A Comparison of Accuracy in Artificial Respiration and Chest Compression Depending on Position, Gender, and Weight of a Victim Given Cardiopulmonary Resuscitation (심폐소생술 시행 시 피구조자의 위치, 성별, 체중에 따른 가슴압박과 인공호흡 정확도 비교)

  • The purpose of this study was to increase accuracy in performing cardiopulmonary resuscitation through comparing accuracy between chest compression and artificial respiration depending on position, gender, and weight of a victim given conducting cardiopulmonary resuscitation. This study randomly sampled 72 college students in G Province (36 in the experimental group and 36 in the control group) to conduct a research from November 5 to 19, 2009. The collected data were analyzed by using an SPSS WIN 12.0 Version program. Since the results showed that most regions got more effective artificial respiration and chest compression on the right side than on the left side, it is necessary to apply cardiopulmonary resuscitation on the right side in terms of how to implement and instruct cardiopulmonary resuscitation in the future. However, it seems that continuous researches are necessary to see if cardiopulmonary resuscitation is more effective on the right side in terms of the survival rate in an actual emergency.

A Prospective Clinical Study of Crystalloid and Colloid Solutions as Priming Additive Fluids for Cardiopulmonary bypass of the Small Children (소아에서 인공심폐기 충전액의 첨가용액으로서 사용한 crystalloid와 colloid 용액에 관한 임상연구)

  • Searching for the clinical effects of colloid solutions that used to increasing the oncotic pressure of priming solutions at the cardiopulmonary bypass, 29 patients [who were diagnosised as simple VSD around 10kg of body weight and scheduled to be operated from June 1990 to December 1990 at Sejong General Hospital] were divided randomly and prospectively to the two groups: A group [15] was received 4gm% albumin as addition to the priming solutions and B Group [14] the same amount of Ringer`s lactated solution. 34 clinical parameters [Body weight, sex, age, body surface area, Qp/Qs, pulmonary arterial pressure, cardiopulmonary bypass time, anesthetic time, intraoperatively infused crystalloid and colloid amount, hemoglobin, hematocrit, serum sodium concentration, serum osmolarity, urine osmolarity, urine specific gravity, serum concentration, serum osmolarity, urine osmolarity, urine specific gravity, serum protein, serum albumin concentration, urine output, central venous pressure, postoperatively infused colloid amount, immedediate post-operative peak inspiratory pressure, cardiac index, blood pressure and pump flow during cardiopulmonary bypass, inotro-pic assist, diuretics, extubation period, total drain amount, duration of ICU] were measured and compaired between the two groups. There were no differences of preoperative and operative clinical parameters. And postoper-atively, practically there were no nearly differences at the clinical outcomes between the two groups, but some parameters [cardiac index, PIP, BP and pumpflow during CPB, etc] contributed to being preferable to the Group A at certain times [P<0.05]. Conclusively, it might be thought that the priming solution of cardiopulmonary bypass added by colloid solution had some beneficial effects on the patients, especially younger and associated with complex anomaly to be expected taken longer time of cardiopulmonary bypass, and more studies about the neonatal and complex anomaly cases were needed in that points.

A study on the cardiopulmonary resuscitation by the emergency medical dispatcher (구급상황관리사에 의한 심폐소생술 안내 실태 연구)

  • Purpose: The purpose of the study is to check up the status of 119 emergency control centers usage. Therefore, the status of use of 119 emergency control centers and the incidence of pre-hospital cardiac arrest patients were investigated. Methods: The emergency activity daily reports and first aid diaries of 119 emergency control centers from January to December 2018 were reviewed. For more accurate status analysis, Among the first aid guidance received in the emergency rescue standard system, the cardiopulmonary resuscitation guide log was reviewed. Results: In 2018, the total usage of the 119 emergency control centers was 1,358,356 calls, hospital guidance werethe most commom (n=629,676, 46.4%), followed by first aid (n=428,027, 31.5%), disease consultation (n=170,238, 12.5%), medical oversight (n=111,188, 8.2%), and interhospital transfer (n=5,052, 0.4%). Regarding the user number per 1,000 persons, Jeju was the greatest at 48.0, whereas Changwon was the lowest at 13.0. A total number of dispatcher-assisted cardiopulmonary resuscitation was 12.181. The time from report to chest compression were 156.2±80.8 seconds for those with previous cardiopulmonary resuscitation training and 168.0±79.3 seconds for those without such training (p<.05). Conclusion: The ratio of first aid instructions, including dispatcher-assisted cardiopulmonary resuscitation, among total usage of the 119 emergency control centers increased. Therefore, additional efforts are required to improve the quality and expertise of information provided through the 119 emergency control centers.

