Study of Smoking Booth Design for the Treatment of Hazardous Pollutants (유해오염물질 처리를 위한 흡연부스의 설계)

  • The purpose of this study was to develop a Eco smoking booth that can effectively reduce hazardous pollutants generated during smoking and evaluate the efficiency and effectiveness of removing hazardous pollutants. The design and manufacture of an eco-friendly automatic smoking booth equipped with deodorizing facilities, such as inlet - HEPA filter - electrostatic precipitator (EP) - impregnated activated carbon - exhaust port, etc., and the efficiency of removing hazardous pollutants from inside and outside was measured and evaluated. The complex odor removal efficiency was 95.37% inside the smoking booth, and 97.38% at the exit of the preventive facility. The carbon monoxide removal efficiency was 94.25% in the inside and 98.32% in the outlet. In addition, the removal efficiency of particulate matter, (PM1, PM2.5, and PM10) inside the smoking booth was 98.59%, and 98.85% at the outlet. The total volatile organic compounds (TVOCs) decreased from $26,000{\mu}g/m^3$ to $5,203{\mu}g/m^3$ in the smoking booth, resulting in 79.99% removal efficiency. After the ventilator was operated, the measured effluent concentration was $5,019{\mu}g/m^3$, and the removal efficiency was 80.70%. Therefore, the smoking booth designed and manufactured through this study can be applied to the removal of harmful pollutants even in the small working environment in the future.

Difference in Management Between Native Koreans and Foreigners with Penetrating Wounds In the Emergency Room (관통상으로 응급실을 내원한 내국인과 외국인 환자의 진료의 차이)

  • Purpose: With the increasing numbers of foreign residents in Korea, the need for an emergency medical care system for foreign patients seems to be growing. Sometimes, a foreigner admitted to an emergency room is not treated sufficiently due to the absence of insurance, facility in the Korean language, and a guardian. The management of a foreigner with trauma in the ER is difficult due to various problems such as social and economic status. The purpose of this study was to investigate the current management status of foreigners with penetrating wounds in the emergency room. Methods: This study is an analysis of 580 patients that were diagnosed with penetrating wounds in one teaching Hospital from Jan. 1, 2008 to Dec. 31, 2008. We analyzed results according to nationality, alcohol ingestion, intentional or accidental trauma, trauma mechanism, injury severity, management time in the ER, and outcome in the ER. Results: Of the total 580 patients, 486 patients (83.8%) were native Koreans and 94 patients (16.2%) were foreigners. According to the Revised Trauma Score, the average score of native Korean patients was 7.808, and the average score of foreign patients was 7.638. Of native Korean patients, 22.6% had knife wounds while 38.3% of foreign patients did. Of native Korean patients, 17.3% experienced intentional trauma while 33.0% of the foreign patients did. Of native Korean patients, 22.5% had ingested alcohol while 49.4% of the foreigners had. Of native Korean patients, 10.5% were admitted while 7.6% of the foreign patients were. Of native Korean patients, 14.2% were discharged against medical advice (DAMA), while 18.5% of foreign patients were. Of native Korean patients, 1.2% ran away while 8.7% of the foreign patients did. Conclusion: Stabbing was the most common cause of penetrating wounds in foreigner patients in this study. Intentional trauma was more common in foreigners with penetrating wounds than in native Koreans. The severity was higher in foreigners with penetrating wounds than it was in native Koreans, and patients who ran away or were discharged against medical advice were more commonly foreigners with penetrating wounds. Social insurance or policy is needed for the management of foreigners with penetrating wounds.

Effect of Horticultural Therapy on the Stress and Serum Cortisol of Demented Elders (원예치료가 치매노인의 스트레스 및 혈중 코티졸 호르몬에 미치는 영향)

  • This study was conducted in order to examine the effects of horticultural therapy on stress in demented elders, and for this purpose, we analyzed stress-related physiological changes in demented elders under the care of an elderly facility who had not taken medication and consented to blood tests. The horticultural therapy program included only flower decoration, which was preferred by the participants, aiming ultimately to reduce stress. First, a one-to-one session was given for forming rapport and the horticultural therapist had many conversations about plants with the elders. Furthermore, the participants were induced to be absorbed into the horticultural activities, though a short while, so that they might forget troubles. Evaluation was made by measuring subjective stress and the blood cortisol hormone concentration before and after the horticultural therapy and examining differences between them. Subjective stress increased from 12.88 to 17.88 in the control group, but decreased significantly from 13.88 before the therapy to 6.38 after in the experimental group (p = 0.007). When the blood cortisol hormone level was measured as a physiological indicator of stress, it decreased significantly from $11.33{\mu}g{\cdot}L^{-1}$ before the horticultural therapy to $9.85{\mu}g{\cdot}L^{-1}$ after (p = 0.037). These results suggest that horticultural therapy reduces the blood cortisol hormone level and is effective in easing stress.

