  • With the development of information and communication technologies, the growing volume of data is increasing exponentially, raising interest in big data. As technologies related to big data have developed, big data is being collected, stored, processed, analyzed, and utilized in many fields. Big data analytics in the health care sector, in particular, is receiving much attention because they can also have a huge social and economic impact. It is predicted that it will be able to use Big Data technology to analyze patients' diagnostic data and reduce the amount of money that is spent on simple hospital care. Therefore, in this thesis, patient data is analyzed to present to patients who are unable to go to the hospital or caregivers who do not have medical expertise with close care guidelines. First, the collected patient data is stored in HDFS and the data is processed and classified using R, a big data processing and analysis tool, in the Hadoop environment. Visualize to a web server using R Shiny, which is used to implement various functions of R on the web.

Prenatal, Perinatal and Infancy History of Autism Spectrum Disorder (자폐 스펙트럼 장애의 산전 및 주산기 합병증)

  • Objectives: The purpose of this study was to evaluate the prenatal, perinatal, and infancy history of children with autism spectrum disorder (ASD) as compared to unaffected siblings (SIB) and typically developing children (TC). Methods: Subjects with ASD, their SIB, and TC were recruited. All subjects were assessed using both the Korean version of Autism Diagnostic Interview-Revised (K-ADI-R) and the Korean version of Autism Diagnostic Observation Schedule (K-ADOS) and were subsequently identified as affected or unaffected. Prenatal, perinatal, and infancy history was obtained from the primary caregivers and each facet was compared in those with ASD, the SIB, and the TC groups using SPSS ver. 17.0 (p<.05). Results: 70 individuals with ASD (63 males, 87.94${\pm}$37.8months), 53 SIB (27 males, 85.4087.94${\pm}$48.06 months), and 32 TC (19 males, 104.1987.94${\pm}$23.409 months) were analyzed. The ASD group showed significantly higher rates of insufficient vaccination as they aged age ($x^2$=15.54, p=.000). Among the scheduled vaccinations, the DPT vaccination ($x^2$=10.08, p=.006) was insufficient in ASD groups. The ASD group also showed higher rates of sleep disturbances from infancy. Differences in maternal/paternal age at conception, gestational age, and growth parameters at birth were not significantly difference among the three groups. Conclusion: These results do not support the previous controversies regarding the relationship between prenatal/perinatal complications and ASD. However, these results indicate that perinatal and prenatal factors may contribute to the development of ASD.

A Study on the Home Nursing Care Need Trajectory of the patients with chronic illnesses after discharged from Hospital (만성질환자의 퇴원후 가정간호 요구변화 추이에 관한 조사 연구)

  • The traditonal inpatient acute hospital setting is organized primarily for the intensive management of disease, but not well-suited for continuity of care for the chronically ill patients after being discharged from hospital. For the planning of the continuity of care, firstly, it is necessary to assess the home care needs of the discharged pateints in the context of the nursing diagnosis. Therefore, this study is designed to identify the home nursing care need trajectory of the patients with chronic illness after discharged from one of the the General Hospitals in Seoul, Korea. The subjects are the patients with chronic illness such as stroke, musculoskeletal disease, hypertension, cancer etc., in average age of 52. 2 years old. The findings of this study are as follows : 1) The limitaion of ADL has been constantly facing to the subjects and has not been changed 4 weeks after being discharged. And the sense of with-drawal was getting worse at 4th weeks than the 1st week after being discharged. 2) The lists of the patient's problems are the impairment of mobility, elimination pattern, inactivity, impairment of skin integrity, ineffective airway clearance, and potential anxiety, self concept deficit, ineffective family coing, etc. Those problems were diminished in quantity at the first week after discharged, but at the 4th week, those problems were getting worse. 3) The need of specialized nursing care such as tube feeding, ostomy care, $O_2$ inhalation, IV therapy, teaching and exercise are considered as the most consisting problems facing to the subjects. 4) In general, the chronically ill patients and their caregivers have not been adapted well even at the 4th weeks after being discharged. 5) Considering those findings, the basic care for patients should be given and the trainging for process of the adaptation after discharged should be encouraged prior being discharged from hostital. For this suggestion, the systematic discharge planning should be carried and the hospital based home nursing model should be implemented at the general hospital for the chronically ill patients.

