Delayed Diagnosis of Cerebral Infarction after Complete Occlusion of ICA due to Blunt Head Trauma: A Case of Report (두부둔상 후 내경동맥손상으로 인한 뇌경색의 지연진단: 증례보고)

  • Blunt cerebrovascular injury is defined as a vertebral or carotid arterial structural wall injury resulting from nonpenetrating trauma. Complete traumatic internal carotid artery occlusion is very rare condition accounting for 0.08~0.4 0f all trauma patients and believed to be associated with the greatest risk of ischemic stroke reported in 50~90% in a few small series. A 55-year-male was admitted with drowsy mentality and severe headache after a fall down accident. Brain computed tomography showed a subdural hematoma at the both frontal area with a fracture of the occipital skull bone. Two days after admission, he suddenly complained with a right side hemiparesis of motor grade 2. Brain magnetic resonance diffusion demonstrated multiple high flow signal changes from the left frontal and parietal lesion. Computed tomographic angiogram (CTA) revealed absence of the left ICA flow. Trans femoral cerebral angiography (TFCA) showed complete occlusion of the left internal carotid artery (ICA) at ophthalmic segment in the left ICA angiogram and flows on the left whole hemispheric lesions through the anterior communicating artery in the right ICA angiogram. We decided to conduct close observations as a treatment for the patient because of acute subdural hematoma and sufficient contralateral cerebral flow by perfusion SPECT scan. Two weeks after the accident, he was treated with heparin anticoagulation within INR 2~4 ranges. He recovered as the motor grade 4 without another neurologic deficit after 3 months.

An Analysis of Cases over which Administrative Litigation was made regarding Cerebral and Cardiovascular Diseases due to Occupational Cases (업무상 뇌심혈관질환 관련 행정소송을 수행한 판례 분석)

  • This study collected 217 cases of court ruling statements for the cases over which administrative litigations were made regarding the acknowledgment of cerebral and cardiovascular diseases arising out of duty against Korea Labor Welfare Corporation and analyzed the factors of occurrence of cerebral hemorrhage and infarction and the Court cases of cancelation and dismissal of the litigation. As a result, due to seasonal factors, cerebral hemorrhage occurred more in the winter while cerebral infarction, in spring. The incidences for each age group were the higest in people in their 40s for cerebral hemorrhage while in those in their 50s for cerebral infarction, it turned that the incidence inside the places of business was the highest. The average days from application for care and family benefits until the confirmation of the case was 31 months on average for 34 canceled cases while 23 months for 183 dismissed ones, and the average number of months working until the occurrence of accident was 80.8 months for the 34 canceled cases while 77.6 for the 183 dismissed ones. This study has a significance in that it analyzed leading cases of confirmed administrative litigations in some cases applied for diseases due to occupational cases after the occurrence of cerebral and cardiovascular diseases but not approved, through which it is expected to be used as the basic data to reduce time and economic loss generated by the litigations to judge the acknowledgment of diseases due to occupational cases.

Comparison of the Effects of Meridian Massage and Hand Massage on the Affected Upper Extremity of Stroke Patients (뇌졸중 환자를 위한 환측 상지 경혈지압마사지와 손마사지의 효과 비교 연구)

  • Purpose: In this study, the effects of meridian massage and hand massage on the affected upper extremity of patients with a stroke were identified. Method: A nonequivalent control group non-synchronized design was used. Participants were 84 (Experimental. 1: 28, Experimental. 2: 28, Control.: 28) patients with a stroke who were hospitalized in K oriental medical center. Experimental treatment for patients in experimental group 1 was the meridian massage for 10 minutes daily for 2 weeks, and for those in experimental group 2, hand massage for 2 minutes, 30 seconds each, daily for 2 weeks. Outcome variables were upper extremity function (grip power, shoulder pain, edema, ROM), ADL, and depression. Data were analyzed using SPSS PC+ version. Results: The score for upper extremity functions and ADL were significantly higher in experimental group 1 who had meridian massage than for experimental group 2 or the control group. Experimental group 1 showed less depression than experimental 2 or the control group. Conclusion: Meridian massage can be an effective nursing intervention to improve upper extremity function and ADL, and to decrease depression in patients stroke who have had a stroke.

  • Abdominal aortic aneurysm has traditionally been treated by open repair. Aortic endovascular stent grafting has recently been introduced as a new modality. We report here on three cases of endovascular stent grafting that were performed by cardiovascular surgeons for the treatment of abdominal aortic aneurysm in the high risk patients with multiple comorbidities such as old age, hypertension, renal failure, cerebrovascular accident and immobility.

The Effects of Plantar Foot Pressure and Muscular Activity on Treadmill Gait Training in Stroke Patients (트레드밀 보행훈련이 편마비 환자의 족저압과 하지 근활성도에 미치는 영향)

  • Purpose:This study was performed for effects of gait training on treadmill and stable surface which influenced on the lower limbs muscular activity needed in gait, plantar foot pressure with hemiplegic patients caused by cerebrovascular accident. Methods:Two groups of adult hemiplegia(n=20) were allocated randomly in this study: treadmill gait training group and control group. The gait training program was provided to experimental groups for 8 weeks (5 times a week). Measurements of pre and post experiment were plantar foot pressure. For measuring muscular activation rectus femoris, biceps femoris, tibialis anterior, gastrocnemius were detected. Results:The results of this study showed that in comparison of pre and post changes of gait training, the treadmill gait training group has noticeable changes than other groups in activity of rectus femoris and tibialis anterior, the control group revealed statistically significant differences in plantar foot pressure Toe2-5, M1, M3, M5, MF area, activity of gastrocnemius. Conclusion:These results mean gait training resulted by treadmill, stable surface provides effective muscle activation and plantar foot pressure with stroke.

