A Wrist Watch-type Cardiovascular Monitoring System using Concurrent ECG and APW Measurement

  • A wrist watch type wearable cardiovascular monitoring device is proposed for continuous and convenient monitoring of the patient's cardiovascular system. For comprehensive monitoring of the patient's cardiovascular system, the concurrent electrocardiogram (ECG) and arterial pulse wave (APW) sensor front-end are fabricated in $0.18{\mu}m$ CMOS technology. The ECG sensor frontend achieves 84.6-dB CMRR and $2.3-{\mu}Vrms$-input referred noise with $30-{\mu}W$ power consumption. The APW sensor front-end achieves $3.2-V/{\Omega}$ sensitivity with accurate bio-impedance measurement lesser than 1% error, consuming only $984-{\mu}W$. The ECG and APW sensor front-end is combined with power management unit, micro controller unit (MCU), display and Bluetooth transceiver so that concurrently measured ECG and APW can be transmitted into smartphone, showing patient's cardiovascular state in real time. In order to verify operation of the cardiovascular monitoring system, cardiovascular indicator is extracted from the healthy volunteer. As a result, 5.74 m/second-pulse wave velocity (PWV), 79.1 beats/minute-heart rate (HR) and positive slope of b-d peak-accelerated arterial pulse wave (AAPW) are achieved, showing the volunteer's healthy cardiovascular state.

Evaluation of the Diagnostic Performance and Efficacy of Wearable Electrocardiogram Monitoring for Arrhythmia Detection after Cardiac Surgery

  • Background: Postoperative atrial fibrillation (A-fib) is a serious complication of cardiac surgery that is associated with increased mortality and morbidity. Traditional 24-hour Holter monitors have limitations, which have prompted the development of innovative wearable electrocardiogram (ECG) monitoring devices. This study assessed a patch-type wearable ECG device (MobiCARE-MC100) for monitoring A-fib in patients undergoing cardiac surgery and compared it with 24-hour Holter ECG monitoring. Methods: This was a single-center, prospective, investigator-initiated cohort study that included 39 patients who underwent cardiac surgery between July 2021 and June 2022. Patients underwent simultaneous monitoring with both conventional Holter and patchtype ECG devices for 24 hours. The Holter device was then removed, and patch-type monitoring continued for an additional 48 hours, to determine whether extended monitoring provided benefits in the detection of A-fib. Results: This 72-hour ECG monitoring study included 39 patients, with an average age of 62.2 years, comprising 29 men (74.4%) and 10 women (25.6%). In the initial 24 hours, both monitoring techniques identified the same number of paroxysmal A-fib in 7 out of 39 patients. After 24 hours of monitoring, during the additional 48-hour assessment using the patch-type ECG device, an increase in A-fib burden (9%→38%) was observed in 1 patient. Most patients reported no significant discomfort while using the MobiCARE device. Conclusion: In patients who underwent cardiac surgery, the mobiCARE device demonstrated diagnostic accuracy comparable to that of the conventional Holter monitoring system.

Evaluation of Circulating Endothelial Progenitor Cells in Abdominal Aortic Aneurysms after Endovascular Aneurysm Repair

  • Background and Objectives: Circulating endothelial progenitor cells (EPCs) participate in vascular repair and predict cardiovascular outcomes. The aim of this study was to investigate the correlation between EPCs and abdominal aortic aneurysms (AAAs). Methods and Results: Patients (age 67±9.41 years) suffering from AAAs (aortic diameters 58.09±11.24 mm) were prospectively enrolled in this study. All patients received endovascular aneurysm repair (EVAR). Blood samples were taken preoperatively and 14 days after surgery from patients with aortic aneurysms. Samples were also obtained from age-matched control subjects. Circulating EPCs were defined as those cells that were double positive for CD34 and CD309. Rat models of AAA formation were generated by the peri-adventitial elastase application of either saline solution (control; n=10), or porcine pancreatic elastase (PPE; n=14). The aortas were analyzed using an ultrasonic video system and immunohistochemistry. The levels of CD34+/CD309+ cells in the peripheral blood mononuclear cell populations were measured by flow cytometry. The baseline numbers of circulating EPCs (CD34+/CD309+) in the peripheral blood were significantly smaller in AAA patients compared with control subjects. The number of EPCs doubled by the 14th day after EVAR. A total of 78.57% of rats in the PPE group (11/14) formed AAAs (dilation ratio >150%). The numbers of EPCs from defined AAA rats were significantly decreased compared with the control group. Conclusions: EPC levels may be useful for monitoring abdominal aorta aneurysms and rise after EVAR in patients with aortic aneurysms, and might contribute to the rapid endothelialization of vessels.

원격측정장치를 이용한 설치류의 생체신호 측정 및 분석 (Measurement and Analysis of Rodent Biological Signals using Telemetry System)

  • Telemetry techniques of rats have been used for assessing safety pharmacology of drugs and chemicals. Biological signals including blood pressure and heart rate measured under anesthesia were significantly different from those obtained under normal conditions. The stress of restraint in awake animals can also affect the accuracy of physiological evaluation. This paper details the surgery required to allow key cardiovascular parameters to be determined. The telemetric measurement of cardiovascular parameters such as blood pressure, heart rate, electrocardiograph(ECG) established. We carried out the continuous monitoring of cardiovascular parameters using the telemetry system in F344 rats. During the measurement, no significant changes were observed in the heart rate and blood pressure. ECG signals and body temperature were also constant during the measurement of biological signals. With the results of this study, we conclude that this telemetry system can be applied usefully for the assesment of biological parameters in the rats.