An Analysis of Nursing Research on Child Rearing in Korea (자녀양육에 대한 국내 간호학분야 연구논문 분석)

  • Purpose: The purpose of this study was to analyze papers on child rearing in Korea and to suggest future research and nursing practice directions for child rearing. Methods: An on-line search via 5 web sites yielded 201 research papers. The analysis of key concepts was based on the attributes of parenting and type of knowledge held by the parents. Results: The research papers found in the search were 27.3% experimental and 72.7% non-experimental. Among the participants, 77.0% of parents raising the child were mothers and the children were mostly young children under the age of one year. Types of knowledge related to child rearing varied widely, but explanatory knowledge (55.2%) was the most common, followed by prescriptive knowledge (30.0%) and descriptive knowledge (14.8%). For attributes of parenting, reaction to parenting (49.9%) followed by parenting behavior/practice (43.1%) was most common. Conclusion: Findings of this study indicate that further research is necessary regarding fathers and surrogate caregivers and on prescriptive knowledge for health promotion of children at different developmental stages. Also the results suggest a need for research toward developing child care policies.

A Study on the Usage Behavior of Elderly Welfare Facilities for the Elderly with Moderate Dementia: Focusing on Satisfaction with Memory School Users (경증치매노인 노인복지시설의 이용행태에 관한 융합연구: 기억학교 이용자만족도를 중심으로)

  • The purpose of this study is to provide basic data to confirm the necessity and effectiveness of the memory school by investigating the perception of users and caregivers about the services and programs provided by the memory school, a social welfare institution for the elderly with minor dementia. For this purpose, 363 students and students in Daegu City were surveyed about the satisfaction and attitude of using memory school. As a result of the analysis, it was found that the use of the memory school is very helpful to the users, and at the same time, it is very meaningful for the decrease of the caregiver burden of the caregiver. Thus, it can be seen that the memory school has the effect of appropriately matching the installation purpose of the facility. In particular, it was found that the degree of satisfaction with the program level, type of service, and future sustainability of the memory school exceeded 90%. Therefore, this study aims to provide policy implications for the social welfare response for the elderly with mild dementia and for the basic data on the memory school for this purpose.

Informational Needs of Postoperative Gastric Cancer Patients (위암 수술 후 환자의 정보 요구)

  • Purpose: Adequate health-related information provided by health professionals may help cancer patients overcome their uncertain situation and manage their healthcare. To provide information effectively, there is a need to understand the content of the patients' essential information. The purpose of this study was to identify recent informational needs of postoperative gastric cancer patients. Materials and Methods: Data were collected from 190 postoperative gastric cancer patients who attended the Stomach Cancer Patients' Day ceremony (18 November 2008) held by the Stomach Cancer Center of Samsung Medical Center with the use of a questionnaire which measured informational needs. A questionnaire with 37 items was comprised of domains of diagnostic tests, cancer therapy, prognosis, follow-up, sexual activity, stomach cancer-related information, and 7 single items. Results: The priorities of informational needs were the domains of prognosis, ways of healthcare during treatment (a single item), follow-up, stomach cancer-related information, cancer therapy, and diagnostic tests (in descending order). Items related to prognosis, diet, and management of their healthcare ranked in the top 10 informational need scores. As age decreased, the degree of informational needs about diagnostic tests, cancer therapy, sexual activity, and stomach cancer-related information increased. Conclusion: We suggest that prognosis-related information based on the accumulated institutional therapeutic outcomes and objective prognosis data should be incorporated in the current education program. Health professionals should provide comprehensible information content to cancer patients and caregivers and encourage patients to participate in their therapy with a more positive attitude.

  • Using the 1998-2004 Health and Retirement Study(HRS), this study explored the determinants of private long-term care insurance(LTCI) ownership and the first home care use. To account for the interaction between LTCI purchase and home care use, this study used two-period utility model as theoretical framework. Discrete time model was used as an empirical model to incorporate the time-dependent feature of LTCI ownership. And this study accounted for the endogeneity of LTCI ownership and home care use by employing full information maximum likelihood estimation. This study indicated insignificant effects of private LTCI ownership and Medicaid eligibility on the home care use. Also, the effects of income and assets on home care utilization were negligible. Those who have poor health condition and who do not have potential informal caregivers were more likely to use home care. For private LTCI ownership, income and assets have positive relationship with LTCI purchase, and poor health status and age were negatively related to LTCI purchase. The elderly living with children and those who have more siblings were less likely to have private LTCI, and those who lived with spouses with no children were more likely to buy private LTCI. Based on the findings, this study provides implications to design long-term care(LTC) policies in the U.S. and to develop LTC planning education programs.

