MicroRNAs: promising biomarkers for diagnosis and therapeutic targets in human colorectal cancer metastasis

  • Colorectal cancer (CRC) is the third most common cancer and the fourth most common cause of cancer-related death worldwide. Distant metastasis is a major cause of mortality in CRC. MicroRNAs (miRNAs) are small non-coding RNA molecules involved in the post-transcriptional and translational regulation of gene expression. Many miRNAs are aberrantly expressed in cancer and influence tumor progression. Accumulating studies suggest that multiple miRNAs are actively involved in the CRC metastasis process. Thus, we aim to introduce the role of miRNAs in multi-steps of CRC metastasis, including cancer cell invasion, intravasation, circulation, extravasation, colonization, angiogenesis, and epithelial-mesenchymal transition (EMT). Moreover, we suggest the potential application of miRNAs as biomarkers for CRC patients with metastasis. [BMB Reports 2015; 48(4): 217-222]

Mortality and Epidemiology in 256 Cases of Pediatric Traumatic Brain Injury : Korean Neuro-Trauma Data Bank System (KNTDBS) 2010-2014

  • Objective : Among pediatric injury, brain injury is a leading cause of death and disability. To improve outcomes, many developed countries built neurotrauma databank (NTDB) system but there was not established nationwide coverage NTDB until 2009 and there have been few studies on pediatric traumatic head injury (THI) patients in Korea. Therefore, we analyzed epidemiology and outcome from the big data of pediatric THI. Methods : We collected data on pediatric patients from 23 university hospitals including 9 regional trauma centers from 2010 to 2014 and analyzed their clinical factors (sex, age, initial Glasgow coma scale, cause and mechanism of head injury, presence of surgery). Results : Among all the 2617 THI patients, total number of pediatric patients was 256. The average age of the subjects was 9.07 (standard deviation${\pm}6.3$) years old. The male-to female ratio was 1.87 to 1 and male dominance increases with age. The most common cause for trauma were falls and traffic accidents. Age (p=0.007), surgery (p<0.001), mechanism of trauma (p=0.016), subdural hemorrhage (SDH) (p<0.001), diffuse axonal injury (DAI) (p<0.001) were statistically significant associated with severe brain injury. Conclusion : Falls were the most common cause of trauma, and age, surgery, mechanism of trauma, SDH, DAI increased with injury severity. There is a critical need for effective fall and traffic accidents prevention strategies for children, and we should give attention to these predicting factors for more effective care.

The Effect of Cognitive Impairment on the Association Between Social Network Properties and Mortality Among Older Korean Adults

  • Objectives: This study investigated the effect of cognitive impairment on the association between social network properties and mortality among older Korean adults. Methods: This study used data from the Korean Social Life, Health, and Aging Project. It obtained 814 older adults' complete network maps across an entire village in 2011-2012. Participants' deaths until December 31, 2020 were confirmed by cause-of-death statistics. A Cox proportional hazards model was used to assess the risks of poor social network properties (low degree centrality, perceived loneliness, social non-participation, group-level segregation, and lack of support) on mortality according to cognitive impairment. Results: In total, 675 participants (5510.4 person-years) were analyzed, excluding those with missing data and those whose deaths could not be verified. Along with cognitive impairment, all social network properties except loneliness were independently associated with mortality. When stratified by cognitive function, some variables indicating poor social relations had higher risks among older adults with cognitive impairment, with adjusted hazard ratios (HRs) of 2.12 (95% confidence interval [CI], 1.34 to 3.35) for social nonparticipation, 1.58 (95% CI, 0.94 to 2.65) for group-level segregation, and 3.44 (95% CI, 1.55 to 7.60) for lack of support. On the contrary, these effects were not observed among those with normal cognition, with adjusted HRs of 0.73 (95% CI, 0.31 to 1.71), 0.96 (95% CI, 0.42 to 2.21), and 0.95 (95% CI, 0.23 to 3.96), respectively. Conclusions: The effect of social network properties was more critical among the elderly with cognitive impairment. Older adults with poor cognitive function are particularly encouraged to participate in social activities to reduce the risk of mortality.

