낭상 뇌동맥류 혈류유동에서 비뉴우토니안 유체 모델의 영향 (EFFEECTS OF NON-NEWTONIAN FLUID MODEL ON HEMODYNAMICS IN CEREBRAL SACCULAR ANEURYSMS)

  • The importance of shear thinning non-Newtonian blood rheology on the hemodynamic characteristics of idealized cerebral saccular aneurysms were investigated by carrying out CFD simulations assuming two different non-Newtonian rheology models (Carreau and Ballyk models). To explore effects of vessel curvature, a straight and a curved vessel geometry were considered. The wall shear stress(WSS), relative residence time(RRT) and velocity distribution were compared at the different phases of cardiac cycle. As expected, blood entered the aneurysm at the distal neck and created large vortex in both aneurysms, but with higher momentum on the curved vessel. Hemodynamic characteristics such as WSS, and RRT exhibited only minor effects by choice of different rheological models although Ballyk model produced relatively higher effects. We conclude that the assumption of Newtonian fluid is reasonable for studies aimed at quantifying the hemodynamic characteristics, in particular, WSS-based parameters, considering the current accuracy level of medical image of cerebral aneurysm.

노화에 따른 두개내 혈류의 혈류 동력학적 변화에 관한 연구 (A Study of Intracranial Hemodynamic Change with Aging)

  • The purpose of this study was to evaluate hemodynamics of intracranial blood flow for obtain clinically useful reference values and assess cerebral hemodynamics change with aging. 81 normal Korean subjects(age, 14$\thicksim\∼\thicksim$70 years) examined who han no history of neurologic disease and the subjects divided into group A(age, 14$\∼$39 years), group B(age, 40$\∼$59 years) and group C(age, 60$\∼$70 years). Transcranial doppler was use for measured the maximum velocity(Vmax), mean velocity(Vmean), pulsatility index(PI), resistive index(RI), stenosis index(SI) and depth of sample volume. Vmax was 99.1 1cm/s, Vmean was 63.57cm/s, PI was 0.85, RI was 0.56, SI was 31.94 and depth of sample volume was 52.35 in middle cerebral artery. Vmax was 85.54cm/s, Vmean was 52.52cm/s, PI was 0.82, RI was 0.55, SI was 34.48 and depth of sample volume was 73.62 in anterior cerebral artery. Vmax was 75.45cm/s, Vmean was 45.60cm/s, PI was 0.82, RI was 0.58, SI was 36.14 and depth of sample volume was 62.35 in posterior cerebral artery. Vmax was 70.44cm/s, Vmean was 47.07cm/s, PI was 0.87, RI was 0.58, SI was 29.83 and depth of sample volume was 75.23 in basilar artery Vmax was 63.92, Vmean was 42.42, PI was 0.89, RI was 0.58, SI was 29.89 and depth of sample volume was 66.65 in vertebral artery. Vmax and Vmean was significantly decreased with increasing age in middle cerebral artery, anterior cerebral artery, posterior cerebral artery, basilar artery and vertebral artery And PI and RI was significantly increased with increasing age in basilar artery and vertebral artery. And I suggest that transcranial doppler sonography can be used as one of useful clinical tool for detection of cerebral hemodynamics.

임상 경두개 도플러 검사 (Clinical Transcranial Doppler)

  • The transcranial Doppler(TCD) is a technique for measuring blood flow velocity of intracranial and extarcranial arteries. This examination based on Doppler effect which was first formulated in 1842 by the Austrian physicist Christian Doppler. In 1982, Rune Aaslid first maked 2MHz pulsed probe and recording intracranial vessels with transcranially. There are six criteria utilized in gaining positive identification of the intracranial vessels. The six criteria are as follows l)acoustical windows 2)depth of sample volume 3)direction of flow 4)spatial relationship of ACA and MCA bifurcation 5)mean velocity and 6)response common carotid artery compression and/or oscillation test. The affected factors for TCD examination are angle of insonation, posture of subject, age, gender, hematocrit, metabolic factors, and cardiac output. Clinical application of TCD are detection of stenosis, occlusion, emboli, thrombsis in intracranial and extracranial arteries and evaluation of cerebral arterovenous malformation, collateral capacity in the circle of Willis, ischemia cerebrovascular disease, stroke patient and vertebrobasilar system.

