중심성 뇌교 수초용해증 환자에 관한 한양방 복합치료 1례 (A Case Report of a Central Pontine Myelinolysis Patient Treated with Korean and Western Medicine)

  • Objectives : The purpose of this study is to report the clinical effect of Korean and Western medical treatment on a hemiplegic inpatient with central pontine myelinolysis(CPM). Methods : A patient who was diagnosed with central pontine myelinolysis(CPM) was treated with acupuncture, herbal medicine, Korean physical therapy, along with receiving continuous Western medication and rehabilitation treatment. The patient was evaluated through Manual Muscle Testing(MMT), measurement of grip strength, dysarthria, and dysuresia. The Korean and Western medical treatment was conducted for 4 weeks. Results : After the 4 weeks of Korean and Western medical treatment, MMT grade improved from 2 to 4 and grip strength improved from 6 to 13. Dysarthria and dysuresia also improved. Conclusion : According to these results, this report suggests that Korean and Western medical treatment could be effective in the treatment of central pontine myelinolysis(CPM) patients.

만성 알콜중독증 환자에서 발생한 중심성 뇌교 수초용해증 치험 1례 (A Case of Central Pontine Myelinolysis in a Chronic Alcoholic)

  • Objective : This study was case report to show the good effects of herbal medicine (Daekumeum-ja) and acupuncture complex therapy for central pontine myelinolysis (CPM). Method : A 44-year-old man suffering from CPM was chronic alcoholic without electrolyte disturbance. He showed symptoms of dysarthria, dysphagia, dizziness, weakness of lower extremities and gait disturbance. CPM was diagnosed at his former hospital by MR imaging. He had no positive response to western treatments. We treated the patient with herbal extract (Daekumeum-ja) and acupuncture. Results : After our treatments, there was no change of Brain CT. But overall symptoms of CPM were improved. Conclusion : This study suggests that herbal medicine (Daekumeum-ja) and acupuncture complex therapy is probably effective in the treatment of CPM.

  • 중심성 뇌교 수초용해 (CPM) 및 뇌교의 수초용해(EPM) 은 대사 이상을 수반하는 여러 질환에서 뇌 세포 내외의 삼투질농도의 급속한 변화와 관련하여 발생하는 신경학적 질환이다. 저자들은 당뇨병성 신중에 의한 만성 신부전으로 신장이식을 받은 43세 남자 환자에서 발현한 CPM과 EPM 증례를 보고하였다. 환자는 망상, 연상이완, 환각, 부적절한 정동, 공격성, 기억장애 등을 수반한 정산병적 증상과 언어실조를 특징적으로 보인 경우로서, CPM과 EPM에서 비교적 드물게 발생하는 정신증상, 특히 정신병적 증상을 보인 증례이기에, 정선과적으로 중요한 임상적 의의를 가진다고 판단하여 문헌고찰과 함께 보고하는 바이다.

  • Objective: The purpose of this study was to show the effects Oriental medicine and treatment for aftereffects of Central pontine myelinolysis (CPM). Methods: The 52-year-old woman suffering from CPM showed symptoms of weakness of both extremities, dysarthria, dysphagia and gait disturbance. We treated the patient with oriental medical therapy. Results: After the treatments, overall symptoms of CPM were improved. Conclusions: This study suggests that oriental medical therapy is probably effective in the treatment of CPM.

  • Introduction: Central pontine myelinolysis (CPM) is a rare neurological disorder marked by symmetric nerve fiber damage, commonly following rapid hyponatremia correction, with no established treatment and a poor prognosis. The condition, often linked to alcoholism, malnutrition, and various diseases, lacks comprehensive studies on its impact on gait. This research aims to quantitatively analyze gait changes in CPM patients receiving both traditional Korean and conventional rehabilitation treatments, addressing a gap in current understanding of CPM management. Case presentation: A 56-year-old male diagnosed with CPM following an initial misdiagnosis underwent combined electoracupuncture and rehabilitative treatment at ○○ university Korean medical center, resulting in significant gait improvements. A treadmill gait analysis system was used to measure changes in key gait parameters at 2-week intervals, and the patient's progress was documented. Conclusions: The quantitative analysis revealed significant gait improvements. Foot rotation decreased from 8.9° to 6.4° (right) and from 11.1° to 7.2° (left); lateral symmetry improved from -7.8 to 0.8; step length increased from 21 cm to 44 cm (right) and from 19 cm to 44 cm (left); and velocity increased from 1.2 m/s to 2.7 m/s. These findings highlight decreased foot rotation and lateral symmetry, along with increased step length and velocity, suggesting a positive outcome of the treatment regimen. Notably, the patient experienced no adverse effects related to the treatments. Despite limitations, including the singe case focus and lack of prior gait-focused CPM research, this case report provides valuable insights into effective CPM management strategies, paving the way for future research in this domain.

Neonatal Seizures with Diffuse Cerebral White Matter Lesions on Magnetic Resonance Imaging Associated with Rotavirus Infection: A Report of Three Cases

  • Rotavirus is the major cause of gastroenteritis in children under the age of 5. Rotavirus infection may lead to several neurological complications as meningitis, encephalitis, convulsion, encephalopathy, hemorrhagic shock, central pontine myelinolysis, Guillain-Barre syndrome, and Reye's syndrome. Further, some reports have described diffuse cerebral white matter lesions on diffusion-weighted magnetic resonance imaging (MRI) in neonates with rotavirus induced seizures. Here, we report on three neonates with rotavirus induced seizures with cerebral white matter abnormalities on MRI.
