갑상설관낭에 의한 상대정맥증후군 -1례 보고- (Superior Vena Cava Syndrome Bue to Thyroglossal Duct Cyst)

  • This is a case report of superior vena cava syndrome due to thyroglossal duct cyst, which was surgically treated. The patient was 61 year old male who progressively complained exertional dyspnea for about 5 months and right ptosis, facial flushing, and nasal stuffiness for about 1 month before admission. The CT scan of the thorax revealed the right paratracheal cystic mass that compressed and displaced the trachea to leftward and SVC to rightward. The resection of the cystic mass was performed through the right posterolateral thoracotomy. The pathologic result was compatible with thyroglossal duct cyst. The postoperative status of the patient was uneventful.

Esophageal Cancer Staging

  • Accurate staging of esophageal cancer is very important to achieving optimal treatment outcomes. The AJCC (American Joint Committee on Cancer) first published TNM esophageal cancer staging recommendations in the first edition of their staging manual in 1977. Thereafter, the staging of esophageal cancer was changed many times over the years. This article reviews the current status of staging of esophageal cancer.

다발성 늑골골절에 의한 동요흉에서 늑골고정판을 사용한 늑골고정술 (Surgical immobilization using Judet`d strut for flail chest with multiple rib fractures)

  • We experienced 2 cases of surgical immobilization with Judet`s strut for flail chest with multiple rib fractures. The patients were undertaken explorative thoracotomy for unevacuated hematoma and chest wall fixation for paradoxically moving segments of ribs after initial trials of internal fixation, i.e. mechanical ventilation with endotracheal intubation. Immediately after operation, the patient`s general condition and respiratory status were improved remarkably. Mechanical ventilation and endotracheal intubtion were removed on postoperative second day without any events.

좌심실 부전증에서의 IABP 치험 - 5예 보고- (Intra-Aortic Balloon Pump in the Left Heart Failure)

  • From July 1986 to June 1987, five patients were underwent IABP [intra aortic balloon pulsation] because of sever left heart failure in spite of maximum medication. These patients were reviewed as prophylactic IABP [1 patient], During operation [3 patients] and postoperative IABP [1 patients]. All patients were showed stable hemodynamic status with improved LV function during and after IABP. there was no IABP related complication or mortality. Advanced disease needs more effective methods of mechanical circulatory assistance and heart replacement.

경피적 관상동맥 성형술후의 응급 관상동맥 우회술 (Emergency Surgery after Failed Percutaneous Transluminal Coronary Angioplasty)

  • From Octorber 1984 through September 1991, 480 patients underwent percutaneous tra-nsluminal coronary angioplasty and 7 of these[1.4%] required coronary artery bypass surgeries in the emergency base. The principal indications for surgery were coronary dis-sections[n=2], acute coronary occlusions[n=3], and ventricular arrhythmias[n=2]. There were two early deaths and one late death. No patient who developed cardiac arrest and had not been resuscitated before surgery survived the operation. The outcome of surgery was related to the preoperative clinical status of patients.

Modified Fontan 술식의 치험 3례 (Modified Fontan Operation [Report of 3 Cases])

  • Since the first report on a successful method of correcting tricuspid atresia by Fontan and Baudet in 1971, several alternative surgical techniques have been described. Bjrk`s modification was performed three patients, 3, 8, and 9 years of age, in our Hanyang University Hospital. The diagnoses were tricuspid atresia type lb, tricuspid atresia type lib and d-TGA with huge VSD and valvular PS. None had previous palliative procedures. The patients were operated on with good results. The CVP after operation was 10-14 mmHg and early extubation was done. Postoperatively, none had peripheral edema or ascites. Postoperatively, all three were acyanotic and had improved physical capacity compared with their preoperative status.

Allocation of Donor Lungs in Korea

  • The expansion of indications for lung transplantation, the growth of the waiting list, and donor shortages are increasing the waiting list mortality rate in Korea. The current lung allocation system in Korea is based mainly on urgency, but outcomes should also be considered to avoid futile transplantation. This review describes the current status of, and issues with, the lung allocation system in Korea including donors, the waiting list, and transplant outcomes in the context of an aging society, in which the frequency of end-stage pulmonary disease is increasing.

Health Empowerment of Older Adults with High-risk of Cardio-cerebrovascular Diseases

  • Purpose: This study was to identify factors explaining health empowerment of older adults with high-risk of cardio-cerebrovascular diseases. Methods: Using a structured questionnaire, data were collected from 148 older adults residing in two areas who have one or more of the following diseases; hypertension, diabetes mellitus, hyperlipidemia, and atherosclerosis. Data were analyzed using descriptive statistics, independent t-test, analysis of variance (ANOVA), Pearson correlation analysis, and hierarchical multiple regression. Results: Health empowerment had a positive correlation with health literacy and social support. Perceived health status and social activity participation showed significant contribution in explaining health empowerment. Conclusion: This study showed that enhancing perceived health status and social activity participation are vital in an effort to improve health empowerment of the population. Main findings of this study can contribute to developing health empowerment interventions to improve health behavior practices of the older adults.

성인의 선천성 식도 기관지루 -수술치험 1례 보고- (Congenital Esophgo-bronchial Fistula without Esophageal Atresia in the Adult -A Case Report-)

  • Congenital esophago-bronchial fistula without atresia is very rare and usually demonstrate more insidious clinical effects, and patients may reach adult life before the diagnosis. We had been experienced a typical case of esophago-bronchial fistula without atresia and document the case. A 42-year-old women was admitted to our hospital due to frequent choking symptom and attacks of pneumonia, and patient status was in chronic pulmonary infection status such as most probably bronchiectatic state of right middle & lower lobe including significant destructive changes. We had confirmed esophago-bronchial fistula without atresia with esophagoscopy, esophagogram, and chest CT, and performed esophago-bronchial fistulectomy and right middle & lower lobectomy. Its belongs to type II of Braimbridge`s classification for congenital esophago-bronchial fistula. Postoperation course was smooth and uneventful.