합곡(合谷) 및 족삼이(足三里) 혈(穴) 자침이 인영혈(人迎穴)부위의 뇌혈류에 미치는 영향 (Effects of Hap-Kok(LI4) and Jok-Sam-Lee(ST36) Acupuncture on Cerebral Blood Flow in In-Young(ST9))

  • Objective : This study was performed to evaluate the effects of Hap-Kok(LI4) and Jok-Sam-Lee(ST36) acupuncture on blood flow in In-Young(ST9). Method : Monitoring of TCD was examined in each 25's healthy men and wemen in 20s before and after acupucture on both(right and left) Hap-kok(LI4) and was done on both(right and left) Jok-Sam-Lee(ST36) by different group with same condition. Mean velocity and Pulsatility index analyzed from TCD at both In-Young(ST9). Both group was acupuctured for 15-20 minutes laying at bed. Results : The results showed a significant($p{\le}0.05:$ Paired T-test) decrease in mean velocity and increase in pulsatility index at both group. Conclusion : These results suggest that acupuncture could have a specific effect on blood flow called In-Young acupoint.

Effects of Acupuncture with a Combination of GV20 and EX-HN1 Acupoints on CO2 Reactivity in the Anterior and Middle Cerebral Arteries during Hypercapnia in Normal Subjects: A Randomized Crossover Trial

  • Objectives: To investigate the effects of acupuncture at GV20 and EX-HN1 on cerebral blood flow (CBF) velocity and cerebrovascular reactivity (CVR) in the middle cerebral arteries (MCA) and anterior cerebral arteries (ACA) and to compare the effects to acupuncture at GV20. Methods: The study was a randomized, crossover trial that included 10 healthy men aged 20 to 29 years who underwent acupuncture treatment four times with a washout period of one week. The CBF velocity and CVR were measured by transcranial Doppler sonography (TCD) on both MCAs at the first and second visits, and both ACAs at the third and fourth visits. Participants were randomly assigned to one of two groups (A and B) before the first and third visits. Group A received two phases of acupuncture intervention at a single GV20 point and a combination of GV20 and EX-HN1 acupoints. Group B received the same interventions, but in the reverse order. Results: The increase in CO2 reactivity was significantly higher for the combination acupoints of GV20 and EX-HN1 than for the GV20 single acupoint in both MCAs (Right: 136 to 178, p=0.017; Left: 127 to 191, p=0.017) and ACAs (Right: 133 to 158, p=0.013; Left: 122 to 168, p=0.025). No significant change was noted in the corrected velocity at PETCO2 40 mmHg, blood pressure, or heart rate. Conclusions: The findings suggest that improvement of the CBF in the MCA and ACA after GV20 acupuncture increases when acupuncture is also performed at EX-HN1. These results clinically support the combined use of EX-HN1 and GV20 to treat disorders of MCA and ACA circulation.

Is Transcranial Doppler Ultrasonography Old-fashioned?: One Institutional Validity Study

  • Objective : The purpose of this study is to investigate the correlation between various transcranial Doppler (TCD) ultrasonography parameters and clinical vasospasm after aneurysmal subarachnoid hemorrhage (SAH). Methods : This study enrolled 40 patients presented with aneurysmal SAH between September 2006 and August 2007. We measured differences of mean blood flow velocity (BFVm), highest systolic blood flow velocity (BFVh), and Lindegaard ratio (LR) in the middle cerebral artery on TCD examination. These parameters were evaluated for correlation with clinical vasospasm by univariate analysis and the receiver operating characteristic analysis. Results : Twelve patients (30%) developed clinical vasospasm. The best TCD parameters for the detection of clinical vasospasm were revealed to be differences of BFVm, BFVh, and LR values between $1^{st}$ TCD test and $3^{rd}$ TCD (7 cm/s. 11.5 cm/s, 0.45 respectively). The positive predictive value of anyone of three parameters was 60% and the negative predictive value was 100%. Conclusion : TCD is still considered a useful tool for screening clinical vasospasm. To confirm the predictive value of the above parameters. further prospective study will be needed.