고엽제 노출이 폐렴의 치료 결과에 미치는 영향 (The Effects of Agent Orange in Patient with Pneumonia)

  • Purpose: Agent Orange (AO) is a herbicide and defoliant used by the United States and its military allies during the Vietnam War. Pneumonia is a common cause of death among Vietnam veterans in our hospital. There have been no previous studies researching any association between AO exposure and the prognosis for pneumonia. The primary objective of this study was to investigate associations between AO exposure and 30-day mortality due to pneumonia. The secondary objective was to examine the clinical factors associated with therapeutic outcomes in veterans with pneumonia, and to assess the prevalence of combined diseases in AO-exposed veterans. Methods: This study retrospectively included veteran patients diagnosed with pneumonia in the emergency department and hospitalized between February 2014 and March 2018. The enrolled patients were grouped according to their defoliant exposure history, and the clinical information of defoliant-exposed and non-defoliant-exposed groups were compared. Patients were divided according to 30-day mortality, and significant factors influencing mortality were evaluated by using univariate analysis and multivariate analysis. The final multivariate model revealed the effect of AO exposure on therapeutic outcomes of pneumonia. Results: A total of 1006 patients were analyzed. Of these, 276 patients had a history of AO exposure, whereas 730 patients had not been exposed. Factors positively associated with 30-day mortality were malignancy, respiratory rate, blood urea nitrogen, and albumin which was negatively associated with mortality. Conclusion: Exposure to defoliant is not associated with 30-day mortality in patients with pneumonia. However, veterans with defoliant exposure are associated with a high prevalence of diabetes mellitus, hypertension, cerebrovascular accident, malignancy, and chronic kidney disease.

장래인구추계를 위한 사망률 예측 (Mortality Forecasting for Population Projection)

  • 장래인구추계방법으로 코호트 요인법(cohort component method)을 적용하기 위해서는 장래출생, 사망, 이동을 정확하게 예측하여야 한다. 이 연구에서는 기존의 사망률예측방법을 검토하고, 다양한 모델 중에서 과거의 사망률 추세에 수학적 곡선을 적합시켜서 외삽 연장하는 대표적인 관계적 모형인 Lee-Carter법을 선택하여 우리나라의 성, 연령별 장래사망률을 추정하였다. 성, 연령별 생명표와 평균수명의 추세를 검토하고 기존 자료와 비교한 결과 lee-Carter 모형을 우리나라 사망력 예측에 적용하였을 경우 설명력이 매우 높았다. 실측자료와 lee-Carter 모형을 이용한 추정자료를 비교한 결과 남녀 모든 연령층에서 전 비교기간(1971-2003) 동안 모두 차이가 크지 않았다. Lee-Carter 모형을 이용하여 추정한 장래 평균수명은 2051년에 남여 각각 82.73년과 89.41년으로 그 차이는 2005년의 7.06년에서 6.68년으로 감소하였다. 다만, 우리나라 영아사망률자료의 제약으로 일본의 2050년 추계자료를 우리의 2051년 목표치로 사용하였다. 앞으로 충분한 시계열자료가 확보되면 우리나라의 자료를 직접 이용하여 장래예측이 가능하게 될 것이다. 그리고 성, 연령별 사망원인을 극복한다는 가정을 도입할 수 있다면 장래 사망률 예측에 설명력을 더욱 높일 수 있을 것이다. 이것은 우리나라 사망원인 통계의 정확성과 사망원인별 생명표의 시계열 확보가 이루어질 때 가능할 것이다.

