Analysis on the Acupuncture Contents of the Domestic Neck Pain and HIVD-Cervical Spine Clinical Studies : a literature review

  • Objectives : Cervical herniated intervertebral disc (HIVD) are common diseases. They can be managed with acupuncture, but the evidence for effectiveness is uncertain. This review analyzed the acupuncture studies of domestic neck pain and cervical HIVD; the purpose of this study was to provide basic data useful for future research. Methods : We investigated acupuncture treatments for neck pain and cervical HIVD by searching 5 Korean Internet databases. The keywords used were "neck pain", "HIVD-cervical spine", and "nuchal pain". A total of 53 research papers (17 case reports, 16 clinical data analyses, 11 randomized controlled trials, and 9 non-randomized controlled trials) were found and analyzed according to the publication year, type of study, treatment, use of filiform needles, and type of pharmacopuncture used acupoint. The effectiveness of acupuncture treatment was determined. Results : 1. Filiform needles have been primarily used in domestic research and were used in at least half of published pharmacopuncture studies. 2. In 51 papers using filiform needles, many studies used only local acupoints; few studies used only distant acupoints. 3. All studies using pharmacopuncture were performed using local acupoints. In particular, the studies based on A-shi point, trigger point, and radiologic lesion sites were useful for multiple purposes. Conclusion : In this study, we analyzed the acupuncture contents of the domestic neck pain and HIVD-cervical spine clinical studies. This study considers the assessment of the quality and efficacy of each study, which is likely to require research that reflects the future.

A Case Report on HIVD-Cervical Spine Failed Back Surgery Syndrome Applied Chuna Treatment (추나 요법을 적용한 경추 추간판 탈출증 척추 수술 실패 증후군 환자의 경과관찰 1례)

  • Objects : This study was to report a clinical effect of Chuna Treatment for HIVD-cervical spine Failed Back Surgery Syndrome(FBSS) patient. Methods : In order to alleviate both arm tingling, numbness and neck stiffness, the patient was treated by acupuncture therapy, cervical traction technique of Chuna treatment and conservative managements. To evaluate the effect of the treatment, Recovery rate of Hirabayashi, Verbal Numerical Rating Scale(VNRS) and Neck Disability Index(NDI) score were used. Results : VNRS and NDI were improved and Recovery rate was 100%. Conclusions : Korean Treatment can be effectively used for a patient with HIVD-cervical spine FBSS patient. Further clinical studies are needed to verify the findings.

Investigation of Beck's Depression Inventory Score of Patients with Cervical and Lumbar Herniation of Intervertebral Disc (만성 경추, 요추 추간판 탈출증 질환군의 우울 척도(BDI) 비교 연구)

  • Objective : It Is known that depression disorder has been related to chronic pains such as HIVD and physical harm. We propose that chronic cervical and lumbar herniations of intervertebral disc patients have emotional and psychiatric problems, therefore we compared it to Beck's Depression Inventory scores. Method : We divided them into two groups: The groups consisted of cervical and lumbar HIVD patients. We then requested them to fill out BDI research questionnaires, and evaluated patients according to the information and results. Result : The Beck's Depression Inventory mean score for the cervical - lumbar HIVD group was $14.00{\pm}5.80$, $10.83{\pm}$5.64 each. Higher Scores were recorded for cervical HIVD group than the lumbar HIVD group. Conclusion : There is statistical significance among two groups.(p<.05) however, the two groups' BDI score were lower than the defined boundary line of Depression disorder(16 points).

The Effects of Proprioceptive Neuromuscular Facilitation for Pain, Grip Strength on Cervical Herniated Intervertebral Disc (HIVD) (고유수용성신경근촉진법이 경추추간판탈출증 환자의 통증 및 손의 악력에 미치는 효과)

  • Purpose: The current study aims to identify the effects of proprioceptive neuromuscular facilitation (PNF) on pain and grip strength in patients with cervical herniated intervertebral disc (HIVD) and provide effective interventions for such patients. Methods: Included in this study were 20 subjects (10 men, 10 women) in their 30s-50s who had been diagnosed with cervical HIVD. Intervention methods included PNF neck patterns, dynamic reversal of antagonists, and a combination of isotonics. The visual analogue scale (VAS) was used to measure pain and dynamometers were used to measure grip strength. Results: After four weeks of muscle strengthening exercises, neck pain was significantly reduced, and hand grip strength significantly increased. Conclusion: PNF can be utilized as an effective intervention to reduce the pain of patients with cervical HIVD and to increase grip strength.

  • Objectives : The purpose of this study is to report the image changes of five cases of Cervical intervertebral disc Herniation after Oriental medicine Treatment. Patient & Methods : We examined 5 patients with Cervical intervertebral disc Herniation (HIVD of C-spine) who showed changes on MRI images before/after the treatment among HIVD of C-spine patients who visited Cervical Spine center, Jaseng Hospital of Oriental Medicine from March, 2006 to May, 2007. Results & Conclusions : In this study, the first MRI examination of HIVD of C-spine patients was performed at the first visit and re-examination of MRI was done after the treatment. We assessed clinical symptoms by using Numerical rating scales(NRS). In each case, the size of the disc herniation was considerably reduced in MRI image. NRS was also reduced significantly.

