Changes in Dermatomal Somatosensory Evoked Potentials according to Stimulation Intensity and Severity of Carpal Tunnel Syndrome

  • Objective : To investigate the change of latency of cervical dermatomal somatosensory evoked potential (DSEP) according to stimulation intensity (SI) and severity of carpal tunnel syndrome (CTS). Methods : Stimulation sites were the C6, C7, and C8 dermatomal areas. Two stimulation intensities $1.5{\times}$sensory threshold (ST) and $2.5{\times}ST$ were used on both normal and CTS patients. Results : In moderate CTS, the latencies of C6 and C7 DSEP during $1.5{\times}ST$ SI and those of C7 DSEP during $2.5{\times}ST$ SI were significantly delayed compared with the values of normal subjects. Significant correlation between the latency of C7 DSEP of $2.5{\times}ST$ stimulation and the median sensory nerve conduction velocity was observed. Conclusion : We suggest that these data can aid in the diagnosis of cervical sensory radiculopathy using low stimulation intensity and of those who have cervical sensory radiculopathy combined with CTS patients.

Arthroscopic Anterior Acromioplasty for the Treatment of Chronic Impingement Syndrome of the Shoulder (관절경적 전방 견봉 성형술을 이용한 만성 견관절 충돌 증후군의 치료)

  • Purpose : The purpose of this study was to evaluate clinical efficacy of the arthroscopic anterior acromioplasty for the treatment of chronic impingement syndrome of the shoulder. Materials and Methods : Between July 1995 and December 1997, twenty seven consecutive shoulders of 26 patients with chronic impingement syndrome of the shoulder were treated by arthroscopic anterior acromioplasty. The patients who had severe osteoarthritis of the shoulder full thickness tear of the rotator cuff, and nonoutlet impingement were excluded. The clinical results were evaluated by using UCLA shoulder rating scale. The average follow-up was 2years 3months(range, 1year 7months to 3years 1 11months). Results : Twenty three patients$(85.2\%)$ were rated as excellent or good results, while four patients$(14.8\%)$ were fair. Twenty six cases$(96.3\%)$ were satisfied with the results of the operations, while one case$(3.7\%)$, who had Parkinsonian syndrome, ossification of posterior longitudinal ligament(OPLL) of the cervical spine, and spinal stenosis of the 5th and 6th cervical spine was not satisfied. Conclusion : Arthroscopic anterior acromioplasty was an effective treatment method, especially for relief of pain, for the treatment of chronic impingement syndrome of the shoulder. If the patient has the combined lesions in the cervical spine and the shoulder, and systemic lesions, these lesions may influence the results of treatment after operation, and cause the unpredictable results.

The Effects of a Deep Cervical Flexion Exercise for Sedentary Workers with VDT Syndrome (VDT 증후군을 가진 사무직 근로자를 위한 심부경부굴곡 운동의 효과 연구)

  • The aims of this study were to compare and analyze two types of deep cervical flexion exercises, craniocervical flexion and cervical flexion, and to ascertain their relative effects on neck and shoulder pain and functional improvement. The participants of the study were individuals who work in sedentary jobs in an office environment. 54 appropriate subjects were chosen and randomly divided into two groups: one group underwent a craniocervical flexion exercise program and the other a cervical flexion exercise program. The six week exercise program consisted of home exercises performed by each subject five times a week and education once a week. Neck-shoulder pain, muscle strengthening, cervical alignment were measured prior to commencement of the exercise programs and again after six weeks. At completion of the six week, both exercise programs decreased neck pain (p<.05). Forward head postures were reduced, and the craniocervical flexion exercise program was more effective in reducing forward head posture (p<.05). The maximum muscle strength and 50% of maximum muscle strength maintaining time of the deep cervical flexor muscles were significantly increased. There were no significant changes of the cervical lordotic curve. The results of this study showed deep cervical flexion exercise was effective in the treatment of neck and shoulder pain, however craniocervical flexion exercise was more effective than cervical flexion exercise.

