An Unusual Presentation of Schwannoma in the Interatrial Space

  • We report the case of a 69-year-old woman who was diagnosed with intracardiac schwannoma without symptoms. Preoperative echocardiography and cardiac magnetic resonance imaging showed a mass attached to the interatrial septum. The initial diagnosis was a myxoma or a bronchogenic cyst. The tumor was successfully excised under cardiopulmonary bypass. However, the pathology of the excised tumor was consistent with schwannoma. We suggest that cardiovascular surgeons consider schwannoma to be a possible differential diagnosis for a mass close to the interatrial septum.

  • A primary malignant lymphoma that originates in the heart is extremely rare. A 68-year-old male patient was admitted due to aggravated dyspnea. After echocardiography and chest computed tomography evaluation, a huge mass in the right atrium and the right ventricle was detected. We decided to perform emergency surgery due to a high risk of infarction and hemodynamic disturbance. After the near total removal of the huge mass in the right cardiac chamber, the interatrial septum and antero-lateral part of the right atrium were reconstructed by the use of a bovine pericardial patch. The final pathological diagnosis was a primary malignant lymphoma. The patient and his guardians refused chemotherapy (including radiotherapy), and the patient was discharged to his home, where the prognosis was hopeless.

  • We report a case of invasive pulmonary aspergillosis invading the mediastinum and the left atrium. A 70-year-old woman was hospitalized for dyspnea. She had been well controlled for her diabetes mellitus and hypertension. The chest X-ray disclosed mediastinal widening, and the computed tomography scan of the chest showed that there was a large mediastinal mass and this lesion extended into the left atrium and right bronchus. The cardiac echocardiography showed that a huge mediastinal cystic mass compressed in the right atrium and a hyperechoic polypoid lesion in the left. The pathology from the bronchoscopic biopsy observed abundant fungal hyphae which was stained with periodic acid-Schiff and Gomori's methenamine silver. Despite the treatment with antifungal agents, she died from cardiac tamponade after three months. Invasive pulmonary aspergillosis, which involves the mediastinum and the heart, is very rare in immunocompetent patients.

  • A Case of main pulmonary artery aneurysm in a 9-year-old boy with patent ductus arteriosus is presented. In this case presented with a huge mass density on the chest X-ray, radionuclide cardiac angiography showed a vascular lesion, which was confirmed as an aneurysm of the main pulmonary artery at roentgenologic angiogram. The aneurysm appeared following an episode of bacterial endocarditis and pulmonary hypertension. A successful aneurysmectomy with multiple ligation of ductus arteriosus was performed.

  • Papillary fibroelastomas are benign, and they are the second most common primary cardiac tumors usually involving the cardiac valve. Papillary fibroelastoma attached to the free margin of right coronary cusp of the aortic vlave was found incidentally during the work-up of a 51-year-old woman, who was presented with palpitation and dyspnea. During the operation, the tumor mass was excised without leaving defect on the aortic valve leaflet.

Inhibition of Histone Deacetylase Activity Diminishes Pressure Overloaded Cardiac Hypertrophy in Mice

  • To explore the role of histone deactylase (HDAC) activation in an in vivo model of hypertrophy, we studied the effects of Trichostatin A (TSA). TSA subjected to thoracic aortic banding (TAB)-induced pressure stress in mice. In histological observations, TAB in treated mice showed a significant hypertrophic response, whereas the sham operation remained nearly normal structure with partially blunted hypertrophy. TSA treatment had no effect (measured as HW/BW) on sham-operated animals. TAB animals treated with vehicle manifested a robust ~50% hypertrophic response (p<0.05 vs sham). TAB mice treated with 2 mg/kg/day TSA manifested a blunted growth responses, which was significantly diminished (p<0.05) compared with vehicle-treated TAB mice. TAB mice treated with a lower dose of TSA (0.5 mg/kg/day) manifested a similar blunting of hypertrophic growth (~25% increase in heart mass). Furthermore, to determine activity duration of TSA in vitro, 1 nM TSA was added to H9c2 cells. Histone acetylation was initiated at 4 hr after treatment, and it was peak up to 18 hr, then followed by significantly reduced to 30 hr. We also analyzed the expression of p53 following TSA treatment, wherein p53 expression was elevated at 4 hr, and it was maintained to 24 hr after treatment. ERK was activated at 8 hr, and maintained till 30 hr after treatment suggesting an intracellular signaling interaction between TSA and p53 expression Taken together, it is suggested that HDAC activation is required for pressure-overload growth of the heart. Eventually, these data suggest that histone acetylation may be a novel target for therapeutic intervention in pressure-overloaded cardiac hypertrophy.

Association between Exercise Capacity and Cardiovascular Risk Factors among Obesity Types in Adult Man

  • Increased waist circumference has shown to be more strongly associated with cardiovascular disease risk factors. The purpose of this study is to investigate the association between exercise capacity and cardiovascular risk factors among obese types in adult men. The subjects of this study were a total fifty-four obese persons and obesity criteria is body mass index $(BMI){\geq}25kg/m^2$. Diagnostic criteria for obesity was defined as a waist circumference of ${\geq}90cm$. The BMI in the obese subjects, as judged by the presence or absence of abdominal obesity, were classified into two groups (non-AO: without abdominal obesity group, AO: with abdominal obesity group). AO presented lower total exercise time, metabolic equivalents (METs) than Non-AO. AO showed slow HRR (heart rate recovery) response. HRR was negative correlated with BMI, body fat mass, waist circumference. AO had a high heart rate and a low cardiac output in submaximal exercise stage 1~2. In conclusion, AO's (with abdominal obesity groups) total exercise time, METs and HRR are lower than Non-AO. HRR is related with BMI, body fat mass and waist circumference.

  • A chest x-ray of 68 year old male showed pneumonic consolidation of right lower lung field with blunting of right cardiac border. Computerized tomography of chest revealed infiltrative mass with V-shaped calcification just below right main bronchus. This finding has to be made into differential diagnosis of numerous pulmonary diseases including, mycobacterial disease, neoplasm, lymphadenopathies, and foreign bodies. Initial bronchoscopic findings suggest endobronchial mass lesion on right intermediate bronchus but endobronchial biopsy fail to prove malignant cell or underlying illness. But repeated endobronchial biopsy shows metallic material in the right Intermediate bronchus and we remove it with alligator jaws forcep under bronchoscopy. He was well after discharge.

  • A 60-year-old male was admitted because of dyspnea and angina like chest pain. Noninvasive findings showed a mass at the posterior aspect of the left atrium. The mass was removed under cardiopulmonary bypass. An encapsulated adipose mass, which originated from the left atrium without any invasion to the pericardium, was completely excised. Histological examination showed mature adipose tissue with partial necrosis, confirming the diagnosis of lipoma. We report a rare of case of massive left atrial lipoma occupying the pericardial space.
