간 전이를 동반한 결장암 환자에서 한방치료의 효과 : 8년간의 추적조사를 통한 증례보고 (Constant State of Sigmoid Colon Cancer with Liver Metastasis by Herbal Medicine : A Case with an 8-year Follow-up)

  • Objectives : The aim of the present study is to derive further studies evaluating the effectiveness of oriental medical treatment on colon cancer patients. We present a case of a patient diagnosed with colon cancer with colon cancer with liver metastasis who has survived more than 8 years. Methods : We followed all of treatments and examination. We prescribed the patient to take HangAm-dan(HAD) three times a day for 5 years and 1 month from January, 2000 to February, 2005. Abdominal computed tomography(CT) was performed to evaluate the therapeutic efficacy and measuring concentration of carcinoembryonic antigen(CEA) in blood serum was also performed to monitor therapeutic response. Results : The patient was diagnosed with liver metastasis in September of 1999 but has survived for over 8 years since. Abdomen CT show no interval change. Conclusions : This case may give us a possibility that oriental medical treatment offers potential benefits for patients with colon cancer.

The clinical outcomes of second-line chemotherapy in patients with advanced pancreatic cancer: a retrospective study

  • Background: Despite recent advances in first-line chemotherapy for advanced pancreatic cancer, standard treatment after the failure of initial chemotherapy has not been established. Hence, we aimed to retrospectively analyze the clinical characteristics and outcomes of second-line chemotherapy in patients with advanced pancreatic cancer. Methods: We reviewed the clinical data of patients with advanced pancreatic cancer who underwent palliative chemotherapy at Kosin University Gospel Hospital between January 2013 and October 2020. Results: Among 366 patients with advanced pancreatic cancer who had received palliative chemotherapy, 104 (28.4%) underwent at least one cycle of second-line chemotherapy. The median age of the patients at the time of initiating second-line treatment was 62 years (interquartile range, 57-62 years), and 58.7% (61 patients) of them were male. The common second-line chemotherapy regimens were 5-fluorouracil (FU) plus leucovorin, irinotecan, and oxaliplatin (33 patients, 31.7%); gemcitabine/nab-paclitaxel (29, 27.9%), gemcitabine±erlotinib (13, 12.5%); and oxaliplatin and 5-FU/leucovorin (12, 11.5%). The median overall survival (OS) and progression-free survival were 6.4 months (95% confidence interval [CI], 4.5-8.6 months) and 4.5 months (95% CI, 2.7-6.3 months), respectively. In a multivariate analysis, poor performance status (PS) (hazard ratio [HR], 2.247; p=0.021), metastatic disease (HR, 2.745; p=0.011), and elevated carcinoembryonic antigen (CEA) levels (HR, 1.939; p=0.030) at the beginning of second-line chemotherapy were associated with poor OS. Conclusion: The survival outcome of second-line chemotherapy for advanced pancreatic cancer remains poor. However, PS, disease extent (locally advanced or metastatic), and CEA level may help determine patients who could benefit from second-line treatment.

Relationship Between Expression of Gastrokine 1 and Clinicopathological Characteristics in Gastric Cancer Patients

  • The aim of the study was to clarify the role of gastrokine 1 in the process of formation and development of gastric cancer. The expression of gastrokine 1 in gastric cancer and corresponding non-cancerous gastric tissues of 52 gastric cancer patients was assessed with the real-time fluorescence quantitative polymerase chain reaction (RT-PCR) and immunohistochemistry. We also analyzed the relationship between the expression level and clinicopathological characteristics. Gastrokine 1 gene and protein expression in gastric cancer tissues was in both cases significantly lower than in corresponding non-cancerous gastric tissues (both P<0.01), but no significant relationship was found with clinicopathological parameters including tumor location, depth of invasion, differentiation, lymph node metastasis, stage, gender, age and carcinoembryonic antigen (CEA), and carbohydrate antigen 19-9 (CA19-9) level in peripheral blood preoperation of patients (P>0.05, respectively). Furthermore, gastrokine 1 gene expression was markedly lower in gastric cancer tissues of Helicobacter pylori (HP)-positive patients than negative ones (P<0.05). The result of the study showed that gastrokine 1 might play a significant role in the process of formation and development of gastric cancer as an anti-oncogene. Its effect might be weakened by HP infection.