Epidemiologic Study of Geriatric Cerebrovascular Accident Inpatients (노인 뇌졸중 입원환자의 역학적 연구)

  • This study was done to research the general information, causes of stroke, risk factors, complications during admission of geriatric CVA inpatients. We retrospectively studied 208 above 65 years old geriatric CVA inpatients treated in Ajou university hospital from 1994 to 2003 by medical record. These were divided into two groups by following years. We were gathered information about sex, onset age, cause of stroke, number of stroke, complications, housing, family, insurance, smoking, alcohol. Most of incidence of the stroke was noted in the group of young old age (65~74 years old) and old age (75~84 years old). The occurrence rate of male stroke (38.5%) was less than of female stroke (61.5%) and the ratio of male to female was 1:1.6. The occurrence rate of ischemic stroke (72.6%) was higher than of hemorrhagic stroke. The occurrence rate of ischemic stroke increased more and more at the late stage. The most common risk factors for stroke was hypertension and complications during hospitalization were neurogenic bowl and bladder. These results of epidemiologic study may help above 65 years old geriatric CVA early treatment and prevention, rehabilitation and use basic data for multiple prospective study using stroke registry.

  • The present experiment was designed to understand the effect of Boganhwan on the cardiovascular system in experimental animals. And thus the Blood pressure, isometric movement of atrium,Mg++-Ca++-ATPasc activity of sarcoplasmin reticulum. liver function, prothrombin time, and changes of blood composition were measured in the presence of Boganhwan. The results obtained here were as following: 1. The blood pressure decreased in the presence of SAMOOLTANG, and the decreasing effect was more stimulated by adding Bangpoong and Ganghwa(Boganhwan) 2. The oral administration of the drug for 7days also demonstrated the decreasing of blood pressure and the effect was stimulated by adding Bangpoong and Kanghwal. 3. The autonomic nerve blocking agents such as atropine and regitine did not demonstrate the effect on the drug action. 4. Boganhwan inhibited the cardiac isometric movement and rate by stimulating the Mg++-Ca++-ATPase activity of the heart sarcoplasmins reticulum. 5. Boganhwan increased the number of red blood cell, hematocrit percentage, hemoglobin concentration, and prothrombin time. 6. The drug stimulated the liver metabolism by stimulating the total ATPase activity. According to the results. Boganhwan demonstrated the decreased blood pressure and it also increased the hemoglobin concentration and the hematocrit percent. These effects stanches the hypertension. anemia, and cerebrovascular accident.

  • Intracranial tuberculoma is very rare nowadays. It is very difficult to differentiate tuberculoma from other space-occupying lesions. CT and MRI and intensive systemic review is very useful to diagnose intracranial tuberculoma. This case of patient had several symptoms of hemiparesis, headache. And we supposed that his symptoms were due to cerebrovascular accident at entry. But he was diagnosed as tuberculoma with brain edema in cerebrum. After treating him with mannitolization, acupuncture and herb medicine, his symptoms were remarkably improved. So we report this case of tuberculoma in cerebrum.

  • This research study has been conducted to identify and evaluate the effectiveness of Sanjoin-Tang(酸棗仁湯) for insomnia relief. The clinical testing of Sanjoin-Tang(酸棗仁湯) was carried out at Oriental Medical Center of Dong-Eui University with the participation of 30 patients. The results of Sanjoin-Tang(酸棗仁湯) treatments show as follows: Change of total amount of sleep showed 1.75±2.23 hours of increase.(p-value=0.001, respectively) Change of average delaying bedtime before and after the treatment showed 1.57±2.50 hours of decrease.(p-value=0.001, respectively) Change of average bedtime arousal before and after the treatment showed 0.96±1.46 times of decrease. (p-value=0.004, respectively) Change of sleep quality after the treatment has improved. According to statistical results, Sanjoin-Tang(酸棗仁湯) has proven effective for reducing sleep disorders, and this, restoring normal sleep patterns.

Ginseng radix Suppresses Ischemia-induced Increase in c-Fos Expression and Apoptosis in the Hippocampal CA1 Region in Gerbils

  • Ginseng radix, the root of Panax ginseng C.A.Meyer (Araliaceae), has traditionally been used for the treatment of various disorders including cerebrovascular accident (CVA). In the present study, the effect of Ginseng radix on c-Fos expression and apoptosis in the hippocampal CA1 region of gerbils following transient global ischemia was investigated via immunohistochemistry for c-Fos and caspase-3 and terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL) assay. Enhanced c-Fos-, TUNEL-, and caspase-3-positivities were detected in the hippocampal CA1 region in ischemic gerbils. Administration of the aqueous extract of Ginseng radix suppressed this ischemia-induced increment in the numbers of c-Fos-, TUNEL-, and caspase-3-positive cells. These results suggest that Ginseng radix has an inhibitive effect on the induction of c-Fos expression and apoptosis seen following transient global ischemia.
