제 2형 당뇨병 환자에서 효능기대증진 프로그램을 적용한 운동요법이 자기효능과 심폐기능, 당대사 및 지질대사에 미치는 영향 (The effects of exercise therapy applied in an efficacy expectation promoting program on self-efficacy, cardiopulmonary function and metabolism in type 2 diabetes mellitus patients)

  • This study was conducted to identify the effects of exercise therapy applied in an efficacy expectation promoting program that was based on Bandura's self efficacy model on self-efficacy, cardiopulmonary function and metabolism in type 2 diabetes mellitus patients. The study design was nonequivalent pre-test post-test control design. 34 type 2 diabetes mellitus patients who received follow-up care regularly through the diabetic out-patient clinic were randomly sampled for this study. Twenty patients were assigned to the experimental group and fourteen patients were assigned to the control group. To the experimental group, exercise therapy applied in an efficacy expectation promoting program that is composed of individualized exercise prescription for 12 weeks was provided. In case of the control group, they were instructed to continue their usual lives. Data collection period was from March 1998 to June 2000 Data were analyzed using SPSS/WINDOW 10.0 program. The results were as follows. In experimental group, The score of self efficacy has increased from 64.20 to 66.65 after exercise therapy applied in an efficacy expectation promoting program and it was statistically significant(t=2.07, p=.04). The anaerobic threshold has increased from $18.20\;m{\ell}$/kg/min to $19.07\;m{\ell}$/kg/min and it was statistically significant(t=2.05, p=.04). Level of fasting blood sugar has decreased from $188.20\;mg/d{\ell}$ to $155.55\;mg/d{\ell}$ after exercise therapy applied in an efficacy expectation promoting program and it was statistically significant.(t=-2.69, p=.01). For the lipid metabolism, percent body fat has decreased from 27.16% to 26.57% after exercise therapy applied in an efficacy expectation promoting program. In conclusion, the exercise therapy applied in an efficacy expectation promoting program showed positive effect of self-efficacy, cardiopulmonary function and glucose and lipid metabolism.

  • 본 연구는 비장애인 휠체어 농구 선수 11명을 대상으로 휠체어트레드밀과 암 에르고미터의 점증부하운동검사 시 심폐기능, 운동자각도 및 젖산 농도를 비교하는데 목적이 있다. 연구방법으로는 휠체어트레드밀과 암 에르고미터의 점증부하운동검사 시 호흡가스분석기와 무선 심박수 측정기를 사용하여 심폐기능을 알아보았으며, 보그 스케일을 이용하여 운동자각도를 측정하였고, 젖산 분석기를 사용하여 안정 시, 운동직후, 회복기 2분, 4분, 6분, 10분의 혈중 젖산 농도를 측정하였다. 측정된 데이터는 SPSS 18.0 프로그램을 통하여 종속 t-검증(paired t-test)을 실시하여 분석하였고 통계적 유의수준은 .05로 설정하였다. 이 연구를 통하여 얻어진 결과는 다음과 같다. 첫째, 심폐기능 변인 중 최대산소섭취량은 암 에르고미터가 높게 나타났으며, 호흡교환율은 휠체어트레드밀이 높게 나타났으며, 최대 심박수는 휠체어트레드밀이 높게 나타났다. 둘째, 주관적 운동 강도에서는 종료 점에서 암 에르고미터에서 높게 나타났다. 셋째, 혈중 젖산 농도는 운동 직후 암 에르고미터에서 높게 나타났다.

흉부압박 피드백 기능이 포함된 기본소생술 앱 개발 (Development of the Basic Life Support App Including Chest Compression Feedback)

  • This study is to develop a basic life support (BLS) app using the android based smartphone and to evaluate the function of the app. Suggested app contains chest compression feedback function, the map of automated external defibrillator (AED), direct emergency call and the basic knowledge of BLS. Using the accelerometer of the smartphone, we implemented a real-time algorithm that estimates the chest compression depth and rate for high quality cardiopulmonary resuscitation (CPR). The accuracy of algorithm was evaluated by manikin experiment. We made contents which were easy to learn the BLS for the layperson and implemented a function that provides the AED location information based on the user's current location. From the manikin experiment, the chest compression depth and rate were no significant differences between the manikin data and the app's feedback data (p > 0.05). Developed BLS app was uploaded on Google Play Store and it was free to download. We expected that this app is useful to learn the BLS for the layperson.