Preliminary Evaluation of Clinical Utility of CYFRA 21-1, CA 72-4, NSE, CA19-9 and CEA in Stomach Cancer

  • Background: Although various tumor markers have been utilized in management of stomach cancer (SC), only a few reports have described relevance of examples such as CYFRA 21-1 and neuron-specific enolase (NSE). The purpose of this study was to evaluate the potential diagnostic performance of carcinoembryonic antigen (CEA), CA 19-9, CA72-4, CYFRA 21-1 and NSE in patients with SC. Materials and Methods: Ninety-six SC patients with pathologic confirmation between 2012 and 2013 were enrolled. Serum levels of five tumor markers were analyzed using a solid-phase immunoradiometric assay. Receiver operating characteristic (ROC) curves were plotted for the five tumor markers to investigate their diagnostic powers and adjusted cutoff values derived from analysis of ROC curves were evaluated to calculate the sensitivity of each for SC with recommended cutoff values. Results: Based on two different cutoff values (recommended and adjusted), CYFRA 21-1 (${\geq}2.0$ and 1.2 ng/ml) had a respective sensitivity of 50% and 78.1%, compared with 8.3% and 18.8% for CEA (${\geq}7.0$ and 3.9 ng/ml), 15.6% and 18.8% for CA 19-9 (${\geq}37$ and 26.7 ng/ml), 28.1% and 9.6% for CA 72-4 (${\geq}4.0$ and 13 ng/ml) and 7.3% and 7.3% for NSE (${\geq}14.7$ and 15.0 ng/ml) in the initial staging of primary SC. The area under the curve (AUC) for CYFRA 21-1, with a value of 0.978 (95% confidence interval, 0.964-0.991) was comparatively the highest. Univariate analysis revealed significant relationships between tumor marker level and lymph node involvement, metastasis and staging with CYFRA 21-1, CA 72-4 and NSE. Conclusions: CYFRA 21-1 was the most sensitive tumor marker and showed the most powerful diagnostic performance among the five SC tumor markers. NSE and CA 72-4 are significantly related to lymph node involvement, metastasis or stage. Further evaluations are warranted to clarify the clinical usefulness and prognostic prediction of these markers in SC.

Prognostic Model Built on Blood-based Biomarkers in Patients with Metastatic Colorectal Cancer

  • Background: We had previously showed that the neutrophil lymphocyte ratio (NLR), ${\gamma}$-glutamyl transpeptidase (GGT) and carcinoembryonic antigen (CEA) are prognostic factors for metastatic colorectal cancer (mCRC) patients. In this study we developed a prognostic model based on these three indices. Materials and Methods: A total of 243 patients who were initially diagnosed as mCRC between 2005 and 2010 in the Sun Yat-sen University Cancer Center were studied. The endpoint was overall survival (OS). Results: NLR>3, elevated GGT and elevated CEA were confirmed as independent risk factors which could predict poor prognosis. Patients could be divided into three groups according to the number of risk factors they had. Those with two or three were defined as the high risk group, individuals with one risk factor as the modest risk group and patients without risk factor as the low risk group. The OS values for these three groups were 16.2 months (2.80~68.8), 24.2 months (4.07~79.0), and 37.2 months (12.6~87.8), respectively (p<0.001). Conclusions: We developed a simple but useful model based on NLR, GGT and CEA to provide prognostic information to clinical practice in highly selected mCRC patients. Further prospective and multi-center studies are warranted to test our model.

Exosomal miR-181b-5p Downregulation in Ascites Serves as a Potential Diagnostic Biomarker for Gastric Cancer-associated Malignant Ascites

  • Purpose: Peritoneal carcinomatosis in gastric cancer (GC) patients results in extremely poor prognosis. Malignant ascites samples are the most appropriate biological material to use to evaluate biomarkers for peritoneal carcinomatosis. This study identified exosomal MicroRNAs (miRNAs) differently expressed between benign liver cirrhosis-associated ascites (LC-ascites) and malignant gastric cancer-associated ascites (GC-ascites), and validated their role as diagnostic biomarkers for GC-ascites. Materials and Methods: Total RNA was extracted from exosomes isolated from 165 ascites samples (73 LC-ascites and 92 GC-ascites). Initially, microarrays were used to screen the expression levels of 2,006 miRNAs in the discovery cohort (n=22). Subsequently, quantitative reverse transcriptase-polymerase chain reaction (qRT-PCR) analyses were performed to validate the expression levels of selected exosomal miRNAs in the training (n=70) and validation (n=73) cohorts. Furthermore, carcinoembryonic antigen (CEA) levels were determined in ascites samples. Results: The miR-574-3p, miR-181b-5p, miR-4481, and miR-181d were significantly downregulated in the GC-ascites samples compared to the LC-ascites samples, and miR-181b-5p showed the best diagnostic performance for GC-ascites (area under the curve [AUC]=0.798 and 0.846 for the training and validation cohorts, respectively). The diagnostic performance of CEA for GC-ascites was improved by the combined analysis of miR-181b-5p and CEA (AUC=0.981 and 0.946 for the training and validation cohorts, respectively). Conclusions: We identified exosomal miRNAs capable of distinguishing between non-malignant and GC-ascites, showing that the combined use of miR-181b-5p and CEA could improve diagnosis.

