노인요양병원 입원실의 실내디자인 특성에 대한 사용자 평가 - 간호사와 간병인 및 보호자를 대상으로 - (Users' Evaluation of Interior Design Features of Patients Rooms in Geriatric Hospital - From the perspectives of Nurses and Care-Givers -)

  • Demands for geriatric hospital have increased in an era of rapidly aging population. Most of aged patients tend to stay in institutions for long terms. This means that the patient rooms of geriatric hospital should be given different considerations from those of normal hospital in designing interior features. They should be a homelike places for the aged patients and designed to take care of specific needs of the aged. However, most of geriatric hospitals are designed with little attention to such point. They appear almost same to normal ones. This study attempts to examine how users evaluate patients' rooms. The users are nurses, care-givers and family members of aged patients in six geriatric hospitals in Busan. They rated 12 features of patient rooms from 0 point to 100 points and described reasons why they rated in that way. Also, the walk-through was done for these six hospitals. 12 features are sizes of patient rooms, sizes and fixtures of bathrooms, sizes and locations of windows, bed layout, numbers and types of chairs, sizes and types of closet, lighting, color scheme, finishes of floor and wall, and interior design tone. Followings are findings : The users evaluated patients' rooms relatively positive. However, extra chairs for visitors, closet in patients rooms and storage in bathroom, and sizes of patients rooms and bathrooms were evaluated relatively negative.

노인장기요양보험제도의 현금급여 도입 필요성 - WTA를 통한 적정 현금급여액 추정 - (Estimating the Willingness-To-Accept for Cash Benefit of Long-Term Care Insurance)

  • 본 연구는 일차적으로 노인장기요양보험 급여방식 중 현물급여와 현금급여의 선택에 있어서 비공식 가족수발자들의 현금급여 선호도를 알아보고, 이에 영향을 미치는 요인을 파악함으로써 현금급여 도입의 필요성을 인식하는데 목적이 있다. 이차적으로는 조건부 가치측정법을 이용하여 비공식적 가족수발에 대한 적정 현금급여액을 추정함으로써 미래 현금급여 도입 시 수가산정을 위한 기초 자료로 제시하고자 한다. 본 연구를 위한 조사는 서울시 소재 3차 의료기관 3곳에서 현재 환자를 돌보고 있는 환자의 보호자 300명을 대상으로 하였다. 경증과 중증 두 가지 유형의 가상의 시나리오 및 구조화된 설문지를 제시한 후 현금급여를 수용할 최소한의 보상금액(Willingness-To-Accept, WTA)을 이중경계 양분선택형 질문으로 구하였으며, 일대일 직접 면접법을 이용하였다. 연구결과, 경증노인에 대해서는 52.7%가, 중증노인에 대해서는 26.0%만이 현금급여 서비스를 선호하였다. 경증노인에 대한 적정 현금급여액은 월 평균 62.8만원, 중증 노인에 대한 적정 현금급여액은 월 평균 107.2만원인 것으로 나타났다. 본 연구는 비공식적 수발자가 직면하는 각기 다른 상황에 대한 실제 현금급여 선호도를 확인함으로써 현금급여 도입의 필요성을 인식하였으며, 잠재적 가족수발자들의 시간손실 보상액으로서 수발자가 받아들일 수 있는 주관적 수발비용을 적정 현금급여액으로 추정함으로써 현행 특별현금급여액의 적정성을 평가하는데 보다 현실적인 기준을 마련하였다는데 함의가 있다고 하겠다.

생활시설 종사자가 경험한 남성노인의 성적욕구 표출에 관한 연구 (A Study on Sexual Desire Outburst of the Male Elderly Residents Experienced by the Workers in Living Facilities)

  • This study has been performed to explore verbal and behavioral expression of sexual desires among male elderly residents who have been living in long tenn care facilities. There are three topics covered in this study; first, in what situations and how seriously do care workers encounter expression of sexual desires of the elderly residents? Second, what kind of negative consequences do they believe those sexual behaviors will lead to? Third, how can we implement defensive measures against the sexual behaviors? In this study, twenty three care workers working full time in five retirement and care facilities were asked about their experience and perception of the above study agendas, and answers of the qualitative interview were drawn as follow; first, the elderly residents apparently show a variety of sexual harassment and provoking behaviors such as sticking to specific women, physically touching and attacking, and induce obscenely activities against female residents, care workers, and volunteer visitors. Second, their sexual behaviors are often influenced by their isolated and abandoned emotionality as well as living situation in rural areas. Third, their sexual behaviors often critically affect care work plan and facility managements by severely discouraging female care givers and community supporters. Therefore in this study, suggestions and defensive measures were made as follows: first, education and counselling programs toward female workers and volunteers need to be developed, and the programs should cover psychological and behavioral mechanism of sexuality in later life. Second, self control plans need to be empowered toward the elderly residents; in the plans, the elderly residents shall be encouraged to evaluate primary cause and proper solutions of sexual behaviors of their peering residents. Third, combination of healthy housing and care facilities for frail elderly need to be integrated in a neighboring location, so that when residents and workers encounter extreme episodes of sexuality of healthy residents in a housing facility, the problematic sexual residents are partially transferred into a neighboring care facility and thereafter other residents and cafe givers are relieved from stressful contacts with the extremely sexual residents.