Tobacco Chewing and Adult Mortality: a Case-control Analysis of 22,000 Cases and 429,000 Controls, Never Smoking Tobacco and Never Drinking Alcohol, in South India

  • Background: Tobacco is consumed in both smoking and smokeless forms in India. About 35-40% of tobacco consumption in India is in the latter. The study objective was to describe the association between chewing tobacco and adult mortality. Materials and Methods: A case-control study was conducted in urban (Chennai city) and rural (Villupuram district) areas in Tamil Nadu state in South India. Interviewed in 1998-2000 about 80,000 families (48,000 urban and 32,000 rural) with members who had died during 1995-1998. These were the cases and their probable underlying cause of death was arrived at by verbal autopsy. Controls were 600,000 (500,000 urban, 100,000 rural) individuals from a survey conducted during 1998-2001 in the same two study areas from where cases were included. Results: Mortality analyses were restricted to non-smoking non-drinkers aged 35-69. The age, sex, education and study area adjusted mortality odds ratio was 30% higher (RR:1.3, 95%CI:1.2-1.4) in ever tobacco chewers compared to never chewers and was significant for deaths from respiratory diseases combined (RR:1.5, 95%CI:1.4-1.7), respiratory tuberculosis (RR:1.7, 95%CI:1.5-1.9), cancers all sites combined (RR:1.5, 95%CI:1.4-1.7) and stroke (RR:1.4, 95%CI:1.2-1.6). Of the cancers, the adjusted mortality odds ratio was significant for upper aero-digestive, stomach and cervical cancers. Chewing tobacco caused 7.1% of deaths from all medical causes. Conclusions: The present study is the first large study in India analysing non-smoking non-drinkers. Statistically significant excess risks were found among ever tobacco chewers for respiratory diseases combined, respiratory tuberculosis, stroke and cancer (all sites combined) compared to never tobacco chewers.

The Pyramid of Injury: Estimation of the Scale of Adolescent Injuries According to Severity

  • Objectives: Due to their developmental characteristics, adolescents have a higher probability than other age groups of experiencing injuries caused by accidents, violence, and intentional self-harm. The severity and characteristics of injuries vary by the intentionality and mechanism of injury; therefore, there is a need for a national-level estimate of the scale and the severity of injuries in adolescents that takes these factors into account. Methods: By using data from the Emergency Department-based Injury In-depth Surveillance Data, National Emergency Department Information System, the Korean National Hospital Discharge In-depth Injury Survey, and cause of death statistics, we calculated the emergency department (ED) visit rate, hospitalization rate, and death rate of injuries per 100 000 adolescents for each injury mechanism. The calculated rates were used to generate the injury pyramid ratio (ratio of death rate to hospitalization rate to ED visit rate) to visualize the scale and the severity of the injury. Results: The mortality rate in adolescents due to injury was 10/100 000; the corresponding rates for hospitalization and ED visits were 1623 and 4923, respectively, resulting in an injury pyramid ratio with the general pyramid form, with a 1:162:492 ratio of deaths to hospitalizations to ED visits. The mortality rate due to suicide/intentional self-harm was 5/100 000, while 35 were hospitalized for this reason and 74 made ED visits. The pyramid ratio of 1:7:15 for intentional self-harm/suicide showed a steep pyramidal form, indicating considerable lethality. The mortality rate due to motor vehicle collisions (MVCs) was 3/100 000; 586 were hospitalized for this reason, while 1023 made ED visits. The pyramid ratio of 1:195:341 for MVCs showed a gradual pyramid form, indicating that the lethality was low and the scale of injury was high. Conclusions: The main categories of injuries in adolescents were visualized in pyramid form, contributing to an understanding of the scale of each injury by mechanism in terms of levels of death, hospitalization, and ED visits. These findings will be helpful for understanding how to prioritize injuries in adolescents.

전국 결핵 신환자 의료빅데이터를 이용한 경쟁위험모형 적합 (Fitting competing risks models using medical big data from tuberculosis patients)

  • 결핵은 높은 이환과 사망을 일으키는 질병으로 현대의학의 발달에 따라 발생률과 사망률은 감소하고 있다. 그러나 한국은 아직까지 OECD 국가 중 결핵 발생률과 사망률이 가장 높다. 이에 따라 한국은 결핵의 예방 및 통제를 위해 여러 정책 사업을 실시하고 있다. 본 연구에서는 공공민간협력(public-private mix) 결핵관리사업이 치료결과에 미치는 영향을 분석하고 결핵환자의 치료 성공에 영향을 미치는 요인을 확인하고자 한다. 질병관리본부에서 관리하는 결핵환자 신고 자료를 이용하여 2012-2015년 전국 결핵 신환자 코호트 약 13만명을 대상으로 분석하였다. 누적 발생 함수(cumulative incidence function)를 이용하여 요인별로 누적 치료 성공률을 비교하였으며. 주 관심사건(치료성공) 및 경쟁사건(사망)을 고려한 두 가지 경쟁위험모형(cause-specific Cox's proportional hazards model and subdistribution hazard model)을 사용하여 분석 결과를 비교하였다.