Eagle's Syndrome Presenting Unilateral Facial Nerve Palsy: Case Report (편측 안면신경 마비를 동반한 Eagle 증후군 치험례)

  • Eagle's syndrome is the manifestation of elongated styloid process that is not a common entity in dental clinics. Without the knowledge of this syndrome misdiagnosis can be made and therefore differential diag-nosis with other diseases of oral and maxillofacial area is required. The symptoms of elongated styloid process are cervical pain, foreign body sensation, dysphagia and pharyngeal pain. A 27 years old male vis-ited our hospital. He had cervical pain with unilateral facial nerve palsy. Along with meticulous clinical examinations, 3D-CT, Plain radiographic studies were used to make the diagnosis. In computed tomogra-phy, Lt. styloid process was elongated and fractured. Patient have undergone surgical resection of elongat-ed styloid process and have been carefully observed. Characteristically, preoperative facial nerve palsy showed rapid postoperative recovery. The other clinical symptoms that were present in the initial visit were diminished. The purpose of this article is to report a case of Eagle's syndrome presenting unilateral facial nerve palsy that was treated with transoral surgical approach with literature review.

A Study on the Use of Dong-Si Acupuncture Points at Movement System Impairment Syndrome of Shoulder and Cervical Spine (견(肩).경항부(頸項部) 동태손상증후군에서의 동씨침 혈위 활용 방안)

  • Objectives : The purpose of this study was to present clinical utility of therapeutic exercise on the neck and shoulder parts based on the movement system impairment syndrome(MSIS) as Dong-Qi therapy of the Dong-Si Acupuncture and was to examine which Dong-Si acupoints were most effective and non-invasive when performing therapeutic exercise of the MSIS. Methods : Totally eight therapeutic exercises correspondent to eight neck and shoulder MSIS were summarized and tabulated from the Diagnosis and Treatment of Movement Impairment Syndrome and Movement System Impairment Syndromes Of The Extremities, Cervical and Thoracic Spines by Sahrmann SA. Together with the MSIS summaries, acupuncture points and Dong-Qi therapy were summarized and tabulated from Yangweijiequanji 1 and Yangweijiequanji 2 by Yangweijie. According to the posture and movement of the MSIS exercise, effective and non-invasive acupoints were selected. Thereafter, clinical pilot study which five normal volunteers participated in were performed to examine whether these acupoints resulted in any side effects of acupuncture therapy such as pain and distortion of the needle during the MSIS exercises. Results : Through clinical pilot study, ZhongZi, ZhongXian, ZhengJin, ZhengZong and MuLiu in a sitting position, and HuaGuYi, ZhongZi and ZhongXian in a supine position were finally determined as safe and non-invasive Dong-Si acupoints to treat cervical pain. In terms of shoulder pain, ZuQianJin, ZuWuJin, HuaGuEr and JianZhong in a supine position, SiHuaZhong, ZuQianJin, ZuWuJin, QuLing, JianZhong, ShenGuan and JiuLi in a standing position, and ZuQianJin, ZuWuJin, HuaGuEr, QuLing and JianZhong in a prone position were finally accepted as safe and non-invasive Dong-Si acupoints. Conclusion : It is concluded that Dong-Si acupoints can be safely and non-invasively used together with therapeutic exercises of the MSIS to treat cervical and shoulder pains.

Changes in blood flow at the mandibular angle and Horner syndrome in a rat model of superior cervical ganglion block

  • Background: A stellate ganglion block (SGB) causes increased blood flow in the maxillofacial region, exhibiting the potential for regenerative effects in damaged tissue. The focus of this study was to understand the efficacy of SGB for regenerative effects against nerve damage. A rat model of the superior cervical ganglion block (SCGB) was created instead of SGB, and facial blood flow, as well as sympathetic nervous system function, were measured. Methods: A vertical incision was made on the left side of the neck of a Wistar rat, and a 5-mm resection of the superior cervical ganglion was performed at the back of the bifurcation of the internal and external branches of the left common carotid artery. Blood flow in the skin at the mandibular angle and mean facial temperature were measured using a laser-Doppler blood flow meter and a thermographic camera, respectively, over a 5-week period after the block. In addition, the degree of ptosis and miosis were assessed over a period of 6 months. Results: The SCGB rat showed significantly higher blood flow at the mandibular angle on the block side (P < 0.05) for 3 weeks, and significantly higher skin temperature (P < 0.05) for 1 week after the block. In the SCGB rat, ptosis and miosis occurred immediately after the block, and persisted even 6 months later. Conclusions: SCGB in rats can cause an increase in the blood flow that persists over 3 weeks.

