  • Background: Cataracts are the leading cause of impaired vision or blindness in dogs. There are many antioxidants that can prevent cataract progression, but whether they are clinically effective in dogs has not been established. Objectives: To analyze the delaying or preventing effect of oral antioxidants on canine senile cataracts through retrospective analysis. Methods: Medical records of dogs from January 1, 2015 to July 10, 2020 were reviewed. Dogs that were 8 yr of age or older with senile cataracts were included in this study. The dogs were divided into two treatment groups (dogs administered with Ocu-GLO supplement and dogs administered with Meni-One Eye R/C supplement) and a control group (dogs that were not administered any supplement). Dogs with incipient and immature cataracts were included in this study. Altogether, 112 dogs (156 eyes) with incipient cataracts and 60 dogs (77 eyes) with immature cataracts were included. The period of time that cataracts progressed from incipient to immature, and from immature to mature was recorded for each dog. Results: There was no significant delaying effect on the progression of incipient cataracts. However, both Ocu-GLO (hazard ratio = 0.265, p = 0.026) and Meni-One (hazard ratio = 0.246, p = 0.005) significantly delayed the progression of immature cataracts compared to the control group. Conclusions: Although there was no significant delaying effect of oral antioxidants on incipient cataract progression, antioxidants could be used to delay the progression of senile immature cataract.

  • Background Obesity, the prevalence of which is increasing due to the lack of exercise and increased consumption of Westernized diets, induces various complications, including ophthalmic diseases. For example, obesity is involved in the onset of cataracts. Methods To clarify the effects and mechanisms of midodrine, an α1-adrenergic receptor agonist, in cataracts induced by obesity, we conducted various analytic experiments in Otsuka Long-Evans Tokushima Fatty (OLETF) rats, a rat model of obesity. Results Midodrine prevented cataract occurrence and improved lens clearance in OLETF rats. In the lenses of OLETF rats treated with midodrine, we observed lower levels of aldose reductase, tumor necrosis factor-α, and sorbitol, but higher levels of hexokinase, 5'-adenosine monophosphate-activated protein kinase-alpha, adenosine 5'-triphosphate, peroxisome proliferator-activated receptor-delta, peroxisome proliferator-activated receptor gamma coactivator 1-alpha, superoxide dismutase, and catalase. Conclusion The ameliorating effects of midodrine on cataracts in the OLETF obesity rat model are exerted via the following three mechanisms: direct inhibition of the biosynthesis of sorbitol, which causes cataracts; reduction of reactive oxygen species and inflammation; and (3) stimulation of normal aerobic glycolysis.

  • Our purpose is to ascertain, if possible, whether atomic bomb survivors with cataracts and epilation were more radiosensitive than those survivors with cataracts but without epilation. A major ophthalmologic survey was conducted in Hiroshima and Nagasaki in 1963-64. At that time, 2125 individuals were examined. Among these individuals, estimated eye organ doses, based on the DS86 dosimetry system, and information on the occurrence of epilation within the first 60 days following the bombings are available on 1742. In the analysis of these data we have assumed that each individual represents a sample of one from a binomial distribution, and that the occurrence of cataracts and epilation are independent biological phenomena. We got following results. The threshold for cataract induction and its 95% confidence limits have been estimated from data on the occurrence of cataract and epilation. Among the 1742 study subjects, 40 had both cataracts and severe epilation. The estimated threshold based on these cases is 0.98 sievert(Sv), with 95% lower and upper confidence bounds of 0.72, and 1.32 Sv, respectively, and is highly statistically significant. Among the 27 cases of cataracts where severe epilation was not reported, the estimated threshold is 1.74 Sv with 95% lower and upper confidence bounds of 1.21 Sv, and "not estimable". The difference between these two estimates is not statistically significant although the effect of dose is highly significant in both instances. The potential importance of biases in the DS86 dose estimates is discussed. The difference between the threshold estimated from cataract cases with epilation and that from cases without epilation is not statistically significant at the 5% or 10% level, and thus affords no support for the notion of increased radiosensitivity.

