROM of Cervical spine has improved. VAS score was also decreased.

The Surgical Management of Traumatic C6-C7 Spondyloptosis

  • A case of traumatic spondyloptosis of the cervical spine at the C6-C7 level is reported. The patient was treated succesfully with a anterior-posterior combined approach and decompression. The patient had good neurological outcome after surgery. A-51-year-old female patient was transported to our hospital's emergency department after a vehicle accident. The patient was quadriparetic (Asia D, MRC power 4/5) with severe neck pain. Plain radiographs, computerize tomography and spinal magnetic resonance imaging (MRI) showed C6-7 spondyloptosis and C5, C6 posterior element fractures. Gardner-Wells skeleton traction was applied. Spinal alignment was reachived by traction and dislocation was decreased to a grade 1 spondylolisthesis. Then the patient was firstly operated by anterior approach. Anterior stabilization and fusion was firstly achieved. Seven days after first operation the patient was operated by a posterior approach. The posterior stabilization and fusion was achieved. Postoperative lateral X-rays and three-dimensional computed tomography showed the physiological realignment and the correct screw placements. The patient's quadriparesis was improved significantly. Subaxial cervical spondyloptosis is a relatively rare clinical entity. In this report we present a summary of the clinical presentation, the surgical technique and outcome of this rarely seen spinal disorder.

  • Objects : This study was to report a clinical effect of Chuna Treatment for HIVD-cervical spine Failed Back Surgery Syndrome(FBSS) patient. Methods : In order to alleviate both arm tingling, numbness and neck stiffness, the patient was treated by acupuncture therapy, cervical traction technique of Chuna treatment and conservative managements. To evaluate the effect of the treatment, Recovery rate of Hirabayashi, Verbal Numerical Rating Scale(VNRS) and Neck Disability Index(NDI) score were used. Results : VNRS and NDI were improved and Recovery rate was 100%. Conclusions : Korean Treatment can be effectively used for a patient with HIVD-cervical spine FBSS patient. Further clinical studies are needed to verify the findings.

  • This study was undertaken to analyze the displacement and stress distribution in the mandible according to the pulling directions during mandibular first molar cervical traction after mandibular second molar extraction. The 3-dimensional finite element method(FEM) was used for a mathematical model composed of 594 elements and 1019 nodes. An orthodontic force, 450 gm, was applied to the each mandibular first molar in parallel, and below the occlusal plane by $7^{\circ}\;and\;25^{\circ}$ and meet the midsagittal plane by $40^{\circ}$ toward posterior direction. The results were as follows: 1. Mandibular teeth were displaced in more downward, posterior and lateral direction. Especially high stress was noted in case of parallel pull than in case of below the occlusal plane by $7^{\circ}\;and\;25^{\circ}$. 2. Mandibular first molar was moved bodily. 3. Generally, alveolar bone, mandibular body, ascending ramus and mandibular angle portion were displaced in downward, posterior and lateral direction. But coronoid process was displaced in downward, forward and lateral direction, and anterior and inner middle portion of condyle head and neck were displaced in downward, forward and medial direction, and posterior and outer middle portion of condyle head and neck were displaced in upward, forward and medial direction. 4. Maximum stress was observed at the condyle head and neck portion. With steeper direction of force, condyle head and neck showed more stress than parallel relation to the occlusal plane.

경추부 전산화단층촬영에서 선속 경화 인공물을 최소화하기 위한 견부 강제 견인법에 대한 임상적 유용성 평가 (Evaluation of Clinical Availability for Shoulder Forced Traction Method to Minimize the Beam Hardening Artifact in Cervical-spine Computed Tomography (CT))

  • 본 연구는 최근 자동노출제어장치에 의한 X선질 보정 및 다양한 수학적 보정 알고리즘 적용이 가능한 전산화단층촬영 장치를 이용하여 견부 강제 견인용 밴드의 사용 유 무에 따라 영상의 질과 환자의 편의성 및 안정성 측면에서 견부 강제 견인법에 대한 임상적 유용성을 제시하고자 하였다. 이를 위하여 경부 통증을 호소하는 환자 79명을 대상으로 견부 강제 견인용 밴드를 사용하기 전 후의 측면 투영 scout 영상과 횡단면 영상을 획득하여 횡단면 영상의 일정 크기의 관심영역 내의 화소 및 평균 HU 값을 비교하여 정량적 분석을 하였고 인공물과 해상도 및 분해능에 대한 임상 영상평가를 정성적으로 분석하였으며 환자가 느끼는 불편 정도를 자가 진단 설문 평가하였다. 결과적으로 측면 투영 scout 영상에서 견부 강제 견인용 밴드를 사용한 경우 묘출되는 경추의 수가 증가되었으나 횡단면 영상의 관심영역 내에서 견부 강제 견인용 밴드를 사용하기 전 후에 대한 화소 및 평균 HU 값의 변화는 거의 없는 것으로 판단되었으며 인공물과 해상도 및 대조도와 관련된 정성적 분석 결과에서 관찰자간 특이한 결과는 보이지 않았다. 따라서 견부 강제 견인용 밴드의 사용에 대한 자가 진단 설문 평가에서 환자의 82.27%는 불편함을 호소하였으며 정량적 및 정성적으로 영상의 질을 분석한 결과에서 사용에 따른 영상의학적 이점은 없는 것으로 판단되었다. 최근 다양한 수학적 보정 알고리즘에 의한 전처리 필터 과정의 적용과 더불어 절편 두께의 감소 및 자동노출제어장치 등에 의한 선질 보정 등이 가능한 전산화단층촬영 장치가 보급되면서 선속 경화에 의한 영상의 잡음은 문제가 되지 않을 것으로 판단되어 영상의 질에 영향을 주지 않으면서 환자에게 불편함을 주거나 추가적 위험성이 있는 견부 강제 견인용 밴드의 사용은 더 이상 임상적 유용성이 없는 것으로 판단되었다.
