Clinical Study of Inguinal Hernia in Pediatric Patients (소아 서혜부 탈장에 대한 임상적 고찰; 단일병원 10년간 1244예 후향적 고찰)

  • This study is a retrospective analysis of 1244 cases of the inguinal hernia in children under the age of fifteen years who were operated at the department of pediatric surgery, Inje University Busan Paik Hospital from March, 1997 to February, 2007. The ratio of male to female was 3.6:1. The type of hernia was indirect in all of the cases. The hernia was on the right side in 656 cases (53.9 %), left side in 467 cases (37.5 %), and bilateral in 121 cases (9.7 %). The hernia presented most frequently in infants under age 12 months; 364 cases (29.2 %). Fifty-nine cases (21.7 %) were in female and 305 cases (31.3 %) in male. There were 428 cases (33.6 %) in 1-3 years age group, 295 cases (23.7 %) in 4-6 years, 112 cases (9.0 %) in 7-9 years, 39 cases (3.1 %) in 10-12 years and 16 (1.2 %) in 13-15 years. The content of hernia sac was small bowel (59 %), omentum (31 %) in males and the ovary and tube (54 %) and small bowel (26 %) in female. The incidence of combined operation at the time was 3.2 %, and consisting of orchiopexy (67.5 %), frenulotomy (12.5 %), appendectomy (10 %), circumcision (5 %), and fistulotomy (5%). The incidence of combined disease was 2.8 % and consisting of undescended testis, Hirschsprung's disease, idiopathic hypertrophic pyloric stenosis, imperforate anus, and congenital heart disease. After unilateral inguinal hernia repairs, contralateral hernias developed in 34 patients. The laterality of the primary site of hernias were left in 19 cases (55.8 %), and right 15 cases (44.1 %). The 936 cases (75.2 %) were operated under general anesthesia; Mask bagging 663 cases (53.2 %), endotrachea intubation 257 cases (20.6 %), and laryngeal mask 16 cases (1.2 %). The remainder 308 cases (24.7 %) were operated under regional caudal anesthesia.

The Efficacy of New Hyaluronic Acid Filler (HyaFilia) (새롭게 개발된 히알루론산 필러(히아필리아)의 유용성)

  • Purpose: Currently, soft tissue filler products based on hyaluronic acid are widely used. They are safe, effective, and convenient to use compared with bovine collagens. However, all commercially available hyaluronic acid based fillers in Korea are imported ones. The purpose of this study is to evaluate efficacy of a new hyaluronic acid filler (HyaFilia; CHA bio&Diostech Co., Seoul), which has been recently developed in Korea. Methods: Three kinds of soft tissue fillers, including Restylane (Q-Med, Uppsala, Sweden), HyaFilia (CHA bio & Diostech Co., Seoul), Juvederm (Allergan, CA, USA), were injected subdermally into the back of hairless mice at six sites. The 2 cephalic sites composed Restylane groups, the 2 middle sites HyaFilia groups, and the 2 caudal sites Juvederm groups. Six hairless mice were included in the study, therefore, a total of 12 injections per group were performed. After 16 weeks after injection, the nodular swellings that resulted from the injections were excised wide enough to include skin beyond the swelling points down to the panniculus carnosus layer using 8mm punches. Volumes and weights were measured using a stereoimage optical topometer system and a weighting machine. Histologic comparisons were also carried out. Results: The mean volumes of the Restylane, HyaFilia and Juvederm groups were $3698{\times}103$, $4820{\times}103$, and $1435{\times}103$ PI, respectively. The mean weights of the Restylane, HyaFilia and Juvederm groups were 36.08, 37.83, and 24.66 mg, respectively. Histologic examination between the 3 groups showed no significant difference in tissue compositions and inflammatory reactions. Conclusion: The results of this study shows that HyaFilia is superior to Restylane and Juvederm in longavity of filling effect. Therefore, HyaFilia may be an effective replacement for the imported materials including Restylane and Juvederm.

A Taxonomic Revision of the Family Bagridae (Pisces, Siluriformes) from Korea (한국산 동자개과 어류의 분류학적 재검토)

  • The morphological characters of the family Bagridae living in the river systems flowing into the south and western seas of Korea were reviewed and a taxonomic revision was made. The family Bagridae was classified into two genera: Pseudobagrus and Leiocassis. Species corresponding to genus Pseudobagrus were P. fulvidraco, P. koreanus Uchida, sp. nov., P. brevicorpus, and genus Leiocassis were L. nitidus, L. ussuriensis, and L. longirostris. Among them Pseudobagrus koreanus Uchida, sp. nov. and P. brevicorpus are endemic species of Korea. As the intergeneric taxonomic characters, the outstanding morphological differences between genera Pseudobagrus and Leiocassis were distinguished in following characters; the serrated structure of pectoral spine, the length of barbels, the number of pectoral fin rays, the structure of cranial lateral line system and the fused state of 3-4 hypurals. Pelteobagrus fulvidraco and Coreobagrus brevicorpus recorded previously in Korea were classified as Pseudobagrus fulvidraco and P. brevicorpus, respectively. Pelteobagrus nitidus and Pseudobagrus ussuriensis are also changed to genus Leiocassis. The characters of interspecific classification belonging to genera Pseudobagrus and Leiocasis were the shape of caudal fin, the form of body, the length of body, the number of anal fin ray and the number of gill rakers. Pseudobagrus sp. was named into Pseudobagrus koreanus Uchida, sp. nov..

