Review of the effect of PNF application on low back pain patient (요통환자에 대한 PNF 적용 효과에 관한 고찰)

  • Purpose : When the pliability of the Lumbar is degraded, the kinesiology function undergoes an influence and it receives an overload. Finally, it is connected with a Lumbar muscular injury. When it does the regular exercise treatment the lumbago patient the balance and function of changeover are improved. The purpose of this study is to review of effect of the PNF application for lumbago patient Methods : By using internet, we research the PubMed, Science Direct, KISS, DBpia and the article on the effect of PNF. We selected the article between 1970 and 2007. Key words were low back pain, lumbago, PNF. Results : PNF is a therapeutic exercise for central nervous system(CNS) patient and the peripheral nervous system(PNS) patient and it is widely applied from sports medicine in the last. According to research of the Jung Young Jo(2007) and Han Kou Soo(2002), PNF technique improves the pain and a condition of the lumbago patient. And the affection balanced change and dynamic balance change and spontaneous movable scope are improved. And it improved to daily life activity. According to research of Moore and Hutton(1980), Lee Kyoung Hye(1999), PNF stretching where it is one of muscular relaxation is good in lumbar muscle fascia ache solutions, and PNF stretching that muscle is relaxed and increases ROM by Reflective system of the vertebra. These researches is relation all each other. it have appeared many report of research about Pliability, muscular power, balance, mobility improve ADL movement of attendance ability and functional action improved excise therapy put effect about lumbago patient recently, it have appeared research about therapy of lumbago patient through PNF. But PNF technique is not application about lumbago patient in clinic, therefore also it is not enough for case study about this therapy. Conclusion : PNF improve combination operation of muscle, inconvenience decrease from daily life activity, ROM(range of movement) of lumbar is increase and Lumbago reduce. So we suggest that PNF will be applies on the lumbago patient and must do a many research.

  • Background and Purpose : Most diagnostic method for lumbago were invasive and complex. So we need to simplify and objectify diagnostic method for lumbago. Some study with EAV which is one of Physiological Signal Measuring Instruments, report significantly result as objective diagnostic method for other clinical symptom. By using EAV, we have obtained some physiological signal data from meridian-acupoints of 31 lumbago cases. Objective and Methods : This study researched into the clinical statistics for 31 case who ware in lumbago, and they ware treated with oriental medical care at the Dong-shin university oriental hospital during 6 month from June 1 2001 to November 10 2001. The data were analyzed and interpreted to compare with traditional differentiation of symptom-complexes, then further evaluated as the Five Evolutive Phases to make them differentiated. The EAV valus of Five Evolutive Phases were identified with the sequence of wood(木), fire(火), earth(土)steel(金), water(水). Results and Conclusion : These values of physiological signal were identical with standard differentiation of symptom-complexes of lumbago which is the main cause of deficiency of Qi and blood of the kidney and bladder. Among Five Evolutive Phases, Earth and wood values were increased, steel, fire and water were decreased significantly. This data imply the possibility of somewhat generalization from measuring instruments.

  • Objective : The purpose of this report is to use more Shin Jong Gyuk of Ohaeng Acupuncture acupunture treatment clinically by comparing the Improvement of Back pain and Sciatica between Common Acupuncture treatment Group and Common Acupuncture with Shin Jong Gyuk of Ohaeng Acupuncture treatment Group. Methods : This study was carried out on 40 patients with Back pain and Sciatica who were hospitalized in Dongshin Univ. Oriental Hospital from 1st, April, 2001 to Tenth, November, 2001. Group A of 20 patients were taken both common acupunture treatment and Shin Jong Gyuk of Ohaeng Acupuncture treatment. And Group B of 20 patients were only taken common acupunture treatment. And after 10 days of admission, we checked and compared VAS(Visual Analogue Scale) and improvement index out of these two groups. SPSS(Statistical Program for Sdcial Science) for Window was used for a statistical analysis and the independent T-test was performed to gauge the improvement of VAS(Visual Analogue Scale) and improvement index out of these two groups, in which case, value of P below 0.05 is considered as useful. Results : The result of the VAS(Visual Analogue Scale) and improvement index of comparision analysis between two groups after 10 days, shows that the improvement of Back pain and Sciatica in Common Acupuncture treatment with Shin Tong Gyuk of Ohaeng Acupuncture treatment Group is more effective than that in Common Acupuncture treatment Group. (independent T-test improvement index P=0.21, VAS P=0.00)

