Direct Carotid Cavernous Fistula of an Adult-Type Persistent Primitive Trigeminal Artery with Multiple Vascular Variations

  • We report a case of spontaneous right carotid-cavernous fistula (CCF) in a proximal segment of persistent primitive trigeminal artery (PPTA) and combined vascular anomalies such as left duplicated hypoplastic proximal posterior cerebral arteries and a variation of anterior choroidal artery supplying temporal and occipital lobe. A 45-year-old male presented with progressive right exophthalmos, diplopia, and ocular pain. With manual compression of the internal carotid artery, a cerebral angiography revealed a right CCF from a PPTA. Treatment involved the placement of detachable non-fibered and fibered coils, and use of a hyperglide balloon to protect against coil herniation into the internal carotid artery. A final angiograph revealed complete occlusion of PPTA resulted in no contrast filling of CCF.

  • 대동맥궁 줄기의 다양한 기시변이는 잘 알려져 있으나 속목동맥과 바깥목동맥의 독립적인 기시와 동반된 척추동맥의 대동맥궁 직접 기시에 대한 보고는 매우 드물다. 저자들은 10세 여아에서 왼쪽 온목동맥의 무형성과 동측 속목동맥, 바깥목동맥 및 척추동맥이 대동맥궁에서 직접 기시하는 매우 드문 복합 변이를 증례로 보고하고자 한다. 또한, 이 증례 보고를 통해 해당 변이의 발생학적 기전과 임상적 의의를 살펴보았다.

  • Extracranial carotid artery dissection may manifest as arterial stenosis or occlusion, or as dissecting aneurysm formation. Anticoagulation and/or antiplatelet therapy is the first-line treatment, but because it is effective and less invasive than other procedures, endovascular treatment of carotid artery dissection has recently attracted interest. We encountered two consecutive cases of trauma-related extracranial internal carotid artery dissection, one in the suprabulbar portion and one in the subpetrosal portion. We managed the patient with suprabulbar dissection using a self-expandable metallic stent and managed the patient with subpetrosal dissection using a balloon-expandable metallic stent. In both patients the dissecting aneurysm disappeared, and at follow-up improved luminal patency was observed.

파노라마방사선사진에서 석회화가 발견되는 경동맥의 협착 정도 (Stenosis of calcified carotid artery detected on Panoramic Radiography)

  • Purpose : This study aimed to investigate the luminal stenosis of the internal carotid artery with calcification detected on panoramic radiographs. Materials and Methods : This study used fifty carotid arteries of 36 dental patients whose panoramic radiograph and computed tomography angiography (CTA) revealed the presence of carotid artery calcification. A neuroradiologist interpreted CTA to determine the degree of stenosis of the internal carotid arteries. The degree of stenosis was stratified in four stages; normal (no stenosis), mild stenosis (1-49%), moderate stenosis (50-69%) and severe stenosis (70-99%). Results : Among the fifty carotid arteries with calcification detected on both panoramic radiography and CTA, 20 carotid arteries (40%) were normal, 29 carotid arteries (18%) had mild stenosis, 1 carotid artery (2%) had moderate stenosis, and there was none with severe stenosis. Conclusion : Sixty percent of the carotid arteries with calcification detected on both panoramic radiography and CTA had internal luminal stenosis, and two percent had moderate stenosis. When carotid atheroma is detected on panoramic radiograph, it is possible that the dental patient has luminal stenosis of the internal carotid artery.

머리의 전방 이동이 경동맥과 추골동맥 혈류량의 변화에 미치는 영향: 사례연구 (Effect of Blood Flow on Carotid and Vertebral Artery During Forward Shift on Head: Case Study)

  • Background: The purpose of this study was to identify the effects of the blood flow on carotid and vertebral artery during the forward shift on head. Methods: A 20-year female healthy subject participated in this study. This study was set up the forward shift on head at the 3cm and 6cm in a shoulder midline (acromion). Measurement method were using duplex ultrasound with colour doppler imaging for the blood flow on carotid and vertebral artery. Results: Carotid artery was increased the blood flow and vertebral artery was decreased the blood flow during forward shift on head. Conclusions: These findings suggest that carotid and vertebral artery changes to the blood flow during forward shift on head. Therefore, we should be consider that hemodynamic factor when apply to the therapeutic exercise for patients of forward head posture.

Urgent Intracranial Carotid Artery Decompression after Penetrating Head Injury

  • We describe a case of intracranial carotid artery occlusion due to penetrating craniofacial injury by high velocity foreign body that was relieved by decompressive surgery. A 46-year-old man presented with a penetrating wound to his face. A piece of an electric angular grinder disc became lodged in the anterior skull base. Computed tomography revealed that the disc had penetrated the unilateral paraclinoid and suprasellar areas without flow of the intracranial carotid artery on the lesion side. The cavernous sinus was also compromised. Removal of the anterior clinoid process reopened the carotid blood flow, and the injection of glue into the cavernous sinus restored complete hemostasis during extraction of the fragment from the face. Digital subtraction angiography revealed complete recanalization of the carotid artery without any evidence of dissection. Accurate diagnosis regarding the extent of the compromised structures and urgent decompressive surgery with adequate hemostasis minimized the severity of penetrating damage in our patient.

