Fistula of Ascending Aorta and Right Atrium Following Percutaneous Transcatheter Atrial Septal Defect Closure (경피적 카테타 심방중격결손 폐쇄술 후 발생한 상행 대동맥-우심방루)

  • Percutaneous transcatheter closure of atrial septal defects as a therapeutic alternative in appropriate patients provides superior cosmetic results, is less invasive, and allows for shorter hospital stays. Unfortunately, however, such percutaneous procedures can be associated with catastrophic procedure complications that may require immediate surgical intervention. We report a case of aorta-to-right atrial fistula two months after transcatheter occlusion of an atrial septal defect by an Amplatzer septal occluder. Revealed by dyspnea, palpitation and hemolysis, this complication needed an emergency surgical operation. The fistula between the noncoronary Valsalva sinus of the aorta and the right atrium was repaired. The atrial septal defect was closed by patch. The cause of this serious complication appears to be erosion into the aorta by the right atrial disk.

Temporal augmentation with calvarial onlay graft during pterional craniotomy for prevention of temporal hollowing

  • Background: Atrophy of muscle and fat often contributes to temporal hollowing after pterional craniotomy. However, the main cause is from the bony defect. Several methods to prevent temporal hollowing have been introduced, all with specific limitations. Autologous bone grafts are most ideal for cranial defect reconstruction. The authors investigated the effectiveness of bony defect coverage and temporal augmentation using pterional craniotomy bone flap. Methods: This study was conducted in 100 patients who underwent brain tumor excision through pterional approach from 2015 to 2016. Group 1 underwent pterional craniotomy with temporal augmentation and group 2 without temporal augmentation. In group 1, after splitting the calvarial bone at the diploic space, the inner table was used for covering the bone defect and as an onlay graft for temporal augmentation. The outcome is evaluated by computed tomography at 1-year follow-up. Results: The mean operative time for temporal augmentation was 45 minutes. The mean follow-up was 12 months. The ratio of temporal thickness of operated side to non-operated side was 0.99 in group 1 and 0.44 in group 2, which was statistically different. The mean visual analogue scale score was 1.77 in group 1 and 6.85 in group 2. Conclusion: This study demonstrated a surgical technique using autologous bone graft for successfully preventing the temporal hollowing and improved patient satisfaction.

  • Cystic intrathoracic lesions of foregut origin are now well recognized and account for approximately 10% of lesions presenting as mediastinal tumors. The terminology used to describe mediastinal endodermal cysts has been confused and sometimes ambiguous. The embryological derivation of these lesions has been the cause of much speculation. It Is suggested that these lesions should be classified Into three main categories based on embryology bronchogenic cyst[resulting from a defect of lung budding], Intramural esophageal cyst[true duplication], and enteric cyst[resulting from the split notochord syndrome]. This communication describes a 26 year old man with intramural esophageal cyst who was diagnosed as posterior medlastlnai tumor preoperatively and cured with extirpation of the cyst.

  • Defect of the Cable is one of the most frequent cause of fire accident for years. It also take a big part of the fire load for train. In this study, we reviewed the standard code of other countries for cable fire test. Oxygen index, flame propagation test, smoke test and toxicity test codes were investigated. We also suggest the our national code for train cables.

  • In Korea mid chord offset method has been used to measure the track irregularities. This method has been also used all over the world widely because of its accessibility and the operational excellence for measured data. But it has been pointed out recently that track irregularities reconstructed from measured data have unexpected noise in the high frequency area as the defect of this way. This study shows the cause of noise which occurs during reconstructing process and mentions the solutions briefly.

  • Objective: The aim of this study was to investigate the concept of purpura in the western and oriental medicine. Method: We surveyed the western and oriental medical books from to recent published books. Result and Conclusion: The cause of purpura is devided into two branchs. One is sthenia syndrome includes stomach-heat, blood-heat and abnormal weather in four seasons. The other is asthenia syndrome includes deficiency of stomache energy, spleen asthenia and deficiency of Yin. In children, the cause of purpura is improper diet, inherent defect and the above things. Brain somatic mutations in MTOR leading to focal cortical dysplasia

  • Focal cortical dysplasia type II (FCDII) is a focal malformation of the developing cerebral cortex and the major cause of intractable epilepsy. However, since the molecular genetic etiology of FCD has remained enigmatic, the effective therapeutic target for this condition has remained poorly understood. Our recent study on FCD utilizing various deep sequencing platforms identified somatic mutations in MTOR (existing as low as 1% allelic frequency) only in the affected brain tissues. We observed that these mutations induced hyperactivation of the mTOR kinase. In addition, focal cortical expression of mutant MTOR using in utero electroporation in mice, recapitulated the neuropathological features of FCDII, such as migration defect, cytomegalic neuron and spontaneous seizures. Furthermore, seizures and dysmorphic neurons were rescued by the administration of mTOR inhibitor, rapamycin. This study provides the first evidence that brain somatic activating mutations in MTOR cause FCD, and suggests the potential drug target for intractable epilepsy in FCD patients.

  • ACSR of the catenary wires is corrosion degradation progressed by the effect of atmospheric pollution. ACSR which consists of galvanized steel stranded aluminum. The inside of Steel Reinforced is hot-dipped zinc coating steel wire and it takes charge of tension. If ACSR is exposed in atmosphere, the galvanic corrosion is occurred because it is contacted with aluminum. It is occurred the chemical reaction rapidly so that the local a defect is also occurred. If the catenary wires are exposed in atmosphere of pollution conditions, it may cause to reduce the mechanical strength by corrosion degradation and may cause to damage the wires by micro cracks. Accordingly, this study presents the effects of mechanical properties through the corrosion of ACSR.

  • Failure cause of the fractured engine parts was analyzed by microstructural observation. These parts were failed far earlier than the expected service life. By the stereoscope and SEM examinations of the fractured surface, the fracture modes have been identified as wear and fatigue failure. From the observation of microstructure and microhardness measurements of the failed gears, the probable cause for failures are internal oxidation during using and retained austenite and carbide networks due to heat-treatment, respectively. These defected structures at near surface contributed to the wear and fatigue failure.
