X선 영상 기반 치아와동 컴퓨터 보조검출 시스템에서의 동적윤곽 알고리즘 비교 (A Comparison of Active Contour Algorithms in Computer-aided Detection System for Dental Cavity using X-ray Image)

  • Dental caries is one of the most popular oral disease. The aim of automatic dental cavity detection system is helping dentist to make accurate diagnosis. It is very important to separate cavity from the teeth in the detection system. In this paper, We compared two active contour algorithms, Snake and DRLSE(Distance Regularized Level Set Evolution). To improve performance, image is selected ROI(region of interest), then applied bilateral filter, Canny edge. In order to evaluate the algorithms, we applied to 7 tooth phantoms from incisor to molar. Each teeth contains two cavities of different shape. As a result, Snake is faster than DRLSE, but Snake has limitation to compute topology of objects. DRLSE is slower but those of performance is better.

Kodak Insight 구내필름의 치아우식증 진단능에 대한 연구 (Diagnostic accuracy of Insight intraoral film on dental caries)

  • Purpose: To compare the diagnostic accuracy of Kodak Insight film with other intra-oral films in the detection of dental caries. Materials and Methods : Periapical radiographs of 99 extracted human teeth with sound proximal surfaces and interproximal artificial cavities were made on Kodak Ultra speed, Ektaspeed, Agfa Ektaspeed and Kodak Insight films and automatically processed. Six dentists examined the presence of dental caries using a five-point confidence rating scale and compared the diagnostic accuracy by ROC (Receiver Operating Characteristic) analysis and ANOVA test. Results: The sensitivity of Kodak Ultra speed, Ektaspeed, Agfa Ektaspeed and Insight film were 0.84, 0.77, 0.75 and 0.79 respectively. The specificity of Kodak Ultra speed, Ektaspeed, Agfa Ektaspeed and Insight film were 0.97. 0.95. 0.96 and 0.94 respectively. The mean ROC areas (Az) of Kodak Ultra speed, Ektaspeed, Agfa Ektaspeed and Insight film were 0.917, 0.910, 0.894, 0.909 respectively. There was no significant differences between Az of Insight film and other films (p = 0.178). Conclusion: Theses results suggested that Kodak Insight film have the comparative diagnostic accuracy of dental caries with Ultraspeed and Ektaspeed films.

  • 영유아기 치아우식증은 복잡하고 다인자적 질환으로 숙주 및 미생물, 식이, 시간 등의 위험요소에 의해 질환이 진행된다. 이 중 미생물로 대변되는 Streptococcus mutans(S. mutans)는 우식 개시에 있어 중요한 역할을 하며, 어머니를 통한 수직 감염이 잘 알려져 있다. 영유아기의 어머니를 통한 조기 감염은 향후 유치열기 우식 발생과 높은 연관성을 나타내므로 우식위험요소에 대한 조기 파악은 미래의 치아우식증 예방에 있어 매우 중요하다. 최근 치아우식증의 원인 및 이에 미치는 요소들에 대한 연구가 활발하게 이루어지면서 우식 진행에 대한 정확한 예측을 위한 여러 가지 우식위험도평가(caries risk assessment) 도구가 개발되었는데, 이 중 대표적인 한 가지가 Cariogram이다. Cariogram은 개인의 우식활성도 및 우식위험요소(caries risk factor)의 상호작용을 그래프로 나타낸다. 이번 연구의 목적은 3${\sim}$5세의 중증 유아기우식증(Severe Early Childhood Caries; S-ECC)에 해당되는 어린이들과 그들의 어머니 사이에서 우식관련타액검사 및 최근 개발된 Cariogram을 이용하여 S. mutans의 수준 및 우식활성도에 대한 상관관계가 존재함과 우식위험요소 간의 연관성을 파악해 보았는데, 이를 통해 다음과 같은 결론을 얻을 수 있었다. 1. 중증 유아기우식증을 보이는 어린이에서 구강 내 존재하는 Streptococcus mutans의 수준은 비우식군보다 높았다(p<0.05). 2. 모자간 구강 내 존재하는 Streptococcus mutans 수준은 상관관계를 보였다(p<0.05). 3. 모자간 우식활성도를 Cariogram을 통해 측정해 본 결과 상관관계를 보였다(p<0.05). 4. 우식활성도에 미치는 우식위험요소는 어린이에서 숙주요소가 가장 큰 영향을 주는 것으로 나타났다. 이상의 결과로 S. mutans가 향후 치아우식증 발생 가능성을 예측하는데 있어 유용한 우식위험요소가 될 수 있으며, Cariogram을 통한 향후의 우식진행과 관련된 여러 가지 우식위험요소가 예측 가능하므로 이에 대한 조기 대처 및 예방적 처치가 가능하리라 사료된다.