An Analysis of Treatment and Economic Evaluation on the Part of Cervical HIVD Inpatients at Korean Medicine Hospital (한방병원에 입원한 경추 추간판 탈출증 환자의 치료 및 경제성 평가 연구)

  • Objectives Herniated Intervertebral Disc of C-spine is one of the most common diseases that causes posterior neck pain. This study was designed to analyze the general distribution and hospital cost by day and case of Korean medical treatment for Cervical Herniated Intervertebral Disc (HIVD). Methods The 132 impatients for treatment of HIVD were analyzed according to the distribution of sex, age, the duration of HIVD, the contributory factors, the Clinical grade at admission, the clinical findings at admission, the duration of hospitalization, the clinical grade at admission and the hospital cost per day and case. Results 1) The total hospital cost per case averaged 1,985,600 Won, which was consisted of room charge 584,044 Won (29.41%), performance fee 511,463 Won (25.76%), herbal medication 381,517 Won (19.21%), Korean medical physiotherapy 296,310 Won (14.92%), food expenses 199,997 Won (10.07%) in order. 2) The total hospital cost per day averaged 137,285 Won, which was consisted of room charge 39,036 Won (28.43%), performance fee 33,594 Won (24.47%), herbal medication 30,642 Won (22.32%), food expenses 12,870 Won (9.37%), and the average duration of hospitalization was 15.1 days. 3) There was statistically significant difference in the consultation fee, room charge, and herbal medication on the part of sex. 4) There was statistically significant difference not only in the performance and consultation on the part of duration of hospitalization but also the in the duration of average duration of hospitalization. 5) There was statistically significant difference in the Korean medical physiotherapy and herbal medication on the part of clinical findings. 6) There was statistically significant difference in the Korean medical physiotherapy and herbal medication on the part of duration of HIVD. 7) There was statistically significant difference in the Korean medical physiotherapy and herbal medication on the part of clinical outcome. 8) There was not only statistically significant difference in the performance, but also in the herbal medication on the part of clinical grade at admission. Conclusions This study provides plenty of information to design out the specific terms of Korean medical expenses of Cervical HIVD inpatients hospitalized at Korean medicine hospital.

An Analysis of the State of Cervical Disc Herniation According to Cervical Lordosis and Age (경추 전만 정도 및 연령에 따른 경추 추간판 탈출 양상 분석 연구)

  • Objectives : The aim of this study is to investigate the state of cervical disc herniation according to the degree of cervical lordosis and age. Methods : 67 records of inpatients who were diagnosed with herniated intervertebral disc(HIVD) of the cervical spine were analyzed. Cobb's method, Jochumsen method and the Ishihara index were used to measure the degree of cervical lordosis. The state of the cervical disc was identified using magnetic resonance imaging(MRI) of the cervical spine. Then correlations among cervical lordosis, age and cervical disc herniation were analyzed. Results : Disc bulging was associated with hypolordosis and disc protrusion was associated with hyperlordosis and age. Disc extrusion was not associated with either cervical lordosis or age. The number of disc herniations in the cervical spine was correlated with age significantly, but not with cervical lordosis. Conclusions : Cervical disc herniation had a tendency to correlate with age and cervical lordosis, although this is not definite. Future studies that analyze more radiographic images of patients with HIVD of the cervical spine might be necessary to identify the influence of cervical lordosis on cervical disc herniation.

  • Objectives : The purpose of this clinical study was to evaluate the effect of spinal decompression with Korean medicine treatment on the herniation of an intervertebral disc (HIVD) in a patient after fusion surgery of a cervical segment. Methods : A single patient was treated with spinal decompression, acupuncture, pharmacoacupuncture, and herbal medication. We measured the Numerical Rating Scale (NRS), Neck Disability Index (NDI), European Quality of Life-5 Dimension (EQ-5D), Cervical Range of Motion (cervical ROM) and the Beck Depression Inventory (BDI) score to evaluate the treatment effects. Results : The patient's post neck pain & pain in both arms improved significantly as suggested by the scores on the NRS, NDI, EQ5D, cervical ROM and BDI. Conclusions : Thus, spinal decompression, including Korean medicine treatment, could be taken into consideration for HIVD patients after fusion surgery.

  • Objective : To report cases of HIVD of C&L-Spine successfully treated with conservative Korean Traditional Medicine treatment with Megadose Pharmacoacupuncture. Methods : Four patients diagnosed with HIVD of C&L-Spine were treated with Megadose Pharmacopuncture, acupuncture and herbal medicine. Numeric Rating Scale(NRS), Neck Disablility Index(NDI) and Oswestry low back pain Disablility Index(ODI) scores were collected before and after treatment for comparison. Results : There was a significant decrease in NRS, NDI and ODI scores. The mean NRS score decresed from 9 to 2.25, NDI from 45 to 14 and ODI from 46.66 to 11.33. Conclusion : The result suggests Megadose Pharmacoacupuncture can be an effective treatment on patients with HIVD of C&L-Spine.

The Relationship between Cervical Intervertebral Space in X-ray and Herniated Disc in CT (X-ray상 경추 추체간 간격과 CT상 경추 추간판 탈출증의 상관관계에 대한 고찰)

  • Objectives To investigate the relationship between cervical intervertebral space narrowing in the X-ray and HIVD level of cervical spine in the CT image. Methods We studied 101 subjects who were taken cervical spine X-ray and CT. Intervertebral space were measured at the anterior, middle and posterior portion of each cervical disc in the X-ray. Considering individual difference, intervertebral space was set as value divided by upper width of lower vertebral body. We analyzed statistically adjusted intervertebral space of normal group and herniated disc group using student' t t-test. Results Intervertebral space of cervical spine was narrowed in the herniated disc group. But there was no significant difference at C2~C3. Conclusions We found that narrowing intervertebral space of cervical spine is associated with herniated disc. Further clinical observation is needed.