대사증후군 환자군과 정상군의 뇌혈류 측정 비교를 통한 뇌졸중 위험인자에 대한 TCD의 진단적 가치 고찰 (Comparison of Cerebral Blood Flow between Patients with Metabolic Syndrome and Normal Group to Evaluate Diagnostic Value of Transcranial Doppler Ultrasound)

  • Objectives: The purpose of this study was to evaluate diagnostic value of Transcranial Doppler Ultrasound about risk factor of stroke by comparing blood flow between patients with metabolic syndrome(MS group) and Normal group. Methods: 62 metabolic syndrome patients and 106 healthy adults were selected who had no cerebrovascular diseases, cardiovascular diseases and other systemic diseases. We measured the mean velocity(Vm), peak systolic velocity(Vs) and pulsatility index(PI) of MCA, ACA, PCA, VA, ICA in two groups using TCD. All subjects were divided by gender and age. Results: In comparing Ms group with normal group, Vm in the MCA, ACA, PCA, ICA and Vs in the MCA, ICA were lower in MS group. In all vessels, PI of MS group were higher than that of Normal group. In all vessels, Vm and Vs revealed negative correlation with age and PI revealed positive correlation with age. In 20-39 year olds, there was decrease in the Vs and Vm and increase of PI of MS group in comparison with normal group. There was significant difference in the Vm of PCA, ICA, Vs of MCA, PCA, ICA and PI of MCA, ACA. In 40-59 year olds, Vm in the MCA, ACA, ICA and Vs in the MCA, ACA were lower in MS group. PI in the MCA, ACA, PCA, ICA were higher in MS group. In 60-79 year olds, Vm of MCA, PCA, ICA was decreased in MS group than Normal group with no statistical signification. Vs in the MCA was lower and PI in the PCA was higher in MS group. In male, Vm of PCA and Vs of MCA were lower and PI of MCA, ACA, PCA, ICA were higher in MS group. In female, Vm of MCA, PCA, ICA and Vs of MCA, ICA were lower and PI of ACA, PCA, VA, ICA were higher in MS group. Conclusions: The significant difference in Vm, Vs, PI between MS group and normal group suggests hemodynamic disorder. Screening and prognosing high risk group can be done through TCD and this can be used to prevent stroke. More detailed study will be needed.

Changes in Cerebral Hemodynamics and Hematological Aspects Following Scuba Diving at 5 Meters of Seawater

  • The present study was designed to clarify whether scuba diving at 5 meters of seawater influences cerebral hemodynamics, hematological and biochemical variables. Twenty healthy young men well trained scuba diving participated in this study. The blood flow velocity in the right and left middle cerebral arteries (L-MCAV and R-MCAV), blood pressure (BP), heart rate (HR), CBC and differential count, prothrombin time (PT), activated partial thromboplastin time (aPTT), biochemical variables, D-dimer and interleukin-8 (IL-8) levels were determined before, immediately after scuba diving for 30 min, and after 30 min of rest (Pre-scuba, Scuba and R-30m, respectively). L-MCAV and R-MCAV tended to increase, but the only significant increase was in L-MCAV in Scuba. SBP and HR significantly declined in R-30m compared with those of Pre-scuba and the Scuba. IL-8 levels were elevated in Scuba and R-30m compared with that of Pre-scuba. In Scuba and R-30m, hematological variables except PT and biochemical parameters excluding glucose and lactic acid did not significantly changed in comparison with those of Pre-scuba. PT level at Scuba and glucose level at R-30m significantly declined in Scuba, while lactate level at R-30m increased compared with each in Pre-scuba. However, PT level at Scuba was within a normal range. These results suggest that scuba diving at 5 m of seawater for 30 min has no adverse effects, is safe and useful for improving health. However, further study must be performed to clarify the mechanism of elevated IL-8 level following scuba diving.