A Study on the Convergence of Spatial Equity of Medical Welfare Facilities for Older Persons and Services (노인의료복지시설과 서비스의 공간적 형평성 융합 연구)

  • The study aims to measure and analysis the spatial equity of Medical welfare facilities for older persons and services, and, based on this, to seek the plan to secure the fairness. To this end, the research was carried out by converging the studies of geography and regional development for the equity of social welfare studies and space arrangement on types and functions of Medical welfare facilities for older persons. The main results of the study showed that, first, in case of the spatial arrangement(desire-to-service), Medical welfare facilities for older persons are located in all areas of cities(Si) and counties(Gun) but mostly existing in cities. Second, in case of the equity of regional distribution of Medical welfare facilities for older persons, it can say the equity in Gun is higher than Si, comparing the regions of Si and Gun. Third, in case of spatial equity of sanatorium for older persons, the spatial equity of care facilities for older persons showed statistical difference depending on the time required to reach the facility, but no difference on distance. This study made various suggestions based on the results of the above research, and suggested the necessity of convergence studies grafting technologies such as AI and the Internet of Things.

Validation of the Scale of Attitudes towards Disabled Person(SADP and ATDP-O) in Korea (장애인에 대한 태도 측정도구(SADP and ATDP-O)의 타당도 검증)

  • The purpose of the present study was to develop a Korean version of the scale of attitudes for disabled Persons and attitudes towards disabled person-original by translating and modifying the scale proposed by Antonak(1982). In order to test validation, this study was focused on processing survey and statistic data analysis and tried to find a way to applicate in Korea. The data were collected from 500 non-disabled persons divided into two groups, which were consist of target group (public officials, health care providers, teachers, journalists and rehabilitation facility staffs) and general group. Based on the descriptive analysis, exploratory factor analysis and confirmatory factor analysis of the data, validity and reliability were examined. Also to find the statistic significant difference on attitudes towards disabled person between target group and general group, t-test was proceed. The result of the exploratory factor analysis drew 15 items representing 3 factors (Pessimism-Stereotype, Optimism-Human Rights, Derogatory Personality Stereotype). A confirmatory factor analysis results provided the construct validity of this scale as good and reasonable fitness. Cronbach's alpha ranged from .67 to .85 showed internal consistencies of the sub-scales. The subscales were significantly correlated with convergent with DFS(Disability Factor Scale) variables. And the target group had more positive attitudes toward disabled person than general group. It is suggested this scale more suitable for various disability types be elaborated, and get a practical implication on attitudes towards disabled person.

  • The rapid development of the current information and communication brings big changes and progress in the health service delivery system. And it is becoming the worldwide trend increasingly. As the name of medical information, by more rapid, detail and more quickly to the patients and diagnosis of the disease it provides not only a high level of health care services but also hospitals and related institutions are making increase the efficiency of the work. Among them, the Telemedicine, that system has many advantage which can expect the shorten the waiting time and the uniform high level of medical, etc. without visiting medical institutions. Especially, the most advantage is it can increase the accessibility of information about extensive medical, without regard to the time and place. But this is the reality, which compared speed the development of modern science and technology with lack of operational regulations and mindset. Current in our Medical Law, it regulates the Telemedicine, but it has Institutional, facility, and environmental constraints. Because, there is no detailed legal relationship. And it takes that in terms of a special form called by a non-face-to-face contact with medical practice rather than the scene. Therefore, in this paper will find a way out to activate the Telemedicine by presupposes the development potential is infinite and find the legal issues and improvements.

A Field Study on Managing System of Maternity Clinic at Public Health Centers in Seoul (서울시 보건소 모성실 운영실태에 관한 현장 연구)

  • The study is to grasp the problems related to operation of Maternity clinic of public health centers in seoul and needs for public health of community in relation to consumers and providers in order to improve efficiency of community public health for mothers and children. Four pregnancy woman, who receive medical care at the maternity clinic of M public health centers in seoul and understand the purpose of this study, and one nurse who works at the were the objects of this field study. Participating observation and intensive interviews were conducted to collect data. All of them were performed as necessary from time to time since December, 1994, and not during a specific period. Through an data analysis in the order of sector analysis and classification analysis, the data were classified into specific patterns and the results are the following; 1. All of the subjects were using both private hospitals and public clinics, but managing activities prior to delivery were not carried out in accordence with theories for those activities. 2. The subjects showed two types of response to utilizing maternity clinic. they answered that the advantages of the clinic were 'short waiting time for medical treatment', 'medical treatment by female doctors' and 'economical benefit.' Meanwhile, they gave negative response to the problems of 'non-implementation of delivery' 'uncleanness and insufficient facilities', 'limited time of treatment', 'lack of expertise' and 'want of public health education for materity.' 3. Problems related to operation of maternity clinic were 'lack of experts', 'irrational facility structure' and 'absolutely lack budget'. In terms of the status of managing the subjects, 'programs only aimed at attaining the central-government-assigned objects' and 'limited management before and after delivery by non-implementing delivery' were pointed out to be problems. Regarding public health education before delivery and PR relations, 'superficial public health education for maternity' and 'absence of PR programs' were named. In planning and evaluation, 'absence of autonomous planning and evaluation by the clinic itself' was a major problem in operating the clinic. 4. 'Substantial health education and PR', 'supplementation of facilities and eqipment', 'development' and supply of demanded service by the subjects', 'implementation of autonomous programs', and 'reinforcement of supplementary education' were presented as alternatives for efficient opration of maternity clinics.