  • Conventional intraoral radiography continues to be the most widely used for the diagnosis of dental caries. But conventional intraoral film has several shortcomings, including processing error, increased radiation dose, etc. Recently, various digital radiographs substitude for conventional radiography to overcome these disadvantages. The digital radiographies are numerous advantages, including elimination of processing errors, lower radiation dose, image quality enhancements such as contrast and density modulation.(omitted)

구강세균 유전자 검사(easy perio test)법을 이용한 치아우식 검사 (The study of caries activity test by multiplex-quantity real time PCR with easy perio test)

  • The aim of this study was to evaluate the competency of the Easyperio test, a genetic test method based on real time PCR for the detection of bacteria that cause dental caries and periodontal disease. To verify the validity of this text, various dental health evaluations were administered to 33 boys between the ages of 12 to 14, as this age group commonly experiences dental caries. These evaluations included a dental caries experience survey, a first molar health evaluation, the Dentocult Streptococcus mutans (SM) strip mutans, the Dentocult Lactobacillus spp (LB) test, and the Easyperio test. The correlation coefficients between the level of the Dentocult SM strip mutans and the dental caries experience were DT (R=0.570, p=0.001), DMFT (R=0.376, p=0.031), and first molar health (R=-0.395, p=0.023). The correlation coefficients between the amount of SM in the Easyperio test and dental caries experience were DT (R=0.528, p=0.002), DMFT (R=0.369, p=0.035), and first molar health (R=-0.426, p=0.013). The correlation coefficients between the level of the dentocult SM strip mutans and the SM amounts of the Easyperio test were S.mi (R=0.564 p=0.001) and S.mu (R=0.621, p=0.002). The correlation coefficients between the level of the Dentocult LB test and the SM amount of Easyperio test was S.mi (R=0.495, p=0.003). In conclusion, Easyperio test may be an easy and effective method for the differentiation and diagnosis of dental caries through quantitative and qualitative analysis of oral bacteria.

초기우식병소에 대한 레이저 fluorescence의 광학적 탐지감도 (OPTICAL SENSITIVITY OF LASER FLUORESCENCE FOR INCIPIENT CARIES DETECTION)

  • The aim of this study was to evaluate the optical density of laser fluorescence for detection of incipient caries. Prepared and polished bovine enamel specimens were demineralized in a STPP solution for varying periods of time between 3 hrs. and 60 hrs. with an area of sound enamel retained on each specimen. The randomized specimens were analyzed for optical density of enamel demineralization using laser fluorescence. The specimens were sectioned and examined lesion depth by polarizing light microscope. Results were analyzed statistically with SAS program. The results from this study can be summarized as follows: 1. Optical density measured by laser fluorescence and lesion depth measured by polarizing light microscope was increased as demineralization time was increased(p<0.001). 2. Between optical density measured by laser fluorescence and lesion depth measured by polarizing light microscope was correlated highly(${\gamma}{\geq}0.74956$, p<0.001). 3. Regressive equation was obtained in this study as follows. Y=[X-0.260851]/0.000271(R-square:0.5618, p<0.001) (X:DENSITY, Y:DEPTH) In summary, optical density measured by laser fluorescence would be within the range of possibility to quantitatively presume demineralization amount of incipient caries lesion

The effects of noise reduction, sharpening, enhancement, and image magnification on diagnostic accuracy of a photostimulable phosphor system in the detection of non-cavitated approximal dental caries

  • Purpose: Contrast, sharpness, enhancement, and density can be changed in digital systems. The important question is to what extent the changes in these variables affect the accuracy of caries detection. Materials and Methods: Forty eight extracted human posterior teeth with healthy or proximal caries surfaces were imaged using a photostimulable phosphor (PSP) sensor. All original images were processed using a six-step method: (1) applying "Sharpening 2" and "Noise Reduction" processing options to the original images; (2) applying the "Magnification 1:3" option to the image obtained in the first step; (3) enhancing the original images by using the "Diagonal/"option; (4) reviewing the changes brought about by the third step of image processing and then, applying "Magnification 1:3"; (5) applying "Sharpening UM" to the original images; and (6) analyzing the changes brought about by the fifth step of image processing, and finally, applying "Magnification 1:3." Three observers evaluated the images. The tooth sections were evaluated histologically as the gold standard. The diagnostic accuracy of the observers was compared using a chi-squared test. Results: The accuracy levels irrespective of the image processing method ranged from weak (18.8%) to intermediate (54.2%), but the highest accuracy was achieved at the sixth image processing step. The overall diagnostic accuracy level showed a statistically significant difference (p=0.0001). Conclusion: This study shows that the application of "Sharpening UM" along with the "Magnification 1:3" processing option improved the diagnostic accuracy and the observer agreement more effectively than the other processing procedures.

