Huge Immature Teratoma of Mediastinum - A case report - (종격동에 발생한 거대 미성숙기형종 -1례 보고-)

  • Primary mediastinal immature teratoma is a rare germinal tumor which includes various elements of mature teratoma, choriocarcinoma, yolk sac carcinoma, embryonal carcinoma, and seminoma in some proportions. The tumor is virtually restricted to young man and the response to surgery and radiotherapy are poor. Recently, we experienced a case of primary mediastinal immature teratoma with elevated serum [-HCG and [-fetoprotein in 18 years old man. The well-encapsulated mass, weighing 4.5 kg, was completely resected and then adjuvant combination chemotherapy was tried with Vincristine, Bleomycin, and Cisplatin. Radical excision of tumor and adjuvant chemotherapy would appear to produce better result than have been reported in other cases. The postoperative course was uneventful and the tumor markers were returned to normal range.

  • Case markers in Korean are omissible in colloquial speech. Previous discourse studies of Caseless bare NPs in Korean show that the information structure of zero Nominative not only differs from that of overt Nominative but it also differs from that of zero Accusative in many respects. This paper aims to provide a basis for these semantic/pragmatic properties of Caseless NPs through the syntactic difference between bare subjects and bare objects: namely, the former are left-dislocated NPs, whereas the latter form complex predicates with the subcategorizing verbs. Our analysis will account for the facts that (i) the distribution of bare subject NPs are more restricted than that of bare object NPs; (ii) bare subject NPs must be specific or topical; (iii) Acc-marked NPs in canonical position tend to be focalized.

Long-Term Follow-up of Pancreatic Cancer Patients with Lung Metastasis Using Integrated Cancer Treatment: A Case Report (통합암치료를 적용한 폐전이 동반 췌장암 환자의 장기간 추적관찰 증례보고)

  • Objective: This long-term case report details a case of pancreatic cancer with lung metastasis suppressed by integrative cancer medicine (IMT). Methods: A 64-year-old woman diagnosed with pancreatic cancer visited a Korean medicine hospital complaining about the side effects of chemotherapy for lung metastasis. She received IMT involving Korean traditional medicine and Western immunotherapy from May 2017 to June 2023. Tumor dimensions were assessed through computed tomography (CT) and positron-emission tomography/CT scans, while tumor markers and safety were monitored by laboratory tests. Results: IMT suppressed the progression of cancer, as observed by imaging and laboratory tests. The patient achieved 5-year survival, even after discontinuing chemotherapy. Conclusion: This case presents a potential therapeutic alternative for patients who are ineligible for chemotherapy or surgical interventions.

Five-Year Follow-up of an Ovarian Cancer Patient with Brain and Vertebral Metastasis Using Integrative Cancer Treatment: A Case Report (통합암치료를 적용한 난소암 뇌, 척추전이 환자의 5년 추적관찰 증례보고)

  • Objectives: This long-term case report presents the case of an ovarian cancer patient with brain, cervical lymph node, and vertebral metastasis suppressed by traditional Korean medicine in combination with cytokine-induced killer (CIK) cell-based immunotherapy. Methods: The patient received acupuncture, moxibustion, GunChil-go, Hangam-dan, and CIK cell-based immunotherapy. The Eastern Cooperative Oncology Group and tumor markers were used to evaluate the treatment effects. Results: Integrative cancer treatment suppressed the progression of cancer, and the patient achieved eight-year survival. The performance status improved, and the tumor marker level was maintained. Conclusions: We suggest that an integrative cancer treatment that includes traditional Korean medicine can be a meaningful treatment option for advanced ovarian cancer.