Serum Carbohydrate Antigen 19-9 as an Indicator of Liver Metastasis in Colorectal Carcinoma Cases

  • Purpose: The liver is the organ to which colorectal carcinomas (CRCs) most commonly metastasize, and surgical resection has been established as the most effective and potentially curative treatment for CRC with liver metastasis (LM). Therefore, surveillance of LM is vital for improvement of prognosis of CRC patients. In this study, we aimed to explore the potential value of carbohydrate antigen 19-9 (CA 19-9), carcinoembryonic antigen (CEA), and marker enzymes in indicating LM with CRC. Methods: Three groups of eligible patients with metastatic cancers were retrospectively included: CRC patients with LM (CRC-LM) or without LM (CRC-NLM), and non-CRC patients with LM (NCRC-LM). All metastatic lesions were identified by CT or MRI. Data on characteristics of the patients, the primary site, the locations of metastasis, CA 19-9, CEA, and biochemical parameters were collected for analysis. Results: A total of 493 patients were retrospectively included. More alcohol consumption was found in CRC-LM than CRC-NLM. Some biochemical enzymes were found to be significantly higher in groups with LM than without (CRC-LM or NCRC-LM v.s CRC-NLM). Both CEA and CA 19-9 were much higher in CRC-LM than CRC-NLM or NCRC-LM. For CRC patients, CA 19-9, ${\gamma}$-glutamyl transpeptidase, CEA and alcohol consumption were identified as independent factors associated with LM. Conclusion: Our analysis suggested the CA 19-9 might be a potential valuable indicator for LM of CRC in the clinic.

항산균 도말 검사, 세포진 검사가 음성이고, ADA와 CEA가 낮은 림프구성 흉막 삼출증의 임상 양상과 예후 (Clinical Characteristics and Prognosis of Lymphocyte Dominant Exudative Pleural Effusion with Low ADA, Low CEA, Negative Cytology and Negative AFB Smear)

  • 연구 배경 : 흉수의 항산균 도말과 세포진 검사가 음성이며, 흉수의 ADA 가 40 IU/L 이하, CEA가 10 ng/ml 이하인 림프구성 흉막 삼출증의 임상적 특징과 예후를 살펴보고자 한다. 연구 방법 : 2000년 1월 1일부터 2001년 8월 31일 사이에 서울대학교 병원을 방문한 림프구성 흉막 삼출증 환자들 중 흉수의 항산균 도말과 세포진 검사가 음성이며, 흉수의 ADA가 40 IU/L 이하, CEA가 10 ng/ml 이하인 환자들의 의무 기록을 후향적으로 분석하였다. 연구 결과 : 43명의 림프구성 흉막 삼출증 환자들 중 31예에서 임상적으로 추정되는 흉막 삼출증의 원인 질환이 발견되었는데, 악성 종양이 21예로 가장 많았고, 나머지 12예에서는 흉막 삼출증의 원인을 찾을 수 없었다. 12예 중 7예에서 흉막 삼출증이 자연 관해 되었으며 2예에서는 항생제 치료를 하면서 흉막 삼출증이 소실 되었고, 1예에서는 소량의 흉막 삼출액이 지속되었다. 결 론 : 흉수의 항산균 도말 검사, 세포진 검사가 음성이고 흉수의 ADA 와 CEA 가 낮은, 원인 질환이 뚜렷하지 않은 림프구성 흉막 삼출증은 양호한 예후를 보이므로 임상의는 주기적인 추적 관찰을 통해 주의 깊게 임상 경과를 살펴볼 수 있다.

