  • Apoplexy is a disease of a morbid condition manifested as sudden syncope, unconciousness, distortion of face, hemiplegia and dysphasia, usually seen in the middle-aged. The symptoms and signs before sudden onset are headache, dizziness, numbness of extremities, palpitation, etc. This study was performed to investige causes of disease, therapies and prescriptions by insect medicine through the successive medical literatures, recent chinese medical literatures and chinese medical journals. The results are as follows; 1. The treatment of apoplexy is divided into stage of attack and sequela. In stage of attack, the principal therapies of apoplexy are calming the liver, suppressed Yang, dissipate phlegm and elimination weatness. In sequela, the principal therapies of apoplexy are invigorating Qi, promote blood circulation and dredge collateral. 2. Insects medicine have more strong effect than herbal medicine, because apoplexy is a kind of critically desease. 3. Insects medicine is effective in a convalscent stage and sequela of apoplexy. The proper dosage for stage of attack is a small dose of insects medicine(about 2-4g), increse gradually. In convalscent stage, about 4g, in sequela, patients need a large dose of insects medicine(about 8g). 4. Hirudo used to remove stagnated blood and to disperse swelling for the treatment of severe cases of blood stasis, such as cerebral infarction, sequela of cerebrovascular accident, contused wounds. Lumbricus used to for the treatment of convulsions due to high fever, and for hemiplegia and hypertension. Scolopendra used to subdue the endogenous wind for the treatment of various kinds of tics, convulsions and tetanus, and it's character is strong because it will be effective Sthenia-Syndrome of apoplexy. Scorpio used to subdue the endogenous wind for the treatment of various kind of tics, convulsions, tetanus and sequela of cerebrovascular accident.

  • Objectives : To determine the contribution of cranial magnetic resonance angiography(MRA) for the evaluation of patients with ischemic cerebrovascular accident. Methods : Magnetic resonance image(MRI) and MRA studies performed on 34 patients with ischemic stroke were retrospectively reviewed with the clinical records. Results : There were 9 transient ischemic attacks and 25 completed strokes in our series. Twenty-three of 34 MRA examinations(68%) were positive for stenosis or occlusion. The distribution of stenotic or occlusive vascular lesions were correlated with the location of infarction in 22 of the 23 positive cases(96%). MRA provided additional information not obtained from the MRI in 19 cases(56%). Conclusions : Vascular lesions demonstrated on intracranial MRA show a high correlation with infarct distribution. MRA provided information adjunctive to conventional MRI in a majority of cases. We conclude that MRA is an important noninvasive component of the complete evaluation of ischemic stroke.

  • Background and Purpose : The aim of the present study was to investigate the prodromes of stroke in already attacked patients and to prove the traditional hypothesis that some symptoms were to be prodromes of stroke in the oriental medicine. Methods : The questionnaire which was based on symptoms of traditional hypothesis was distributed to subacute stroke patients who were confirmed by Brain CT or MRI in Oriental Medical Hospital, Kyung Hee University, except patients with indistinct onset, symptoms proved any other disease. Results : Eighty-three patients(82.2%) felt some prodromal symptoms and fifty-nine patients (58.4%) underwent a change in their life within one week before onset. Most common prodromal symptoms was the weakness or numbness of limbs, single or unilateral. Conclusion : Our results suggest that the prodromal symptoms before stroke can be regarded as predicting sign. And we think that these research may contribute to preventing stroke and relapse.