Factors Affecting Knowledge about Cardiopulmonary Resuscitation of Older Persons (노인의 심폐소생술 지식에 영향을 미치는 요인)

  • The purpose of this study is to examine factors affecting knowledge about cardiopulmonary resuscitation of older persons. For the purpose of the study, 2022 Social Survey conducted by Statistics Korea was utilized and 8,862 older persons aged over 65 years old were analyzed. Results of the analyses showed that individual resources and social resources of older persons had significant effects on knowledge about cardiopulmonary resuscitation. Regarding individual resources, a higher level of education, a higher level of income, a higher level of subjective health status, and a higher level of cognitive function increased the level of knowledge about cardiopulmonary resuscitation of older persons. Regarding social resources, having a spouse and a higher level of satisfaction of relationship with children increased the level of knowledge about cardiopulmonary resuscitation of older persons. Based on results of this study, various interventions are needed to improve the level of knowledge about cardiopulmonary resuscitation among vulnerable older groups with regard to the coping knowledge with emergency situation.

Changes of Interleukin-10 level in Patients Undergoing cardiopulmonary Bypass (체외순환에 따른 혈중 Interleukin-10의 변화)

  • Background: Cardiopulmonary bypass during open heart surgery causes systemic inflammatory respose. IL-10 is an anti-inflammatory cytokine that inhibits inflammatory process and protects organ function by down regulation of pro-inflammatory cytokine release and maintenance of blood level balance with pro-inflammatory cytokines. Mateial and Method: Plasma IL-10 levels were measured and analyzed in 22 patients who underwent open heart surgery (11 cases of coronary artery bypass graft, 11 cases of valve replacement) under cardiopulmonary bypass since 1988 January to July at Department of Thoracic and Czardiovascular surgery, Yeungnam University Hospital. 1g of methylprednisolone was administrated to thirteen patients randomly. Blood samp.es were taken and collected at the time of induction of anesthesia, 10 min before cardiopulmonary bypass, 10 min after starting of CPB, 10 min aftr aortic cross clamping, 10 min after ACC release, and 10 min, 2 hours, and `5 hours after CPB respectively. The plasma levels of IL-10 were determined by enzyme-linked immunosorbent assays(ELISA). Wilcoxon-Raule Sum test was used for statistical analysis. Result: In all 22 patients, cardiopulmonary bypass time was used for statistical analysis. Result: In all 22 patients, cardiopulmonary bypass time was 171$\pm$41.4 min and aortic cross clamp time was 118$\pm$36.5 min. Peak IL-10 level was achieved at 10 min after ACC(361.0$\pm$52.81pg/ml) and was decreased sharply at 2 hours after CPB. Peak IL-10 level was correlated positively with aortic cross clamp time(p=0.011); however, it did not correlated with bypass time(p=0.181). In valve replacement group, mean IL-10 level at peak point was 567.89$\pm$107.69 pg/ml and was significantly higher than that of coronary artery bypass group(205.67$\pm$192.70 pg/ml)(p<0.001). ACC time in valve replacement group was significantly longer than that of coronary artery bypass group(p<0.01), however, bypass time was not(p=0.212). Thirteen patients with steroid pretreatment before starting of CPB showed relatively higher plasma IL-10 level than in control group, however, no statistical significance was noted(p=0.19). Conclusion: plasma level of IL-10 was increased in association with cardiopulmonary bypass and revealed peak at 10 min after ACC release. IL-10 level was correlated positively with ACC time. Therefore, systemic inflammatory respeonse in association with cardiopulmonary bypass could be decreased by reducing ACC time during cardiac surgery.