Taxonomic and Morphological Characteristics in the Juvenile Cottus(Cottidae) Fishes from Korea (한국산 둑중개속(Cottus)의 분류학적 재검토와 자어의 형태적 특징)

  • In order to study the taxonomic status of Cottus species from Korea, 19 morphological characters, egg number and size and hatched juvenile form in C. poecilopus and C. hangionsis were compared. The two species of C. poecilopus and C. hangionsis overlap each other in morphometric characters mostly, but snout length, caudal peduncle length and pectoral fin ray number were distinguished between them. Moreover the C. poecilopus and C. hangionsis specimens were differ from each other in the number of egg and size. The newly hatched juvenile form of C. hangiongensis produced many small eggs from which pelagic larvae were formed. In contrast, C. poecilopus produced well - developed juveniles. Two species have distinctly different life - cycles ; amphidromous for C. hangionsis and fluvial land - locked for C. poecilopus.

First Record of a Filefish, Thamnaconus tessellatus (Monacanthidae: Tetraodontiformes) from Jeju Island, Korea (한국산 쥐치과 미기록종, Thamnaconus tessellatus의 최초 보고)

  • A single specimen (273.1 mm in standard length) of the monacanthid Thamnaconus tessellatus was caught by one-boat trawl from western Jeju Island and then collected at Busan Cooperative Fish Market (BCFM) on 30 May 2015. The specimen is characterized by both head and body with many dark brown spots densely, posterior margin of caudal fin no black, first dorsal spine originates the posterior half of eye, and 34~37 dorsal fin rays. This is the first record of T. tessellatus in Korea; we therefore add the species to the Korean fish fauna. According to the Yamada et al. (1995), we propose the Korean name, "Nam-byeol-jwi-chi" for this species.

Comparison on postural control between abdominal draw-in maneuver and abdominal expansion maneuver in persons with stroke

  • Objective: The effect of abdominal expansion maneuver (AEM) and abdominal draw-in maneuver (ADIM) on postural control in an unsupported position in stroke patients. Design: Randomized controlled trial. Methods: A total of 36 persons with hemiplegic stroke participated in this study. The subjects were randomly divided into an AEM experimental group (n=12), an experimental ADIM group (n=12), and a control group (n=12). We collected the general characteristics of all subjects and the pre-test results before the intervention and after 4 weeks of the intervention. The trunk stabilization training of the ADIM and AEM group were performed 15 minutes a day, 3 times a week for 4 weeks, and general physical therapy was performed 2 times a day, 30 minutes per session, 5 times a week for all three groups. The control group received joint mobilizations, muscle strengthening, endurance strengthening, and gait exercises along with treatment of the central nervous system, such as neuro-developmental treatment, mat, and gait training. The AEM is an inspiratory phase of tidal breathing expanding the lateral lower ribcage in a lateral direction with minimal superior movements of the chest. Then the lower abdomen expands and the navel moves in an anterior-caudal direction. The ADIM is a repeated contraction and relaxation of the anal sphincter during inspiration. The navel pulls the lower abdomen to the direction of the spine without the movement of the trunk and pelvis. Results: Before and after the interventions, medial-lateral axis movement distance, anterior-posterior axis movement distance, sway mean velocity, and sway area 95% was a statistically significant change in all three groups (p<0.05). The post-hoc test showed a significant improvement in medial-lateral axis movement distance, anterior-posterior axis movement distance, sway mean velocity, and sway area in the AEM group compared with the control group, and in the ADIM group compared with the control group (p<0.05). Conclusions: In conclusion, both AEM training and ADIM training are necessary interventions to maintain the independent sitting position according to the characteristics of the patient.

Studies on the Glochidial Encystment in Host Fish during the development of Anodonta arcaeformis flavotincta (작은대칭이, Anodonta arcaeformis flavotincta의 유생 발생 중 숙주어류내에서 글로키디움 유생의 피낭 형성과정에 관한 연구)

  • A scanning electron microscopic study on the glochidial encystment and excystment during the development of Anodonta arcaeformis flavotincta on Carassius auratus, a common natural host fish, was carried out. The glochidia were attached to the fins, buccal cavity and gills of the host fish within 30 minutes. In this study, the fins of host fish infected with the glochidia were examined in a time series. The attachment rates of the glochidia on the pectoral fins, caudal fin and pelvic fins of the host fish were 30%, 22%, and 17%, respectively. The glochidia which attached to the fish became encysted within 27 hrs. The process of encystment progressed slowly. Ti took 24 to 27 hours in the formation of the primary cyst, and after 5 to 6 days, the larvae was covered completely with the epithelial cels of the host tissues. The process of detachment of juvenile clam was observed on the 8th day after host infection. Most of the juvenile clams have sloughed from the cyst of the host within 15 days. No significant size difference was observed in the glochidia and the juvenile which were found before attachment and after detachment from the cyst of the host fish.