하퇴 절단 환자의 족부 종류에 따른 심폐 기능의 비교 (A Comparison of Cardio-Pulmanary Function by the use of a Single Axis Foot and SACH Foot in Below-Knee Amputation)

  • This study was to measure whether single axis foot or the SACH foot applied to below-knee amputation patients would bring positive changes to the patients' cardiopulmonary function, and help them to overcome their disability. The experiment took place at Asan Medical Center, University of Ulsan, from July 20th to November 20th, 1999 with 10 below-knee amputation patients. The patients were asked to equip single axis foot and SACH foot by turns and lead them to walk on a treadmill which was designed to increase its steep slope. Patients heart rate and blood pressure were recorded before and after their walking, The heart rate, systolic blood pressure, diastolic blood pressure of the patients who were equipped with, either SACH foot or single axis foot, have been increased with as the slant becoming steeper. The heart rates during the experiment did not show relevant changes according to the kind of foot used and the gradient, but the changes occurred before and after the walking(p=0.0001), The similar result of systolic blood pressure was found during the waking(p=0.01). Below-knee amputation patients are expected to walk and perform the routines better, no matter what type of foot is used, as long as they wear an artificial foot properly and taking features of foot product into consideration

몸통 안정화 운동이 단기 금연자의 허파기능, 최대 산소 섭취량, 심장박동수에 미치는 영향 (Effects of trunk stabilization exercise on pulmonary function, maximal oxygen uptake, and heart rate of short-term non-smokers)

  • 본 연구에서는 흡연자와 비흡연자를 대상으로 단기 금연 동안 실시하는 몸통 안정화 운동이 단기 금연자의 허파기능, 최대 산소 섭취량, 심장박동수, 회복 시간에 미치는 영향에 대해 알아보고자 하였다. 본 연구는 충남 소재 N대학교의 20대 남성 26명으로 흡연자 13명과 비흡연자 13명을 선정하였다. 대상자의 신체적 특성 파악 후 사전 측정 시 허파기능(FVC, SVC, MVV)과 최대 산소 섭취량를 측정하였으며 3일 간 심장박동수와 회복 시간을 측정하였다. 측정 후 3일 금연과 함께 몸통 안정화 운동 프로그램을 실시하였다. 사후 측정으로 허파기능과 최대 산소 섭취량을 측정하였고, 당일 운동 후 즉시 1일차, 2일차 3일차의 심장박동수, 회복 시간을 측정하였다. 본 연구의 자료는 SPSS(Statistical Package for the Social Sciences) Ver. 23.0 for windows를 이용하여 분석되었다. 몸통 안정화 운동이 흡연자와 비흡연자의 심장허파기능에 미치는 영향을 분석한 결과, 흡연자 집단과 비흡연자 집단 모두 운동 전후 허파기능(FVC, SVC, MVV), 최대산소섭취량, 회복 시간에 유의한 차이가 나타났다. 이는 몸통 안정화 운동이 짧은 기간이라도 비흡연자뿐만 아니라 단기 금연자의 심장허파기능에 긍정적인 영향을 미칠 수 있는 것으로 생각되며, 금연 활동을 더한 몸통 안정화 운동이 심장허파기능 감소 예방 및 향상을 위한 더욱 안전하고 효과적인 방법이 될 수 있을 것으로 사료된다.

1년간의 복합 운동프로그램이 남성 치매환자의 운동능력과 인지기능에 미치는 영향 (The Effects of One Year Exercise Program on Exercise Capacity & Cognitive Function in Male Patients with Dementia)

  • 1년간의 운동이 남성 치매환자의 운동능력과 인지기능에 미치는 영향을 연구한 결과, 운동군의 경우 심폐지구력, 근력, 근지구력 및 MMSE는 운동전에 비해 운동중과 운동후 유의하게 증가하였고, 평형성과 민철성은 운동후 유의하게 증가하였으나 유연성은 유의한 차이가 없었다. 반면, 통제군은 모든 시기에서 유의한 차이가 나타나지 않았다. 이상의 결과를 볼 때, 1년간의 운동은 남성 치매환자의 운동능력과 인지기능을 향상시켜 환자들에게 자신감과 성취감을 얻을 수 있는 기회를 주었고, 나아가 독립적인 일상생활을 영위하고 삶의 질을 높이는데 크게 이바지 할 것으로 기대된다.

과당음료 투여가 최대운동부하시 운동수행력, 심폐기능, 젖산 및 염증반응에 미치는 영향 (Effects of Fructose-containing Drinks on Cardiopulmonary Function, Lactate Levels, and Inflammatory Markers during Maximal Exercise Testing)

  • 본 연구는 과당 위주의 스포츠음료 투여가 최대운동부하시 심폐기능과 젖산 및 염증반응에 미치는 영향에 대해 비교 분석하기 위해 실시되었다. 연구대상자는 D 대학교 체육학과 학생 8명으로 구성하였으며 실험시기를 2차로 나누어 스포츠 음료 투여 전과 투여 후 시기에 심폐기능과 젖산 및 염증반응 등을 비교 분석하였다. 본 연구의 결과 스포츠 음료 섭취 전과 섭취후 운동시간과 V02max, AT HRmax가 유의하게 증가하였고(p<0.05), 스포츠 음료 투여 후 회복기 20분에서 젖산 농도가 유의하게 감소되었다(p<0.05). 따라서 고강도 최대운동 전 과당위주의 스포츠 음료 투여는 운동지속시간을 늘려주고 VO2max를 높여주며, 운동 후 피로물질인 젖산축적의 빠른회복에 도움을 줄 수 있는 것으로 사료된다. 비록 본 연구에서 염증 반응에 대한 효과를 완벽하게 규명하지는 못했지만 스포츠 음료 투여시 CRP의 농도가 감소하는 경향이 나타나 염증반응 개선에도 어느 정도 영향을 미칠 수 있을 것이라 생각된다. 추후 과당위주의 스포츠 음료투여시 염증 반응에 대한 다양한 운동 강도와 상황에서의 기전적 연구가 수행되어져야 할 것이다.