왜도 예측을 이용한 Lee-Carter 모형의 주택연금 리스크 분석 (Actuarial analysis of a reverse mortgage applying a modified Lee-Carter model based on the projection of the skewness of the mortality)

  • 주택연금은 계약기간이 확정되어 있지 않기 때문에 계약 종료 시점에 대한 확률분포 예측이 장수리스크 관리를 위하여 중요하다. 따라서 고령화의 주요인인 기대수명의 연장은 연금 재정건전성에 심각한 영향을 끼칠 수 있기 때문에 사망률의 개선 추세가 적절히 반영된 사망률 예측 연구가 선행될 필요가 있다. 본 연구에서는 Lee-Carter (LC) 모형과 연생모형을 이용하여 주택연금 계리모형에 사망률 개선 효과를 반영하였다. 전통적 LC 모형을 통한 사망률 예측 방식은 미래 사망률이 지나치게 개선되는 현상을 보이고 있기 때문에 사망률 개선효과를 조금 더 적절한 수준으로 보정하고자 본 연구에서는 사망확률 분포의 편중을 나타내는 왜도를 활용한 LC 모형을 적용하였다. 왜도 예측 방식을 LC 모형에 적용한 방법론을 사용하여 주택연금 월 지급금을 산출해본 결과 전통적 LC 모형의 사망률 예측보다 사망률 개선효과를 더 적게 반영하여 더 큰 월 지급금이 산출되었고, 왜도 활용 LC 모형에 의한 이러한 결과는 장수 리스크를 덜 왜곡한다는 데 의의가 있다고 볼 수 있다. 본 연구 결과는 사망률 감소 추세를 적절하게 반영한 위험률을 계산하여 주택연금의 발행기관 및 보증기관의 적정한 월 지급금 지급과 차후 월 지급금의 과대지급으로 인한 지급불능을 방지할 수 있는 리스크 관리 방법으로 이용될 수도 있다.

The Incidence and Mortality of Liver Cancer and its Relationship with Development in Asia

  • Background: Liver cancer (LC) is the sixth world most common cancer and the second leading cause of cancer death. Due to the importance and necessity of awareness about the incidence and mortality of diseases to perform prevention programs, this study focused on data for LC and its relationship with the human development index (HDI) and its components in Asia in 2012. Materials and Methods: This ecological study was based on GLOBOCAN data for Asian countries. We assessed correlations between standardized incidence rates (SIR) and standardized mortality rates (SMR) of LC with HDI and its components using of SPSS18. Results: A total of 582,420 incident cases and 557,097 deaths were recorded in Asian countries in 2012. The five with the highest SIR were Mongolia, Lao PDR, Vietnam, Republic of Korea and Thailand and those with the highest SMR were Mongolia, Lao PDR, Vietnam, Cambodia and Thailand. A negative relation was observed between HDI and LC for SIR of 0.049 (P=0.748) and for SMR of 0.07 (P=0.645), with life expectancy at birth a positive relation for SIR of 0.061 (P=0.687) and a negative relation for SMR of 0.079 (P=0.603), with the average years of education a negative relation fo SIR of 0.476 (p=0.952) and for SMR of 0.032 (P=0.832), and with the country income level per person a negative relation for SMI of 0.11 (p=0.465) and for SMR of 0.113 (P=0.455). Conclusions: The incidence of LC is more in less developed and developing countries but statistically significant correlations were not found between standardized incidence and mortality rates of LC, and HDI and its dimensions.