핵의학 체외검사실에서 시약 lot간 parallel test 시 변이 분석 (Analysis of Variation for Parallel Test between Reagent Lots in in-vitro Laboratory of Nuclear Medicine Department)

  • 핵의학 체외 검사실 에서는 시약 Lot가 변경될 때, Lot 간의 결과가 신뢰성이 있는지를 판단하기 위해 Lot 간 동등성 검사(comparability test between reagent lots) 또는 시약 병행 검사(reagent parallel test)를 시행하는데, 다수의 국내 검사실에서는 두 lot 간 결과 차이로부터 %difference를 구하여 저농도에서는 20% 이내, 중 고농도에서는 10% 이내로 설정하고 있으며 범위를 벗어 날 경우 재검사 시행으로 범위를 맞추는 실정이다. 따라서 본원의 핵의학 체외 검사실에서 시행되는 몇 가지 검사를 선정하여 parallel test의 결과를 분석해보았고, 검사별 맞춤 %difference 값 선정에 도움 될 만한 참고 자료를 마련해 보고자 하였다. Thyroid-stimulating hormone(TSH), Free thyroxine(FT4), Carcinoembryonic antigen(CEA), CA-125, Prostate-specific antigen(PSA) 그리고 HBs-Ab, insulin, 7종목에 대해 2018 1월부터 2018년 11월까지의 기간 동안의 시약 lot 변화에 따른 정도 관리 물질의 결과를 분석하였다. TSH, F-T4, CEA, CA-125, PSA의 측정에는 IRMA의 원리를 이용한 RIA-MAT 280 system이 사용되었고, Insulin의 측정에는 TECAN 자동화 분주 장비와 GAMMA-10 측정 장비가 사용되었다. HBs-Ab의 측정에는 HAMILTON 자동화 분주 장비와 GAMMA-10 측정 장비가 사용되었다. 각각 전용 시약과 전용 칼리브레이터, 전용 정도 관리 물질이 사용되었다. 1. HBs-Ab [%diffrence Max / Mean / Median] (P-value by t-test > 0.05) C-1(중농도) [9.6 / 3.7 / 2.7 / 0.2] C-2(고농도) [8.9 / 4.1 / 3.6 / 0.3] 7. insulin [%diffrence Max / Mean / Median] (P-value by t-test > 0.05) C-1(중농도) [8.7 / 3.1 / 2.4 / 0.9] C-2(고농도) [8.3 / 3.2 / 1.5 / 0.1] 모두 정도 관리 물질의 lot 변경 시에도 유의미한 차이가 없었으며 표본 수가 늘어남에 따라 검사실과 검사 종목 별 맞춤 허용 기준을 설정할 수 있을 것이라 기대할 수 있었다. 면역 방사 계수 측정법에서 비교적 검출률이 높은 종목들을 선정해서 일 것이라 판단되며 여러 번 재 측정된 결과 값이기 때문일 수도 있겠다. 대부분의 검사 결과에서 허용 기준인 10%에 크게 못 미치는 차이를 보였으며 저농도 target 값을 가진 경우에도 허용 기준인 20%에 가까운 수치를 보이진 않았다. 더 오랜 기간 동안의 관찰과 연구를 통해 평균의 균질화가 이루어진다면 종목 별 검사실 맞춤 허용 기준을 얻을 수 있을 것으로 판단되며 더 다양한 변수를 고려한 관찰과 연구도 필요할 것이다.

Biological Functions and Identification of Novel Biomarker Expressed on the Surface of Breast Cancer-Derived Cancer Stem Cells via Proteomic Analysis

  • Breast cancer is one of the most common life-threatening malignancies and the top cause of cancer deaths in women. Although many conventional therapies exist for its treatment, breast cancer still has many handicaps to overcome. Cancer stem cells (CSCs) are a well-known cause of tumor recurrences due to the ability of CSCs for self-renewal and differentiation into cell subpopulations, similar to stem cells. To fully treat breast cancer, a strategy for the treatment of both cancer cells and CSCs is required. However, current strategies for the eradication of CSCs are non-specific and have low efficacy. Therefore, surface biomarkers to selectively treat CSCs need to be developed. Here, 34 out of 641 surface biomarkers on CSCs were identified by proteomic analysis between the human breast adenocarcinoma cell line MCF-7 and MCF-7-derived CSCs. Among them, carcinoembryonic antigen-related cell adhesion molecules 6 (CEACAM6 or CD66c), a member of the CEA family, was selected as a novel biomarker on the CSC surface. This biomarker was then experimentally validated and evaluated for use as a CSC-specific marker. Its biological effects were assessed by treating breast cancer stem cells (BCSCs) with short hairpin (sh)-RNA under oxidative cellular conditions. This study is the first to evaluate the biological function of CD66c as a novel biomarker on the surface of CSCs. This marker is available as a moiety for use in the development of targeted therapeutic agents against CSCs.