The effectiveness of Cardiopulmonary Resuscitation training in elementary students (일부 초등학생의 심폐소생술교육의 효과)

  • Purpose: With a view to providing basic data to develop cardiopulmonary resuscitation education suitable for elementary students, the cardiopulmonary resuscitation education was conducted to grasp students' knowledge, skills accuracy and the attitude change before and after the education. Methods: Convenience sampling was made on fourth and fifth graders(total-35 students) of S elementary school located in K city, Chungcheongnam-do, and this was a pre-experiment research designed before and after choosing a single group. In terms of methods, specifically we, researchers ; 1) Handed out questionnaires to students directly to make them fill in firsthand and collected the questionnaires. 2) Utilized PPT materials based on 2005 AHA guideline and DVD materials of AHA, to give students theoretical education of cardiopulmonary resuscitation. We used Anne/SkillReporter$^{(R)}$ torso produced by Leardal Inc, and Little Anne to conduct practical education individually. 3) Asked students to give Anne/SkillReporter$^{(R)}$ torso cardiopulmonary resuscitation five times with the ratio of 30 : 2, and then one of researchers filled in the evaluation sheet individually. 4) Evaluated the accuracy of students' ability to perform the resuscitation based on the record of Anne/SkillReporter$^{(R)}$ integrated printer(which was the objective tool to grasp students' skills accuracy). 5) Gave out questionnaires to make students fill them in and then collected them. after completing the practical evaluation. Results: 1) In case of the attitude about cardiopulmonary resuscitation, Students' confidency rose from 19.28%(before the education) to 93.57(after the education)- which is a positive change. 2) As the result of the education, some elementary students scored 11 points (full score-16 points), up from 5 points before the education, in terms of the knowledge about cardiopulmonary resuscitation. The average point also reached 13.14 points(after the education), jump from 8.37(before the education), which was the rise of 29.8%. 3) When it comes to the practical performance, the skills accuracy was 80.93% on average, and the calculation method was as follows: total items were 16, and each item was marked form 0 to 2 points, meaning the full score was 32 points. The minimum score was 19 points and the maximum was 32($M{\pm}SD=25.90{\pm}2.88$), which was calculated based on percentage. 4) Regarding skills accuracy, respiration accuracy(%)($M{\pm}SD=30.20{\pm}27.16$) was higher than pressure accuracy(%) ($M{\pm}SD=15.34{\pm}25.27$). Conclusion: The result showed that students' attitude on cardiopulmonary resuscitation changed positively. and meaningful difference(p = .00) existed in the change of students' knowledge. In terms of skills accuracy. chest compression and airway control showed high accuracy, but the result of Anne/SkillReporter$^{(R)}$ performance showed that the accuracy of chest compression was lower than that of mouth-to-mouth resuscitation.

The Effectiveness of Basic Cardiopulmonary Resuscitation training in female college students (일부 여대생의 기본심폐소생술 교육 효과)

Results : 1) In case of the attitude(confidence) about basic cardiopulmonary resuscitation, students' confidency rose from 14%(before the education) to 55.5%(after the education)- which was a positive change. 2) In case of the attitude(educational necessity) about basic cardiopulmonary resuscitation, students' confidency rose from 94.7%(before the education) to 100%(after the education)- which was a positive change. 3) As a result of the education, some female college students' scored knowledge about basic cardiopulmonary resuscitation. The average point also reached 88.95%(after the education), jump from 63.51%(before the education), which was the rise of 25.44%. 4) Regarding skill accuracy, pressure accuracy(%)($M{\pm}SD=91.37{\pm}14.16$) was higher than respiration accuracy(%)($M{\pm}SD=61.55{\pm}26.13$). Conclusion : The result showed that students' attitude(confidence, on basic cardiopulmonary resuscitation changed positively, and meaningful difference(p= .000) existed in the change of students' knowledge. Anne/Skill Reporter$^{(R)}$ performance showed that the accuracy of mouth-to-mouth resuscitation was lower than that of chest compression.