  • Background: Long-term comparisons of phacoemulsification with topical medication are limited in canine diabetic cataracts. Objectives: To compare outcomes of eyes submitted to phacoemulsification with those of topical medication for canine diabetic cataracts and identify risk factors for complications. Methods: Through medical records review, 150 eyes (76 dogs) with diabetic cataracts were included; 58 eyes (31 dogs) underwent phacoemulsification (phaco-group) and 92 eyes (48 dogs) received ophthalmic solution alone (medication-group). The medication-group was divided into owner-led and vet-led groups depending on who elected not to perform surgery. Comparisons involved time-to-complications, vision, and the number and type of ophthalmic solutions administered. The association between complications and pretreatment clinical findings was investigated. Results: No difference was found in complication risk between the phaco and owner-led medication groups. Conversely, the vet-led medication-group had a higher complication risk than the other groups. At the last follow-up, 94.8% of the phaco-group had vision, whereas 7.6% of the medication-group restored some visual axis. Poor glycemic control in the medication-group and younger age in the phaco-group increased complication risk. At 1-year post-treatment, the average number of ophthalmic solutions administered was 1.7 and 2.6 in the phaco and medication groups, respectively. The medication-group used anti-inflammatories the most throughout the follow-up, whereas the phaco-group used anti-inflammatories the most until 1-year post-treatment and lacrimostimulants at 1.5-year post-treatment. Conclusions: For canine diabetic cataracts, phacoemulsification is recommended because it is superior to topical management alone in terms of maintaining vision and reducing the number of ophthalmic solutions required in the long term.

  • Naturally occurring coumarin compounds have received substantial attention due to their pharmaceutical effects. Esculetin is a coumarin derivative and a polyphenol compound that is used in a variety of therapeutic and pharmacological strategies. However, its effect on aldose reductase activity remains poorly understood. In this study, the potential beneficial effects of esculetin on lenticular aldose reductase were investigated in galactose-fed (GAL) rats, an animal model of sugar cataracts. Cataracts were induced in Sprague-Dawley (SD) rats via a 50% galactose diet for 2 weeks, and groups of GAL rats were orally treated with esculetin (10 or 50 mg/kg body weight). In vehicle-treated GAL rats, lens opacification was observed, and swelling and membrane rupture of the lens fiber cells were increased. Additionally, aldose reductase was highly expressed in the lens epithelium and superficial cortical fibers during cataract development in the GAL rats. Esculetin reduced rat lens aldose reductase (RLAR) activity in vitro, and esculetin treatment significantly inhibited lens opacity, as well as morphological alterations, such as swelling, vacuolation and liquefaction of lens fibers, via the inhibition of aldose reductase in the GAL rats. These results indicate that esculetin is a useful treatment for galactose-induced cataracts.

  • The Wonyenaejang is equivalent to the (senile)cataract in western medicine. The word cataract is used to describe the natural lens that has turned cloudy. As the natural lens of the eye becomes cloudy, it does not allow light to pass through it. Cataracts usually start as a slight cloudiness that progressively grows more opaque. As the cataract becomes more mature(increasingly opaque and dense), the retina receives less and less light. The light that does reach the retina becomes increasingly blurred and distorted. This causes gradual impairment of vision. If left untreated, cataracts can cause needless blindness. Although there are many kinds of cataracts, a senile cataract is the most common one. We chose the oriental medicine textbooks and the oriental medicine journals that were dealing with the symptoms, etiology, and internal/external treatments. The results were as follows : 1. The main causes of this disease are weak liver and kidney, burning up of the wind and heat in the liver and gall, weak spleen and stomach. 2. As the internal treatment of the Cataract, Geegukjihwangtang is mostly prescribed. 3. As the external treatment of the Cataract, (l) In the field of medicine for external application is commonly prescribed (2) In the field of drug action, frequently used treatments are as follows. emission of the evil, alleviation of fever, removal of lump of blood, and the medicine for external applications. (3) In the field of four Qi, cold medicine is commonly prescribed. (4) In the field of five tastes, bitter/hot/sweet mdicine are commonly prescribed. (5) In the field of toxicity, non-togic medicine is commonly prescribed. (6) In the field of channel distribution, most of the medicine belong to liver channel.