Male Nurses' Experiences of Being Rejected in Nursing Practice (간호행위를 거부당한 남자간호사의 경험)

  • Purpose: The purpose of this study is to examine and share experiences of male nurses who have been rejected during their nursing practice by their patients. Methods: The participants were 12 male nurses who have worked in several hospitals. Data were collected through personalized in-depth interviews. Collected data were analyzed with the content analysis method. Results: The results can be categorized into three main themes. 1. Rejection based on gender stereotypes of nurses' roles. 2. Nurses' reactions when nursing was rejected 3. Reestablishing the role as a nurse. The results of this study showed that male nurses were struggling to maintain their own positions as professional nurses. They were refused by their patients and they experienced a lack of skill and knowledge in nursing practice. They were harmed physically and psychologically from being turned down, and sometimes they had serious conflicts with female nurses. Meanwhile, the male nurses tried to be faithful to their role as professional nurses. Conclusion: The results of this study show the following findings. 1. The male nurses' experiences where mostly ones of understanding and cooperation with patients' caregivers. 2. The need for public relations advertising and systematic support from the media. 3. The need for improving gender equality for nurses. 4. Strengthening male nursing students' endeavors for sound nursing professionalism.

The Effects of Orofacial Exercises Program using Smart Phone on Swallowing Function and Tongue Strength in Acute Stroke Patients with Dysphagia (스마트폰을 사용한 구강안면 운동 프로그램이 삼킴장애가 있는 급성 뇌졸중 환자의 삼킴기능과 혀 근력에 미치는 효과)

  • The aim of this study was investigated the effect of orofacial exercise program using smart phone on swallowing function and tongue strength in acute stroke patients with dysphagia. This study participated in sixteen acute stroke patients with dysphagia. All subjects allocated that randomized each eight patients in experimental and control groups. Subjects of both group received to conventional dysphagia therapy during 30 min/day, 5 times per week, for 4 weeks. Experimental group performed additionally that orofacial exercise program using smart phone, supervised under caregivers, during 30 min/day. The outcome measures were the IOPI(: Iowa Oral Performance Instrument), VR(: Vellecula Residue), PSR(: Pyriform Sinuses Residue), MIP(: Maximum Isometric Pressures). In results of study. Both group showed significant improvements after intervention in all assessments(p<.05). In comparison of change score between two groups, experimental group showed significant improvements than control group in PSR and MIP(p<.05). Orofacial exercise program using smart phone suggested that expected to positive effects the reduction of residue in pharynx and improvements of tongue strength in acute stroke patients with dysphagia.

The Study on Investigation of Advantages and Disadvantages of Acupoint Sticking in Dog-Days and Side Effects of Acupoint Sticking in Dog-Days (삼복첩의 장단점에 대한 인식조사와 삼복첩 부작용에 대한 연구)

  • Objectives The purpose of this study is to understand what caregivers think advantageous and disadvantageous regarding Acupoint sticking Dog-days. Also, to know whether Acupoint sticking in Dog-days affect vital signs, and to investigate if occurrence of side effects are related to application duration, sweat, or compliance to precautionary measures. Methods Survey has been handed out to parents of the children who took Acupoint sticking in Dog-days. 10 days after the first dose was administered, survey was performed to examine any side effect occurrences, characteristics of sweat, duration of application, parents' thought about this therapy. Also, vital signs were checked before the first dose, and 10 days after to see any differences in vital signs in regards to this therapy. Results Preferable opinions about Acupoint sticking in Dog-days are 'convenient than herbal medications' (76.5%), 'less reluctance of child' (47.1%), 'reasonable price' (20.6%), 'good effectiveness' (17.6%), 'less side effect' (14.7%), 'no worry about agrochemicals or heavy metals uptake' (11.8%). Downsides were 'difficult to admit to hospital right on the Dog-days' (73.5%), 'no conspicuous effectiveness' (11.8%), 'expensive' (5.9%), 'reluctance of child' (2.9%). Vital sign differences were minimal when examined before and after Acupoint sticking in Dog-days. Side effects occurred 5 times in 34 cases, and they were erythema, itchiness, and hot sensation. There were no scars or blisters reported. All 5 cases were resolved without any interventions within 3 days. Conclusions Acupoint sticking in Dog-days doesn't affect vital signs. Severe side effect is rare, reported side effects were relatively mild, suggesting that it may be safe and well-tolerated. 'Convenience' (76.5%) is biggest advantage of Acupoint sticking in Dog-days and 'less reluctance of child' is also big advantage of Acupoint sticking in Dog-days. 'Difficulty to admit to hospital right on the Dog-days' (73.5%) is biggest disadvantage of Acupoint sticking in Dog-days.