  • When open reduction of maxilla fractures is postponed due to concurrent life-threatening injuries, delayed union may result with malunion or nonunion. If delayed malunion is occurred, significant facial deformity may result, including a dished-out face, irregular retromaxillism with Angle's class III malocclusion, open anterior bite, nasal collapse, telecanthus and malar flattening. The treatment planning for this problem includes cephalometric evaluation anterior and lateral tomograms, dental casts, orthodontic planning, dental planning and use of impression tray to rupture the fibrous tissue casts, orthodontic planning, dental planning and use of impression tray to rupture the fibrous tissue attachment at the fracture site. In this paper, one case presented a 58-year-old female patient with maxilla retrusion after comminuted fracture, who was treated with orthodontic methods of maxillary protraction headgear and Plaster headcap, whereas the other two cases were about male patients who were treated principally with surgically open reduction or Le Fort I-controlled transverse osteotomy with iliac bone graft.

  • Radical paring of the cleft edge during primary cleft lip operation or repeated secondary surgeries can result in tightness of the upper lip. In case, the degree of the resulting side-to-side tension is very severe, the possibility of a lip switch flap must be considered. When the lip tightness accompanies a loss of more than two-thirds of the Cupid's bow, an Abbe flap is an alternative. The disadvantages of Abbe flap are scar formation on the lower lip, design of incision line on the upper lip, disharmony of colors, and the dysfunction of the orbicularis muscle. These problems have been recognized in the literature and extreme discretion has been advised in its application. We experienced four cases of Abbe flap operation which were designed differently to correct the secondary unilateral or bilateral cleft-lip nasal deformities. The Abbe flap operations resulted in removal of the scars and tightness of the upper lip, reconstruction of the Cupid's bow, lengthening of the columella, and therefore secondary cleft-lip nasal deformity could be corrected. It is thought that carefully applied Abbe flap is an appropriate method to relieve horizontal tightness or flattening of the upper lip which occured after primary operation of cleft lip.

  • Microstomia can be occurred as a result of direct injury to tissues such as chemical, thermal and electrical burns, and animal bites. It also may be secondary to contracture of burned perioral skin, or may result from scarring after reconstructive lip surgery. Narrowing of the oral aperture is not only disfiguring, but also limiting the oral access needed for introduction of food, insertion of dentures, oral hygiene, and dental treatment. Limited mouth opening may also interfere with mastication and speech. Few reports exist regarding correction of microstomia and reconstruction of the corners of the mouth. A 16-year-old girl with a bilateral cleft lip and palate presented with the limited mouth opening (approximately 20 mm), the esthetic problem due to the small lip, and the cleft lip-nasal deformity. The microstomia was corrected by bilateral commissuroplasty using "over and out" buccal mucosa flaps proposed by Converse. The intercommissure distance was increased from the preoperative 40 mm to the postoperative 60 mm. The one-year postoperative intercommissure distance was 54 mm, because the 6 mm relapse was occurred. The bilateral commissuroplasty using "over and out" buccal mucosa flap could increase the width and general size of the oral aperture and improve the lip appearance.

  • 1. The author investigated on the flora of Yongzong island especially on the point of view of the sociological studies. 2. The author could distinguish kinds of the plant formtion in this island(1). Seaside plant formation (2). Mountain plant formation. 3. The seaside plant formation could be subdivided as follows: (1). Association of Triglochin maritimum LINNE. (2). Association of Suaeda japonica MAKINO. (3). Association of Scirpus triqueter LINNE. (4) Association of Phragmites prosturatus MAKINO. (5). Association of Zoysia japonica STEUDEL. (6). Association of Zoysia sinica HANCE var. tenuis NAKAI ex YAMAMOTO. (7). Assocation of Carex pumila THUNBERG. (8). Association of Rosa rugosa THUNBERG. (9). Association of Pinus Thunbergii PARLATORE. (1), (2), (3), (4), (5) associations can grow under the high tide, so when the high tide comes they soaked in the sea water. 4. The mountain plant formation can be subdivided as follows: (1). Association of Quercus aliena BLUME and Platycarya strobilacea STEBOLD & ZUCCARINI. (2). Association of Quercus acutissima CARRUTHERS and Platycaryastrobilacea SIEBOLD & ZUCCARINI. (3). Association of Robinia Pseudoeacia LINNE and Platycarya strobilacea SIEBOLD & ZUCCARINI. 5. The plants at the abandoned mine regions are replacing by the association of Robinia Pseudacacia LINNE and Platycarya strobilacea SIEBOLD & ZUCCARINI. 6. Generally I can find that soil which is developed by the seaside plant formation under sea water (when high tide comes, they soaked in the sea water) is over the pH 8.0, so it shows strong alkaline. But the sandy soil behind the tide line shows near the neutral in spite of the same seaside plant formation. In the case of mountain plant formation the soil where they developed show near the pH 6.0, so it is acidity.