A Case Report of Recurrent Cervical Intraepithelial Neoplasia Treated by Korean Traditional Medicine and Mistletoe Extract Inject (미슬토 추출물 투여와 병행한 한방치료를 통해 호전된 재발성 자궁경부상피내종양 증례보고)

  • Objectives: The purpose of this study is to report the improvement of recurrent cervical intraepithelial neoplasia (CIN) and chief complaint including dysmenorrhea, premenstrual syndrome after Korean medicine treatment. Methods: The patient who diagnosed CIN even after undergoing two times of loop electrosurgical excision procedure (LEEP) and complained dysmenorrhea with premenstrual syndrome was treated by acupuncture, moxibustion and herbal medicine as Ojeok-san-gami-bang along with mistletoe extract injection (Abnobaviscum®). The effect of treatment was evaluated by the results of liquid based cytology and HPV genotyping. Other symptoms were evaluated according to the patient's subjective complaint. Results: Before the treatment, the result of cytology was low grade squamous intraepithelial lesion and a low-risk group for HPV was detected. At the first examination after treatment, cytology showed negative for intraepithelial lesion or malignancy and the HPV genotyping was negative. The result showed negative findings in 3 consecutive follow-up tests. In addition, the chief complaint and general conditions were improved. Conclusion: This study shows that the recurrent cervical intraepithelial neoplasia (CIN) was improved after the Korean traditional treatment and it can be effective medical alternatives or options for patients receiving mistletoe injection during follow-up.

Immediate Effects of Moving Myofascial Decompression Therapy for Young Adults with Nonspecific Neck Pain

  • Objective: The purpose of this study was to investigate the effects of myofascial decompressiontherapy using moving suction on body temperature, pain, neck disability index, and cervical rotation for young adult with nonspecific neck pain. Design: Two-group pretest-posttest design. Methods: The subjects were randomly assigned 22 patients with chronic cervical pain who met the study conditions to the experimental group (n=11) and the control group (n=11). In the experimental group, the myofascial decompressiontherapy (MDT) was performed for 10 minutes using moving suction withnegative 15 mmHg pressure from the insertion to the origin of the upper trapezius muscle, while the control group without negative pressure. In order to investigate the effects of the intervention, an infrared thermometer, a visual analogue scale, neck disability index, and goniometer were used. Results: As a result of comparing the pre- and post- changes in each group according to the intervention, skin temperature, pain, neck disability index, and cervical rotation in both the experimental and control groups were significantly improved (p<0.05). Comparison of pre- and post- changes between the experimental and control groups showed significant differences for pain and cervical rotation (p<0.05), but no significant difference was found in the body temperature and neck disability index. Conclusions: Based on the results, MDT using moving suction was effective in reducing pain and increasing of cervical rotation for young adult with nonspecific neck pain.

HORNER'S SYNDROME AFTER ORTHOGNATHIC SURGERY (악교정수술 후 발생한 호르너 증후군)

  • Horner's syndrome as a complication of orthognathic surgery is given little attention of head and neck surgery and is a relatively benign and transient condition. A 18-year-old male referred to our department with long and anteriorly projected chin. The cephalometric evaluation revealed a skeletal Class III relationship. A 10-mm setback of the mandible to eliminate Class III relationship and 4-mm vertical reduction genioplasty were performed. Three weeks after operation, the patient was recognized anhidrosis in left face and the head, and ptosis of left eye. The trauma to cervical sympathetic nerve during left sagittal split ramus osteotomy was thought to be the cause of Horner's syndrome. Patient was treated by dermatologic and opthalmologic care. Follow-up examination 8 months later, he was recovery of horner's symptom.

  • Thoracic outlet syndrome refers to compression of the subclavian vessels and brachial plexus at the superior aperture of the thorax. it was previously designated according to presumable etiologies such as scalenus anticus, costoclavicular, hyperabduction, cervical rib and first thoracic rib syndromes. We experienced a case of thoracic outlet syndrome[costoclavicular syndrome] which was caused by posttraumatic left clavicular fracture. Patient had suffered from swelling and cyanosis of left forearm and hand. preoperative vascular doppler test, angiography and venography were performed. First rib resection was done with transaxillary approach. After operation preoperative cyanosis and swelling of left forearm and hand were disappeared. Postoperative course was uneventful.