헤테로토피아로서의 이주 공간: 텔아비브 필리핀 노인돌봄노동자들의 '주말아파트'를 중심으로 (Constructing a Heterotopia of Migrant Space: 'Weekend Flat' of Filipino Migrant Care-givers in Tel Aviv, Israel)

  • 본 논문은 초국적 이주노동자들이 거주국 내에서 수행하는 다층적인 사회적 관계와 공간적 실천을 살펴봄으로써 '차이의 공간'이 생산되는 과정을 고찰하고, 이러한 이질적인 공간을 단순히 외부 세계로부터 분리되고 타자화된 공간이 아니라 이주자들의 일상적 경험이 외부 세계와 상호작용하는 과정에서 새로운 의미와 질서가 재생산되는 헤테로토피아 공간으로 분석한다. 필자는 입주돌봄노동자로서 이스라엘에 체류하는 필리핀 이주자들의 일상 리듬이 주중-작업장, 주말-아파트로 패턴화된다는 사실에 주목하고, 이들이 주말마다 점유하는 텔아비브 네베셰아난의 '아파트'에 분석의 초점을 둔다. 이러한 주말아파트는 비록 주말에만 일시적으로 존재하는 공간이지만 필리핀 이주민사회를 구성하는 최소한의 사회적 단위인 동시에 경제, 사회, 종교적 활동의 핵심 장소로서 기능하며, '위험하고 적대적인 외부 세계'로부터 분리된 '안식처(shelter)'로 기능함으로써 거주국에서 배제되고 주변화된 이주노동자들이 "그들만의 세계"를 구축하는 것을 가능케 한다. 이 논문에서 이주노동자들의 일상적 실천을 통해 유의미한 공간으로 전환되는 '주말아파트'는 이주노동자 삶의 역동성을 이해하고 지배집단의 통제 속에서도 능동적으로 공간을 전유하는 이주자들의 적극적인 행위성을 드러내는 데 유용한 통로로 분석된다. 주말아파트는 이주자들의 필요를 충족시켜주는 다양한 기능을 가진 독특한 공간으로서 다른 공간들과 경계가 뚜렷하고 철저하게 분리된 공간으로 나타나지만, 그것을 둘러싼 외부 공간과의 관계 속에서 형성되는 헤테로토피아 공간으로 해석될 수 있다.

  • Government came to enforce long-term care insurance system in preparation for the rapid aging society. Whether this system is successful or not depends on the professionalism of long-term care-givers who are professional population in charge of care service. Currently in the early stage of enforcement, such problems as a race cutting fee resulted from numerical increase of educational facilities, insolvent operation, degradation of education level resulted from unprofessional instructor, are pointed out. As a mean of manpower-training on long-term care insurance system, this study is to research public-private-university network model of the Academy of Continuing Education attached to Ko Sin University which is the case of Yong Do Gu Busan city. Networking between the vision and development strategy of Yong Do Gu on continuing education city, education system on community manpower-training supported by Ko Sin University, and service field of welfare for the elderly can not only contribute to the professionalism of long-term care-givers but also play an ideal role in manpower-utilization and arrangement of community. Through this networking, high quality of education level and circumstance, using the existing infra, manpower-training and utilization for continuing education of Yong Do Gu can be accomplished. Additionally, the connection with facilities related with welfare for the elderly can contribute to professionalism and accountability of manpower-networking.

영적간호의 개념분석 (Concept Analysis of Spiritual Care)

  • Purpose: The purpose of this study was to identify attributes of the concept of spiritual care. Method: Walker & Avant's concept analysis framework was employed to review the clinical guidelines, nursing text books, and nursing research articles which were related to spiritual care and published from 1985 to 2005. Result: The attributes of the concept of spiritual care were a three stage process such as spiritual assessment, spiritual intervention, and spiritual evaluation. Spiritual care included three dimensions of relationships such as transpersonal, interpersonal, and intrapersonal. The quality of spiritual care was dependent upon characteristics of care-givers such as perception and knowledge of spiritual care, and the clinical environment. The antecedents of spiritual care was spiritual needs due to the prompt events. The consequence of spiritual care was spiritual well-being. Conclusion: This concept analysis of spiritual care contributed to promote performance of spiritual care in clinical fields by removing conceptual ambiguity and confirming the true meaning of spiritual care.

요양보호사의 직장폭력 경험실태 (Workplace Violence Experiences among Care Helpers)

  • Purpose: This is a descriptive research that reports incidents of workplace violence among care helpers. Methods: The subjects were 181 care helpers from elderly care facilities and domiciliary elderly welfare centers in Daejeon. Data were collected through self-administered questionnaires from July to November 2011. Subjects were asked to report incidents of violence within the previous six months. Data analysis included one-way ANOVA, and logistic regression analysis. Results: Forty-seven percent of care helpers reported verbal violence, 16% reported being physically threatened, more than 21 % reported sustaining a physical injury with 2% reporting severe physical injuries. Further, 18.8% of the care givers reported being sexually harassed by client. There were differences in reported workplace violence based on service types, service hours and whether there were policies about workplace violence. Conclusion: To prevent workplace violence for carehelpers, it is necessary to make a policy for preventing violence and develop a violence prevention program to meet service characteristics of facility-based and domiciliary care helpers.