Effects of the Health Management Importance Awareness on Occupational Performance and Basic Fitness for the People with Intellectual Disabilities Participated in the Muscle Strengthening Exercise (건강관리 중요도 인식이 시설 거주 지적장애인들의 작업수행과 기초체력에 미치는 영향)

  • This study performed to provide the basic information about the effects of the health management importance awareness on the occupational performance and basic fitness for the people with intellectual disabilities. 16 people with intellectual disabilities participated in muscle strengthening exercise. To assess health management importance awareness, the Canadian Occupational Performance Measure was used to identify the importance of the health management. According to the results of the importance, they were divided into two groups: Extreme and Neutral Importance Group in order to analyze the changes of the occupational performance and basic fitness. In-group analysis of the performance, satisfaction level, strength, and flexibility found that all measures improved in both groups following completion of the program. Between-group analysis following completion of the program indicated group differences in the performance and satisfaction level, but no group differences were found in the muscle strength and flexibility. Thus, this study showed the positive effects from the results of the program participation targeting the people with intellectual disabilities, and highlighted the importance of the awareness for them regarding the need for the health management (as evidenced by the improvement in occupational performance). It is expected that effective health management through participation in programs such as this will help maintain a healthy lifestyle and contribute to improve the quality of life.

A Qualitative Study on Coping strategies of Older adults with Depression: Focused on the Experience of Coping with Depression in Older Adults Living in Seoul (노인의 우울 대처 전략에 관한 질적 연구: 서울시 거주 도시 노인의 우울 대처 경험을 중심으로)

  • The purpose of this study is to explore qualitatively what kind of coping strategy the Korean older adults use when they are depressed and why. Participants were users, older than 60, of an elderly welfare center in Seoul, and answers of 34 respondents who experienced depression were included in the analysis. The collected data were analyzed through content analysis. The results of the analysis showed that strategies to cope with depression used by participants were composed of 6 domains and 11 sub-domains: health behavior (medical approach / exercise and diet), family and social contact (social interaction / going out and going on a trip / communication with family), Religious activities (Religious activities), lifelong education (hobbies / educational activities), productive activities (labor), health risk behaviors (drinking and gambling / resignation). The depression coping strategies of the older adults and their characteristics are as follows. First, older adults used diverse problem-focused coping strategies to cope with depression. Second, older adults considered various coping strategies together and used them simultaneously. Third, the facility space for older adults functions as a shelter. Fourth, although there were cases where medical approach was used, the intention to reuse was very low. Through this study, the following suggestions were made to help older adults cope with depression more successfully. First, access to various coping strategies should be promoted. Second, more places where older adults can spend their time comfortably need to be provided. Third, resistance to mental health care should be resolved.

Clinical Microscopy: Performance, Maintenance and Laser Safety (임상에서의 현미경: 작동, 유지보수 및 레이저 안전)

  • A microscope is the fundamental research and diagnostic apparatus for clinical investigation of signaling transduction, morphological changes and physiological tracking of cells and intact tissues from patients in the biomedical laboratory science. Proper use, care and maintenance of microscope with comprehensive understanding in mechanism are fully requested for reliable image data and accurate interpretation for diagnosis in the clinical laboratory. The standard operating procedure (SOP) for light microscopes includes performance procedure, brief information of all mechanical parts of microscopes with systematic troubleshooting mechanism depending on the laboratory capacity. Maintenance program encompasses cleaning objective, ocular lenses and inner optics; replacement and calibration of light source; XY sample stage management; point spread function (PSF) measurement for confocal laser scanning microscope (CLSM); quality control (QC) program in fluorescent microscopy; and systematic troubleshooting. Laser safety is one of the concern for medical technologists engaged in CLSM laboratory. Laser safety guideline based on the laser classification and risk level, and advisory lab wear for CLSM users are also expatiated in this overview. Since acquired image data presents a wide range of information at the moment of acquisition, well-maintained microscopes with proper microscopic maintenance program are impulsive for its interpretation and diagnosis in the clinical laboratory.