Biomonitoring Human Exposure to VOCs : Using Individual Susceptibility Markers

  • In this study, biomonitoring methods were developed to measure BTEXs exposure level in the air, metabolites of benzene and toluene in human urine, individual susceptibility markers in human blood for evaluation of the health effects about environmental pollution. We have also performed a small-scaled molecular epidemiology study on residents in Chuncheon and workers in workplace for these method applications. The workers in workplace were surveyed as study areas, and the residents in Chuncheon which is in the suburban area were surveyed as comparative areas in this study. Actually, 31 workers in as target group and 33 residences in as control group this epidemiological study. The results obtained from this study were as follows: 1. Benzene is a well-known carcinogen, it's median concentrations were 0.00024∼0.02057ppm at suburban area and 0.002∼00.654ppm at work place, These benzene concentrations were not exceed the OSHA(Occupational Safety and Health Administration) threshold benzene level of 1ppm in the states. 2. Metabolites product of benzene(t,t-Muconic Acid) and toluene(Hippuric Acid) were not significant both in suburban and workplace area. The median concentration of t,t-MA and HA were 0.0122, 1.44277g/g creatinine, respectively. 3. In the case of individual susceptibility markers as CYPlAl, 41.8% of them has homozygous wild type(W) and who has heterozygous variant type(H) was 35.4% and 22.8% of homozygous variant type(M) genetic type. In the case of CYP2E1, 62.82% of them has homozygous wild type(D) type, 34.62% of each has heterozygous variant type (DC) and 2.56% of them has homozygous variant type (CC). Who doesn't have GSTM1 gene was 46.25% and who has GSTM1 gene was 53.75%. Who doesn't have GSTT1 gene was 40.0% in study groups and who has GSTT1 gene was 60.0%. Who has W genetic type, which is homozygous wild type of GSTP1, was 69.18% and H genetic type, which is heterozygous variant type was 28.4%. M genetic type which is homozygous variant type was 2.4%. 4. Concentration differences of metabolites such as t,t-MA and HA in urine, which is generated by individual susceptibility marker of GSTM1, GSTT1, GSTP1 gene of Phase I and CYP1A1, CYP2E1 gene of Phase II, was examined. As a result, GSTP1 and GSTM1 indicate slight differences depend on the amount of metabolites in urine, it was not statistically significant.

Evaluation of MT1XT20 Single Quasi-Monomorphic Mononucleotide Marker for Characterizing Microsatellite Instability in Persian Lynch Syndrome Patients

  • Background: Colorectal malignancies with high microsatellite instability (MSI-H), either hereditary (Lynch syndrome) or sporadic, demonstrate better prognosis and altered response to 5FU chemotherapy. It is now recommended to perform MSI testing for all new cases of colorectal cancer regardless of being categorized as hereditary or sporadic. For MSI detection, immunohistochemistry or PCR-based protocols using a cohort of various sets of STR markers are recommended. Here we aimed to evaluate a simplified protocol using just a single STR marker, MT1XT20 mononucleotide repeat, for detection of MSI in Lynch syndrome patients. A Promega five-marker MSI testing panel and immunohistochemistry (IHC) were used as the gold standard in conjunction with MT1XT20. Materials and Methods: Colorectal patients with a positive history of familial cancers were selected by evaluating medical records. Based on Amsterdam II criteria for Lynch syndrome 20 families were short listed. DNA was extracted from formalin fixed paraffin embedded tumour and adjacent normal tissues resected from the index case in each family. Extracted DNA was subjected to MT1XT20 mononucleotide marker analysis and assessment with a commercially available five marker MSI testing kit (Promega, USA). IHC also was performed on tissue sections and the results were compared with PCR based data. Results: Eight (40%), seven (35%) and five (25%) cases were MSI positive using with the Promega kit, IHC and MT1XT20, respectively. Among the markers included in Promega kit, BAT26 marker showed instability in all 8 samples. NR24 and NR21 markers showed instability in 7 (87.5%), and BAT25 and MONO 27 in 6 (75%) and 5 (62.5%). Conclusions: Although MT1XT20 was earlier reported as a valid standalone marker for MSI testing in CRC patients, we could not verify this in our Iranian patients. Instead BAT26 among the markers included in Promega MSI testing kit showed instability in all 8 MSI-H CRC samples. Therefore, it seems BAT26 could act well as a single marker for MSI testing in Iranian CRC patients.