심경부 감염과 동반된 총경동맥 가성동맥류 1례 (A Case of Pseudoaneurysm of the Common Carotid Artery secondary to Deep Neck Infection)

  • The complications of deep neck infection have become much less common in the antibiotic era. The vascular complications of deep neck infection can have devasting consequences. Most commonly, the internal carotid artery is involved, although the common carotid and external carotid artery can also be affected.0 the cases of patients with a protracted course, recurrent bleeding, cranial neuropathies, or trismus, the presence of vascular complications must be considered. Appropriate imaging should be carried out to allow the localization of the infection and ascertain the status of the vessels in the neck The vascular structures can be imaged with duplex doppler or color doppler flow ultrasound to see the flow between the mass and vessels. Also angiography plays a key role in the diagnosis and management of vascular complication of deep neck infection. Prompt diagnosis and treatment of these patients is necessary to prevent significant hemmorrhagic complications. We experienced a case of pseudoaneurysm of the common carotid artery secondary to deep neck infection treated successfully with surgical excision in 45-year-old-male.

External Carotid Artery Angioplasty and Stenting Followed by Superficial Temporal Artery to Middle Cerebral Artery Anastomosis

  • A 31-year-old man presented with right hemiparesis, and magnetic resonance imaging revealed a small infarct at left basal ganglia. Digital subtraction angiography showed left cervical internal carotid artery (ICA) occlusion and severe stenosis of the ipsilateral external carotid artery (ECA) with collateral cerebral circulation fed by ECAs. Based on the results of a functional evaluation of cerebral blood flow, we performed preventive ECA angioplasty and stenting for advanced ECA stenosis to ensure sufficient blood flow to the superficial temporal artery. Eight weeks later, superficial temporal artery to middle cerebral artery (STA-MCA) anastomosis was performed. His postoperative course was uneventful and no additional transient ischemic attacks have occurred. To our knowledge, this is the first report of preventive angioplasty and stenting for advanced narrowing of an ECA before STA-MCA anastomosis for ipsilateral ICA occlusion.

The Clinical Outcomes of 75 Consecutive Patients with Cervical Carotid Artery Stenosis Treated by Carotid Artery Stenting

  • Objective : The purpose of this study was to analyze the clinical outcome of 75 consecutive patients with cervical carotid artery stenosis and who were treated by carotid artery stenting (CAS) only. Methods : From February 2003 to June 2008, there were 78 stents placed in 75 symptomatic patients (mean age : 67.3 years); 69 patients had carotid stenosis ${\geq}70%$, and 6 patients had asymptomatic stenosis ${\geq}80%$. No carotid endarterectomy (CEA) was performed during the same period. The patients were clinically followed-up for a mean of 20.1 months. Results : The procedures were technically successful in all cases. Three (3.8%) patients had procedure-related complications. During the 30-day postprocedural period, there were no restenosis or major stroke. Minor stroke was noticed in 3 (3.8%) patients and 1 (1.3%) of the 75 patients suddenly expired 2 days after discharge. There were no new neurological symptoms that developed during the clinical follow-up period. The results of our series were not inferior to those the previously published in CAS studies, and in fact they were better. Conclusion : Our results suggest that CAS may be safe and useful for the treatment of cervical carotid artery stenosis when it is used as the first line treatment in those institutions that lack enough experience with CEA.

Takayasu`s arteritis의 수술치험 1례 (Surgical Treatment of Takayasu`s Arteritis; Report of One Case)

  • Takaysu`s arteritis is an arteritis of unknown etiology involving larger elastic arteries. The end stage pathologic feature is vascular obstructive change and the resulting clinical manifestations are local ischemic symptoms such as syncope, visual disturbance, claudication of extremities, hypertension, and angina. Recently we have experienced one case of Takayasu`s arteritis involving aortic arch, left common carotid artery and left subclavian artery. The patient was 27 year-old female and she was admitted because of headache and neck pain. Aortogram revealed fusiform dilatation of left common carotid artery with focal narrowing on it`s distal portion. The patient underwent surgical resection and replacement of Dacron tube graft between distal and proximal left common carotid artery. 3 months after operation, she was readmitted because of shoulder pain and headache. Aortogram revealed focal narrowing of proximal left common carotid artery and total obstruction of left subclavian artery which caused subclavian steel syndrome. Aorto-left common carotid and aorto-left subclavian bypass graft replacement were done.