한복 치마허리 치수가 인체의 압력과 심폐기능에 미치는 영향 (Effect of the Korean Skirt-Band Size on Pressure and Cardiopulmonary Function of Human Body)

  • Korean clothings are admired for their beauty and traditional value. On the other hand, many of Korean women complain of chest restriction. Korean skirts may give high garment pressure to the wearers because the Korean skirt-band do not resolve physical tensions in size or in stretch. This paper aims at the identification of the garment pressure caused by the Korean skirt-band and the cardiopulmonary change caused by garment pressure. The Korean skirt-band were made in 3 kinds of size, and 21 women were selected for wearing test. The garment pressure was measured in front, side, and back parts of the body. The measured cardiopulmonary parameters were vital capacity, respiratory rate, heart rate, and blood pressure. The results were as follows: 1. The smaller the size of the Korean skirt-band, the greater the garment pressure. The pressure during inspiration was significantly greater than the pressure during expiration. 2. The pressure in side part was the greatest of the three measurements and the pressure in back part was the smallest. 3. The small size of the Korean skirt-band revealed low vital capcity and great respiratory rate. 4. As the small size grement was dressed, the respiratory rate and the heart rate during walking and rest after walking were significantly high. 5. Subjects felt uncomfortable when they wore small sized garment.

Respiration Rate and Oxygen Intake by Change of Wheelchair Backrest Angle

  • This study was purposed to provide basic information on the correct application of a wheelchair's backrest angle by investigating the change in cardiopulmonary function according to backrest angle during propulsion. This study examined the effects of the wheelchair's backrest angle on the cardiopulmonary function by varying the angle to $0^{\circ}$, $10^{\circ}$ and $20^{\circ}$ with a propulsion velocity of 60 m/min. The experimental parameters were respiration rate, oxygen consumption rate and oxygen consumption rate/kg which were measured by a portable wireless oxygen consumption meter (COSMED, $K4b^2$). The results of the study were as follows: 1) There were no statistically significant differences in respiration rates due to changes in the wheelchair backrest angle (p>.05). 2) There were statistically significant differences in oxygen consumption rates due to changes in the wheelchair backrest angle (p<.05). 3) There were also statistically significant differences in the oxygen consumption rate/kg due to changes in the wheelchair backrest angle (p<.05). In conclusion, changes in the backrest angle of wheelchairs during propulsion influences oxygen consumption rates and heart rates, while respiration rates are not affected. Therefore, a training program for good seating and posture needs to be provided, and the wheelchair seating system should be equipped with the unadjustable-angle wheelchair to reduce the functional load on the cardiopulmonary system.

Thoracic Epidural Anesthesia and Analgesia (TEA) in Patients with Rib Fractures

  • Background: We analyzed the results of surgical reduction and fixation of ribs under thoracic epidural anesthesia and analgesia (TEA) in patients who had no more than 3 consecutive rib fractures with severe displacement to examine the clinical usefulness of this method. Materials and Methods: From May 2008 to March 2010, 35 patients underwent surgical reduction and fixation of ribs under TEA. We reviewed the indications for this technique, number of fixed ribs, combined surgical procedures for thoracic trauma, intraoperative cardiopulmonary events, postoperative complications, reestablishment of enteral nutrition, and ambulation. Results: The indications of TEA were malunion or nonunion of fractured ribs in 29 (82.9%; first operation) and incompletely ribs under previous general anesthesia in 6 (17.1%; second operation). The average number of fixed ribs per patient was 1.7 (range: 1~3). As a combined operation for thoracic trauma, 17 patients (48.6%) underwent removal of intrathoracic hematomas, and we performed repair of lung parenchyma (2), wedge resection of lung (1) for accompanying lung injury and pericardiostomy (1) for delayed hemopericardium. No patient had any intraoperative cardiopulmonary event nor did any need to switch to general anesthesia. We experienced 3 postoperative complications (8.6%): 2 extrapleural hematomas that spontaneously resolved without treatment and 1 wound infection treated with secondary closure of the wound. All patients reestablished oral feeding immediately after awakening and resumed walking ambulation the day after operation. Conclusion: Thoracic epidural anesthesia and analgesia (TEA) may positively affect cardiopulmonary function in the perioperative period. Moreover, this technique leads to an earlier return of gastrointestinal function and early ambulation without severe postoperative complications, resulting in a shortened hospital stay and lowered costs.