사향소합원(麝香蘇合元)이 정상인의 뇌혈류역학에 미치는 영향 (Effects of Sahyangsohap-won on Cerebral Hemodynamics in Healthy Subjects)

  • Background and Purpose : Transcranial doppler ultrasonography(TCD) is a noninvasive and nonradioactive technique for evaluation of the hemodynamics in large cerebral vessels. Sahyangsohap-won(SS) has been considered to be effective for the treatment of various disease, especially cerebrovascular, cardiovascular, and psychosomatoform disorders. But, there is no study about the effect of SS on the cerebral hemodynamics in humans. The aim of this study was to assess the effect of SS on the changes in cerebral hemodynamics and the dose-dependant effect by using TCD. Subjects and Methods : 30 healthy subjects were randomly divided into three group: group 1 took no drug, group 2 took SS one pill, and group 3 took SS 2 pills. Changes in the mean blood flow velocity(MBFV) and pulsatility index(PI) in the middle cerebral artery were evaluated by means of TCD. We obtained hypercapnia with breath-holding and evaluated cerebrovascular reactivity with the breath-holding index(BHI). Systolic blood pressure(SBP), diastolic blood pressure(DBP), and heart rate(HR) were measured by means of ambulatory blood pressure monitoring. In group 2 and group 3, the evaluations were performed during the baseline and were repeated at 20, 40, and 60 minutes after SS administration. In group 1, the evaluation was performed at corresponding time intervals. Results : In mean values of MSFV, PI, SSP, DBP, and HR, no stastically significant differences were found between the 3 groups. However, BHI values were significantly lower in groups 2 and 3 than in group 1 at 40 minutes after SS administration(P<0.05, group 1 vs group 2, group 1 vs group 3 by post-hoc analysis: Scheffe's test) but in dose-dependant effect, there was no difference between group 2 and group 3. Conclusion : These results suggest that SS can decrease vascular resistance in cerebral small arteries or arterioles and enhance their distensibility. Further studies on larger numbers of subjects are needed to confirm these effects and the dose-dependant effects.

전기치료가 긴장형 두통환자의 뇌 혈류 속도에 미치는 영향 (Effects of Electrotherapy on Blood Velocity of Cranial Artery in Tension-Type Headache subjects)

  • The aim of study was to evaluated the possible role of cranial artery velocity in headache pathogenesis. The present study was studied of five headache(F=5. Mean $age=29.80\pm6.76yrs$) were compared to 4 controls(F=4, Mean $age=29.00\pm5.48yrs$). Transcranial doppler ultrasonography(TCD) is a new non-invasive and easily applicable method to evaluate flow velocities of the intracranial and extracranial cerebral arteries. TCD was performed with standard method to measure the mean Flow Velocity(MFV) of the middle and posterior cerebral arteries, the internal carotid artery, the vertebral and the basilar artery. We reviewed the whole TCD results performed at Taejon Veterans Hospital from October. 11. 2000 to November. 10. 2000. Mean flow velocities in headaches and controls at their 6 decades are $28.00\pm3.61cm/sec$ and $41.25pm1.71cm/sec$ in lent PCA (P<0.01), $50,000\pm23.07cm/sec$ and $82.75\pm15.59cm/sec$ in right MCA(P<0.05), $26.20\pm4.82cm/sec$ and $45.50\pm4.51cm/sec$ in fight PCA(P<0.01). $26.60\pm4.56cm/sec$ and $38.25\pm4.92cm/sec$ in right VAC(P<0.01). After treatment for 2 weeks, mean of velocity on pre treatment and post treatment and post treatment are $28.00\pm3.61cm/sec$ and $38.20\pm5.81cm/sec$ in left PCA (P<0.05), $26.20\pm4.827cm/sec$ and $39.20\pm5.54cm/sec$ in right PCA(P<0.05), $40.60\pm9.18cm/sec$ in right VA(P<0.01). It is concluded that Electrical Therapy for two weeks was effected to promote Mean Flow of Velocity in cranial artery. Mean of velocity in cranial artery with headaches observed in this study was lower than controls, but MFV was promote after treatment for 2 weeks.