레이저 형광 원리를 이용한 우식 진단 기기의 구치부 인접면 우식 진단 능력 비교 (Comparison of Diagnostic Validity between Laser Fluorescence Devices in Proximal Caries)

  • 이 연구의 목적은 두 가지 형태의 레이저 형광원리를 이용한 우식 진단 기기(DD, DDpen)의 구치부 인접면 우식을 진단 하는 능력을 비교하고, 두 기기의 최적의 절단값을 확인하는 것이다. 이 연구에는 164개의 유구치 인접면과 438개의 영구치 구치부 인접면이 연구 대상에 포함되었다. 각 치면은 순서대로 두 가지 기기를 통한 검사를 시행하였고, 두 기기를 비교할 기준으로서 교익 방사선 사진 촬영을 시행하였다. 방사선 사진 촬영 결과는 우식의 깊이에 따라 유치열 3개의 군($pR_0$, $pR_1$, $pR_2$), 영구치열 4개의 군($PR_0$, $PR_1$, $PR_2$, $PR_3$)으로 분류하였다. 유치열에서 법랑질 우식과 상아질 우식의 AUC 값은 DD가 각각 0.851과 0.890으로 나타났고, DDpen은 각각 0.883과 0.917로 나타났다. 영구치열에서 법랑질 우식과 상아질 우식의 AUC 값은 DD가 각각 0.762과 0.886으로 나타났고, DDpen은 각각 0.828과 0.958로 나타났다. 유치열과 영구치열 모두 구치부 인접면 우식을 검사하는 데에는 DDpen이 DD보다 정확한 결과를 보였다. 하지만 유치열에 대해서는 DD도 인접면 우식 검사에 활용이 가능할 것으로 보인다.

CCD 디지털 방사선사진촬영법의 초기 치아우식증의 진단능 평가에 대한 연구 (Evaluation of diagnostic ability of CCD digital radiography in the detection of incipient dental caries)

  • Purpose : The purpose of this experiment was to evaluate the diagnostic ability of a CCD-based digital system (CDX-2000HQ) in the detection of incipient dental caries. Materials and Methods : 93 extracted human teeth with sound proximal surfaces and interproximal artificial cavities were radiographed using 4 imaging methods. Automatically processed No.2 Insight film (Eastman Kodak Co., U.S.A.) was used for conventional radiography, scanned images of conventional radiograms for indirect digital radiography were used. For the direct digital radiography, the CDX-2000HQ CCD system (Biomedisys Co. Korea) was used. The subtraction images were made from two direct digital images by Sunny program in the CDX-2000HQ system. Two radiologists and three endodontists examined the presence of lesions using a five-point confidence scale and compared the diagnostic ability by ROC (Receiver Operating Characteristic) analysis and one way ANOV A test. Results: The mean ROC areas of conventional radiography, indirect digital radiography, direct digital radiography, and digital subtraction radiography were 0.9093, 0.9102, 0.9184, and 0.9056, respectively. The diagnostic ability of direct digital radiography was better than the other imaging modalities, but there were no statistical differences among these imaging modalities (p > 0.05). Coclusion : These results indicate that new CCD-based digital systems (CDX-2000HQ) have the potential to serve as an alternative to conventional radiography in the detection of incipient dental caries.

인공적 인접면 치아우식증의 구내방사선사진과 디지털 영상의 진단능 평가 (DIAGNOSTIC ABILITY OF THE PERIAPICAL RADIOGRAPHS AND DIGITAL IMAGE IN THE DETECTION OF THE ARTIFICIAL PROXIMAL CARIES)

  • Recently, the digital image was introduced into radiological image. The digital image has the power of contrast enhancement, histogram control, and other digitally enhancement. At the point of the resolution, periapical radiograph is superior to the digital image, but enhanced digital procedure improves the diagnostic ability of the digital image. The purpose of this study was to evaluate the diagnostic ability of artificial proximal caries in conventional radiographs, digital radiographs and enhanced digital radiographs (histogram specification). ROC (Receiver Operating Characteristic) analysis and paired t-test were used for the evaluation of detectability, and following results were acquired: 1. The mean ROC area of conventional radiographs was 0.9274. 2. The mean ROC area of unenhanced digital image was 0.9168. 3. The mean ROC area of enhanced digital image was 0.9339. 4. The diagnostic ability of three imaging methods was not significant difference(p>0.05). So, the digital images had similar diagnostic ability of artificial proximal caries to conventional radiographs. If properly enhanced digital image, it may be superior to conventional radiographs.