The Effects of Table Tennis Program on Self Efficacy, Cardiopulmonary Function, Serum Lipids, Catalase Activity in the Physical Disabilities (탁구프로그램이 지체장애인의 자기효능감, 심폐기능, 혈청지질, Catalase 활성도에 미치는 영향)

  • Purpose: This study was conducted to evaluate the effects of table tennis program on self efficacy, cardiopulmonary function, serum lipids, catalase activity in the physical disabilities. Method: Physical disabilities were allocated to one of two groups: control group (n=7), experiment group (n=8). The experiment group took table tennis program four times a week for 12 weeks. Self efficacy was measured by questionnaire. Serum lipid profiles, catalase and cardiopulmonary function were checked after the exercise program and compared with pre-exercise data. Result: Self efficacy was significantly higher in the table tennis group. Maximum oxygen consumption and forced vital capacity were significantly increased and heart rate at rest was decreased in the table tennis group. Total cholesterol and triglyceride were decreased in the table tennis group. There was no significant change in catalase activity between two groups. Conclusion: These results indicate that table tennis program has positive effects on the health of the physical disabilities by improving the self efficacy and cardiopulmonary function and serum cholesterol profile.

A study on knowledge and competence of hospital nurses inperforming cardiopulmonary resuscitation (심폐소생술에 대한 병원 간호사의 지식 및 수행능력에 관한 연구)

  • Purpose: This study was done to identify the level of knowledge and competence of nurses with regard to cardiopulmonary resuscitation. Methods: The participants were 360 nurses who worked in 3 general hospitals affiliated with universities in K-province. The study was done in May and June, 2005. Analysis was done using SPSS 12.0 and descriptive statistics, t test, ANOVA, Pearson’s correlation coefficients, and multiple regression were used to analyze the data. Results: The results showed that nurses’knowledge and competence levels vary. Mean scores for knowledge were 19.64. Using a score of 1, the mean was 0.65. Mean scores for competence were 96.29. Using a score of 5, the mean was 3.57. Correlation between total knowledge and total competence was r=.49 (p=.00). Conclusion: Lack of knowledge and low competence in nurses should be addressed in cardiopulmonary resuscitation education programs. For more effective education, the type of education should not be limited to lectures only, but should include both lectures and practice. As knowledge and competence were affected by work experience, it is important to provide nurses not only with knowledge but with work experience to increase levels of knowledge and competence in cardiopulmonary resuscitation.

Influence of Smokers's Jogging Exercises on Cardiopulmonary Function of Normal Adults in Their Twenties (20대 성인 흡연자의 조깅운동이 심장호흡기능에 미치는 영향)

  • Background and Purpose : Effects of smokers's jogging exercises on oxygen uptake, ventilation, heart rate, and vital-capacity of normal adults have not been studied in details on. In this study, people that had completed the established jogging exercises was selected for the study. We could examine the effects of imposed jogging in smokers who might be considered by an experienced physical therapist to be well practiced and proficient in this exercises. Subjects and Methods : A total number of 24 young healthy subjects participated in a randomized crossover controlled study and completed the study. The subjects of this study were divided into jogging and controlled groups which were composed 14 people respectively. The members of exercise group had jogging exercises 50 minutes a day, five times a week, for 5 weeks in same conditions, otherwise controlled group did not. Results : The results of this study were as follows. The jogging group demonstrated significant increases in cardiopulmonary function, vital-capacity and exercise capacity, compared with control group, which demonstrated no change in baseline measurements of 5 weeks. Conclusion : Jogging exercises can increase cardiopulmonary function in people after they have undergone successful PA. These results suggested that it was improved on cardiopulmonary function and lung capacity, since jogging exercise had. The results of this study could be applied for valuable criterion cardiopulmonary function. It is necessary to selects in various groups of the purpose of enhancing confidence in of this research.