  • Purpose: The aim of this study is to examine the trends of cataract surgery and the recognition of cataract. Methods: ; It enrolled a total of 150 respondents, comprising 72 men and 78 women, who reside in Busan Metropolitan City, Ulsan Metropolitan City and Kyeong-nam Province. By the age groups, they include 44 people in their 20s, 24 in their 30s, 24 in their 40s, 26 in their 50s and 32 aged 60 years or older. In addition, the current study was conducted by dividing the respondents into two groups: people with cataract or not. Moreover, it was also conducted mainly for younger people. Results: 1. As for age of cataract operation, this study say that 1 patient was in their 20s, 3 patients in their 30s, 3 patients in their 40s, 6 patients in their 50s, and 7 patients in their over 60s. 2. In the question about whether they know about cataract, 100 respondents answered 'I know', 50 answered 'I do not know', 67% answered 'I know', 33% answered 'I do not know'. 3. As for pathways to know about cataract, of the total 120 respondents, 49 (41%) were in the media, 20 (17%) were Internet users, 26 (22%) were acquaintances, and 10 (7%) were specialty books, and 15 (13%) were 'others'. 4. As for cataract causes, 20 respondents (13%) answered 'exposure to sunlight', 12 (8%) answered 'electronic devices', 7 answered 'hereditary' 89 (60%) answered, and 22 (14%) answered 'eye disease'. 5. Twenty (13%) answered 'extremely yes', 40 (27%) answered 'yes', and 31 (21%) answered 'normal' when asked whether the use of smartphone was affected by cataracts. 48 (32%) answered 'little', and 11 (7%) answered 'not at all'. Our results showed that many people aged 50 years or older developed cataract and received surgery. It was also shown, however, that people in their 20s to 40s also developed cataract and received surgery. Conclusions: Our eyes are exposed to many places. Many people are unaware of cataracts, despite the fact that cataracts are gradually getting younger, due to the development of electronic devices and the use of electrical appliances. We should have interest in many information and contents about cataracts, knowing the importance to eyes, and gradually reducing the use of many electronic devices in young people.

  • Objective: In general, macular degeneration, cataracts and glaucoma generally cause visual injury in clinical settings. This study aimed to examine the effects of low visual acuity simulations on hand manual dexterity function and brainwaves in healthy young adults. Design: Cross-sectional study design Methods: This study was an observational, cross-sectional study. Seventy healthy young adults participated in this study. To evaluate the effects of low visual acuity simulations on hand function and brain waves, this study involved four different visual conditions including (1) normal vision, (2) simulated cataracts, (3) simulated glaucoma, and (4) simulated macular degeneration. The hand function was measured to use the Minnesota manual dexterity test (MMDT), and the brainwaves was also measured to use the electroencephalography. Results: In hand function, placing and turning performance on the MMDT in the normal visual condition was significantly different than that in the cataract and macular degeneration conditions (p<0.05), and the placing performance was significantly differred in the normal condition than that in the simulated glaucoma. However, turning was not significantly different in the normal condition than that in the simulated glaucoma. The alpha, beta, and gamma waves did not significantly differ among the four visual conditions (p>0.05). Conclusions: The results suggest that limited visual information negatively affects the ability to perform tasks requiring arm-hand dexterity and eye-hand coordination. However, the effectiveness of low visual acuity on the brainwaves should be further studied for rehabilitative evidence of visual impairment.

  • The purpose of this study is to investigate the incidence rate, surgery tendency, and risk factors of cataracts among the elderly in Korea. The subjects were 40-95 years old men and women using the National Health Services' surgical statistics. Among the 33 major surgeries, which increased by 1.1% per year for the last 20years (1.6 million), the most operated surgery in 2020 was cataract surgery (454,000), the number of which was 858.8 per 100,000 people. Among the surgical patients, cataract patients (7.9% increased) was the third largest among the top (2016-2020) constantly with annually 2.2% increasing for 20 years (19.95 million). The medical expenses of major surgeries (excluding non-benefit) increased by an average rate of 9.3% per year over the 20 years (7,204 billion won) out of a total medical expenses of 7.2 trillion won, and the cataract surgery (813.2 billion won) was the second largest among the medical expenses in 2020 (top 3). This study showed that there was a high correlation between cataract and smoking, low income, and low-educated, whereas being no concentration of cataract patient in tertiary hospitals.