  • Lymphangioleiomyomatosis, a rare disease in women of childbearing age, is the result of benign nodular hypertrophy of the smooth muscle of the lypmhatics and other tissues of the abdomen and thorax. We report a 36-years-old woman with pulmonary and retroperitoneal lymphangioleiomyomat.osis who responded with hormone treatment. She developed vaginal pruritis and a pelvic ultraound was done given her significant past medical history. Ultrasound examination demonstrated a large mass in the right side of her pelvis. Therefore she was admitted to St. Michael's Hospital in Toronto for laparoscopy. Result of cytology was to be consistent with the diagnosis of retroperitoneal lymphangioleiomyomatosis. High resolution CT sacn of the thorax demonstrated multiple small cystic lesions, without associated nodularity compatible with a diagnosis of pulmonary lymphangioleiomyomatosis. She has been taking Provera tablets 100mg po tid since Dec. 15, 1993. We have given her a prescription for Depo provera 500mg IM monthly since she came back to Korea. and made arrangements for regular follow up monthly. We performed chest X-ray, CT of chest(high resolution), abdomen and pelvis, pulmonary function tests and arterial blood gas analysis. Chest X-ray and CT findings showed no significant change since July. 20, 1993.

  • Polymyositis (PM) is a inflammatory connective tissue disease involving predominantly skeletal muscles, characterized by symmetrical, proximal muscle weakness, inflammation, and frequently, degeneration. Interstitial lung disease in association with PM occurs in 5~10% of cases and carries an especially grave prognosis. Although the cause of lung involvement in PM is not known, the underlying pathologic process in the lung is an immune mediated inflammation of alveolar structures, alveolitis. It is of interest, therefore, that cyclophosphamide, an immune modulating agent, has been reported to be effective in the treatment of PM. We report a case of corticosteroid resistant PM associated with interstitial lung disease, successfully treated with cyclophosphamide. A 37-year-old female was presented with 8 months duration of cough, exertional dyspnea, and muscle weakness. She had typical symptoms, physical findings, and elevated muscle enzyme levels in serum with characteristic findings of muscle biopsy. She also had typical interstitial lung disease pattern on chest X-ray and high resolution CT with restrictive pattern on pulmonary function test. The findings of transbronchial lung biopsy was compatible with interstitial lung disease. She failed to respond to corticosteroid initially. Subsequently steroids and cyclophosphamide were given with excellent clinical improvement.

  • This report is an analysis of 60 cases of primary mediastinal tumors and cysts which were experienced in chonbuk National University Hospital, Wonkwang University Hospital and Chonju Presbyterian Medical Center from january 1985 to October 1990. The result was summarized like this; 1) Age distribution was various from ages of 5 to 75 years. There were 26 males and 34 females, sex ratio of patients was 1: 1.3 (M:F). 2) The most common chief compliant was dyspnea (21 cases, 35%) and followed by chest pain, coughing and chest discomfort. 17 patients (28%) were asymptomatic. 3) The most common primary mediastinal tumor was thymoma, which comprise 28% (17 cases) of all our cases and followed by neruogenic tumor (14 cases, 23%) and germ cell tumor (11 cases, 18%). 4) The incidence of malignancy of all case was 22%; 2 cases were asymptomatic and the most common malignancy was malignant lympoma (5 cases, 38.5%). 5) The anterior mediastinum was the most common tumor location and followed by posterior and middle mediastinum. Anterior mediastinal tumors mainly consisted of thymomas and germ cell tumors and posterioc mediastinal tumors mainly did neurogenic tumors. 6) Of 60 cases, 21 cases were confirmed histopathologically by percutaneous transthoracic needle lung biopsy. 7) Of 60 cases, 40 cases were received radical tumor resection.