A Rare Case of Acute Pyelonephritis Leading to Bilateral Subcapsular Renal Hematoma: A Case Report (급성 신우신염에 의한 양측 신피막하혈종 1예)

  • This case underscores the rarity of bilateral subcapsular renal hematoma (SCH) as a complication of acute pyelonephritis, and this is the first reported case in South Korea. SCH can have various underlying causes, including cysts, bleeding tendencies, cancer, vascular diseases and, less commonly, infections. A patient with uncontrolled diabetes mellitus and alcohol dependency presented with weakness. Bilateral SCH from acute pyelonephritis was diagnosed and treated with antibiotics and renal replacement therapy. Despite improved renal function, 30-day bilateral percutaneous drainage was necessary due to a persistent fever and elevated infection markers, resulting in a bloody, pus-like discharge. This case report sheds light on the rare etiology of bilateral SCH as a result of acute pyelonephritis. It serves as a reminder of the critical importance of timely intervention, to prevent deteriorating outcomes.

Resistin and Insulin Resistance: A Link Between Inflammation and Hepatocarcinogenesis

  • Background: Hepatocellular carcinoma (HCC) is the sixth most common cancer and the third leading cause of cancer related death overall. The role of insulin resistance in the development of HCC associated with chronic HCV infection has not been established. Resistin is a polypeptide hormone belonging to the adipokine family which could contribute to tumorigenesis and angiogenesis. Our aim was to study serum resistin and insulin resistance as risk factors for HCC in HCV cirrhotic patients. Materials and Methods: This prospective case controlled study included 100 patients with HCV related liver cirrhosis and HCC, 100 patients with HCV related liver cirrhosis without HCC and 50 apparently healthy participants as controls. For all subjects, liver profile, serologic markers for viral hepatitis, lipid profile, alpha-fetoprotein level (AFP), homeostasis model assessment (HOMA) were examined along with resistin. Results: HCC patients had higher mean values of HOMA-IR and resistin than cirrhotic patients and the control subjects (p<0.01). HOMA and resistin were considered independent risk factors in development of HCC, those patients with resistin > 12 ng/ml and HOMA > 4 being 1.6 times more likely to have HCC. Conclusions: HOMA and serum resistin allow for early identification of patients with cirrhosiswho are at substantially increased risk of HCC. Recommendation: HOMA and serum resistin could represent novel markers to identify HCV cirrhotic patients at greater risk of development of HCC.

  • We Propose a sensor fusion technique for motion capture system. In our system, two kinds of sensors are used for mutual assistance. Four magnetic sensors(markers) are attached on the upper arms and the back of the hands for assisting twelve optical sensors which are attached on the arms of a performer. The optical sensor information is not always complete because the optical markers can be hidden due to obstacles. In this case, magnetic sensor information is used to link discontinuous optical sensor information. We use a system identification techniques for modeling the relation between the sensors' signals. Dynamic systems are constructed from input-output data. We determine the best model from the set of candidate models using the canonical system identification techniques. Our approach is using a simple signal processing technique currently. In the future work, we will propose a new method using other signal processing techniques such as Wiener or Kalman filter.

Characteristic Trend Analysis of Cancer Patients Hospitalized in Shanxi Tumor Hospital for the First Time during 2001 and 2010

  • To observe and analyze the characteristic trend of cancer patients hospitalized for the first time in Shanxi Tumor Hospital from 2001 to 2010, clinical data including case number, age, gender, and frequency of different tumor occurrences were collected and statistically analyzed. Results: (i) From 2001 to 2010, the number of cancer patients hospitalized for the first time increased by 1.3-fold; (ii) The patient overall average age also increased from 51.8 to 54.4, for males from 55.5 to 58.7 and females from 48.4 to 51.1, respectively. (iii) Male patients accounted for 43-48% and females accounted for 52-57% of the total. The percentage of female patients was higher than that of male patients in every year and showed an upward trend over the years, while that of the males showed a downward trend (χ2=7.031, p=0.008); (iv) Among the top 6 most common cancers, lung, cervical, esophageal, colorectal and breast cancers tended to increase over the years (p<0.05), but not gastric cancer (p=0.423). Conclusions: (i) The number of cancer patients hospitalized for the first time during the past 10 years increased year by year, and was higher for female than male; (ii) the average age of patients increased year after year and was greater for male than female; (iii) the number of patients with lung cancer, cervical cancer, esophageal cancer, colorectal cancer and breast cancer increased over years.