  • This study was designed to investigate factors related to recovery from cerebrovascular accidents(CVA). Medical charts of 100 CVA patients(40 males and 60 females) who had been treated at Bul-Guang hospital in Teagu from June to December 1994 were reviewed to assess their recovery from CVA. Not only types of CVA, blood pressure and serum cholesterol and triglyceride were factors affecting recovery from CVA, but also smoking and drinking status and food preference seemed to be important factors. The percentages of recovered patients were higher in the order of cerebral thrombosis(83.3$\%$), subarachnoid hemorrhage(57.1$\%$), cerebral embolism(50.0$\%$), and cerebral hemorrhage(26.7$\%$). Recovery rates of patients with serum cholesterol below 200mg/dl, 200-239mg/dl, over 240mg/dl were 81.8$\%$, 66.6$\%$, and 16.6$\%$ respectively. Recovery rates of patients with serum triglyceride below 160mg/dl, 160-209mg/dl, and oover 210mg/dl were 84.6%, 72.8$\%$, and 35.7$\%$ respectively. Patients with standard weight recovered better than those with overweight or obesity. Recovery rates of underweight, standard weight, overweight and obesity patients were 73.3$\%$, 85.7$\%$, 45.8$\%$, and 31.6$\%$ respectively. Smoking and drinking seemed to be important factors which inhibited recovery from CVA. Patients preferring spiced foods were recovered better than those preferring salty or pungent foods. • The purpose of this case-control study is to investigate the relationship between storke and stroke risk factors, such as family history and past history. The case-control study over clinical data registered from Daejeon Oriental Medical Hospital in the period of November 2006 to July 2010. Study subjects consisted of 108 patients with acute stroke within 1 month as the case group (Cases) and 108 people who visited hospital for health care as the genreral control group (Controls). The participants had been interviewed by residents to find out their family history and past history. Their blood was taken to check the blood lipid level and liver function. 1. The people whose family history included cerebrovascular accident had more probability of stroke than the people who did not have cerebral vascular accident as their family history. 2. The people who suffered from hypertension had more probability of stroke than the people who did not suffer from hypertension. 3. The people who suffered from diabetes mellitus had more probability of stroke than the people who did not suffer from diabetes mellitus. Based on this study, the people who had cerebral vascular accident as their family history should thoroughly treat their blood pressure and blood sugar level to prevent the cerebral vascular accident.

  • Moyamoya disease (MMD) is a progressive occlusive disease of the cerebral vasculature with particular involvement of the circle of Willis and the arteries that feed it. MMD is one of cerebrovacular accident, which is treated with sugical maeuver in pediatic neurosurgery. Moyamoya (ie, Japanese for "puff of smoke") characterizes the appearance on angiography of abnormal vascular collateral networks that develop adjacent to the stenotic vessels. The steno-occlusive areas are usually bilateral, but unilateral involvement does not exclude the diagnosis. The exact etiology of moyamoya disease is unknown. Some genetic predisposition is apparent because it is familial 10% of the time. The disease may be hereditary and multifactorial. It may occur by itself in a previously healthy individual. However, many disease states have been reported in association with moyamoya disease, including the following: 1) Immunological - Graves disease/thyrotoxicosis 2) Infections - Leptospirosis and tuberculosis 3) Hematologic disorders - Aplastic anemia, Fanconi anemia, sickle cell anemia, and lupus 4) Congenital syndromes - Apert syndrome, Down syndrome, Marfan syndrome, tuberous sclerosis, Turner syndrome, von Recklinghausen disease, and Hirschsprung disease 5) Vascular diseases - Atherosclerotic disease, coarctation of the aorta and fibromuscular dysplasia, 6)cranial trauma, radiation injury, parasellar tumors, and hypertension etc. These associations may not necessarily be causative but do warrant consideration due to impact on treatment.(Mainly neurosurgical operation.) The incidence of moyamoya disease is highest in Japan. The prevalence of MMD is 1 person per 100,000 population. The prevalence and incidence of moyamoya disease in Japan has been reported to be 3.16 cases and 0.35 case per 100,000 people, respectively. With regard to sex, the female-to-male ratio is 1.4:1. A bimodal peak of incidence is noted, with symptoms occurring either in the first decade(5-10yr) or in the third and fourth decades (30-40yr)of life. Mortality rates of moyamoya disease are approximately 10% in adults and 4.3% in children. Death is usually from hemorrhage. In aspect of life insurance, MR is 1700%, EDR is 16 per 1000 persons. Children and adults with moyamoya disease (MMD) may have different clinical presentations. The symptoms and clinical course vary widely from asymptomatic to transient events to severe neurologic deficits. Adults experience hemorrhage more commonly; cerebral ischemic events are more common in children. Children may have hemiparesis, monoparesis, sensory impairment, involuntary movements, headaches, dizziness, or seizures. Mental retardation or persistent neurologic deficits may be present. Adults may have symptoms and signs similar to those in children, but intraventricular, subarachnoid, or intracerebral hemorrhage of sudden onset is more common in adults. Recently increasing diagnosis of MMD with MRI, followed by surgical operation is noted. MMD needs to be considered as the "CI" state now in life insurance fields.

  • Bacteremia is the transient presence of bacteria (or other microorganisms) in the blood. Bacteremia may result no symptom to the healthy persons, but it may result symptoms such as pylexia, headache and even septic shock to the patient in immunosuppression. In this case, a 75 years old male patient had a dental caries and cerebrovascular accident ago, therefore was in a bad condition and often caught cough. Suddenly be bad a headache with a high fever caused by Streptococcus Mutans Bateremia. After defervescence, a slight fever and a headache remained.