First Record of Three Uronychia Species (Ciliophora: Spirotrichea: Euplotida) from Korea

  • Three morphospecies of the genus Uronychia, i.e. U. setigera Calkins, 1902, U. binucleata Young, 1922, and U. multicirrus Song, 1997, were collected from the coastal waters of Gumjin-ri on the East Sea and the public waterfront of Incheon on the Yellow Sea in Korea, respectively. These species are described based on live observation, protargol impregnation, silver nitrate impregnation, and their morphometrics. Diagnostic keys for these species are also provided. In addition, their small subunit ribosomal DNA sequences were compared with previously known sequences of Uronychia species. Diagnostics of three Uronychia species are as follows: U. setigera: $50-80\;{\mu}m$ long in vivo, oval-shaped, 2 macronuclear nodules (Ma), 1 spur on the left margin, 11 adoral membranelles (AM) 1, 4 AM2, 1 buccal cirrus (BC), 4 frontal cirri (FC), 3 left marginal cirri (LMC), 2 ventral cirri (VC), 5 transverse cirri (TC), 3 caudal cirri (CC), 6 dorsal kineties (DK), and approximately 23 cilia in the leftmost kinety. U. binucleata: $70-110\;{\mu}m$ long in vivo, oval to slightly rectangular shaped, 2 Ma, 1 micronucleus (Mi), 2 spurs on the posterior region, 11 AM1, 4 AM2, 1 BC, 4 FC, 3 LMC, 2 VC, 5 TC, 3 CC, 6 DK, and approximately 37 cilia in the leftmost kinety. U. multicirrus: $140-200\;{\mu}m$ long in vivo, oval to slightly rectangular shaped, ca. 7 Ma, 1 Mi, 11 AM1, 4 AM2, 1 BC, 4 FC, 3 LMC, approximately 8 VC, 5 TC, 3 CC, and 6 DK. This study presents the first record of this genus in Korea.

Computed Tomographic Diagnosis of Bronchiectasis in a Dog with Chronic Bronchopneumonia (만성 기관지폐염 견에서 컴퓨터단층촬영을 통한 기관지확장증 진단 1례)

  • A 2-year-old castrated male, Cocker spaniel dog with a history of chronic productive cough for 2 to 3 months and with unsuccessful treatment was referred to Veterinary Medical Teaching Hospital, Seoul National University. On thoracic radiographs, there were alveolar infiltrations at left cranial and right caudal lung fields, and soft-tissue opacity round to oval images at overall lung field. The bronchi were dilated, tortuous and not tapered. Abnormal air was accumulated focally in the caudodorsal lung fields. To scrutinize the soft-tissue opacity image and accumulated air, computed tomography (CT) was done. On CT images, severe cylindrical or tubular bronchiectasis was confirmed. And the soft-tissue opacity images were found in the dilated bilated and thought to complexes of mucous plugs, inflammatory cells, necrotic and fibrotic tissue. The dog was dead next day to the CT scan, so necropsy and histopathologic examination were perfermed. On the histopathology, there were cylindrical bronhiectasis and severe diffuse chronic fibrinous necropurulent bronchitis and bronchopneumonia. In this case, it was difficult to diagnose the bronchiectasis only with radiography due to the concurrent lesions, such as pulmonary infiltrations and mucous plugs, which was identified by computed tomography. Thus, computed tomography is considered as a useful modality to confirm tile bronchiectasis camouflaged by the concurrent lesion.

The Use of a Porcine Cancellous Bone Graft for Arthrodesis of Bilateral Antebrachiocarpal Luxation in a Dog (양측 전완앞발목관절 탈구의 관절고정술을 위한 돼지뼈 해면골이식의 적용)

  • A seven-year-old Yorkshire terrier 2.1 kg was admitted with a 2 months history of sever bilateral forelimb lameness and joint instability. Examination and radiographs revealed caudal luxation of bilateral antebrachiocarpal joint. Luxation of bilateral antebrachiocarpal (ABC) joint in a toy breed dog was treated with pancarpal arthrodesis. Modified external skeletal fixator (ESF) (type IIb) with polymethylmethacrylate (PMMA) was utilized in this case. Porcine cancellous bone graft (PCBG) was inserted to fill the joint space in carpus. The clinical and radiographic assessments of joint after the surgical procedure proved that PCBG can be used a promising alternative to Autogenous cancellous bone graft (ACBG).