  • 심혈관질환은 전 세계 주요 사망 원인으로 다양한 원인에 의해 발생한다. 심혈관질환의 가장 높은 위험인자는 고혈압으로 뚜렷한 증상이 없지만 방치할 경우 여러 합병증을 유발한다. 고혈압을 치료하기 위하여 약물치료와 규칙적 운동을 진행해야한다. 고혈압 환자의 경우 과도한 신체 활동 시 심장에 큰 무리가 발생해 심혈관질환으로 이어질 수 있다. 따라서 적정 강도로 운동을 진행할 수 있는 PTT 계측을 통한 운동 강도 모니터링 시스템이 요구된다. 본 연구에서는 심전도와 맥파 계측을 통해 PTT를 산출하여 운동 강도에 따른 PTT 변화 모니터링 시스템을 구현하였다. 구현된 시스템은 심전도의 R-peak와 맥파의 P-peak를 미분하여 peak간의 시간차를 이용하여 PTT를 산출한다. 운동 강도에 따른 PTT 변화 모니터링을 위하여 달리기 실험을 진행하였다. 실험결과 저강도는 0.313s, 중강도는 0.220s, 고강도는 0.188s의 PTT가 측정되었으며, 운동 강도가 증가함에 따라 PTT는 감소하는 것을 확인하였다.

  • In primary prevention for cardiovascular diseases, there are significant barriers to adherence including freedom from symptoms, long latency for therapeutic benefits, life-long duration of treatment, and need for combined lifestyle changes. However, to implement more systematic approaches, the focus on adherence improvement needs to be shifted away from patient factors to the effects of the treatment team and healthcare system. In addition to conventional educational approaches, more patient-oriented approaches such as patient-centered clinical communication skills, counseling using motivational strategies, decision-making by patient empowerment, and a multi-disciplinary team approach should be developed and implemented. Patients should be involved in a program of self-monitoring, self-management, and active counseling. Because most effective interventions on adherence improvement demand greater resources, the health care system and educational or training system of physicians and healthcare staff need to be supported for systematic improvement.

휴대형 u-Health 모니터링 시스템 개발 (Development of Portable u-Health Monitoring System)

  • 본 연구는 무구속 무자각 지향의 센싱 기술을 통해 취득된 환자 데이터를 지능적으로 처리하고 판단하여 개인 맞춤형 의료 서비스를 제공해주는 모바일 기반 휴대형 u-Health 모니터링 시스템을 개발하고자 한다. 이를 위해 USN 기반의 휴대형 모니터링 unit을 구성하였다. 환자의 몸에 부착하여 생체정보를 검출하는 생체계측센서, 그 센서로부터 정보를 수신하고 모니터서버로 정보를 전송하는 휴대용 무선단말기, 그리고 무선 통신망을 통해 전달받은 데이터를 해석하고 처리하기 위한 모니터서버가 그것이다. 또한, 당뇨 및 심혈관 질환 관련 무구속 무자각 지향의 센싱 기술을 개발하고자 한다.

  • 대량의 심낭삼출액을 제거한 뒤 발생할 수 있는 심한 좌심실 부전에 관한 보고는 많지 않다. 심실 간의 용적 차이, 심장의 갑작스런 확장 그리고 교감신경계의 작용 등이 심실기능 부전의 원인으로 생각될 수 있다. 검상돌기하 심낭창을 통해 심장 압전을 완화한 뒤 심실 기능 부전이 발생한 두 증례를 보고하면서, 특히 심장수술 후 심장 압전이 있는 환자의 경우 혈류역학을 잘 관찰하면서 심낭삼출액을 서서히 제거할 것을 제안한다.

Wearable Intelligent Systems for E-Health

  • Due to the increasingly aging population, there is a rising demand for assistive living technologies for the elderly to ensure their health and well-being. The elderly are mostly chronic patients who require frequent check-ups of multiple vital signs, some of which (e.g., blood pressure and blood glucose) vary greatly according to the daily activities that the elderly are involved in. Therefore, the development of novel wearable intelligent systems to effectively monitor the vital signs continuously over a 24 hour period is in some cases crucial for understanding the progression of chronic symptoms in the elderly. In this paper, recent development of Wearable Intelligent Systems for e-Health (WISEs) is reviewed, including breakthrough technologies and technical challenges that remain to be solved. A novel application of wearable technologies for transient cardiovascular monitoring during water drinking is also reported. In particular, our latest results found that heart rate increased by 9 bpm (P < 0.001) and pulse transit time was reduced by 5 ms (P < 0.001), indicating a possible rise in blood pressure, during swallowing. In addition to monitoring physiological conditions during daily activities, it is anticipated that WISEs will have a number of other potentially viable applications, including the real-time risk prediction of sudden cardiovascular events and deaths.

사물인터넷 기반의 다중채널 생체신호 측정 (Acquisition of Multi-channel Biomedical Signals Based on Internet of Things)

  • Internet of Things(IoT)-devices are now expanding inter-connecting networking technologies to invent healthcare monitoring system especially for assessing physiological conditions of the chronically-ill patients those with cardiovascular diseases. Hence, IoT system is expected to be utilized for home healthcare by dedicating the original usage of IoT devices to collect the biomedical data such as electrocardiogram(ECG) and photoplethysmography(PPG) signal. The aim of this work is to implement health monitoring system by integrating IoT devices with Raspberry-pi components to measure and analyze ECG and the multi-channel PPG signals. The acquired data and fiducial features from our system can be transmitted to mobile devices via wireless networking technology to support the concept of tele-monitoring services based on IoT devices.
